Dual-phase (DP) steel is the flagship of advanced high-strength steels, which were the first among various candidate alloy systems to find application in weight-reduced automotive components. On the ...one hand, this is a metallurgical success story: Lean alloying and simple thermomechanical treatment enable use of less material to accomplish more performance while complying with demanding environmental and economic constraints. On the other hand, the enormous literature on DP steels demonstrates the immense complexity of microstructure physics in multiphase alloys: Roughly 50 years after the first reports on ferrite-martensite steels, there are still various open scientific questions. Fortunately, the last decades witnessed enormous advances in the development of enabling experimental and simulation techniques, significantly improving the understanding of DP steels. This review provides a detailed account of these improvements, focusing specifically on (
a
) microstructure evolution during processing, (
b
) experimental characterization of micromechanical behavior, and (
c
) the simulation of mechanical behavior, to highlight the critical unresolved issues and to guide future research efforts.
Previous studies have shown that among populations with a high rate of consanguinity, there is a significant increase in the prevalence of cancer. Single nucleotide polymorphism (SNP) array data ...(Affymetrix, 50K XbaI) analysis revealed long regions of homozygosity in genomic DNAs taken from tumor and matched normal tissues of colorectal cancer (CRC) patients. The presence of these regions in the genome may indicate levels of consanguinity in the individual's family lineage. We refer to these autozygous regions as identity-by-descent (IBD) segments. In this study, we compared IBD segments in 74 mostly Caucasian CRC patients (mean age of 66 years) to two control data sets: (a) 146 Caucasian individuals (mean age of 80 years) who participated in an age-related macular degeneration (AMD) study and (b) 118 cancer-free Caucasian individuals from the Framingham Heart Study (mean age of 67 years). Our results show that the percentage of CRC patients with IBD segments (>or=4 Mb length and 50 SNPs probed) in the genome is at least twice as high as the AMD or Framingham control groups. Also, the average length of these IBD regions in the CRC patients is more than twice the length of the two control data sets. Compared with control groups, IBD segments are found to be more common among individuals of Jewish background. We believe that these IBD segments within CRC patients are likely to harbor important CRC-related genes with low-penetrance SNPs and/or mutations, and, indeed, two recently identified CRC predisposition SNPs in the 8q24 region were confirmed to be homozygous in one particular patient carrying an IBD segment covering the region.

Analysis of relevant diagnoses, medical procedures and G-DRGs in the versions 2004 and 2014 based on the publications of the German DRG Institute (InEK) and the German Institute of Medical Documentation and Information (DIMDI).
The number of G-DRGs in the whole system increased by 45.1 % between 2004 and 2014. The number of G-DRGs in the major diagnostic category (MDC) 08 that contains the majority of orthopedic and trauma surgery categories increased in the same period by 61.6 %. The reduction of variance of inlier costs in the MDC 08 category, a statistical measure of the performance of the G-DRG system, was below the corresponding value of the total system in 2004 as well in 2014. However, the reduction of variance of inlier costs in MDC 08 (+ 30.0 %) rose more from 2004 to 2014 than the corresponding value of the overall system (+ 21.5 %).
Many modifications of the classification systems of diagnoses (ICD-10-GM) and medical procedures (OPS) and the structures of the G-DRG system could significantly improve the quality of case allocation from the perspective of orthopedics and trauma surgery between 2004 and 2014. Th assignment of cases could be differentiated so that complex cases with more utilization of resources were allocated to higher rated G-DRGs and vice versa. However, further improvements of the G-DRG system are necessary. Only correct and complete documentation and coding can provide a high quality of calculation of costs as a basis for a correct case allocation in future G-DRG systems.

The DRG catalog is has grown from 8 to 1,200 G-DRGs. A number of codes for surgical measures have been newly established or modified. Here, the identification and the correct and performance-based mapping of complex and elaborate scenarios was again the focus of the restructuring of the G-DRG system. The G-DRG structure in orthopedics and traumatology has been changed, especially in the areas of spinal surgery and surgery of the upper and lower extremities. The actual impact of the changes may vary depending on the individual hospital services.
For the first time since the introduction of the G-DRG system, the pure numerical changes at the level of DRGs themselves are so marginal that only part of the DRG users in the hospitals will register them. The changes implemented not only a high selectivity between complex and less complex scenarios, but partly also unintended and unjustified revaluation of less complex measures. The G-DRG system has gained complexity again. Especially the G-DRG allocation of spinal surgery and multiple surgical interventions of the upper and/or lower extremities have reached such a complexity that only a few DRG users can follow them.

A number of codes for surgical measures have been newly established or modified - above all in foot surgery, arthroscopic surgery and wound surgery. Here, the identification and the correct and performance-based mapping of complex and elaborate scenarios was again the focus of the restructuring of the G-DRG system. The G-DRG structure in orthopaedics and traumatology is changed, especially for polytraumata.
The allocation of common cases with a standardized treatment pattern appears to be appropriate and the reimbursement adequate. For the less common and more complex cases the 2011 G-DRG system still shows need for further modification (e.g. polytraumata, joint replacement, spine surgery). The proper integration of the modified OPS classification for foot surgery to the appropriate G-DRGs will be essential to maintain the high quality of the reimbursement structure for the future.

Welche Entwicklungen sind seit der Systemeinführung 2004 bis zur G-DRG-Systemversion 2014 für das Fach Orthopädie und Unfallchirurgie eingetreten? Dieser Beitrag zieht eine Zwischenbilanz des Weiterentwicklungsprozesses aus der Perspektive der Leistungserbringer.

Die Anzahl der G-DRGs des Gesamtsystems erhöhte sich zwischen den Versionen 2004 und 2014 um 45,1 %, die der Hauptdiagnosekategorie (MDC), die den Großteil des Fachs Orthopädie und Unfallchirurgie beinhaltet, erhöhte sich im gleichen Zeitraum um 61,6 %. Die Varianzreduktion der Kosten der Normallieger, ein Maß für die statistische Güte des Systems, lag für die MDC 08 in beiden Systemversionen unterhalb des vergleichbaren Werts des Gesamtsystems. Allerdings erhöhte sich die Varianzreduktion der Kosten zwischen den Versionen 2004 und 2014 für die MDC 08 mit + 30,0 % wesentlich stärker als die des jeweiligen Gesamtsystems (+ 21,5 %).
Schlussfolgerungen
Durch zahlreiche Modifizierungen der Klassifikationssysteme für Diagnosen und Prozeduren und der G-DRG-Strukturen konnte die Abbildungsqualität des Fachs Orthopädie und Unfallchirurgie bis zur Systemversion 2014 deutlich erhöht werden. Dies geht mit einer deutlich differenzierteren Fallzuordnung einher, sodass komplexe Leistungen aufgewertet und weniger komplexe Leistungen im Vergleich dazu abgewertet wurden. Allerdings findet sich auch in der Systemversion 2014 noch eine Reihe von Verbesserungsnotwendigkeiten. Nur durch eine vollständige und korrekte Kodierung kann eine sachgerechte Fallzuordnung sichergestellt werden. Dies schafft darüber hinaus auch die Datengrundlage für die Kalkulation zukünftiger G-DRG-Systeme.

The relevant diagnoses, medical procedures and G-DRGs in the versions 2011 and 2012 were analyzed based on the publications of the German DRG Institute (InEK) and the German Institute of Medical Documentation and Information (DIMDI).
Changes were made for the International Classification of Diseases (ICD) coding of complex cases with medical complications, the procedure coding for spinal surgery and for hand and foot surgery. The G-DRG structures were modified for endoprosthetic surgery on ankle, shoulder and elbow joints. The definition of modular structured endoprostheses was clarified.
The G-DRG system for orthopedic and trauma surgery appears to be largely consolidated. The current phase of the evolution of the G-DRG system is primarily aimed at developing most exact descriptions and definitions of the content and mutual delimitation of operation and procedures coding (OPS). This is an essential prerequisite for a correct and performance-based case allocation in the G-DRG system.

Dieser Beitrag analysiert und bewertet die relevanten Weiterentwicklungen der G-DRG-Systemversion 2011 für die Orthopädie und Unfallchirurgie aus medizinischer und klassifikatorischer Perspektive.

Ergebnisse
Eine Reihe von OPS-Kodes zur Kodierung operativer Maßnahmen wurde neu etabliert bzw. modifiziert – insbesondere in den Bereichen Fußchirurgie, arthroskopische Operationen und Wunddébridement. Hierbei stand die Identifikation und die sachgerechte Abbildung von komplexen Fallkonstellationen erneut im Mittelpunkt der Umstrukturierungen. Die G-DRG-Struktur zur Abbildung der Orthopädie und Unfallchirurgie wird v. a. für Schwerverletzte modifiziert. Zukünftiger Anpassungsbedarf findet sich für die Revisionsendoprothetik und der Wirbelsäulenchirurgie.
Schlussfolgerungen
Für häufige Standardfälle ist, abgesehen von wenigen Details, bereits heute eine ausreichende Abbildungsqualität, verbunden mit einer sach- und leistungsgerechten Refinanzierung, gegeben. Für seltenere bzw. komplexere Fallkonstellationen besteht auch 2011 noch ein z. T. nicht unerheblicher Weiterentwicklungsbedarf. Dies betrifft neben Optimierungen im Bereich der Schwerverletztenversorgung auch die Revisionsendoprothetik und die Wirbelsäulenchirurgie. Wesentlich für die zukünftige Abbildungsqualität der Fußchirurgie wird es sein, die Umstellung der OPS-Klassifikation sachgerecht ins G-DRG-System zu integrieren.

Ergebnisse
Veränderungen ergaben sich bei der Diagnosekodierung komplikativer Behandlungsverläufe, bei der OPS-Kodierung operativer Eingriffe an der Wirbelsäule sowie an Hand und Fuß. Auf der Ebene der G-DRG wurden endoprothetische Eingriffe an Sprung-, Schulter- und Ellenbogengelenk differenzierter eingruppiert. Bei modularen Endoprothesen gab es eine zwischen Fachgesellschaft, SEG 4 der MDK-Gemeinschaft und DIMDI konsentierte Klarstellung.
Schlussfolgerungen
Die G-DRG-Systementwicklung für die Orthopädie und Unfallchirurgie erscheint mittlerweile weitgehend konsolidiert. Die aktuelle Phase der Weiterentwicklung des G-DRG-Systems zielt v. a. auf eine möglichst exakte inhaltliche Beschreibung und gegenseitige Abgrenzung der OPS-Kodes. Dies ist eine wesentliche Voraussetzung für eine sach- und leistungsgerechte Abbildung orthopädisch/unfallchirurgischer Leistungen mit unterschiedlichem Ressourcenaufwand im G-DRG-System.
