The statutory discharge management system in German hospitals does not provide adequate transitional care for geriatric patients. The American Transitional Care Model (TCM), where a qualified ...professional supports the patient for a period before, during, and after discharge, could help to address this problem. In this study, we compared both approaches from an economic perspective. Our research questions were: (1) How do treatment costs per person differ when geriatric patients are supported according to the TCM compared with routine discharge management? (2) What are the intervention costs?
The present economic analysis was part of a randomized controlled trial conducted at a hospital in Germany. In this study, geriatric patients in the intervention group received care according to TCM and those in the control group received routine care. We obtained data from the hospital and a health insurance company. In a cost-cost analysis, we compared the treatment costs per patient incurred in both study groups. In cases where higher costs occurred in the intervention group, we also conducted a cost-utility analysis, using the 12-Item Short Form Survey questionnaire to collect quality of life data for the Quality Adjusted Life Years calculation. Intervention costs were calculated on the basis of staff salaries, working hours, and the cost of acquiring the necessary equipment.
The intervention group consisted of 109 geriatric patients, the control group of 119. The average quality of life score was slightly higher in the intervention group, but the difference was not significant. On average, the intervention group was less expensive. However, for individual cost types (e.g., rehabilitation), the intervention group incurred higher costs than the control group. The differences between the study groups were not significant for all cost types (p > 0.05). Intervention costs were estimated to be approximately 800 euros per patient.
The TCM approach leads to savings. However, the amount of savings will depend on intervention costs, which vary in practice. After outlier exclusion, an unfavorable incremental cost-effectiveness ratio is observed for the TCM approach.
From an economic perspective, TCM in its present form provides no additional value to patient care compared with routine discharge management. However, we see an urgent need to optimize the existing discharge management system to ensure better transitional care for those affected. Therefore, we recommend that different TCM components should be tested in further studies to address the questions that could not be clarified in the present study.
Das gesetzliche Entlassmanagement in deutschen Krankenhäusern bietet geriatrischen Patienten keine adäquate transsektorale Versorgung. Das amerikanische Transitional Care Modell (TCM), bei dem eine Fachkraft den Patienten vor, während und nach der Entlassung eine Zeitlang begleitet, könnte bei diesem Problem helfen. In der vorliegenden Studie wurden beide Ansätze aus ökonomischer Perspektive verglichen. Dazu wurden folgende Forschungsfragen gestellt: (1) Wie unterscheiden sich Behandlungskosten bei Patientinnen und Patienten, die nach dem TCM-Ansatz begleitet werden, von denen, welche die Routinebetreuung erhalten? (2) Was kostet die Maßnahme?
Die vorliegende ökonomische Analyse war Teil einer randomisierten, kontrollierten Studie, die an einem Krankenhaus in Deutschland durchgeführt wurde. In dieser Studie wurden die geriatrischen Patienten in der Interventionsgruppe nach TCM betreut und die in der Kontrollgruppe erhielten Routinebetreuung. Die Evaluatoren wurden vom Krankenhaus und von einer Krankenkasse mit Daten versorgt. Im Rahmen einer Kosten-Kosten-Analyse wurden die für Behandlungen angefallenen Kosten pro Patient in beiden Studiengruppen verglichen. Für den Fall höherer Kosten in der Interventionsgruppe wurde mittels des Fragebogens 12-Item Short Form Survey die Lebensqualität der Probanden erhoben, damit diese bei Bedarf in Form von qualitätskorrigierten Lebensjahren in einer Kosten-Nutzwert-Analyse eingesetzt werden kann. Die Kosten der Maßnahme wurden auf der Basis von Personalgehältern, Arbeitszeiten sowie der Anschaffungskosten für die notwendige Ausstattung berechnet.
Die Interventionsgruppe umfasste 109 und die Kontrollgruppe 119 geriatrische Patientinnen und Patienten. Die durchschnittliche Lebensqualität war in der Interventionsgruppe etwas höher. Der Unterschied zur Kontrollgruppe war jedoch nicht signifikant. Im Schnitt war die Testgruppe günstiger. Bei einzelnen Kostenarten (z.B. Rehabilitation) wurden jedoch in der Interventionsgruppe höhere Kosten verursacht. Die Unterschiede zwischen den Studiengruppen waren bei allen Kostenarten nicht signifikant (p > 0,05). Die Kosten der Maßnahme wurden auf rund 800 Euro pro Patient geschätzt.
Der TCM-Ansatz ermöglicht Einsparungen. Sie müssen jedoch um Kosten der Maßnahme reduziert werden, wobei diese in der Praxis variieren. Nach Ausreißerausschluss kann außerdem ein unvorteilhaftes Kosten-Nutzwert-Verhältnis beim TCM-Ansatz vermutet werden.
Aus ökonomischer Perspektive bietet TCM in der getesteten Form keinen Mehrwert im Vergleich zum gesetzlichen Entlassmanagement. Die Autoren sehen jedoch einen dringenden Verbesserungsbedarf im bestehenden Entlassmanagement, um den Betroffenen eine bessere sektorenübergreifende Versorgung zu bieten. Deshalb empfehlen sie, verschiedene Komponenten aus TCM in weiteren Studien zu testen und dabei auf die Fragen einzugehen, die in der vorliegenden Studie nicht geklärt werden konnten.

Hintergrund
Mit Verabschiedung des „Digitale-Versorgung-Gesetzes“ durch den Deutschen Bundestag Ende 2019 wurde unter anderem ermöglicht, dass digitale Gesundheitsanwendungen (DiGA) ...unter bestimmten Voraussetzungen von den gesetzlichen Krankenkassen erstattet werden können. Ziel dieser Arbeit ist die Identifikation von DiGA mit Bezug zur HNO-Heilkunde und die Beschreibung der zugrunde liegenden Evidenz.
Material und Methoden
Es erfolgte eine Analyse des DiGA-Verzeichnisses nach DiGA, deren Indikationsbereich eine Erkrankung aus dem HNO-Bereich betrifft. Es wurden DiGA eingeschlossen, die entweder dauerhaft oder vorläufig aufgenommen wurden oder aktuell gestrichen sind, wenn hierzu weitere Informationen vorlagen. Es erfolgte eine Bewertung der zugrunde liegenden Evidenz nach den Empfehlungen des Oxford Centre for Evidence-Based Medicine für therapeutische Studien.
Ergebnisse
Insgesamt wurden sechs DiGA mit direktem oder indirektem Bezug zur HNO-Heilkunde identifiziert, von denen drei dauerhaft und zwei vorläufig in das Verzeichnis aufgenommen wurden. Eine DiGA ist aktuell vom Hersteller zurückgezogen worden. Den dauerhaft aufgenommenen DiGA liegt eine Evidenz Grad 1b zugrunde.
Schlussfolgerung
Die Einführung von DiGA wird teilweise auch kritisch diskutiert, dennoch stellt ihre Einführung einen innovativen Ansatz dar, und gerade für den HNO-Bereich sind bereits verschiedene DiGA mit hoher zugrunde liegender Evidenz verfügbar.
Zusammenfassung Hintergrund und Zielsetzung Vergleichsportale können helfen, einen geeigneten medizinischen Leistungserbringer, wie z.B. eine Hebamme, zu finden. Das Ziel dieser Arbeit ist, solche ...Portale in Deutschland zu analysieren und ihren Nutzen für Schwangere zu beurteilen. Methodik Über die Suchmaschine Google wurden 25 deutsche Webseiten mit Hilfe der PRISMA-Systematik identifiziert, die eine Hebammensuche in Deutschland ermöglichen. Anhand einer Stichprobe aus 550 Hebammenprofilen wurden dort der Inhalt (Struktur-, Prozess- und Ergebnisqualität gemäß dem Qualitätsmodell von Avedis Donabedian) und die Funktionen der Webseiten (z.B. Filtern, Sortieren, Vergleichen) untersucht sowie der Nutzen der Vergleichsportale anhand einer Checkliste mit fünf Kategorien (z.B. Sozialkompetenz) evaluiert. Ergebnisse Die Webseiten konzentrieren sich in den meisten Fällen ausschließlich auf Hebammensuche (92 %) und werden durch Hebammen (60 %) oder ihre Verbände (16 %) mit Informationen versorgt. Ein Drittel ( n = 8) der Webseiten gibt keine Informationsquelle an. Überwiegend sind Elemente der Strukturqualität und Möglichkeiten zum Suchen und Filtern von Informationen vorhanden. Mehrere bedeutende Informationen (z.B. Qualifikation, Ergebnisqualität) und Funktionen (z.B. Vergleichen, Bewerten) sind ungenügend vorhanden. Bei der Nutzenbewertung erreichten die Webseiten insgesamt 114 von 400 möglichen Punkten, was einem Abdeckungsgrad von weniger als 30% entspricht. Diskussion und Fazit Insgesamt konnte festgestellt werden, dass Vergleichsportale in der aktuellen Form für Schwangere bei der Suche nach einer geeigneten Hebamme nicht hilfreich sind und nicht das Potenzial haben, die Versorgung zu verbessern. Für eine adäquate Qualitätsberichterstattung auf Vergleichsportalen, müssen Informationen aus unabhängigen Quellen konsequent dargelegt werden. Neben strukturellen Informationen müssen auch „harte Fakten“ veröffentlicht werden und Bewertungen durch Patientinnen sollten möglich sein. Darüber hinaus sollten für eine bessere Handhabung der Informationen weitere Komplexität reduzierende Funktionen eingebaut werden.
Abstract Background and objective Comparison portals for health care providers search can help pregnant women to identify a suitable midwife. In the present study, we aimed to analyze such websites in Germany and to evaluate their benefit for pregnant women. Methodology Via search engine Google and using the PRISMA reporting statement, we identified 25 German websites that allow for a midwife search in Germany. After drawing a sample of 550 midwife profiles on those websites, we collected data on their content (structural, process, and outcome quality dimensions suggested by Avedis Donabedian) and functions (e.g., filtering, sorting, comparing), and evaluated the benefit of the websites via a five-category-checklist (e.g., social competence). Results Most websites focus exclusively on midwife search (92%) and are provided with information by midwives (60%) or their associations (16%). One third ( n = 8) of the websites do not provide information sources. Predominantly, elements of structural quality and options for searching and filtering of information are available. Several relevant information (e.g., qualification, outcome quality) and functions (e.g., comparing, ratings) are insufficiently available. In the benefit assessment, the websites achieved a total of 114 out of 400 points possible, which is equivalent to a coverage rate of less than 30%. Discussion and conclusion In the current form, comparison portals are not only less helpful for pregnant women when choosing a midwife, but also lack the potential to improve the quality of care. For appropriate quality reporting on comparison portals, information should be independent and transparent. In addition to structural quality information, “hard facts” must also be displayed and patient ratings should be possible. Furthermore, additional complexity-reducing functions should be integrated to make the information easier to use.

Multiple interventions are available for the treatment of actinic keratosis (AK). However, most randomized clinical trials and meta-analyses focus on short-term efficacy outcomes.
To investigate and ...synthesize the long-term efficacy (≥12 months) of interventions for AK from parallel-arm randomized clinical trials.
Searches in MEDLINE, Embase, and Central were conducted from inception until April 6, 2020. The reference lists of the included studies and pertinent trial registers were hand searched. The study was completed February 27, 2021.
Two reviewers screened the titles and abstracts of 2741 records. Finally, 17 published reports (original studies and follow-up reports) referring to 15 independent randomized clinical trials with an overall sample size of 4252 patients were included.
Two reviewers independently extracted data on study, patient, and intervention characteristics. Network meta-analysis (NMA) of each outcome was conducted with a frequentist approach. The Grading of Recommendations Assessment, Development, and Evaluation (GRADE) guidance for NMA was used to assess the certainty of evidence. The revised Cochrane risk-of-bias tool for randomized clinical trials was used to evaluate the methodologic quality.
Participant complete clearance, participant partial clearance, and lesion-specific clearance were the outcomes, with each assessed at least 12 months after the end of treatment.
Data from 15 independent randomized clinical trials including 4252 patients were extracted and synthesized. Ten studies were included in an NMA for the outcome of participant complete clearance, with photodynamic therapy with aminolevulinate (ALA-PDT) showing the most favorable risk ratio (RR) compared with placebo (RR, 8.06; 95% CI, 2.07-31.37; GRADE, moderate), followed by imiquimod, 5% (RR, 5.98; 95% CI, 2.26-15.84; GRADE, very low), photodynamic therapy with methyl aminolevulinate (MAL-PDT) (RR, 5.95; 95% CI, 1.21-29.41; GRADE, low), and cryosurgery (RR, 4.67; 95% CI, 1.36-16.66; GRADE, very low). Similarly, ALA-PDT had the highest RR in the NMA for lesion-specific clearance (RR, 5.08; 95% CI, 2.49-10.33; GRADE, moderate). No NMA was possible for participant partial clearance owing to poor reporting of this outcome.
This systematic review and network meta-analysis found that therapy including ALA-PDT, imiquimod, 5%, MAL-PDT, and cryosurgery was associated with significant long-term efficacy in the NMA. This study provides data for a possible use in an evidence-based framework for selecting interventions with sustained lesion clearance.

Selective literature search.
All permanent residents in Japan are enrolled in one of more than 3,000 compulsory health funds. Employees and public servants are covered through company or government-related health insurance schemes. Independent workers, the unemployed and the pensioners are usually assigned to health insurance plans managed by local city governments. The elderly over 75 years are insured through special health funds managed at the prefectural level. To correct the fiscal disparities among the health insurance programs, a risk adjustment is realized by compensatory financial transfers between the funds and substantial subsidies from the central and local governments. The statutory benefits package that is identical for all insurance plans is regulated in a single comprehensive schedule. All the covered health services and products are listed with the fees and compensations, and the conditions for the service providers to be remunerated are also stated. This fee and compensation schedule is regularly revised every 2 years under the leadership of the Ministry of Health, Labor and Welfare. The revisions are intended to contain health expenditures and to set incentives for the achievement of health policy goals.
The funding of the Japanese health care system and the risk adjustment mechanisms among health funds are well established and show a rather static character. The short- and mid-term development of the system is mainly controlled on the side of the expenditures through the unique and comprehensive fee and compensation schedule. The regular revisions of this schedule permit to react at relatively short notice to evolving situations, and through a policy of small improvements, target an optimization of the system as a whole.
Abstract Objectives Stroke unit care has been shown to be beneficial but costly. In an own previous study, the resource utilization of stroke unit care has been evaluated. Since the resource ...utilization on regular neurological wards is widely unknown, we determined the costs for acute stroke care on regular neurological wards to compare both treatment settings. Methods and patients We included 253 consecutive in-patients with the diagnosis of ischemic stroke (IS), intracerebral hemorrhage (ICH) or transient ischemic attack (TIA) treated on regular wards at a German University Department of Neurology, between 1 January and 30 June 1998. The modified Rankin scale (mRS) was used to assess outcome. Costs of stroke care were calculated from the perspective of the healthcare provider (hospital) by using a bottom-up approach. Resource utilization was compared to stroke unit care as determined in a previous study. Prices of 2002 were used (in Euros). Results IS was present in 78% ( n = 196), TIA in 13% ( n = 34), and ICH in 9% ( n = 23) of patients. Length of stay was 11.1 ± 8.9 (mean ± S.D., IS), 11.1 ± 6.5 (TIA), and 16.9 ± 15.5 (ICH) days ( p > 0.05). Mean costs of stroke care were €3060 (US$ 3180) for TIA, € 3070 (US$ 3200) for IS and € 5210 (US$ 5430) for ICH ( p < 0.05, ICH versus IS and TIA). The highest costs were due to non-medical care (46%) and personnel (25%). The mRS improved during hospitalization from 3.0 ± 1.6 to 2.2 ± 1.8 ( p < 0.01). Compared to care on regular neurological wards, mean costs per admission with treatment on stroke units increased by 7.0%, mean costs per day by 15.6%. Conclusion The comparison – considering a potential bias of patient selection – shows that acute stroke unit care is approximately 16% more costly than treatment on regular neurological wards due to higher resource use of personnel and diagnostic procedures. Stroke unit treatment tends to decrease post-acute in-patient care costs.

Fragestellung Erfragt wurde das Interesse der Patient*innen an einer Ergänzung der Tumornachsorge durch Apps zur Erfassung der Lebensqualität (patient-reported outcome PRO). Material und Methode Dazu erfolgte die Auswertung eines selbstständig erstellten Fragebogens zur Erfassung des Interesses von Tumorpatient*innen an einer digitalisierten Form der Tumornachsorge ( n = 110) Ergebnisse In der Studie zeigten sich Tumorpatient*innen zum Großteil interessiert an einer App-Nutzung zur Tumorerkrankung. Unter Einbeziehung des Alters stieg die Anzahl erwartungsgemäß bei Patient*innen < 60 Jahren noch weiter. Dazu passend zeigten für humanes Papillomavirus (HPV-)positive Patient*innen ein signifikant größeres Interesse ( p = 0,021). Schlussfolgerung Die Einführung einer App wird vom Großteil der Tumorpatient*innen befürwortet. Neben der Erfassung des PRO könnten hier weitere Anwendungen (Terminerinnerung, Patientenakte, Sport‑/Ernährungsprogramme) integriert werden.
