Beta-ketotiyolaz L-izolösin katabolizması ve keton cisimlerinin metabolizmasında yer alan bir enzimdir. 13 aylık kız hasta huzursuzluk, hızlı nefes alma şikayetlerinden 12 saat sonra hızla gelişen ...derin koma nedeniyle başvurdu. Ağır ketoasidoz ve hafif hiperamonyemi tespit edilen hastaya beta-ketotiyolaz eksikliği tanısı konuldu. Akut atak tedavisi yapılan olgu nadir görülmesi nedeniyle sunulmuştur.
Beta-ketothiolase deficiency is a rare inborn error of L-isoleucine catabolism and ketone body metabolism. We present a 13-month-old, girl with beta-ketothiolase deficiency who was admitted with rapidly progressive coma. Severe ketoacidosis and mild hyperammonemia was documented. This rare case was treated successfully during the acute episode.
Amaç: Çocuk diyabetlilerde retinopati varlığına genellikle pek önem verilmemiştir. Genellikle 30 yaş altı jüvenil diyabetliler değerlendirilip çocuk diyabetliler ihmal edilmiştir. Tip 1 diabetes ...mellitus (IDDM) tanılı hastalarda, göz bulgularının yıllara göre değişiklikler gösterdiği bilinmektedir. Kan-retina engelinin etkinliği IDDM'li hastalarda ergenliğe kadar sabit kalır. Daha sonra kan-retina engelinde ilerleyici azalma görülür. Diyabetik retinopati bir mikroanjiyopati tablosudur ve görme kaybına yol açabilmektedir. Tanı konduktan 3-5 yıl sonra IDDM hastaları, düzenli olarak retina muayeneleri ile takip edilmelidirler. Kliniğimizde takip edilen ergenlik sonrası IDDM olguları da fundus bulguları yönüyle incelenmek istenildi. Gereç ve Yöntem: En az bir yıldır IDDM tanısı ile izlenen 53 hasta aynı göz hekimi tarafından değerlendirildi. Hastaların görme düzeyleri Snellen eşeli ile ölçüldü. Ön segment biyomikroskopik muayeneleri ve gözbebeğinin genişlemesinden sonra endirekt oftalmoskopi ile retina muayeneleri yapıldı. Bulgular: Hastaların yaş ortalaması 15,3 yıl ve hastalık süreleri 5,1 yıldı. Ortalama görme düzeyleri sağ gözde %86,6, sol gözde ise %85,2 olarak saptandı. Göz dibi muayenelerinde 33 (%62.3) hastada sağ fundus ve 34'ünde (%64) sol fundus doğaldı. Başlangıç retinopati toplam 4 (%7,5) olguda gelişmişti. Bu hastaların hepsi 5 yıldan kısa süreli izlenen hastalardı. İlerleyici olmayan retinopati toplam bir (%1,9) hastada gelişmişti. Bu 15 yıldır izlenen bir hastaydı. İlerleyici retinopati toplam üç (%5,7) hastada görüldü. Bunların hepsi beş yıldan uzun süreli tanı almışlardı. Maküla ödemi toplam olarak sekiz (%15,1) hastada saptandı. Optik atrofi üç (%5,7) hastada saptandı. Hastaların üçünde (%5,7) katarakt saptandı. Bunlardan ikisi (%66,7) 10 yılın üzerinde izlenmiş hastalardı. Çıkarımlar: Retinopati oranı IDMM hastalarında hastalık süresi arttıkça yükselmektedir. Retina muayenesi ile düzenli aralıklarla izlem, görsel işlevlerin korunması açısından önemlidir.
Aim: While juvenile diabetics are followed more regularly, the importance of retinopathy in diabetic children is generally underestimated. Ophthalmologic examination may present some differences in patients with Type I diabetes mellitus (IDDM) according to the age of the patient. The effectiveness of blood-retina barrier is stable in patients with IDDM until the age of puberty. A progressive decrease is observed in the blood-retina barrier after this period. The diabetic retinopathy is a kind of microangiopathy which can lead to blindness. Patients with IDDM should be followed regularly with a retinal examination, starting 3- 5 years after the first diagnosis. In this study, we presented the fundus findings of our cases with post-pubertal IDDM. Material and Method: A total of 53 patients with IDDM who had been followed for more than a year were evaluated by an ophthalmologist. Visual acuity of the patients was determined by using Snellen test. Biomicroscobic examination of the frontal segment, indirect ophthalmoscopy following pupillary dilatation and retina examinations were performed. Results: The mean age of the patients and the duration of the disease were 15.3 and 5.1 years, respectively. The mean visual acuity was 86.6% in the right and 85.2% in the left eyes. Ophthalmoscopic examination revealed a normal left and right fundus in 33 (62.3%) and 34 (64.1%) of the patients, respectively. Four patients (7.5%) developed a background retinopathy. These patients were those who were followed less than five years. Non-proliferative retinopathy was observed in only one patient (1.9%) who had been followed for a period of 15 years. Proliferative retinopathy was recorded in three patients (5.7%) who were followed for more than five years. Edema of the macula and optic atrophy were detected in eight (15.1%) and three patients (5.7%), respectively. Three patients had cataract (5.7%), of these two (66.7%) had been followed over 10 years. Conclusions: The retinopathy incidence in IDDM patients increases when the duration of the disease is prolonged. Examination of the retina and periodic follow-up are of importance in terms of protection and maintenance of the visual functions.
