Chiari malformation is a structural defect in the skull that causes part of the brain to push into the spinal canal. Chiari malformations are almost always present at birth, though symptoms may not ...develop until later in childhood. In adult years, problems can include persistent headaches, neck pain, and weakness and/or numbness and tingling in the arms and legs, which makes it a disease of interest to present as a clinical case with a nursing model care plan.
This is a 43-year-old female patient with type I Chiari malformation. She has neck and shoulder pain, stiffness in the hands, as well as hypotonus and hypotrophy in the upper limb. The patient underwent foramen magnum decompression surgery.
We used Roy's adaptation model for the nursing care plan. The main needs we find are respiration (risk for ineffective breathing pattern), mobility (impaired physical mobility), sensation (acute pain), protection (risk for infection, and risk for bleeding), fluid and electrolyte (risk of fluid and electrolyte imbalance), and self-concept (anxiety).
The Roy's adaptation model approach was useful in this case as it focuses on the patient's adaptability and is suitable for use in perioperative nursing management of type I Chiari malformation in adults. A collaborative approach is essential for planning and carrying out care.
La malformación de Chiari es un defecto estructural en el cráneo que hace que parte del cerebro empuje hacia el canal espinal. Las malformaciones de Chiari casi siempre están presentes al nacer, aunque es posible que los síntomas no se desarrollen hasta más tarde en la infancia. En la edad adulta, los problemas pueden incluir dolores de cabeza persistentes, dolor de cuello y debilidad y/o entumecimiento y hormigueo en brazos y piernas, lo que la convierte en una enfermedad de interés para presentar como caso clínico con un plan de cuidados modelo de enfermería.
Se trata de una paciente de 43 años con malformación de Chiari tipo I. Experimenta dolor de cuello y hombros, rigidez en las manos, así como hipotonía e hipotrofia en el miembro superior. La paciente fue sometida a una cirugía de descompresión del foramen magnum.
Usamos el plan de cuidados de enfermería del Modelo de Adaptación de Roy. Las principales necesidades que encontramos son respiración (riesgo de patrón respiratorio ineficaz), movilidad (movilidad física alterada), sensación (dolor agudo), protección (riesgo de infección y riesgo de hemorragia), líquidos y electrolitos (riesgo de desequilibrio de líquidos y electrolitos) y autoconcepto (ansiedad).
El enfoque del Modelo de Adaptación de Roy fue útil en este caso, ya que se centra en la adaptabilidad del paciente adecuada para su uso en el manejo perioperatorio de enfermería de la malformación de Chiari tipo I en adultos. Un enfoque colaborativo es esencial para planificar y llevar a cabo la atención.

This qualitative study adopted the directed content analysis approach based on the Roy nursing model. The participants were nine children with cancer aged 6-18 years old, five family members, four nurses, one doctor, one teacher, and two charity association members, recruited by purposive sampling method. The information was collected via individual semi-structured interviews, a focus group discussion, and field notes. The data were analyzed simultaneously with data collection using the Elo and Kyngäs method. The study rigor was ensured based on the Guba and Lincoln criteria.
Of the four categories of physical challenges, fragile self-concept, the difficulty of role transition, and disruption of the path to independence, the theme of Falling and rising in the cancer vortex was abstracted.
Based on the Roy model, the children in the present study were at the compensatory level of adaptation. This research demonstrates that the adaptation of children being treated for cancer is fragile and not constant. With each hospitalization and exacerbation of the disease, they made efforts to adapt to their disease using regulatory and cognitive subsystems. Paying attention to different stimulants and the effects of support systems on physical challenges, fragile self-concept, difficult role transition, and disruption of the path to independence for each child, as well as providing individualized care for these children, can help their adaptation to and healthy transition from the vortex of cancer. The Roy adaptation model was helpful and efficient for elucidating the adaptation of children with cancer. Providing care for children by healthcare specialists, especially nurses, should be theory-based and individualized.

main visible negative aspects and although he receives family support, he sometimes feels that it
is a burden. In conclusion, the quality of life is progressively deteriorated in terms of the effects of
kidney disease and its treatment.
El propósito del estudio fue analizar el nivel de calidad de vida de un paciente renal con tratamiento de hemodiálisis en base a la teoría de callista Roy y el test de Enfermedad Del Riñón (KDQOL™-36). Para la presente investigación se utilizó un enfoque cualitativo, de tipo descriptivo, utilizando el método clínico, a través del estudio de caso, ya que se analizó las experiencias y vivencias de un paciente con Insuficiencia Renal Crónica; para la recolección de los datos se utilizó instrumentos como encuesta test destinada a identificar la calidad de vida en el proceso de su enfermedad y también como está relacionada su personalidad y estado de salud mental. La investigación permitió identificar que a pesar de que el paciente percibe su calidad de vida como regular, tiene afectaciones en su rutina diaria y que, aunque cuenta con conocimientos de autocuidado en la nutrición no lleva una dieta adecuada para su condición, el dominio de salud física es poco limitada, puesto que el deterioro físico es uno de los principales aspectos negativos visibles y aunque recibe apoyo familiar en ocasiones siente que es una carga. En conclusión, la calidad de vida se ve deteriorada progresivamente en cuanto a los efectos de la enfermedad renal y su tratamiento.

La asfixia neonatal se denomina como la deficiencia de oxígeno con inadecuada perfusión tisular cerebral que se presenta con acidosis metabólica, pH menor a 7 en la sangre del cordón umbilical, puntuación de Apgar de 0-3 por más de 5 min y alteraciones neurológicas o también falla orgánica múltiple, es una de las principales problemáticas de salud pública debido a su alta tasa de morbimortalidad. Tiene como objetivo reportar un caso de recién nacido con asfixia neonatal para determinar las intervenciones de enfermería relacionadas con la teoría de Callista Roy que permitirán una mejor adaptación del individuo y familia. La metodología utilizada en la presente investigación fue de tipo cualitativa, descriptiva, analítica de corte transversal, se recolectó información mediante la revisión de la historia clínica del paciente. Se lo realizó en Hospital de Machala en un recién nacido de 2 horas de vida ingresa al área de neonatología con diagnóstico RN a Término más peso adecuado para la edad gestacional más Asfixia del nacimiento más Síndrome de dificultad respiratoria, se realiza intervenciones de enfermería y seguimiento del caso. Se concluye que enfermería tiene una gran responsabilidad de asegurar el tratamiento de los neonatos que sufren de asfixia durante el parto, para así contribuir de manera positiva en su evolución que además de ayudar a enriquecer y fomentar la investigación científica contribuyeron a homogeneizar la práctica profesional, mejorar la calidad y dar continuidad a la asistencia mediante las actividades independientes del enfermero.

This study examined the physiological and behavioral effects of a gentle human touch nursing intervention on medically fragile preterm infants (27 to 32 weeks gestational age). The Roy adaptation ...model of nursing was the framework for the study. The results of this study suggest that the immediate and short-term effects of a gentle human touch nursing intervention were not aversive or stressful to preterm infants of 27 to 32 weeks gestational age; furthermore, the findings document several positive, beneficial behavioral effects of the intervention on preterm infants and indicate this type of touching may be appropriate for infants in the neonatal intensive care unit.
