Materiál: Z celkového počtu 231 osob, které bylo sledovaných s diagnózou chronické pankreatitidy, 56 nemocných bylo iniciálně léčeno s akutní pankreatitidou (24,2 %). V intervalu 12–24 měsíců od prvé ataky akutní pankreatitidy jejich stav postupně progredoval do obrazu pankreatitidy chronické. Osoby zařazené do studie po 1. atace akutní pankreatitidy abstinovaly, během období jejich sledování nebyla recidiva akutní pankreatitidy prokázána. Výsledky: V etiologii akutní pankreatitidy převládal alkohol (55,3 %), biliární etiologie byl prokázána v 35,7 %. Dle revidované Atlantské klasifikace byla u 69,6 % nemocných stanovena pankreatitida těžká, ostatní nemocní splnili kritérium formy intermediární, nemocní s lehkou formou nebyli zařazeni. Závěr: Významnými přítomnými rizikovými faktory bylo kouření a obezita a u 18 %, resp. u 25,8 % nemocných byl zjištěn pankreatogenní diabetes mellitus. Přitom 88,1 % nemocných trpících akutní pankreatitidou byli kuřáci. Největší počet osob s chronickou pankreatitidou po atace pankreatitidy akutní byl v produktivním věku, tj. 25–50 let. Do chronické pankreatitidy nepřechází pouze akutní pankreatitida alkoholická, ale nalezli jsme tento přechod i u pankreatitidy biliární etiologie.- Introduction: One of the diseases involving a potential risk of developing chronic pancreatitis is acute pancreatitis. Material: Of the overall number of 231 individuals followed with a diagnosis of chronic pancreatitis, 56 patients were initially treated for acute pancreatitis (24.2 %). Within an interval of 12- 24 months from the first attack of acute pancreatitis, their condition gradually progressed to reached the picture of chronic pancreatitis. The individuals included in the study abstained (from alcohol) following the first attack of acute pancreatitis and no relapse of acute pancreatitis was proven during the period of their monitoring. Results: The etiology of acute pancreatitis identified alcohol as the predominant cause (55.3 %), biliary etiology was proven in 35.7 %. According to the revised Atlanta classification, severe pancreatitis was established in 69.6 % of the patients, the others met the criterion for intermediate form, those with the light form were not included. Conclusion: Significant risk factors present among the patients were smoking, obesity and 18 %, resp. 25.8 % had pancreatogenous diabetes mellitus identified. 88.1 % of the patients with acute pancreatitis were smokers. The majority of individuals with chronic pancreatitis following an attack of acute pancreatitis were of a productive age from 25 to 50 years. Cardiovocal syndrome is characterised by vocal cord paralysis due to a cardiovascular disease. A wide spectrum of conditions can result in this syndrome. Here we present a case of cardiovocal syndrome in association with primary pulmonary hypertension. Je indikována v terapii diabetiků 2. typu se zachovalou endogenní produkcí inzulinu, nejčastěji v kombinaci s perorálními antidiabetiky či bazálním inzulinem. Aplikuje se v 1 denní dávce podkožně. Kromě základního antidiabetického efektu s protekcí B buněk je výhodný i podpůrný účinek na redukci hmotnosti. Pozitivní efekt na kardiovaskulární systém je očekávaný, výsledky klinických studií ovšem aktuálně nejsou k dispozici. Z nežádoucích účinků se v běžné klinické praxi nejčastěji setkáme s mírnými projevy dyspepsie horního typu při zahájení terapie, které odeznívají v řádu dnů. Celkově jde o velmi dobře tolerovaný preparát s potenciálem redukce glykovaného hemoglobinu většinou do 15 mmol/l a zároveň minimálním rizikem vzniku hypoglykemie.- Victoza (liraglutide) is a modern antidiabetic drug of GLP1 (glucagone like peptide) analogue group; since 2009 registered in Czech republic. VICTOZA is used in therapy of type 2 diabetic patients with preserved endougenous insulin secretion, usually in combination with other peroral antidiabetic drugs or with basal insulin. VICTOZA is applied once a day subcutaneously. The characteristical antidiabetic effect with β cell protection is enhanced with supportive weigth reduction effect. Positive cardiovascular influence is expected, however data from recent studies are not available at this time. In clinical practice, the mild upper dyspepsia after initiation of therapy is the most common side effect, usually subsiding in several days. These episodes carry on for hours or days. The patient is healthy between the episodes and has no clinical finding. For the treatment of the CVS, antiemetic, antimigraine and sedative medications were used. However, in some cases CVS treatment is very difficult. M/T rose sharply in 1987 in central-eastern European countries following the Chernobyl accident in 1986. This study analyses M/T and births for the former Soviet Republics and for the countries most contaminated by the event. METHODS: Annual birth data was obtained from the World Health Organisation. The countries with the highest exposure levels (by 137Cs) were identified from an official publication of the International Atomic Energy Agency. All of the former Soviet states were also analysed and the periods before and after 1986 were compared. RESULTS: Except for the Baltic States, all regions in the former USSR showed a significant rise in M/T from 1986. There were significant rises in M/T in the three most exposed (Belarus, Ukraine and the Russian Federation). The birth deficit in the post-Soviet states for the ten years following Chernobyl was estimated at 2,072,666, of which 1,087,924 are accounted by Belarus and Ukraine alone. DISCUSSION: Chernobyl has resulted in the loss of millions of births, a process that has involved female even more than male fetuses. This is another and oft neglected consequence of widespread population radiation contamination.- V. Pacienti z této sociální vrstvy vyžadují komplexní zdravotně-sociální péči ve větší míře než pacienti z ostatních vrstev. Zdravotně-sociální péče o sociálně vyloučené osoby vyústila v USA a EU v založení oboru pouliční medicína (street medicine). Specifickou problematiku v rámci pouliční medicíny představuje zdravotně-sociální péče o ženy. Ta je v České republice zatížena, tak jako všeobecná lékařská péče o tyto osoby, profesionální nepřipraveností lékařů, zdravotnických zařízení i systému zdravotní péče (chybějí guidelines, chybí koncepce zdravotní péče o tuto sociální skupinu). Nadto u žen z této sociální skupiny chybí vzhledem k dlouhodobě neperspektivní životní situaci motivace ke spoluúčasti na prevenci.- Increased number of socially excluded people in our society emphasizes the need to provide specific health care. Patients of this social group require complex health and social care to a greater extent than those of the other social groups. Health and social care for socially excluded people resulted in the US and the EU in establishing the field of street medicine. The specific issue within the street medicine is health and social care for women. This type of medicine struggles in the Czech Republic as well as the general medical care mainly due to professional unpreparedness of doctors, medical facilities and health care system (no recommendations, no concept of health care in this social group).
