Infeksi jamur menjadi penyebab morbiditas dan mortalitas paling penting pada pasien pasca-transplantasi hati yang mendapat beberapa imunosupresan. Candida species dan Aspergillus species adalah ...infeksi jamur paling invasif. Infeksi Candida species dilaporkan menjadi etiologi penyebab infeksi jamur tertinggi pada transplantasi hati. Selain infeksi jamur, parasit juga dilaporkan menjadi penyebab infeksi pada transplantasi hati khususnya pada pasien berasal dari daerah endemis. Artikel ini bertujuan untuk membuat protokol evaluasi infeksi jamur dan parasit pada transplantasi hati anak.
Aim: Candidemia has high morbidity and mortality rate in critically ill patients hospitalized in intensive care units. Prompt initiation of accurate anti-fungal therapy is essential for survival. In ...this study, a 6-year retrospective candidemia analysis of pediatric intensive care unit (PICU) was performed to review candida species distribution, risk factors for candidemia and change in the antifungal resistance in years.
Material and Method: The candidemia episodes of children followed in the PICU of XXX Hospital between January 2014 and January 2020 were analyzed. The demographic and clinical characteristics, laboratory findings, treatments and outcomes of the patients were obtained from the medical records.
Results: Fifty-nine episodes of candidemia (54.2% female) were reported in six years, median age at diagnosis was 43 months (range 1-225), median hospital stay was 48 days (range 3-664). All patients had used broad-spectrum antibiotics, majority had comorbidities (89.8%), nasogastric tube (84.7), central venous catheter (78.0%), and on mechanic ventilation (76.3%). Type of candida species was identified in 36 episodes of candidemia; 47.2% of these episodes were caused by C. parapsilosis, 38,9% by C. albicans, 8.3% by C. glabrata (8.3%), 2.8% by C. lusitaniae, and 2.8% by C. tropicalis. Length of hospital stay was longer among patients with nonalbicans candidemia, and these patients had higher leucocyte count at diagnosis. The rate of antifungal resistance increased over the years. Overall, thirty-day mortality rate was 16.9%.
Conclusion: For critically ill patients with markedly elevated leucocyte count and long hospital stay, empiric antifungal treatment should cover nonalbicans candida with antifungal resistance.
Amaç: Kandidemi yoğun bakım ünitelerinde yatan kritik hastalarda yüksek morbidite ve mortalite oranına sahiptir. Sağ kalım için doğru anti-fungal tedavinin gecikmeden başlanmasının hayati önemi vardır. Bu çalışmada, kandida türlerinin dağılımını, kandidemi için risk faktörlerini ve yıllar içinde anti-fungal dirençteki değişimi gözden geçirmek için bir Çocuk Yoğun Bakım Ünitesi (ÇYBÜ)’inde gözlenen kandidemi ataklarının 6 yıllık retrospektif analizi yapıldı.
Gereç ve Yöntem: Ocak 2014- Ocak 2020 tarihleri arasında XXX Hastanesi ÇYBÜ’inde yatan hastalarda gözlenen kandidemi atakları incelendi. Hastaların demografik ve klinik özellikleri, laboratuvar bulguları, tedavileri ve sonuçları tıbbi kayıtlarından elde edildi.
Bulgular: Altı yılda 59 kandidemi atağı (%54,2) olmuştu, tanı anındaki medyan yaş 43 ay (aralık 1-225), medyan hastanede kalış süresi 48 gün (aralık 3-664) idi. Hastaların tümü antibiyotik kullanmıştı, çoğunda komorbidite (%89,8), nazogastrik tüp (84,7), santral venöz kateter (%78,0) ve mekanik ventilasyon (%76,3) vardı. Otuz altı kandidemi epizodunda etken olan kandida türleri tanımlanmıştır; bu atakların %47,2’si C. parapsilosis, %38,9’u C. albicans, %8,3’ü C glabrata, %2,8’i C lusitaniae ve %2,8’i C. tropicalis’den kaynaklanmıştı. Non-albicans kandidemili hastalarda albicans kandidemili hastalara göre hastanede kalış süresi daha uzundu ve bu hastaların tanı anında lökosit sayısı daha yüksekti. Yıllar geçtikçe anti-fungal direnç oranında artış vardı, ancak istatiksel olarak anlamlı fark yoktu. Genel olarak, otuz günlük ölüm oranı%16,9 idi.
Sonuç: Belirgin derecede yüksek lökosit sayısı ve uzun hastanede kalış süresi olan kritik hastalarda, başlanacak ampirik antifungal tedavi antifungal direnci olabilen nonalbicans candida'yı kapsamalıdır
Latar belakang. Infeksi kandidiasis invasif meliputi infeksi aliran darah dan infeksi invasif dalam lainnya yang disebabkan oleh spesies Kandida dan merupakan penyebab morbiditas dan mortalitas yang ...signifikan, khususnya pada pasien dengan status immunocompromised, seperti pada kondisi keganasan hematologis, kelainan limfoproliferatif, dan gangguan myeloproliferative.Tujuan. Menginvestigasi peran skor Kandida, dibandingkan dengan kultur darah dan PCR, sebagai alat diagnostik kandidiasis invasif pada pasien dengan neutropenia berat, khususnya pada kasus keganasan.Metode. Penelitian ini menggunakan desain penelitian cross sectional. Data yang diperoleh diolah dan dianalisis menggunakan metode Receiver operating characteristic (ROC) untuk mendapatkan nilai area under curve (AUC). Berdasarkan kurva AUC kemudian dilakukan pencarian titik potong yang paling optimal untuk mendapatkan nilai sensitivitas dan spesifisitasHasil. Hasil penelitian menunjukkan bahwa pada kedua kelompok dengan kultur positif dan negatif tidak didapatkan perbedaan bermakna berdasarkan karakteristik jenis kelamin, usia, berat badan, status gizi, dan diagnosis klinis (uji Mann-Whitney, p>0,05).Kesimpulan. Berdasarkan hasil tersebut, dapat disimpulkan bahwa tidak didapatkan perbedaan pada sensitivitas dan spesifisitas skor Kandida dibandingkan dengan hasil kultur pada pada pasien anak dengan neutropenia berat.
Objective: The aim of this study was to determinate of the
effective time of different denture cleanser tablets on the removal of Candida albicans on various base resins.
Materials
and Methods: ...Conventional
heat cured resin, high impact resin, autopolymerized resin, and polyamide
resin were used. 160 samples were prepared for each resin type. The biofilm of C. albicans was formed on the resins and then exposed to alkaline
peroxide tablet and enzymatic tablets for 3-5-10-20-40-80-160-200 minutes. Cell
viability was assessed by MTT test. 3-way ANOVA was used for statistical
analysis.
Results: The
effect of resin type, tablet type and application time on cell viability of C. albicans were found to be significant
(p<0.05). Cell death was at least
65% on the resins even at the minimum time. Furthermore, as the duration of
administration of both tablets increased, cell viability in all resins tended
to decrease. The administration of both tablets on all resins for 10 min
resulted in approximately 80% cell death. Additionally, the most significant
antimicrobial activities of the tablets were determined on 20th
minutes. The alkaline peroxide tablet on all base resins for application
periods of 3 to 200 minutes was generally more effective than the enzymatic
tablet.
Conclusion:
Alkaline peroxide and enzymatic cleanser tablets showed remarkable anticandidal
activity for all resins. The patients with risk of C. albicans infection should keep their prostheses in cleanser
tablets for at least 20 minutes. Furthermore, the anticandidal effect tended to
increase with prolonged exposure time.
Amaç:
Bu çalışmanın amacı, çeşitli protez kaide rezinleri üzerindeki Candida albicans'ın uzaklaştırılmasında farklı
protez temizleyici tabletlerin etkin sürelerinin belirlenmesidir.Gereç ve Yöntemler: Çalışmada geleneksel ısıyla polimerize olan rezin, yüksek çarpma
dayanıklı rezin, otopolimerize rezin ve poliamid rezin kullanıldı. Her rezin
tipi için 160 numune hazırlandı. C.
albicans'ın biyofilmi rezinler üzerinde oluşturuldu ve daha sonra
3-5-10-20-40-80-160-200 dakika boyunca alkalin peroksit tablet ve enzimatik
tablete maruz bırakıldı. Hücre canlılığı MTT testi ile değerlendirildi.
İstatistiksel analiz için 3 yönlü ANOVA kullanıldı.Bulgular:
Rezin tipi, tablet tipi ve uygulama süresinin rezinler üzerindeki C. albicans'ın hücre canlılığı üzerine
etkisi anlamlı bulundu (p<0.05). Minimum
sürede bile rezinlerde en az % 65 hücre ölümü meydana geldi. Bununla birlikte,
her iki tabletin uygulama süresi arttıkça, tüm rezinlerdeki hücre canlılığı azalma
eğilimi gösterdi. Rezinlerin her iki tablete de 10 dakika maruz bırakılması tüm
rezinlerde yaklaşık % 80 hücre ölümüne neden oldu. Bununla birlikte,
tabletlerin en önemli antimikrobiyal aktiviteleri 20. dakika da görülmüştür. Alkalin
peroksit tablet, tüm protez kaide rezinleri üzerinde 3 ila 200 dakikalık uygulama
süreleri için genel olarak enzimatik tabletten daha etkili olmuştur.
Sonuç:

Aim: Urinary tract infections (UTI) are the most common infections in hospitalized patients. Although the bacteries are the most common agents of UTI, fungal agents are isolated from %10 of all cases ...and Candida species are most common in these cases. This study is aimed to determine the risk factors of candiduria in hospitalized patients. Patients and Method: The study included above-18-year-old patients with positive urine cultures for Candida spp who were hospitalized in Afyon Kocatepe University Medical Faculty Hospital between August 2012 and August 2013. We recorded the general risk factors, physical examination findings, biochemical and microbiological laboratory results for each patient. Candida species which are isolat-ed from the patient identified with Phoenix yeast ID automatize system and antifungal sensitivity tested with integral system yeast plus kit. Results: The study included 90 patients; the study group 45 patients who had candiduria, and control group 45 patient who had bacteriuria. Among investigated risk factors, DM (p=0.057), urinary catheter usage (p=0.015), invasive procedure performed in last 30 days (p=0.024), long term hospitalization (p=0.017), antibiotic usage (p=0.001), history of intensive care unite (ICU) admition (p=0.058) were found more frequent in study group then the control group. The most frequent isolated Candida species was C.albicans. Conclusion: Controlled use of antibiotics, performing urinary catheterization with ap propriate indications, regulation of blood sugar in diabetic patients and the shortest appropriate period of hospitalization are thought to be effective in reducing frequency of candidiuria.
AMAÇ: Bu çalışmada iki farklı yüzey yapısına göre hazırlanan yumuşak astar materyallerinde Candida albicans biyofilm oluşumunun değerlendirilmesi amaçlanmıştır.GEREÇ VE YÖNTEM: Dört yumuşak astar ...materyali (Molloplast B, Permafleks, Elite-Soft, Ufi Gel P) cam veya alçı yüzeyler üzerinde hazırlandı (5x5x2 mm; n=5). Tüm örneklerin yüzey pürüzlülükleri ve temas açıları ölçüldü. Candida albicans ATCC 10231 Sabouraud dextrose besiyeri (SDB) içerisinde 37 °C’de 24 saat süresince çoğaltıldı. Ardından, biyofilm oluşumu için örnekler polistren kuyucuklarda, önceden elde edilen Candida albicans kültürü ve taze SDB ilavesiyle 37 °C’de 48 saat süresince enkübe edildi. Örnekler %0.1 kristal viyole solüsyonu ile boyanarak 65 °C’de kurumaya bırakıldı. Biyofilm oluşumu spektrofotometrik yöntemle incelendi. Test edilen materyallerin yüzey pürüzlülüğü, temas açıları ve biyofilm formasyonlarının karşılaştırılması oneway ANOVA ve Tukey testi ile yapıldı. Cam veya alçı yüzeylerde hazırlanan örneklerin yüzey pürüzlülüğü, temas açıları ve biyofilm oluşumu arasındaki korelasyon Pearson korelasyon analizi ile incelendi.BULGULAR: Alçı yüzeyinde hazırlanan örneklerde yüzey pürüzlülüğü, temas açısı ve biyofilm oluşumu, cam üzerinde hazırlananlara göre istatistiksel olarak daha fazla bulundu (p<0.05). Alçı yüzeyde hazırlanan yumuşak astar materyallerinde biyofilm oluşumu, en fazla Ufi Gel P ve en az Molloplast B örneklerinde bulundu. Cam yüzeyde hazırlanan örneklerde ise en fazla biyofilm oluşumu Ufi Gel P’de bulundu. Candida albicans bağlanması ile hiç- bir yumuşak astar materyalinin yüzey özellikleri (yüzey pürüzlülüğü, temas açısı) arasında korelasyon bulunmadı (p>0.05). Alçı yüzeyde hazırlanan yumuşak astar materyallerinde biyofilm oluşumu, en fazla Ufi Gel P ve en az Molloplast B örneklerinde bulundu. Cam yüzeyde hazırlanan örneklerde ise en fazla biyofilm oluşumu Ufi Gel P’de bulundu. Candida albicans bağlanması ile hiçbir yumuşak astar materyalinin yüzey özellikleri (yüzey pürüzlülüğü, temas açısı) arasında korelasyon bulunmadı (p>0.05).SONUÇ: Test edilen yumuşak astar materyalleri, yüzey topografik özellikleri açısından benzer olsalar da farklı miktarlarda Candida albicans biyofilm oluşumu gösterdiler. Düz yüzeylerin biyofilm oluşumunu azalttığı gözlendi.
OBJECTIVE: The purpose of this study was to evaluate Candida albicans biofilm formation on soft liners prepared against two surfaces with different properties.MATERIALS AND METHOD: Four soft liners (Molloplast B, Permafleks, Elite-soft, Ufi Gel P) were prepared against glass or dental plaster surfaces (5x5x2 mm; n=5). The surface roughness and contact angle of the soft liner specimens were measured. Candida albicans ATCC 10231 was grown in Sabouraud dextrose broth (SDB) at 37 °C for 24 h. For biofilm formation, the specimens were incubated in polystrene wells together with fresh SDB and the C. albicans culture at 37 °C for 48 h. The specimens were stained with 0.1% crystal violet and dehydrated at 65 °C. The quantification of the C. albicans biofilm formation was performed spectrophotometrically. Comparison of the surface roughness, contact angle and biofilm formation between the materials was done using one way ANOVA and Tukey tests. Correlation between surface roughness, contact angle and biofilm formation on the specimens prepared against glass or plaster surfaces was performed using Pearson correlation analysis.RESULTS: Specimens prepared against plaster showed significantly higher surface roughness, contact angle and biofilm formation values in comparison to those prepared against glass surface (p<0.05). Among the specimens prepared against the dental plaster surface, the highest and the lowest biofilm formation values were observed in the Ufi Gel P and Molloplast B specimens, respectively. Among the specimens prepared against the glass surface, the greatest biofilm formation was shown for Ufi Gel P. There was no correlation between the surface properties (surface roughness and contact angle) and the amount of biofilm formation for the soft liner materials tested in this study (p>0.05).CONCLUSION: Although the soft liners tested in this study had similar surface topographic properties, they showed different amounts of C. albicans biofilm formation. Smooth surfaces were found to reduce the biofilm formation.
Onikomikozler yüksek insidans ile birlikte morbidite ve anti-fungal ajanlar ile uzun süren tedavi gerektiren önemli bir halk sağlığı problemidir. Bu çalışma enfeksiyon kontrolünde yardımcı olabilecek ...onikomikoza ait kliniko-mikolojik özellikleri tespit etmek amacıyla planlandı. Bu çalışmanın amacı, risk faktörlerini analiz etmek ve klinik özellikleri tespit edebilmek için, onikomikozun değişik etken ajanlarının prevelansını belirlemekti. Bu çalışma 1 Şubat 2011-31 Ocak 2012 tarihleri arasında 1 yıllık periyotta SKIMS Kaşmir Tıp Fakültesi dermatoloji bölümüne başvuran klinik olarak tırnak enfeksiyonu şüphesi olan 150 hastadan alınan örnekler üzerinde yapılan prospektif bir çalışmadır. Tırnaklar klinik olarak değerlendirildi ve tırnak örnekleri direkt mikroskopi ve kültür yapılarak ile incelendi. Örneklerde %66.6’sı direkt mikroskopi ve kültürde pozitif idi. Erkekler bayanlardan daha fazla enfekte idi. En sık enfekte yaş grubu 21-30 yaş arası idi. El tırnakları ayak tırnaklarından daha fazla enfekte idi ve %66 hastada görülen distolateral subungal onikomikoz en sık klinik tip idi. Etiyolojik ajanlar dermatofit (%52.68), NDM (%29.85) ve yeast (%7.46) idi. Dermatofitler arasında T. Rubrum en sık etiyolojik ajan idi
Onychomycosis is a major public health problem with high incidence, associated morbidity and long lasting treatment with anti-fungal agents. This study was carried out to know the clinico-mycological pattern of onychomycosis which can help in the control of this infection. The aim of this study was to determine the prevalence of various causative agents of onychomycosis, to identify the clinical pattern and to analyze the risk factors. This was a prospective study carried over a period of one year from Ist February 2010 to 31st january 2011 on samples from 150 patients with clinically suspected nail infections attending the dermatology department of SKIMS Medical college, Kashmir. The nails were evaluated clinically and the nail samples were subjected to direct microscopy and culture. 66.6% samples were positive by direct microscopy and culture. Males were infected more than females. The commonest age group infected was 21-30 years. Finger nails were affected more frequently than toe nails and distolateral subungal onychomycosis was the most common clinical type seen in 66% patients. The etiological agents were dermatophytes (62.68%), NDM (29.85%), yeasts (7.46%). Among dermatophytes T. rubrum was the commonest etiological agent.

Ovaj pregledni rad opisuje oralne aspekte spolno prenosivih bolesti (SPB). SPB se mogu vidjeti na oralnoj sluznici, bilo kao uzrok lokalne bolesti ili kao mjesto ulaska opće infekcije. K tomu, bez ...obzira na oralni ili genitalni put prijenosa, neke SPB mogu izazvati karakterističan oralni nalaz, koji često dovodi do postavljanja točne dijagnoze. U pregledu se nalaze osvrti na bakterijske (gonoreja, sifilis, klamidijska infekcija), virusne (HSV-1 i 2, HPV, HIV) i kandidalne infekcije, odnosno na njihove oralne poveznice. Posebna pozornost pridana je kontroverznim stajalištima, kao što su postojanje gonokoknog stomatitisa i uloga oralnog seksa u nastanku karcinoma usne šupljine. Uloga pružatelja oralne skrbi bit će od velike važnosti u multidisciplinarnom pristupu prevenciji i dijagnozi SPB.

Thirty Lactobacillus were isolated from the stool of 5-15 year-old children using MRS agar and identified by API 50CHL. Also, 50 Candida were isolated from blood cultures using SD agar and identified ...by Vitek 2 instrument with YST kit. Antifungal effects of Lactobacillus bacteria on Candida were explored using well diffusion method in SD agar. Lactobacillus had the most prominent antifungal effect against on C. albicans (M29, M36), C. parapsilosis (M25, M26), C. famata (M28) and C. guilliermondii (M38) yeasts. This study shows the presence of antifungal effect of Lactobacillus on Candida isolated from blood cultures.
Otuz adet Lactobacillus, 5-15 yaş grubu çocuk gaitasından MRS agar'da izole edilerek API 50CHL cihazı tanımlanmaları yapılmıştır. Ayrıca 50 adet kan kültürü kaynaklı Candida ise SD agar'da izole edilerek Vitek 2 cihazında YST kiti ile tanımlanmaları yapılmıştır. Lactobacillus'ların Candida'lar üzerindeki antifungal etkileri SD agar'da kuyu difüzyon yöntemi ile tespit edilmiştir. Lactobacillus'lar en fazla C. albicans (M29 ve M36), C. parapsilosis (M25, M26). C. famata (M28) ve C. guilliermondii (M38) mayaları üzerinde antifungal etki göstermiştir. Bu çalışma Lactobacillus'ların, kan kültürlerinden elde edilen Candida izolatları üzerine antifungal etkisinin varlığını göstermektedir.
