In the era of "big data," science is increasingly information driven, and the potential for computers to store, manage, and integrate massive amounts of data has given rise to such new disciplinary ...fields as biomedical informatics. Applied ontology offers a strategy for the organization of scientific information in computer-tractable form, drawing on concepts not only from computer and information science but also from linguistics, logic, and philosophy. This book provides an introduction to the field of applied ontology that is of particular relevance to biomedicine, covering theoretical components of ontologies, best practices for ontology design, and examples of biomedical ontologies in use.After defining an ontology as a representation of the types of entities in a given domain, the book distinguishes between different kinds of ontologies and taxonomies, and shows how applied ontology draws on more traditional ideas from metaphysics. It presents the core features of the Basic Formal Ontology (BFO), now used by over one hundred ontology projects around the world, and offers examples of domain ontologies that utilize BFO. The book also describes Web Ontology Language (OWL), a common framework for Semantic Web technologies. Throughout, the book provides concrete recommendations for the design and construction of domain ontologies.
Miranda Fricker has characterized epistemic injustice as "a kind of injustice in which someone is wronged specifically in her capacity as a knower" (2007, Epistemic injustice: Power & the ethics of ...knowing. Oxford: Oxford University Press, 20). Gaslighting, where one agent seeks to gain control over another by undermining the other's conception of herself as an independent locus of judgment and deliberation, would thus seem to be a paradigm example. Yet, in the most thorough analysis of gaslighting to date (Abramson, K. 2014. "Turning up the lights on gaslighting." Philosophical Perspectives 28, Ethics: 1-30), the idea that gaslighting has crucial epistemic dimensions is rather roundly rejected on grounds that gaslighting works by means of a strategy of assertion and manipulation that is not properly understood in epistemic terms. I argue that Abramson's focus on the gaslighter and on the moral wrongness of his actions leads her to downplay ways in which gaslighters nevertheless deploy genuinely epistemic strategies, and to devote less attention to the standpoint and reasoning processes of the victim, for whom the experience of gaslighting has substantial and essential epistemic features. Taking these features into account reveals that all gaslighting has epistemic dimensions and helps to clarify what resistance to gaslighting might look like.
The mTOR complex 1 (mTORC1) pathway promotes cell growth in response to many cues, including amino acids, which act through the Rag guanosine triphosphatases (GTPases) to promote mTORC1 translocation ...to the lysosomal surface, its site of activation. Although progress has been made in identifying positive regulators of the Rags, it is unknown if negative factors also exist. Here, we identify GATOR as a complex that interacts with the Rags and is composed of two subcomplexes we call GATOR1 and -2. Inhibition of GATOR1 subunits (DEPDC5, Nprl2, and Nprl3) makes mTORC1 signaling resistant to amino acid deprivation. In contrast inhibition of GATOR2 subunits (Mios, WDR24, WDR59, Seh1l, and Sec13) suppresses mTORC1 signaling, and epistasis analysis shows that GATOR2 negatively regulates DEPDC5. GATOR1 has GTPase-activating protein (GAP) activity for RagA and RagB, and its components are mutated in human cancer. In cancer cells with inactivating mutations in GATOR1, mTORC1 is hyperactive and insensitive to amino acid starvation, and such cells are hypersensitive to rapamycin, an mTORC1 inhibitor. Thus, we identify a key negative regulator of the Rag GTPases and reveal that, like other mTORC1 regulators, Rag function can be deregulated in cancer.
Recent literature on epistemic innocence develops the idea that a defective cognitive process may nevertheless merit special consideration insofar as it confers an epistemic benefit that would not ...otherwise be available. For example, confabulation may be epistemically innocent when it makes a subject more likely to form future true beliefs or helps her maintain a coherent self-concept. I consider the role of confabulation in typical cases of interpersonal gaslighting, and argue that confabulation will not be epistemically innocent in such cases even if it does preserve a coherent self-concept or belief-set for the subject. Analyzing the role of confabulation in gaslighting illustrates its role in on-going interpersonal relationships, and augments already growing evidence that confabulation may be quite widespread. The role of confabulation in gaslighting shows that whether confabulation confers epistemic benefits (and so is epistemically innocent) will depend greatly on the interpersonal context in which it is deployed, while whether a coherent self-concept is epistemically beneficial will depend to a great extent on the content of that self-concept. This shows that the notion of an epistemically harmful or beneficial feature of a cognitive process can and should be further clarified, and that doing so has both theoretical and practical advantages in understanding epistemic innocence itself.

In light of this analysis, it introduces the concept of epistemic ...pornography and argues that epistemic agents both can and should avoid consuming and sharing epistemic pornography.

The paper draws on research on epistemic virtue, cognitive biases, social media use and its epistemic consequences, fake news, paternalistic nudging, pornography, moral philosophy, moral elevation and moral exemplar theory to analyze the epistemically pernicious effects of the internet and social media.
Findings
There is a growing consensus that the internet and social media activate and enable human cognitive biases leading to what are here called “failures of epistemic virtue.” Common formulations of this problem involve the concept of “fake news,” and strategies for responding to the problem often have much in common with paternalistic “nudging.” While fake news is a problem and the nudging approach holds out promise, the paper concludes that both place insufficient emphasis on the agency and responsibility of users on the internet and social media, and that nudging represents a necessary but not sufficient response.
Originality/value
The essay offers the concept of epistemic pornography as a concept distinct from but related to “fake news” – distinct precisely because it places greater emphasis on personal agency and responsibility, and following recent literature on moral elevation and moral exemplars, as a heuristic that agents might use to economize their efforts at resisting irrational cognitive biases and attempting to live up to their epistemic duties.
BonJour has proposed background conditions on internalist justification, which Kornblith argues are inconsistent with a core internalist ‘credo’ – that subjects internally alike are justificationally ...alike – signaling the ‘death’ of internalism. The funeral arrangements are premature, though more systematic consideration of background conditions is needed. The majority of BonJour's conditions are not background conditions as their failure makes an epistemically relevant internal difference. This neutralizes Kornblith's criticisms and reveals how the proposed conditions are not departures from internalism. BonJour's background conditions are simply conditions, the credo remains intact, and Kornblith's objections do not spell problems for internalism, let alone death.
Objective: This paper introduces the objectives, methods and results of ontology development in the EU co-funded project Advancing Clinico-genomic Trials on Cancer – Open Grid Services for Improving ...Medical Knowledge Discovery (ACGT). While the available data in the life sciences has recently grown both in amount and quality, the full exploitation of it is being hindered by the use of different underlying technologies, coding systems, category schemes and reporting methods on the part of different research groups. The goal of the ACGT project is to contribute to the resolution of these problems by developing an ontology-driven, semantic grid services infrastructure that will enable efficient execution of discovery-driven scientific workflows in the context of multi-centric, post-genomic clinical trials. The focus of the present paper is the ACGT Master Ontology (MO).
Methods: ACGT project researchers undertook a systematic review of existing domain and upper-level ontologies, as well as of existing ontology design software, implementation methods, and end-user interfaces. This included the careful study of best practices, design principles and evaluation methods for ontology design, maintenance, implementation, and versioning, as well as for use on the part of domain experts and clinicians.
Results: To date, the results of the ACGT project include (i) the development of a master ontology (the ACGT-MO) based on clearly defined principles of ontology development and evaluation; (ii) the development of a technical infrastructure (the ACGT Platform) that implements the ACGT-MO utilizing independent tools, components and resources that have been developed based on open architectural standards, and which includes an application updating and evolving the ontology efficiently in response to end-user needs; and (iii) the development of an Ontology-based Trial Management Application (ObTiMA) that integrates the ACGT-MO into the design process of clinical trials in order to guarantee automatic semantic integration without the need to perform a separate mapping process.

Little is known about platelet transfusions in pediatric critical illness. We sought to describe the epidemiology, indications, and outcomes of platelet transfusions among critically ill children.

Multicenter (82 PICUs), international (16 countries) from September 2016 to April 2017.
Children ages 3 days to 16 years prescribed a platelet transfusion in the ICU during screening days.
None.
Over 6 weeks, 16,934 patients were eligible, and 559 received at least one platelet transfusion (prevalence, 3.3%). The indications for transfusion included prophylaxis (67%), minor bleeding (21%), and major bleeding (12%). Thirty-four percent of prophylactic platelet transfusions were prescribed when the platelet count was greater than or equal to 50 × 10 cells/L. The median (interquartile range) change in platelet count post transfusion was 48 × 10 cells/L (17-82 × 10 cells/L) for major bleeding, 42 × 10 cells/L (16-80 × 10 cells/L) for prophylactic transfusions to meet a defined threshold, 38 × 10 cells/L (17-72 × 10 cells/L) for minor bleeding, and 25 × 10 cells/L (10-47 × 10 cells/L) for prophylaxis in patients at risk of bleeding from a device. Overall ICU mortality was 25% but varied from 18% to 35% based on indication for transfusion. Upon adjusted analysis, total administered platelet dose was independently associated with increased ICU mortality (odds ratio for each additional 1 mL/kg platelets transfused, 1.002; 95% CI, 1.001-1.003; p = 0.005).
The majority of platelet transfusions are given as prophylaxis to nonbleeding children, and significant variation in platelet thresholds exists. Studies are needed to clarify appropriate indications, with focus on prophylactic transfusions.
