Health is a multi-dimensional concept, capturing how people feel and function. The broad concept of Active and Healthy Ageing was proposed by the World Health Organisation (WHO) as the process of ...optimizing opportunities for health to enhance quality of life as people age. It applies to both individuals and population groups. A universal Active and Healthy Ageing definition is not available and it may differ depending on the purpose of the definition and/or the questions raised. While the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) has had a major impact, a definition of Active and Healthy Ageing is urgently needed. A meeting was organised in Montpellier, France, October 20-21, 2014 as the annual conference of the EIP on AHA Reference Site MACVIA-LR (Contre les Maladies Chroniques pour un Vieillissement Actif en Languedoc Roussillon) to propose an operational definition of Active and Healthy Ageing including tools that may be used for this. The current paper describes the rationale and the process by which the aims of the meeting will be reached.
The average life expectancy of the world’s population is increasing and the healthcare systems sooner than later will be compromised by its reduced capacity and its highly economic cost; in addition, ...the age distribution of the population is leading towards the older spectrum. This trend will lead to immeasurable and unexpected economic problems and social changes. In order to face up this challenge and complex economic and social problem, it is necessary to rely on the appropriate digital tools and technological infrastructures for ensuring that the elderly are properly cared in their everyday living environments and they can live independently for longer. This article presents ACTIVAGE IoT Ecosystem Suite (AIoTES), a concrete reference architecture and its implementation process that addresses these issues and that was designed within the first European Large Scale Pilot, ACTIVAGE, a H2020 funded project by the European Commission with the objective of creating sustainable ecosystems for Active and Healthy Ageing (AHA) based on Internet of Things and big data technologies. AIoTES offers platform level semantic interoperability, with security and privacy, as well as Big Data and Ecosystem tools. AIoTES enables and promotes the creation, exchange and adoption of cross-platform services and applications for AHA. The number of existing AHA services and solutions are quite large, especially when state-of-the-art technology is introduced, however a concrete architecture such as AIoTES gains more importance and relevance by providing a vision for establishing a complete ecosystem, that looks for supporting a larger variety of AHA services, rather than claiming to be a unique solution for all the AHA domain problems. AIoTES has been successfully validated by testing all of its components, individually, integrated, and in real-world environments with 4345 direct users. Each validation is contextualized in 11 Deployment Sites (DS) with 13 Validation Scenarios covering the heterogeneity of the AHA-IoT needs. These results also show a clear path for improvement, as well as the importance for standardization efforts in the ever-evolving AHA-IoT domain.
The overarching goals of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) are to enable European citizens to lead healthy, active and independent lives whilst ageing. The ...EIP on AHA includes 74 Reference Sites. The aim of this study was to transfer innovation from an app developed by the MACVIA‐France EIP on AHA reference site (Allergy Diary) to other reference sites. The phenotypic characteristics of rhinitis and asthma multimorbidity in adults and the elderly will be compared using validated information and communication technology (ICT) tools (i.e. the Allergy Diary and CARAT: Control of Allergic Rhinitis and Asthma Test) in 22 Reference Sites or regions across Europe. This will improve the understanding, assessment of burden, diagnosis and management of rhinitis in the elderly by comparison with an adult population. Specific objectives will be: (i) to assess the percentage of adults and elderly who are able to use the Allergy Diary, (ii) to study the phenotypic characteristics and treatment over a 1‐year period of rhinitis and asthma multimorbidity at baseline (cross‐sectional study) and (iii) to follow‐up using visual analogue scale (VAS). This part of the study may provide some insight into the differences between the elderly and adults in terms of response to treatment and practice. Finally (iv) work productivity will be examined in adults.
Purpose
Urban green spaces support people to approach active, healthy ageing, especially in high-density cities where they compensate for limited private living spaces. This research paper aims to ...examine how urban green spaces support active, healthy ageing by exploring correlations between behaviour, physical setting and gender difference among older people in a highly populated urban context.
Design/methodology/approach
Urban parks in older neighbourhoods of Macau were selected for data collection. Photographic documentation was used to collect data, with 1,201 older people photographed identified as valid samples. Each was coded according to labels of behaviour, physical setting and gender. Chi-squared tests were conducted to assess correlations between behaviours and features of physical settings, and differences between genders in behaviours and physical settings. Logistic regression analysis was conducted to further examine associations between older people's behaviours and features of physical settings by gender.
Findings
The research reveals types and frequency of behaviours displayed, preferred environmental features for different behaviours and gender differences in behaviours and preferred environmental features. Design principles targeting active, healthy ageing should consider particular amenities and street furniture, the arrangement of trees and landscapes and the integration of open and secluded places.
Originality/value
Previous studies address older people's behaviour from the perspective of either environmental influence or gender difference, but there have been few studies on gendered behavioural differences among older people in urban green spaces. Analysing the behaviour–physical setting–gender relationship provides more evidence in the field of built environment studies.
Since life expectancy has increased significantly over the past century, society is being forced to discover innovative ways to support active aging and elderly care. The e-VITA project, which ...receives funding from both the European Union and Japan, is built on a cutting edge method of virtual coaching that focuses on the key areas of active and healthy aging. The requirements for the virtual coach were ascertained through a process of participatory design in workshops, focus groups, and living laboratories in Germany, France, Italy, and Japan. Several use cases were then chosen for development utilising the open-source Rasa framework. The system uses common representations such as Knowledge Bases and Knowledge Graphs to enable the integration of context, subject expertise, and multimodal data, and is available in English, German, French, Italian, and Japanese.
We recently proposed a novel intelligent newscaster chatbot for digital inclusion. Its controlled dialogue stages (consisting of sequences of questions that are generated with hybrid Natural Language ...Generation techniques based on the content) support entertaining personalisation, where user interest is estimated by analysing the sentiment of his/her answers. A differential feature of our approach is its automatic and transparent monitoring of the abstraction skills of the target users. In this work we improve the chatbot by introducing enhanced monitoring metrics based on the distance of the user responses to an accurate characterisation of the news content. We then evaluate abstraction capabilities depending on user sentiment about the news and propose a Machine Learning model to detect users that experience discomfort with precision, recall, F1 and accuracy levels over 80%.
Virtual coaching systems show great potential for meeting the challenges of demographic change. However, the proportion of older users in the field of digital technologies is far behind that of ...younger people. As part of the e-VITA project, semi-structured interviews were conducted in Japan, France, Italy and Germany with 58 people aged 65 and over, and the content was analyzed with the aim of obtaining information about how older adults organize their everyday lives, also with regard to the COVID-19 pandemic, how they deal with their health, what role digital technologies play in the lives of the interviewees and why they oppose progressive digitization. Second, the survey asked why the older adults oppose a virtual coach, which is to be developed in the e-VITA project to support older adults in healthy and active aging, and what barriers they see in a possible implementation. It was found that older respondents lead active, varied lives and that the COVID-19 pandemic contributed to the increased use of digital solutions. In addition, respondents were consciously addressing their own health. With regard to a virtual coach, barriers were seen primarily in the area of data security and sharing. It can be concluded from this that heterogeneity among older user groups should be taken into account when developing virtual coaches. In addition, aspects of data security and data protection should be presented in a clearly understandable and transparent manner.
The lack of physical exercise is among the most relevant factors in developing health issues, and strategies to incentivize active lifestyles are key to preventing these issues. The PLEINAIR project ...developed a framework for creating outdoor park equipment, exploiting the IoT paradigm to build "Outdoor Smart Objects" (OSO) for making physical activity more appealing and rewarding to a broad range of users, regardless of their age and fitness. This paper presents the design and implementation of a prominent demonstrator of the OSO concept, consisting of a smart, sensitive flooring, based on anti-trauma floors commonly found in kids playgrounds. The floor is equipped with pressure sensors (piezoresistors) and visual feedback (LED-strips), to offer an enhanced, interactive and personalized user experience. OSOs exploit distributed intelligence and are connected to the Cloud infrastructure by using a MQTT protocol; apps have then been developed for interacting with the PLEINAIR system. Although simple in its general concept, several challenges must be faced, related to the application range (which called for high pressure sensitivity) and the scalability of the approach (requiring to implement a hierarchical system architecture). Some prototypes were fabricated and tested in a public environment, providing positive feedback to both the technical design and the concept validation.
Chronic diseases are diseases of long duration and slow progression. Major NCDs (cardiovascular diseases, cancer, chronic respiratory diseases, diabetes, rheumatologic diseases and mental health) ...represent the predominant health problem of the Century. The prevention and control of NCDs are the priority of the World Health Organization 2008 Action Plan, the United Nations 2010 Resolution and the European Union 2010 Council. The novel trend for the management of NCDs is evolving towards integrative, holistic approaches. NCDs are intertwined with ageing. The European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) has prioritised NCDs. To tackle them in their totality in order to reduce their burden and societal impact, it is proposed that NCDs should be considered as a single expression of disease with different risk factors and entities. An innovative integrated health system built around systems medicine and strategic partnerships is proposed to combat NCDs. It includes (i) understanding the social, economic, environmental, genetic determinants, as well as the molecular and cellular mechanisms underlying NCDs; (ii) primary care and practice-based interprofessional collaboration; (iii) carefully phenotyped patients; (iv) development of unbiased and accurate biomarkers for comorbidities, severity and follow up of patients; (v) socio-economic science; (vi) development of guidelines; (vii) training; and (viii) policy decisions. The results could be applicable to all countries and adapted to local needs, economy and health systems. This paper reviews the complexity of NCDs intertwined with ageing. It gives an overview of the problem and proposes two practical examples of systems medicine (MeDALL) applied to allergy and to NCD co-morbidities (MACVIA-LR, Reference Site of the European Innovation Partnership on Active and Healthy Ageing).
