ABSTRACT Diffuse X-rays from the Local Galaxy (DXL) is a sounding rocket mission designed to quantify and characterize the contribution of Solar Wind Charge eXchange (SWCX) to the Diffuse X-ray ...Background and study the properties of the Local Hot Bubble (LHB). Based on the results from the DXL mission, we quantified and removed the contribution of SWCX to the diffuse X-ray background measured by the ROSAT All Sky Survey. The "cleaned" maps were used to investigate the physical properties of the LHB. Assuming thermal ionization equilibrium, we measured a highly uniform temperature distributed around kT = 0.097 keV 0.013 keV (FWHM) 0.006 keV (systematic). We also generated a thermal emission measure map and used it to characterize the three-dimensional (3D) structure of the LHB, which we found to be in good agreement with the structure of the local cavity measured from dust and gas.
ABSTRACT We report abundances of elements from 26Fe to 40Zr in the cosmic radiation measured by the SuperTIGER (Trans-Iron Galactic Element Recorder) instrument during 55 days of exposure on a ...long-duration balloon flight over Antarctica. These observations resolve elemental abundances in this charge range with single-element resolution and good statistics. These results support a model of cosmic ray origin in which the source material consists of a mixture of % material from massive stars and ∼81% normal interstellar medium material with solar system abundances. The results also show a preferential acceleration of refractory elements (found in interstellar dust grains) by a factor of ∼4 over volatile elements (found in interstellar gas) ordered by atomic mass (A). Both the refractory and volatile elements show a mass-dependent enhancement with similar slopes.
Objective
Chronic joint pain is a major cause of pain and disability. Exercise and self‐management have short‐term benefits, but few studies follow participants for more than 6 months. We ...investigated the long‐term (up to 30 months) clinical and cost effectiveness of a rehabilitation program combining self‐management and exercise: Enabling Self‐Management and Coping of Arthritic Knee Pain Through Exercise (ESCAPE‐knee pain).
Methods
In this pragmatic, cluster randomized, controlled trial, 418 people with chronic knee pain (recruited from 54 primary care surgeries) were randomized to usual care (pragmatic control) or the ESCAPE‐knee pain program. The primary outcome was physical function (Western Ontario and McMaster Universities Osteoarthritis Index WOMAC function), with a clinically meaningful improvement in physical function defined as a ≥15% change from baseline. Secondary outcomes included pain, psychosocial and physiologic variables, costs, and cost effectiveness.
Results
Compared to usual care, ESCAPE‐knee pain participants had large initial improvements in function (mean difference in WOMAC function −5.5; 95% confidence interval 95% CI −7.8, −3.2). These improvements declined over time, but 30 months after completing the program, ESCAPE‐knee pain participants still had better physical function (difference in WOMAC function −2.8; 95% CI −5.3, −0.2); lower community‐based health care costs (£−47; 95% CI £−94, £−7), medication costs (£−16; 95% CI £−29, £−3), and total health and social care costs (£−1,118; 95% CI £−2,566, £−221); and a high probability (80–100%) of being cost effective.
Conclusion
Clinical and cost benefits of ESCAPE‐knee pain were still evident 30 months after completing the program. ESCAPE‐knee pain is a more effective and efficient model of care that could substantially improve the health, well‐being, and independence of many people, while reducing health care costs.
The solar neighbourhood is the closest and most easily studied sample of the Galactic interstellar medium, an understanding of which is essential for models of star formation and galaxy evolution. ...Observations of an unexpectedly intense diffuse flux of easily absorbed 1/4-kiloelectronvolt X-rays, coupled with the discovery that interstellar space within about a hundred parsecs of the Sun is almost completely devoid of cool absorbing gas, led to a picture of a 'local cavity' filled with X-ray-emitting hot gas, dubbed the local hot bubble. This model was recently challenged by suggestions that the emission could instead be readily produced within the Solar System by heavy solar-wind ions exchanging electrons with neutral H and He in interplanetary space, potentially removing the major piece of evidence for the local existence of million-degree gas within the Galactic disk. Here we report observations showing that the total solar-wind charge-exchange contribution is approximately 40 per cent of the 1/4-keV flux in the Galactic plane. The fact that the measured flux is not dominated by charge exchange supports the notion of a million-degree hot bubble extending about a hundred parsecs from the Sun.
The Lunar Environment heliospheric X-ray Imager (LEXI) is a wide field-of-view soft X-ray telescope developed to study solar wind-magnetosphere coupling. LEXI is part of the Blue Ghost 1 mission ...comprised of 10 payloads to be deployed on the lunar surface. LEXI monitors the dayside magnetopause position and shape as a function of time by observing soft X-rays (0.1–2 keV) emitted from solar wind charge-exchange between exospheric neutrals and high charge-state solar wind plasma in the dayside magnetosheath. Measurements of the shape and position of the magnetopause are used to test temporal models of meso- and macro-scale magnetic reconnection. The cerebellar floccular and parafloccular lobes are housed in fossae of the periotic region of the skull of different vertebrates. Experimental evidence indicates that the lobes integrate visual and ...vestibular information and control the vestibulo-ocular reflex, vestibulo-collic reflex, smooth pursuit and gaze holding. Multiple paleoneuroanatomy studies have deduced the behaviour of fossil vertebrates by measuring the floccular fossae (FF). These studies assumed that there are correlations between FF volume and behaviour. However, these assumptions have not been fully tested. Here, we used micro-CT scans of extant mammals (47 species) and birds (59 species) to test six possible morphological-functional associations between FF volume and ecological/behavioural traits of extant animals. Behaviour and ecology do not explain FF volume variability in four out of six variables tested. Two variables with significant results require further empirical testing. Cerebellum plasticity may explain the lack of statistical evidence for the hypotheses tested. Therefore, variation in FF volume seems to be better explained by a combination of factors such as anatomical and phylogenetic evolutionary constraints, and further empirical testing is required.

A systematic search of Cochrane Library, MEDLINE, Embase, PsychInfo, PEDro, BASE, and OpenGrey for published and unpublished systematic reviews of inpatient rehabilitation interventions for older adults following an unplanned admission to hospital from database inception to December 2020. Duplicate screening for eligibility, quality assessment, and data extraction including extraction of treatment components and their respective ingredients employing the Treatment Theory framework. Systematic reviews (n = 12) of moderate to low quality, including 44 non-overlapping relevant RCTs were included. When incorporated in a rehabilitation intervention, there was a large effect of endurance exercise, early intervention and shaping knowledge on walking endurance after the inpatient stay versus comparison. Early intervention, repeated practice activities, goals and planning, increased medical care and/or discharge planning increased the likelihood of discharge home versus comparison. The evidence for activities of daily living (ADL) was conflicting. Rehabilitation interventions were not effective for functional mobility, strength, or quality of life, or reduce length of stay or mortality. Therefore, we did not explore the potential role of treatment ingredients for these outcomes.
Benefits observed were often for subgroups of the older adult population e.g., endurance exercise was effective for endurance in older adults with chronic obstructive pulmonary disease, and early intervention was effective for endurance for those with hip fracture. Future research should determine whether the effectiveness of these treatment ingredients observed in subgroups, are generalisable to older adults more broadly. There is a need for more transparent reporting of intervention components and ingredients according to established frameworks to enable future synthesis and/or replication.

ABSTRACT The mean production factor, or broadband averaged cross-section, for solar wind charge-exchange (SWCX) with hydrogen producing emission in the ROSAT keV (R12) band is count degree−2 cm4. The ...production factor is expected to be temporally variable, and that variation is roughly 15%. These values are derived from a comparison of the long-term (background) enhancements in the ROSAT All-Sky Survey with magnetohysdrodynamic simulations of the magnetosheath. This value is 1.8-4.5 times higher than values derived from limited atomic data, suggesting that those values may be missing a large number of faint lines. This production factor is important for deriving the exact amount of keV band flux that is due to the Local Hot Bubble, for planning future observations in the keV band, and for evaluating proposals for remote sensing of the magnetosheath. The same method cannot be applied to the keV band as that band, being composed primarily of the oxygen lines, is far more sensitive to the detailed abundances and ionization balance in the solar wind. We also show, incidentally, that recent efforts to correlate XMM-Newton observing geometry with magnetosheath SWCX emission in the oxygen lines have been, quite literally, misguided. Simulations of the inner heliosphere show that broader efforts to correlate heliospheric SWCX with local solar wind parameters are unlikely to produce useful results.
The host- and bacteria-derived extracellular polysaccharide coating of the lung is a considerable challenge in chronic respiratory disease and is a powerful barrier to effective drug delivery. A low ...molecular weight 12–15-mer alginate oligosaccharide (OligoG CF-5/20), derived from plant biopolymers, was shown to modulate the polyanionic components of this coating. Molecular modeling and Fourier transform infrared spectroscopy demonstrated binding between OligoG CF-5/20 and respiratory mucins. Ex vivo studies showed binding induced alterations in mucin surface charge and porosity of the three-dimensional mucin networks in cystic fibrosis (CF) sputum. Human studies showed that OligoG CF-5/20 is safe for inhalation in CF patients with effective lung deposition and modifies the viscoelasticity of CF-sputum. OligoG CF-5/20 is the first inhaled polymer therapy, represents a novel mechanism of action and therapeutic approach for the treatment of chronic respiratory disease, and is currently in Phase IIb clinical trials for the treatment of CF.
Purpose
Deinstitutionalisation in Ireland began following the impetus of the successful transfer of psychiatric patients into the community in other countries. This study sought to evaluate the ...quality of life (QoL) and social functioning (SF) of former long-stay institutionalised patients with severe and enduring mental illness who had been relocated into local community settings and followed up 10 years later.
Method
One month prior to hospital closure, 87 former long-stay psychiatric patients, the majority of whom had a diagnosis of schizophrenia, were assessed on a range of QoL and SF measures. Patients were followed-up 10 years later in the community, to evaluate baseline predictors of quality of life and social functioning.
Results
Study completers (

= 35) improved significantly on a range of QoL and SF measures over the 10 year period. Specific improvements were noted in domestic skills (

< 0.001), as well as the activity and social relations measure (

< 0.001). Increased social function (

< 0.001) were noted at follow-up. Linear regression analysis found that less social behaviour problems at baseline predicted QoL 10 years later (

Conclusion
This study demonstrated that transfer into the community from an institutional environment was associated with long-term improvements in quality of life and social functioning, even in those who spent many years in the institution. Those who demonstrated the greatest improvement in QoL had less social behavioural problems at baseline assessment, providing further evidence of the success of community living for former long-stay patients.
