ABSTRACT
The United States has seen unprecedented growth in the number of incarcerated women, most of whom are mothers with minor children. Major public health concerns relate to the reproductive ...health of women in prisons and jails and the well‐being of their infants and young children. In the current article, we use a reproductive justice framework to examine the intersection of incarceration and maternal and child health. We review (a) current research on the reproductive health of incarcerated women, (b) characteristics and experiences of pregnant incarcerated women, (c) outcomes of infants and young children with incarcerated parents, (d) implications of research findings for policy and practice, and (e) the need for increased research, public education, and advocacy. We strongly recommend that correctional policies and practices be updated to address the common misconceptions and biases as well as the unique vulnerabilities and health needs of incarcerated women and their young children.

Estados Unidos ha visto un crecimiento sin precedentes en el número de mujeres encarceladas, la mayoría de las cuales son madres con niños menores. Mayores preocupaciones de salud pública conciernen a la salud reproductiva de mujeres en prisiones y cárceles y el bienestar de sus infantes y niños pequeños. En el presente artículo, usamos un marco de trabajo de justicia reproductiva para examinar la intersección del encarcelamiento y la salud materno‐infantil. Revisamos 1) la investigación actual sobre la salud reproductiva de mujeres encarceladas, 2) las características y experiencias de mujeres embarazadas encarceladas, 3) lo que resulta de infantes y niños pequeños con progenitores encarcelados, 4) las implicaciones de los resultados de la investigación en cuanto a políticas y prácticas, y 5) la necesidad de aumentar la investigación, la educación pública y la defensoría. Recomendamos fuertemente que se actualicen las políticas y prácticas correccionales para incluir los conceptos erróneos y prejuicios acerca de mujeres encarceladas y sus niños pequeños, así como también sus distintivas vulnerabilidades y necesidades de salud.

Les Etats‐Unis d'Amérique ont été les témoins d'une augmentation sans précédent dans le nombre de femmes incarcérées, la plupart étant des mères avec des enfants mineurs. De grands problèmes de santé publique sont liés à la santé reproductive des femmes en prison et au bien‐être de leurs nourrissons et de leurs jeunes enfants. Dans cet article nous utilisons une structure de justice reproductive afin d'examiner l'intersection de l'incarcération et de la santé maternelle et de l'enfant. Nous passons en revue: 1) les recherches actuelles sur la santé reproductive des femmes incarcérées, 2) les caractéristiques et les expériences de femmes incarcérées enceintes, 3) les résultats de nourrissons et de jeunes enfants avec des parents incarcérés, 4) les implications des résulats de recherche pour les lois et la pratique, et 5) le besoin de plus de recherches, plus d’éducation publique, plus de promotion et défense. Nous recommandons fortement que les lois correctionnelles et les pratiques correctionnelles soient mises à jour afin de traiter et de faire face aux conceptions erronées et à la partialité communes, ainsi qu'aux vulnérabilités uniques et aux besoins de santé des femmes incarcérées et de leurs jeunes enfants.
ZUSAMMENFASSUNG
In den Vereinigten Staaten ist die Zahl der inhaftierten Frauen, von denen die meisten Mütter mit minderjährigen Kindern sind, beispiellos gestiegen. Wesentliche Bedenken der öffentlichen Gesundheit betreffen die reproduktive Gesundheit von Frauen in Gefängnissen sowie das Wohlergehen ihrer Säuglinge und Kleinkinder. Im vorliegenden Artikel verwenden wir ein Rahmenkonzept der reproduktiven Gerechtigkeit, um den Schnittpunkt von Inhaftierung und Gesundheit der Mütter und Kinder zu untersuchen. Wir betrachten 1) die aktuelle Forschung zur reproduktiven Gesundheit von inhaftierten Frauen, 2) Merkmale und Erfahrungen von schwangeren inhaftierten Frauen, 3) Ergebnisse von Säuglingen und Kleinkindern inhaftierter Eltern, 4) Implikationen von Forschungsergebnissen für Politik und Praxis und 5) den höheren Bedarf an Forschung, öffentlicher Bildung und Interessenvertretung. Wir empfehlen nachdrücklich, die Korrekturmaßnahmen und ‐praktiken zu aktualisieren, um den häufigen Missverständnissen und Verzerrungen sowie den spezifischen Vulnerabilitäten und Gesundheitsbedürfnissen von inhaftierten Frauen und ihren kleinen Kindern angemessen zu begegnen.

合衆国では収監された女性の数がかつてないほど増加しており、そのほとんどは少人数の子どもを持つ母親である。公衆衛生の主要な関心事は、刑務所や拘置所に収監されている女性の生殖に関連する健康とその乳幼児の福祉に関することである。われわれは、この論文で生殖の権利の枠組みを用いて、収監と母親や子どもの健康に共通する部分について検討した。われわれは、1) 収監された女性の生殖に関連する健康についての現在の研究、2) 収監された妊娠女性の特徴と経験、3) 収監された両親を持つ乳幼児の転帰、4) 政策や取り組みのための研究成果の重要性、そして5) さらなる研究、学校教育、そして擁護活動の必要性、について再検討した。われわれは、収監された女性とその乳幼児の持つ特有の脆弱性と健康の必要性のみならず、よくある誤解や偏見に対処するためにも、懲治政策や取り組みを更新すべきであることを強く推奨する。

العدالة الإنجابية للأمهات السجينات والدفاع عن أطفالهن الرضع وأولادهن الصغار
شهدت الولايات المتحدة نموا لم يسبق له مثيل في عدد السجينات، ومعظمهن أمهات لديهن أطفال قاصرون. وتتمثل الشواغل الرئيسية في مجال الصحة العامة بالصحة الإنجابية للنساء في السجون ورفاهية أطفالهن الرضع والأطفال الصغار. وفي هذه المقالة، نستخدم إطارا للعدالة الإنجابية لدراسة التقاطع بين السجن وصحة الأم والطفل. نستعرض 1) البحوث الحالية حول الصحة الإنجابية للسجينات، 2) خصائص وتجارب السجينات الحوامل، 3) نتائج الرضع والأطفال الصغار مع الآباء المسجونين، 4) تطبيقات نتائج البحوث في السياسة والممارسة، و 5) الحاجة إلى زيادة البحوث، والتثقيف العام، والدعوة. ونوصي بشدة بتحديث السياسات والممارسات الإصلاحية لمعالجة المفاهيم الخاطئة والتحيزات الشائعة، فضلا عن أوجه الضعف والاحتياجات الصحية الخاصة للسجينات وأطفالهن الصغار.
ABSTRACT
This qualitative study aimed to explore mothers’ perceptions of the transition to second‐time motherhood, and advance methodology for supporting mothers during this transition. Fifty‐seven ...pregnant mothers and their firstborn children (M age = 30.3 months) participated in a laboratory procedure in which the mother exposed her child to a brief episode of differential treatment. The episode was designed to simulate an expected experience of caregiving that would occur after the birth of a second child. During a postnatal visit (M=5.72 weeks following childbirth), mothers provided written feedback on their perceptions of the transition and the contribution of the laboratory experience to that transition. Qualitative content analysis revealed that mothers approached the birth of a second child with apprehension. Intrapersonal themes revealed that participation in the procedure shaped mothers’ understanding of their expanded maternal roles by enhancing their sense of preparedness and confidence in their abilities to support their children. Interpersonal themes revealed that participation helped mothers appreciate the magnitude of the challenge that their children would face and gave mothers reason to believe that their children would adjust successfully. Findings are discussed in terms of their potential contribution toward developing methodology for helping mothers prepare for the addition of a second child.

La meta de este estudio cualitativo fue explorar las percepciones de las madres sobre la transición a la maternidad por segunda vez, y adelantar la metodología de apoyo a las madres durante esta transición. Cincuenta y siete madres embarazadas y sus infantes primerizos (M edad = 30.3 meses) participaron en un procedimiento de laboratorio en que la madre expuso a su niño a un breve episodio de trato diferente. Se diseñó el episodio para estimular una esperada experiencia de cuidado que ocurriría después del nacimiento del segundo niño. Durante una visita posnatal, (M = 5.72 semanas después del nacimiento del niño), las madres dieron un reporte escrito acerca de sus percepciones de la transición y la contribución de la experiencia de laboratorio a la misma. Los análisis de contenido cualitativos revelan que las madres se acercaron al nacimiento de un segundo hijo con aprehensión. Los temas intrapersonales revelaron que la participación en el procedimiento dio forma a la comprensión de las madres de la expansión de sus roles maternos incrementando sus sentidos de estar preparadas y tener confianza en sus habilidades de apoyar a sus niños. Los temas interpersonales revelaron que la participación ayudó a las madres a apreciar la magnitud del reto que sus niños enfrentarían y les dio a las madres razones para creer que sus niños se ajustarían exitosamente. Los resultados se discuten en términos de su potencial contribución hacia el desarrollo de una metodología para ayudar a las madres a prepararse para la adición de un segundo niño.

Cette étude qualitative s'est donné pour but d'explorer les perceptions des mères de la transition à une deuxième maternité et de faire avancer la méthodologie afin de soutenir les mères durant cette transition. Cinquante sept mères enceintes et leurs premiers né (M âge = 30,3 mois) ont participé à une procédure en laboratoire durant laquelle la mère a exposé son enfant à un bref épisode de traitement différentiel. L’épisode a été conçu afin de simuler une expérience attendue de mode de soin qui se passerait après la naissance d'un second enfant. Durant une visite postnatale (M = 5,72 semaines après la naissance de l'enfant), les mères ont présenté leurs réactions par écrit, sur leurs perceptions de la transition et la contribution à cette transition de l'expérience en laboratoire. Une analyse de contenu qualitative a révélé que les mères approchaient la naissance d'un second enfant avec appréhension. Les thèmes interpersonnels ont révélé que la participation à la procédure a façonné la compréhension des mères de leurs rôles maternels plus étendus en augmentant leur sens de préparation et leur confiance dans leurs capacités à soutenir et aider leurs enfants. Les thèmes interpersonnels ont révélé que la participation a aidé les mères à apprécier la magnitude du défi auquel leur enfant ferait face et a donné aux mères une raison de penser que leurs enfants s'adapteraient avec succès. Les résultats sont discutés pour ce qui concerne leur contribution potentielle au développement d'une méthodologie permettant d'aider les mères à préparer l'ajout d'un deuxième enfant.

Diese qualitative Studie zielte darauf ab, die Wahrnehmung der Mütter beim Übergang zur zweiten Mutterschaft zu erforschen und die Methodik zur Unterstützung der Mütter während dieses Übergangs zu fördern. 57 schwangere Mütter und ihre Erstgeborenen (M Alter = 30.3 Monate) nahmen an einem Laborverfahren teil, in dem die Mutter ihr Kind einer kurzen Episode einer differentiellen Behandlung aussetzte. Die Episode wurde entworfen, um eine erwartete Erfahrung zu simulieren, die nach der Geburt eines zweiten Kindes auftreten würde. Während eines postnatalen Besuchs (M = 5.72 Wochen nach der Geburt) gaben die Mütter schriftliche Rückmeldungen über ihre Wahrnehmung des Übergangs und den Beitrag der Laborerfahrung zu diesem Übergang. Die qualitative Inhaltsanalyse ergab, dass sich die Mütter der Geburt eines zweiten Kindes mit Besorgnis näherten. Intrapersonelle Themen zeigten, dass die Teilnahme an dem Verfahren das Verständnis der Mütter für ihre erweiterten mütterlichen Rollen veränderte: sie fühlten sich besser vorbereitet und hatten mehr Vertrauen in ihre Fähigkeit, ihre Kinder unterstützen zu können. Interpersonelle Themen zeigten, dass die Teilnahme den Müttern dabei half, die Größe der Herausforderung anzuerkennen, der ihre Kinder gegenüberstehen und gab den Müttern Grund zu glauben, dass sich ihre Kinder erfolgreich anpassen würden. A troubling study of the role that medical racism plays in the ...lives of black women who have given birth to premature and low birth weight infants
Black women have higher rates of premature birth than other women in America. This cannot be simply explained by economic factors, with poorer women lacking resources or access to care. Even professional, middle-class black women are at a much higher risk of premature birth than low-income white women in the United States. Dána-Ain Davis looks into this phenomenon, placing racial differences in birth outcomes into a historical context, revealing that ideas about reproduction and race today have been influenced by the legacy of ideas which developed during the era of slavery.
While poor and low-income black women are often the “mascots” of premature birth outcomes, this book focuses on professional black women, who are just as likely to give birth prematurely. Drawing on an impressive array of interviews with nearly fifty mothers, fathers, neonatologists, nurses, midwives, and reproductive justice advocates, Dána-Ain Davis argues that events leading up to an infant’s arrival in a neonatal intensive care unit (NICU), and the parents’ experiences while they are in the NICU, reveal subtle but pernicious forms of racism that confound the perceived class dynamics that are frequently understood to be a central factor of premature birth.
The book argues not only that medical racism persists and must be considered when examining adverse outcomes—as well as upsetting experiences for parents—but also that NICUs and life-saving technologies should not be the only strategies for improving the outcomes for black pregnant women and their babies. Davis makes the case for other avenues, such as community-based birthing projects, doulas, and midwives, that support women during pregnancy and labor are just as important and effective in avoiding premature births and mortality.
The clinical trial on medicinal products stands in the area of conflict between important scientific progress in favour of future patients on the one hand and the necessary protection of human ...subjects on the other hand. This conflict grows more acute when vulnerable persons – like pregnant women – are included in medical research. Whereas there are specific regulations for minors or incapacitated persons in the German Medicinal Products Act, legal regulations for the inclusion of pregnant women are missing. Based on this uncertain legal situation solutions are developed, which will show a way out of the dilemma. Insofar as the applicable law does not provide a handle for this problem, a need for reform is shown.
Die klinische Prüfung von Arzneimitteln steht im Spannungsfeld zwischen dem wichtigen wissenschaftlichen Fortschritt zugunsten künftiger Patienten auf der einen und dem notwendigen Schutz der Probanden auf der anderen Seite. Dieses Spannungsverhältnis spitzt sich zu, wenn vulnerable Personen – wie schwangere Frauen – in die klinische Erprobung von Arzneimitteln einbezogen werden. Während das AMG für Minderjährige oder andere einwilligungsunfähige Personen besondere Regelungen zur Teilnahme an klinischen Prüfungen vorsieht, fehlen gesetzliche Vorgaben zur Einbeziehung von Schwangeren. Ausgehend von dieser unsicheren Rechtslage werden Lösungsansätze entwickelt, die einen Ausweg aus dem Dilemma weisen. Soweit das geltende Recht hierfür keine Handhabe bietet, wird der Reformbedarf aufgezeigt.
english The feminization of the medical profession, demographic change with an impending shortage of physicians, Generation Y - these issues are new challenges for medical schools in terms of their ...social responsibility and the training of the next generation of highly qualified scientists. This study, conducted by the University Hospital of Ulm throughout Baden-Württemberg via an online survey provides a valuable data basis which can be used to optimise support activities.A disproportionately high percentage of students with children in Tübingen must, as an additional challenge, cope with a lower than average monthly income. Students with children need organisational support in this doubly challenging situation. The Dean's Office can provide valuable assistance as an advocate (contact with the departments/institutions), provide organisational help (course guidance, individual support) and infrastructure (childcare/changing rooms, etc.). In Tübingen, high intrinsic motivation (willingness to engage in self-managed care networks) is also found amongst students, something that needs to be integrated into the development of the existing support networks.german Die Feminisierung des Arztberufes, der demographische Wandel mit drohendem Ärztemangel, Generation Y – diese Themen stellen die Medizinischen Fakultäten in Hinblick auf ihren sozialen Auftrag der Ausbildung eines hochqualifizierten wissenschaftlichen Nachwuchses vor neue Herausforderungen. Die durch das Universitätsklinikum Ulm durchgeführte Baden-Württemberg-weite Online- Befragung liefert eine wertvolle Datengrundlage, welche zur Optimierung der unterstützenden Tätigkeiten herangezogen werden kann.Der überproportional hohe Anteil der Studierenden mit Kind in Tübingen muss als zusätzliche Herausforderung mit einem durchschnittlich niedrigerem monatlichen Einkommen zurechtkommen. Die Studierenden mit Kind benötigen in dieser Doppelbelastung organisatorische Unterstützung. Hier kann das Studiendekanat als Fürsprecher (Kontakt mit den Fachbereichen/Institutionen), organisatorischer Helfer (Studienfachberatung, individuelle Betreuung) und im Bereich der Infrastruktur (Kinderbetreuung/Wickelräume etc.) wertvolle Hilfestellung leisten. Dies trifft in Tübingen auf eine hohe intrinsische Motivation (Bereitschaft zur Beteiligung an selbstverwalteten Betreuungsnetzwerken) auf Seiten der Studierenden, welches es in die Weiterentwicklung des bestehenden Betreuungsnetzes zu integrieren gilt.
