Der akute Verwirrtheitszustand, das Delir und die als Verwirrtheitszustand imponierenden, nicht kognitiven Störungen des Verhaltens bei Demenz, wie z.B. Wahn und Aggression, gehören zu den häufigsten ...psychischen Störungen des alten Menschen. Nosologisch handelt es sich um zwei verschiedene Entitäten, deren Ätiologie bei Konfrontation im klinischen Alltag primär oft unklar ist, insbesondere dann, wenn der Patient gleichzeitig auch von einem dementiellen Abbauprozess betroffen ist. So steht «umtriebiges Verhalten» oft als gemeinsame Endstrecke für eine Vielzahl von Auslösern. Entsprechend der Vielfalt möglicher Einflussfaktoren erfordern sowohl Diagnose und Behandlung, wie auch die Prävention ein interdisziplinäres Vorgehen. Welche pharmakologischen Interventionen sollen in Erwägung gezogen werden, wenn die nicht medikamentösen Massnahmen nicht mehr ausreichen?
Les états de confusion et le comportement nerveux font partie des principaux troubles non cognitifs chez les personnes âgées. Ils décrivent un état d'étiologie non spécifiée, ce qui est un phénomène qui se fonde sur différentes sources. A cause de leur multimorbidité, les personnes âgées montrent une tendence particulièrement élevée à développer des états de confusion et de délire graves, surtout quand ils sont atteints de démence dégénerative. A l'origine de ces maladies, il faut penser à des souffrances cérébrales et extracérébrales ainsi qu'à des traitements médicaux et à des facteurs liés à l'entourage. Vu la diversité des facteurs d'influence, la prévention et le traitement des syndromes de confusion aigus requiert une démarche interdisciplinaire.
Acute confusional state, delirium, occurs in up to 80% of patients in the intensive care unit and is also a common, life-threatening and potentially preventable clinical syndrome among persons who are 65 years of age or older in general hospital. The cause of acute confusional state is typically multifactorial. Delirium and dementia are highly interrelated and dementia is the leading risk factor for delirium. So the key steps to distinguish between delirium and behavioural and psychological symptoms of dementia are to address all evident causes, e.g. dementia, dehydration, infection, polymedication and to prevent complications and treat behavioral symptoms. First nonpharmacologic approaches should be instituted, including a calm, comfortable environment with the use of orienting influences. Pharmacologic management should be reserved for patients whose symptoms would threaten their own safety or the safety of other persons. Therapeutic drug options include modern antidepressants and neuroleptics.
Les troubles du langage oral constituent un problème de santé publique du fait de leur fréquence et de leurs conséquences négatives sur les apprentissages scolaires et sur l’épanouissement ...psychosocial de l’enfant. L’objectif de la présente étude est de déterminer la prévalence des difficultés du langage et des troubles qui y sont associés chez les enfants de trois ans et demi.
Nous avons procédé à la passation, chez 165 enfants intégrés dans les jardins d’enfants de Sfax (Tunisie), du questionnaire de Chevrie-Muller.
Des difficultés du langage et/ou du comportement (score au QLC≥14) ont été relevées dans 17,55 % des cas. Les difficultés langagières (difficultés en production orale et/ou en compréhension) représentaient 16,36 % des cas. Les enfants appartenant à ce groupe présentaient significativement plus de troubles du comportement et de difficultés attentionnelles et motrices par rapport au groupe sans difficultés de langage.
Cette étude a permis d’objectiver la fréquence élevée des difficultés de langage et des troubles du comportement ainsi que des difficultés motrices et attentionnelles associées chez les enfants de trois ans et demi. D’où l’intérêt de la sensibilisation des enseignants à ces difficultés afin de les repérer à temps, d’y remédier et d’aider l’enfant au niveau des apprentissages scolaires ultérieurs.
Oral language is the principal communication tool that develops in close interaction with the development of intelligence and thought. It is logical to think that early difficulties with language are harmful for a good acquisition of school learning. Therefore, the detection of oral language disorders in children 3 to 6years must be systematic in order to identify these problems as soon as possible and treat them early. In Tunisia, there are few studies that have focused on evaluating the frequency of language disorders in clinical population; but there are still no publications on screening for language disorders in general population. This survey aimed to determine the prevalence of language and behavior problems in children aged 3.5years to 3years 9months and frequenting kindergartens by using the questionnaire of Chevrie-Muller (QLC_3.5).
The study was cross-sectional, carried out in 13 kindergartens in the city of Sfax (Tunisia) for a period of two months (November and December 2012). All the children aged between 3.5years and 3years 9months and whose parents consent to participate to this study were included. We have collected 165 children for whom the QLC_3.5 was completed by kindergarten's teachers.
Among these children, 17.55% had language difficulties and/or behavior problems (Score at QLC_3.5≥14), 75.15% had no difficulties (Score between 0 and 9 at QLC_3.5) and 7.3% were to monitor and review (Score between 10 and 13). The principal component analysis, which identifies four partial scores had showed difficulties in oral production in 16.36% of cases, difficulties in comprehension in 9.1% of cases, difficulties in motor skills and attention as well as behavioral problems in 16.36% of cases for each score. According to this analysis, we have identified two groups: a first group of children with language difficulties (difficulties in oral production and/or in comprehension), which represented 16.36% of cases (27 children) and a second group without language difficulties. The comparison between the two groups underlined that children with language difficulties exhibited much more associated problems. Indeed, motor skills and attentions difficulties were noted in 59.3% in the first group vs. 7.97% in the second group, and behavior problems were observed in 48.1% in the first group vs. 10.14% in the second one (P<10−3).
The results of this study confirm the high prevalence of language difficulties in children aged between 3.5years and 3years 9months and point out the common association with behavioral disorders and attentional difficulties. Thereby, the sensibilisation of teachers taking care of children in this age group is crucial to identify those who have language problems early in order to improve their healthcare management and to help them in later school learning.
En partant du principe de précaution et de ce qu’il crée comme effet de leurre, cet article tentera d’extraire une problématique clinique courante dans la prise en charge de l’adolescent adressé pour ...« prise de risque ». L’hypothèse développée serait qu’un certain type de comportement observé à l’adolescence, en se heurtant au principe de précaution, se rapporterait à une recherche de vérité, que les régulations sociales maquilleraient alors en comportement à risque pathologique.
L’analyse bénéficiera d’une illustration issue d’un conte classique dans notre culture : l’aventure initiatique de Pinocchio. Ce personnage en conjuguant la préoccupation sociale, en termes essentiellement éducatifs, et une transformation psychique et physique, permet une mise en perspective de la prise de risque avec celle de la témérité maturative. Son rapport au risqué fait saillir le problème de la vérité.
De ce point de vue, il apparaît que l’on assiste à l’émergence d’une nouvelle construction sociale en lien avec le processus de subjectivation à l’adolescence. C’est pourquoi, sous le vocable du comportement risqué, on ne trouve pas une pathologie clinique, mais une problématique sociale.
Celle-ci consiste essentiellement en la recherche des moyens de maintenir une définition objective du vrai nécessaire à l’évolution gestionnaire de la société, mais en pathologisant le doute. La vérité ainsi maintenue du côté du principe de réalité transforme le rapport au social en épreuve articulée sur un objectif. Or, pour le jeune pubère, au seuil de son départ de la famille, le sexuel ouvre une brèche dans les vérités de l’enfance qui procurent un vécu de déréliction. En croyant trouver refuge dans les vérités sociales, il se trouve pris au piège de ses logiques, jusqu’à se sentir marionnette. L’opération subjective qui lui permet de dépasser cet état liminaire consiste à faire le doute sur les discours d’autorité en privilégiant la vérité phénoménale à celle de la parole objectivante. Corrélativement, elle consiste en un triple meurtre symbolique qui transforme la parole pour qu’elle puisse dire quelque chose de vrai.
Au final, ce que l’on désigne comme conduite à risque peut se comprendre comme mise en scène d’une opération subjective qui passe par le doute.
Starting from the precautionary principle and the illusions it sells, this article will attempt to sketch out the clinical issues arising from the treatment of teenagers seen for “risk-taking”. The hypothesis developed here is that a certain type of teenage behavior, when coming up against the precautionary principle, could in fact be a quest for truth, and that social regulation then assimilates this quest to pathological risk-taking.
Pinocchio's initiatory adventures, a classic tale in our culture, can help to illustrate this phenomenon. This character puts risk-taking behavior in perspective with boldness, which for its part accelerates the maturing process. In the story, social concerns, essentially in terms of education, combine with psychological and physical transformations. The way this character relates to risk sheds a new light on the question of truth.
From this point of view, it can be said that we are witnessing the emergence of a new social development in connection with the process of “subjectivation” in adolescence. This is why what is dubbed “risky behavior” is actually not a new pathology but a social issue.
The main task is thus to define ways to maintain an objective definition of truth, which is required for the management-based evolution of society, but which makes doubt pathological. Keeping truth on the side of the principle of reality turns social relationships into tests hinged on an objective. For the adolescent entering puberty and about to leave his/her family, sexual issues open breaches in the truths of childhood, and this generates feelings of loss and abandonment. By looking for shelter in social truths, the adolescent becomes trapped in his own logic, to the extent that he feels like a puppet. The subjective process that enables him to move beyond this threshold stage consists in doubting the discourse of authority and favouring phenomenal truth rather than objectifying discourse. This amounts to a triple symbolic murder that transforms discourse so that it can tell the truth.
To conclude, what we call “risky behavior” can be understood as setting the stage for a subjective process that transits through doubt.

Solche Störungen lassen sich durch manuelle Funktionsdiagnostik und qualitative Beurteilung der Bewegungssteuerung aufdecken, wobei hier beispielhaft auf die neurophysiologischen Besonderheiten des Treppabgehens und des Einbeinhüpfens eingegangen wird. Störungen solcher Bewegungsleistungen lassen sich mit einer Behandlung der Halspropriozeptoren in Form der Atlastherapie nach Arlen beeinflussen. Regelmäßig kommt es dadurch neben dem Verschwinden der Schwindelsymptomatik und Verbesserung der Körperkontrolle auch zu Fortschritten in der feinmotorischen, sprachlichen und kognitiven Entwicklung.
L'insertion de corps étranger (CE) par l'anus est devenue un motif assez fréquent de consultation aux urgences. Cette introduction du CE est rarement accidentelle et se voit surtout dans le cadre ...d'actes sexuels et/ou parfois criminels survenant sur un terrain de trouble du comportement. Le sexe masculin est le plus concerné et l'admission aux urgences ne se voit qu'en cas d'échec d'extraction à domicile ou de survenue de complications (perforation, péritonite ou syndrome occlusif). Le traitement consiste à l'extraction du CE par la voie anale avec ou sans anesthésie. Il s'agit d'un patient de 50 ans, suivi pour des hémorroïdes récidivantes sous traitement et compliquées d'un prolapsus hémorroïdaire, admis aux urgences pour la prise en charge d'un CE intra-rectal. Les circonstances de cet incident sont particulières : en effet, dans le cadre d'une "auto-médication superstitieuse", le patient s'est mis en intra-rectal un corps étranger (du caoutchou) refoulant ainsi les hémorroïdes. L'évolution a été marquée par le piégeage du CE avec douleurs intenses. L'abdomen sans préparation réalisé aux urgences a objectivé le niveau du CE et l'extraction s'est faite sous sédation avec une bonne évolution et un suivi en gastrologie avec une consultation psychiatrique n'objectivant pas d'anomalie.
Les comportements externalisés sont fréquents chez les enfants d’âge préscolaire. Ils tendent à diminuer progressivement avec l’âge. Une minorité d’enfants (<10 %) vont cependant présenter des ...niveaux élevés et persistants de difficultés comportementales à l’âge de six ans. Sur un plan nosographique, les catégories diagnostiques des troubles externalisés comprennent principalement le trouble oppositionnel avec provocations (TOP), le trouble des conduites (TC) et le trouble déficit de l’attention/hyperactivité (TDA/H). Bien qu’elles présentent de nombreuses limites chez les moins de six ans, les classifications restent indispensables puisqu’elles permettent la reconnaissance du caractère pathologique et la mise en place d’interventions. Les interventions proposées correspondent à des prises en charge globales et multidisciplinaires qui s’adressent aux parents et aux enfants. La psychoéducation parentale représente un outil thérapeutique privilégié. Il existe d’autres pistes telles que les programmes d’interventions scolaires et les interventions directes auprès des enfants.
Externalized behaviors are common in preschool children. They tend to decrease gradually with age. Yet, a minority of children (<10%) is about to present high and persistent levels of behavioral difficulties at age 6 years. From a nosographic viewpoint the diagnostic categories of behavioral disorders include Oppositional Defiant Disorder, Conduct Disorder and Attention Deficit/Hyperactivity Disorder. Despite their limitations with respect to younger children, classifications remain essential since they allow both recognition of the disorder and the implementation of interventions. Interventions address parents and children. The parental psychoeducation represents a privileged therapeutic tool. Further interventions including school interventions programs and direct interventions in children are also developped.
Le nombre d’enfants et d’adolescents diagnostiqués et traités pour une bipolarité a considérablement augmenté alors que seulement quelques uns de ces enfants ont réellement un diagnostic ...s’apparentant à une maladie bipolaire « typique ». La controverse actuelle concerne les enfants et adolescents avec une irritabilité chronique, des crises de colère et une comorbidité élevée pour les troubles de l’hyperactivité avec déficit de l’attention et considérés jusqu’à présent comme souffrant d’une forme développementale de manie. Pour ces enfants, le DSM-5 a proposé un nouveau diagnostic : le trouble sévère de régulation de l’humeur (TSRH). Beaucoup d’enfants diagnostiqués bipolaires ou comme ayant des troubles des conduites, souffrent en réalité de TSRH. Aussi, à travers l’exposé du cas clinique d’un enfant agité, nous avons illustré un tableau clinique de TSRH. Ce patient semblait souffrir de troubles de l’opposition avec provocation, associés à un risque d’évolution psychopathique et présentait finalement cette pathologie qui s’inscrit en réalité dans un trouble de l’humeur. Dans une perspective intégrative, l’instabilité psychomotrice associée au TSRH est analysée ici selon une évaluation clinique standardisée couplée à une investigation des aspects psychopathologiques et psychomoteurs. Notre observation a ainsi abouti à un diagnostic de TSRH associé à une instabilité psychomotrice syndromique. Les perspectives thérapeutiques sont ensuite exposées à partir des connaissances actuelles sur les dimensions psychodynamique et neurophysiologique des instabilités psychomotrices et plus particulièrement de celles liées à un TSRH associé à un trouble de la personnalité.
Children and adolescents have been overdiagnosed with bipolarity over the last decades. However, only a small number of them actually meet the diagnostic criteria for “typical” bipolar disorder. The current controversy targets children and teenagers with chronic irritability and high comorbidity for attention deficit hyperactivity disorders (ADHD) which have been considered until now as suffering from a form of developmental mania. The DSM-5 has introduced a new diagnostic category, disruptive mood dysregulation disorder (DMDD). Many children diagnosed with bipolarity or conduct disorder actually suffer from DMDD or even DMDD with ADHD. Thus in keeping with the clinical case of an agitated child, we reported a clinical case of DMDD. This patient seemed to be suffering from behavior disorders with a risk for psychopathic evolution. This pathology is actually part of the mood disorder spectrum. DMDD will be analysed with an integrative approach including a standardized clinical assessment associated with exploring the psychopathological and psychomotor aspects. Our observation led to diagnosing DMDD associated with psychomotor instability syndrome partly consecutive to an environment unfavorable to the development. We discuss the therapeutic perspectives based on current knowledge with respect to the psychodynamic and neurophysiologic dimensions of psychomotor instabilities and in particular those related to DMDD with a personality disorder.
