Intracranial aneurysm is the enlargement of an artery in the brain which may lead to rupture and result in serious health disorders. The exact mechanism of aneurysm formation is still unclear; ...however, the disturbed hemodynamics take part in the initiation of the vessel enlargement. In this study, a simplified intracranial aneurysm is numerically investigated to elucidate the disturbed flow conditions and the generated sound on the aneurysm wall. In order to determine the generated sound, the pressure fluctuations on the inner wall are obtained using computational fluid dynamics simulations. Large eddy simulation model is employed to find the unsteady flow pressures. The results indicate that the sound levels increase at the proximity of the intracranial aneurysm. The sound levels on the aneurysm are compared to the sound levels on the sites with normal vessel diameter, and it is seen that the aneurysm results in about 10 dB increase in the sound generation. This relative increase in the flow-generated sound is important in terms of the diagnosis of the intracranial aneurysms, which can be used as a diagnostic tool for the early detection of the aneurysm before facing with the serious symptoms.
İntrakraniyal anevrizma, beyindeki bir arterin yırtılmasına ve ciddi sağlık bozukluklarına yol açabilecek bir damar genişlemesidir. Anevrizma oluşumunun kesin nedenleri hala belirsizdir; ancak bozulmuş hemodinamik parametreler ve kan akış koşullarındaki anormallikler damar genişlemesinin başlamasında rol oynar. Bu çalışmada, bozulmuş akış koşullarını ve anevrizma duvarında oluşan sesi incelemek için basitleştirilmiş bir intrakraniyal anevrizma modeli kullanılmıştır. Anevrizmaya bağlı olarak oluşan sesi belirlemek için, hesaplamalı akışkanlar dinamiği simülasyonları kullanılmış ve iç damar duvarındaki basınç dalgalanmaları incelenmiştir. Kararsız akış basınçlarını bulmak için büyük girdap benzeşimi modelleri kullanılmıştır. Sayısal akış simülasyonlarının sonuçları, anevrizma yakınındaki bölgelerde oluşan ses seviyelerinin arttığını göstermektedir. Anevrizma üzerindeki ses seviyeleri, normal damar çapına sahip bölgelerdeki ses seviyeleri ile karşılaştırıldığında, anevrizmanın ses oluşumunda 10 dB civarında bir artışa neden olduğu görülmektedir. Akış kaynaklı sesteki bu göreceli artışın, ciddi semptomlarla karşılaşmadan önce intrakraniyal anevrizmaların teşhisi açısından önemli olduğu öngörülmektedir.
Amaç: Bu çalışmada, amacımız geniş boyunlu paraoftalmik anevrizmaların tedavisi için kullanılan akım çevirici stentlerin erken ve orta dönem klinik sonuçlarını sunmak idi.
Gereç ve Yöntem: ...Hastanemizin girişimsel radyoloji ünitesinde Ekim 2008-Ocak 2020 tarihleri arasında internal karotid arterin paraoftalmik segmentinde yer alan geniş boyunlu anevrizma nedeniyle akım çevirici stent ile tedavi edilen toplam 87 hastanın verileri retrospektif olarak analiz edildi.
Bulgular: Bu çalışmada yaş ortalaması 51,1 ± 8 olan 66’sı (% 75,8) kadın toplam 87 hasta yer aldı. Otuz dört (% 39) hastada birden fazla olmak üzere, toplam 100 paraoftalmik segment yerleşimli anevrizma, akım çevirici stentler ile tedavi edildi. Kırk (% 40) paraoftalmik segment anevrizmanın tedavisi için akım çevirici stente ek olarak koil embolizasyonu da yapıldı. Tüm stentler istenilen lokalizasyona yerleştirildi (Teknik başarı % 100). Bir hastada tedaviden sonra 5. günde intrakraniyal kanama nedeniyle ölüm gerçekleşti (Mortalite oranı: % 1,1). Uzun dönem takiplerde 3 hastada ileri düzeyde (stent lümenin %50 nin üzerinde daralaması) stent stenozu gelişti ve tedavi gerektirdi. Hastalar ortalama 29,8 ± 3 ay takip edildi. Yapılan kontrollerde ilk 3-6. ayda 77 (%81,9) anevrizma ve 6. ay sonrası kontrollerde 88 (%92,6) anevrizma kapalı idi.
Sonuç: Paraoftalmik anevrizmaların endovasküler tedavisinde akım çevirici stent implantasyonu tek başına veya koil embolizasyonu ile birlikte güvenilir ve etkin bir yöntemdir.
Purpose: The aim of this study was to present early and mid-term clinical results of flow diverter stents used for the treatment of wide-necked paraophthalmic aneurysms.
Materials and Methods: Between January October 2008-January 2020, the data of 87 patients treated with flow diverter stent due to a wide-necked aneurysm located in the paraophthalmic segment of the internal carotid artery were retrospectively analyzed.
Results: In this study, a total of 87 patients, 66 of whom (75.8%) were women, with an average age of 51.1 ± 8 were involved. A total of 100 paraophthalmic segment aneurysms were treated with flow diverter stents. For forty (40%) paraophthalmic segment aneurysms, embolization was performed using a flow diverter stent with coil. All stents were placed in the desired location (technical success 100%). In one patient, death due to intracranial bleeding occurred on the fifth day after treatment (mortality rate: 1.1%). During long-term follow-up, 3 patients developed critical stent stenosis (narrowing of the stent lumen by more than 50%) and required treatment. Patients were followed for an average of 29.8 ± 3 months. In follow-up at 3th-6th moth and after 6th moth, 77 (81.9%) and 88 (92.6%) aneurysms were total occluded.
Conclusion: Flow diverter stent implantation alone or in combination with coil embolization is a safety and effective method for endovascular treatment of paraophthalmic aneurysms.
Amaç: Koil paket dansitesinin (KPD) intraktaniyal anevrizma nüksünde önemli bir prediktif faktör olduğu ile ilgili birçok çalışma vardır. Çalışmamızda KPD yoğunluğunun tedavide kullanılan ...endovasküler tekniğe göre, anevrizma stabil okluzyonundaki rolünü göstermeyi amaçladık. Gereç ve Yöntem: 2018-2019 tarihleri arasında balon ya da stent yardımlı koil embolizasyon ile tedavi edilmiş anevrizmaların retrospektif analizi yapıldı. Tedavi sonrası anevrizma okluzyon değerlendirmesinde Raymond-Roy sınıflaması, anevrizmaların paketleme yoğunluğu için ise AngioCalc serebral ve periferik anevrizma hesaplayıcısı kullanıldı. Bulgular: Çalışmaya 30 hastadaki 31intraserebral anevrizma dahil edildi. Balon yardımlı koil embolizasyon yapılan anevrizma sayısı 18 iken, stent yardımlı embolizasyon yapılan anevrizma sayısı ise 13 idi. Yapılan kontrol görüntülemede balon yardımı kullanılan hastaların %38.8’inde, stent yardımı kullanılan hastaların ise %7.7’sinde rekanalizasyon saptandı. Çalışmamızda mortalite izlenmemiş olup, morbidite oranı %3.3 idi. Sonuç: KPD yoğunluğu, Raymond-Roy Class 1 anevrizma okluzyonu sağlamak için oldukça önemli bir faktördür. Bununla birlikte, anevrizma hacmi arttıkça, yüksek KPD sağlamak zorlaşmakta ve bu durum rekanalizasyon riskini arttırmaktadır. Daha büyük hasta grupları ve daha uzun süreli takipler ile yapılacak çalışmalar, stent yardımlı tedavi edilen anevrizmalarda, stent kullanımının da nüks riskini azalttığını gösterilebilir.
Pelvik böbrek nadir görülen bir konjenital anomalidir. Pelvik yerleşimli ektopik böbrek konjenital renal ektopinin en sık görülen şeklidir. Ektopik pelvik böbreğin otopsi serilerinde görülme oranının ...1/2100-3000 oranında olduğu bildirilmektedir. Ektopik böbrek genellikle klinik belirti vermez ve tanı rastlantısal olarak konulur. Anevrizmalar en sık infrarenal abdominal aorta da görülür. İliak arter anevrizmaları daha az sıklıkla görülürler ve genellikle abdominal aort anevrizması ile birliktedirler. Renal anomaliler retroperi- toneal cerrahi prosedürleri komplike hale getirebilir. Abdominal aort cerrahisi uygulanacak hastalarda ameliyat esnasında gelişebile- cek komplikasyonların önlenebilmesi için renal anomalilerin tanımlanması önemlidir. This study aims to report the incidence of acute kidney injury following surgical reconstruction after a failed endovascular aneurysm repair (EVAR) in patients with abdominal aortic aneurysms.
This ...retrospective study included 44 patients (39 males, 5 females; mean age: 70±11.3 years; range, 35 to 84 years) who underwent emergency or elective surgical reconstruction after failed EVAR between February 2015 and January 2019. Patients were divided into two groups: acute kidney injury group and no acute kidney injury group. The primary end-point of the study was to evaluate the development of acute kidney injury following surgery. The secondary end-points included the 30-day and one-year mortality rates.
Surgical reconstruction of the abdominal aorta was performed electively in 29 (65.9%) patients and urgently in 15 (34.1%) patients. Acute kidney injury occurred in 12 (27.3%) patients. The interval from endovascular aneurysm repair to surgical reconstruction was statistically significantly higher in the no acute kidney injury group than in the acute kidney injury group (24.6±11.5 and 18.1±13.3 months, respectively; p=0.145). The mean abdominal aortic aneurysm diameter, neck angulation, and neck diameter were statistically significantly higher in the acute kidney injury group than in the no acute kidney injury group (p=0.001, p=0.009, and p<0.001, respectively). No statistically significant difference was observed between the acute kidney injury and no acute kidney injury groups for the overall 30-day mortality (p=0.185) and oneyear mortality (p=0.999).
Acute kidney injury is not uncommon after the surgical reconstruction of a failed EVAR. Aneurysm-related anatomical factors may have an impact on the development of postoperative acute kidney injury. Comprehensive surgical planning should be performed for open abdominal aortic surgery after a failed EVAR.
The purpose of our study was to compare findings of 64 detector cranial computed tomographic angiography (CTA) and conventional digital subtraction angiography (DSA) mainly in patients with acute ...subarachnoid hemorrhage (SAH) headache and diplopia. Between March 2007 and May 2009, 65 consecutive patients were admitted with clinical symptoms and signs of intracranial aneurysm. 44 of 67 detected aneurysms were confirmed during surgery or endovascular embolization. DSA was performed in the 3 days after CTA. CT angiograms and conventional angiographies were studied by two radiologists, who performed aneurysm detection and characterisation of morphological features. Using CTA and DSA, we detected 67 aneurysms in 50 patients. 15 patients did not have any aneurysms in both DSA and CTA. There was no difference between the two modalities in the number of the detected aneurysms. But two aneurysms were falsely located before surgery with CTA. They were located in posterior communicating artery (PcoA) but, had been thought to be located at the internal carotid artery supraclinoid segment at CTA. CTA has an equal sensitivity as DSA in the detection of intracranial aneurysms. It has 100% detection rate in anterior communicating artery and middle cerebral artery bifurcation aneurysms, but aneurysms at some locations, like the ones at PcoA, were hard to detect with CTA. DSA preserves its diagnostic value for detection of posterior circulation aneurysms.
Çalışmamızın amacı esas olarak akut subaraknoid kanaması, baş ağrısı ve diplopisi olan hastalarda 64 kesitli kranyal bilgisayarlı tomografi anjiyografi görüntüleri ile konvansiyonel dijital çıkarmalı anjiyografi görüntülerinin karşılaştırılmasıdır. Mart 2007 ve Mayıs 2009 tarihleri arasında intrakranyal anevrizma düşündüren klinik semptom ve bulguları olan 65 hasta kabul edilmiştir. Saptanan 67 anevrizmanın 44’ü cerrahi veya endovasküler tedavi ile doğrulanmıştır. Dijital çıkarmalı anjiyografi incelemesi, bilgisayarlı tomografi anjiyografi yapıldıktan sonraki 3 gün içinde gerçekleştirilmiştir. Bilgisayarlı tomografi anjiyografi ve dijital çıkarmalı anjiyografi incelemeleri iki radyolog tarafından değerlendirilmiştir. Bilgisayarlı tomografi anjiyografi ve dijital çıkarmalı anjiyografi incelemelerinde 50 hastada 67 anevrizma saptanmıştır. 15 hastada bilgisayarlı tomografi anjiyografi ve dijital çıkarmalı anjiyografide anevrizma izlenmemiştir. Saptanan anevrizmaların sayısı bakımından her iki inceleme arasında fark izlenmemiştir. Ancak 2 anevrizma bilgisayarlı tomografi anjiyografi ile yanlış lokalize edilmiştir. Posterior komünikan arter yerleşimli olmalarına rağmen internal karotis arter supraklinoid segment yerleşimli oldukları düşünülmüştür. İntrakranyal anevrizmaların saptanmasında bilgisayarlı tomografi anjiyografinin duyarlılığı dijital çıkarmalı anjiyografi ile eşittir. Anterior komünikan arter ve orta serebral arter bifurkasyon anevrizmalarını saptama oranı %100 iken, posterior komünikan arter gibi bazı bölgelerdeki anevrizmaları bilgisayarlı tomografi anjiyografi ile lokalize etmek güçtür. Dijital çıkarmalı anjiyografinin arka dolaşım anevrizmalarının saptanmasındaki tanısal değeri devam etmektedir
To investigate the diagnostic accuracy of spiral CT angiography (CTA) in detection of cerebral aneurysms in cases with acute subarachnoid hemorrhage (SAH).
Spiral CT angiography and DSA examinations ...were performed in 32 cases due to non-traumatic SAH. CTA data were obtained by maximum intensity projection (MIP) method. CTA and DSA findings were evaluated and compared in terms of existence of aneurysm, size and location.
In 32 patients, DSA detected 34 aneurysms with diameters ranging from 3 to 13 mm while four cases were free of aneurysms. With CTA, an aneurysm at anterior communicating artery location could not be demonstrated. In all other cases CTA correlated well with DSA in detecting the site, size and orientation of the aneurysms. It was found that CTA sensitivity was 97% and specificity was 100% in diagnosis of intracranial aneurysms.
Spiral CTA is a highly accurate, cheap and non-invasive imaging method in diagnosis of intracranial aneurysms in cases with SAH and can be used as a safe alternative method to DSA when emergency surgery is needed.
To evaluate our results of stent-graft implantation for the endovascular treatment of peripheral vascular lesions.
Seventeen patients underwent repair of iatrogenic, traumatic, or spontaneous ...vascular lesions by means of endovascular stent-grafts. The study cases consisted of 10 cardiac catheterization-induced femoral arteriovenous fistulas (AVFs) located between the deep femoral artery and the femoral vein, one iatrogenic AVF between the common iliac artery and vein, one penetrating trauma-induced AVF between the superficial femoral artery and vein, two penetrating trauma-induced pseudoaneurysms in the axillary and popliteal arteries, and three spontaneous or atherosclerosis- related iliac artery aneurysms. Balloon-expandable or self-expandable stent-grafts were used in all 17 cases.
All the stent-grafts were deployed successfully. In catheterization-induced femoral AVFs, complete closure of the fistulas was immediately accomplished in 9 of the 10 cases. Blood flow within the pseudoaneurysms of the axillary artery and the popliteal artery, and aneurysms of the iliac arteries were stopped. Partial closure was achieved in two patients with traumatic iliac and femoral AVFs. For the patients who reported subsided complaints, mean follow-up for 24 months with clinical examination and color Doppler ultrasound revealed patency of the stented segments.
Our results suggest that endovascular treatment of various types of peripheral vascular lesions with stent-grafts is a low-risk procedure, which appears to be less invasive than surgery.
