Introduction Les recommandations des sociétés savantes (SFE) proposent la mise sous antithyroïdiens de synthèse (ATS) en première intention dans le cadre des hyperthyroïdies d’origine auto-immune ...(HAI). Les effets secondaires des ATS sont bien connus. Une étude récente rappelle la possibilité d’un traitement alternatif par iodure de potassium (IK) en cas d’effets secondaires (ES) sous ATS. Observation Nous présentons les cas cliniques de 6 patients pris en charge pour une HAI, mis sous IK 130 mg/j comme traitement alternatif au traitement par ATS. Deux indications : groupe 1 : 2 patientes présentant une CI pré-thérapeutique avec neutropénie au diagnostic : la première neutropénie spontanée PNN à 280/mm3 avec biopsie moelle osseuse normale. Sous IK, retour à l’euthyroïdie et correction de la neutropénie, permettant la mise sous ATS à dose minimale efficace. La deuxième en cours de chimiothérapie neutropéniante mise sous IK, jusqu’à la fin de la chimiothérapie puis mise sous ATS. Groupe 2 : 4 patients avec des ES type hépatite, leuco-neutropénie et allergie cutanée sous carbimazole. Retour à l’euthyroïdie sous IK au bout de 3 semaines et disparition des ES. Puis traitement radical, par thyroïdectomie totale dans un délai moyen d’environ 2 mois. Dans tous les cas, il existe une bonne tolérance au traitement par IK sans ES. Conclusion L’iodure de potassium est une alternative efficace permettant de passer un cap aiguë dans certains cas avec un délai rapide.
Introduction La découverte fortuite d’un micro cancer papillaire ≤ 5 mm (mPC≤5 mm) après une chirurgie thyroïdienne est une situation fréquente. Ils représentent en 2009, 30 % des nouveaux cas de ...cancer thyroïdien. Objectif À partir des données du registre des cancers de la thyroïde Marne-Ardennes, préciser l’évolution de l’incidence sur 35 ans, décrire la survenue des évènements évolutifs avec un recul d’au moins 5 ans pour les patients ayant présenté un mCP ≤ 5 mm découvert fortuitement lors de l’examen anatomopathologique après une chirurgie thyroïdienne pour une pathologie bénigne. Méthodes Étude rétrospective. Patients avec un cancer papillaire ≤ 5 mm, pT1a ou pT1am (somme des lésions moins de 10 mm), Np0 ou Nx, M0, sans traitement par131 I. Résultats Sur les 2056 cancers thyroïdiens enregistrés entre 1975 et 2009, 424 (20 %) sont des mPC ≤ 5 mm de découverte histologique fortuite. L’incidence est passée de 0,3 (période 1975–1979) à 3,49 (période 2005–2009) pour 100 000 habitants. Aucun des 424 patients n’a présenté de métastase ganglionnaire ou à distance. Aucun patient n’est décédé de son cancer thyroïdien. Parmi les 165/424patients ayant eu une chirurgie partielle, 8 ont été réopérés ; 2 avaient un micro carcinome < 5 mm et un patient un carcinome papillaire de 25 mm. Conclusion Cette étude confirme l’augmentation importante des carcinomes papillaire ≤ 5 mm de découverte histologique fortuite, leur excellent pronostic sans événement évolutif et sans décès lié au cancer.
Objective
To retrospectively compare frequency‐place mismatch among adult cochlear implant (CI) recipients with lateral wall (LW) and perimodiolar/Mid Scala (PM/MS) arrays, and to quantify the impact ...of these factors on early post‐activation (3 months) speech recognition abilities and CI‐specific quality of life.
Methods
One hundred and twenty‐six adult participants were separated into two groups: (1) 83 participants who underwent CI with a PM/MS array and 43 patients who underwent CI with a LW array. All participants completed the Cochlear Implant Quality of Life Profile (CIQOL‐35 Profile) instrument. Angular insertion depth and semitone mismatch, which contribute to frequency‐place mismatch, were assessed using post‐operative CT scans. Word and speech recognition in quiet were determined using the Consonant‐Nucleus‐Consonant (CNC) and the AzBio tests, respectively (n = 82 patients).
Results
LW arrays were more deeply inserted and exhibited less semitone mismatch compared to PM/MS arrays. No significant relationship was found between semitone mismatch and early post‐operative speech perception scores for either PM/MS or LW arrays. However, greater degrees of semitone mismatch were associated with lower CIQOL‐35 profile scores for PM/MS arrays.
Conclusions and Relevance
The results of this study indicate that both the degree of frequency‐place mismatch, and its impact on CI‐specific quality of life, vary by CI array design.

Hearing outcomes are highly variable and difficult to predict among adult CI recipients. In this study, we found that the shape of CI arrays can impact frequency‐place mismatch and CI‐specific quality of life.
Identify barriers and facilitating factors in cochlear implant (CI) utilization by comparing functional measures between CI candidates who undergo or forgo implantation.
Forty-three participants were ...separated into two groups: (1) 28 participants who underwent CI and (2) 15 participants who elected not to proceed with CI despite meeting eligibility criteria (no-CI). Prior to implantation, all participants completed the CI Quality of Life (CIQOL)-35 Profile and CIQOL-Expectations instrument. They were also surveyed on factors contributing to their decision to either undergo or forgo CI. Word and speech recognition were determined using the Consonant-Nucleus-Consonant (CNC) and the AzBio tests, respectively.
CIQOL-Expectations scores were indistinguishable between groups, but there were substantial differences in baseline CIQOL-35 Profile scores. Compared to the CI group, the no-CI group exhibited higher pre-CI scores in the Emotional (Cohen's d 95% CI = 0.8 0.1, 1.5) and Entertainment (Cohen's d 95% CI = 0.8 0.1, 1.5) domains. Survey data revealed that the most commonly reported barriers to pursuing CI in the no-CI cohort were fear of surgical complications (85%), cost associated with implantation (85%), and perception that hearing was not poor enough for CI surgery (85%).
The results of this study indicate that functional outcome expectations are similar between candidates who elect to receive or forgo CI, yet those who forgo CI have higher baseline CI-specific QOL abilities.

Le carcinome médullaire de la thyroide (CMT) avec mutation du codon 804 de l’exon 14 du proto-oncogène RET est classé, selon l’ATA, à faible risque d’agressivité. Nous avons voulu étudier le ...phénotype des patients porteurs de la mutation 804 de RET en France. Quatre-vingt-sept familles, incluant 372 patients, ont été identifiées (index = 87 ; apparentés = 271) (83 % V804 M ; 16 % V804L ; 0,3 % V804A). L’âge moyen des cas index au diagnostic est de 55 ans 17–83. L’anatomopathologie a diagnostiqué un CMT multifocal, une hyperplasie des cellules C (HCC) et des métastases ganglionnaires dans 62 %, 43 % et 51 % des cas. Les médianes des calcitonines (CT) pré et post opératoires sont de : 313 ng/L ; 141,5 ng/L. La médiane de survie sans récidive est de 3,4 ans (IC95 % 0 ; 8). L’âge moyen des apparentés mutés est de 37 ans 2–91. Cinquante-cinq pourcent ont été opérés et l’anatomopathologie a retrouvé 53 % de CMT (2/3 multifocaux), 35 % d’HCC isolée et 12 % de métastases ganglionnaires. Les CT pré, post opératoires sont indétectables : 41 %, 91 % des cas (médianes CT détectables préop = 21 ng/L 5,8–2096 ; CTpostop = 6,2 ng/L 6–132). La moyenne de survie sans récidive est de 20,3 ans (IC95 % 17 ; 23,6). La mutation V804L est associée à une CT préopératoire plus élevée. Un polymorphisme est plus souvent retrouvé lorsqu’il y a un CMT. On note 4 hyperparathyroïdies et 6 phéochromocytomes. La mutation 804 est certes responsable de CMT d’apparition tardive, mais avec des facteurs pronostiques en relation avec le type de mutation et certains polymorphismes. Ces patients doivent être suivis, contrairement à ce que nous avons constaté : 55 % suivis.

Le cancer médullaire de la thyroïde (CMT) est héréditaire dans 40 % des cas par mutation germinale du proto-oncogène RET. La mutation du codon 804 est classée à faible risque d’apparition du CMT. ...Nous avons voulu étudier le phénotype des patients porteurs de la mutation 804 de RET en France et évaluer son niveau de risque. Cette étude rétrospective du groupe de tumeurs endocrines (GTE), a recruté les sujets index et apparentés via 11 laboratoires de référence, avec recueil des données clinico-biologiques. Trois-cents-dix-neuf patients, de 45 familles porteuses de la mutation 804, sont inclus. Les premières analyses de la série indiquent chez les cas index un âge moyen de découverte du CMT et de thyroïdectomie de 55 ans (IC 95 % = 51-59). Chez les apparentés, la découverte de la mutation était à 37 ans (IC 95 % = 32,9-40,9, Min-Max = 2-91 ans). 58 % des apparentés ont eu une thyroïdectomie, avec une calcitonine préopératoire anormale chez 42,5 % des sujets. L’anatomopathologie a retrouvé respectivement chez les cas index et apparentés : 59 % et 37 % de CMT multifocal, 39 % et 58 % d’hyperplasie des cellules C, 44 % et 22 % de curage ganglionnaire positif. 17,5 % des apparentés non opérés avaient une calcitonine anormale. Dans 3 % des cas, le CMT était associé à un phéochromocytome ou une hyperparathyroïdie. La prévalence d’une double mutation ou d’un polymorphisme de RET était respectivement de 6 % et 17 %. Cette première étude nationale rapporte tous les porteurs de la mutation 804 de RET. L’apparition du CMT est tardive mais les phénotypes sont hétérogènes. L’analyse complète sera rapportée dans la communication.

The cochlear implant (CI) is widely considered to be one of the most innovative and successful neuroprosthetic treatments developed to date. Although outcomes vary, CIs are able to effectively ...improve hearing in nearly all recipients and can substantially improve speech understanding and quality of life for patients with significant hearing loss. A wealth of research has focused on underlying factors that contribute to success with a CI, and recent evidence suggests that the overall health of the cochlea could potentially play a larger role than previously recognized. This article defines and reviews attributes of cochlear health and describes procedures to evaluate cochlear health in humans and animal models in order to examine the effects of cochlear health on performance with a CI. Lastly, we describe how future biologic approaches can be used to preserve and/or enhance cochlear health in order to maximize performance for individual CI recipients.
