ZUSAMMENFASSUNG
Hintergrund:
Knapp 80% aller Menschen sterben in Deutschland in stationärer Versorgung, ca. jeder fünfte von ihnen wird in seiner letzten Lebensphase nochmals in einen anderen ...Versorgungsbereich verlegt. Dies sind mehr als 150.000 Menschen, die oft ohne Indikation verlegt werden. 13 Risikofaktoren wurden für diese nicht indizierten Verlegungen identifiziert.
Methode:
Unter Begleitung der AWMF wurden zwei regional wirksame Leitlinien entwickelt und in einem Krankenhaus der Maximalversorgung sowie einer Pflegeeinrichtung implementiert. In dem Krankenhaus wurde ein palliativer Konsildienst etabliert. In der Pflegeeinrichtung wurde eine umfassende Personal- und Organisationsentwicklung betrieben. Unterschiedliche Kooperationen mit relevanten regionalen Partnern beider Modelleinrichtungen wurden systematisch ausgeweitet.
Ergebnisse und Schlussfolgerungen:
Die Verlegungen konnten trotz kurzer Laufzeit des Projekts signifikant reduziert werden. Gelingen konnte dies auch durch die Etablierung von Entscheidungshilfen und einer digitalisierten Implementierungsunterstützung. Die Ergebnisse der ethischen und sozialen Begleitforschung rechtfertigen das Vorgehen: Die Zufriedenheit von Angehörigen und Mitarbeitern steigt.

Hintergrund
Eigene Studien aus den Jahren 2013 und 2014 hatten darauf hingewiesen, dass es — entgegen formulierter Zielstellungen — auch in der letzten, terminalen Lebenshase eines ...Krankenhauspatienten bzw. Pflegeheimbewohners noch zu vermeidbaren Verlegungen kommt.
Methode
Um solche Verlegungen zu erfassen und zu beschreiben, wurde im Jahr 2016 ein 27 Items umfassender Fragebogen entwickelt und an Mitarbeiter aus Krankenhäusern und Pflegeheimen adressiert. Ermittelt wurden die Häufigkeit der Verlegung, hierfür präferierte Zeiträume, Zielorte und auf die Verlegung einwirkende Faktoren. 101 Krankenhäuser und 65 Pflegeheime unterschiedlicher Größen, Trägergesellschaften und Bundesländer wurden berücksichtigt.
Ergebnisse
In etwa 20% der Versorgung Sterbender in den Krankenhäusern und in ca. 25% der Fälle in den Pflegeheimen kommt es zu solchen Transfers. Dies würde eine Gesamtzahl von ca. 150.000 Patienten bzw. Bewohnern ausmachen. Pflegeheime verlegen sterbende Patienten in Krankenhäuser. In den Krankenhäusern finden die Verlegungen innerhalb der Einrichtung statt. Wochentage oder Tageszeiten, an denen es bevorzugt zu Verlegungen kommt, konnten nicht identifiziert werden. Als zentrale Gründe für Verlegungen werden akute körperliche Zustandsverschlechterungen identifiziert. Im Gegensatz dazu sind mental-psychische Gründe weniger bedeutsam. Eine gute Zusammenarbeit sowohl mit den Angehörigen als auch mit dem Hausarzt und anderen Versorgungspartnern wirkt sich positiv auf das Verlegungsverhalten aus. Schriftliche Patientenvereinbarungen besitzen einen mittleren bis deutlichen Einfluss. Beständige Personalknappheit wirkt sich nachteiliger auf die identifizierte Verlegungspraxis aus als ein akuter Mitarbeiterengpass.
Empfehlungen
Physiologische Zustandsverschlechterungen dürfen nicht zu einer Alarm- und Lösungskette führen, an deren Ende die Verlegung Sterbender steht. Ein spezielles Risikomanagement oder Fallmanagement für diese Gruppe könnte Fehlverlegungen verhindern. Auch das 2015 durch das Hospiz- und Pallitativgesetz auf den Weg gebrachte „Advance Care Planing“ sollte die Verlegungsfrequenz senken.

The results indicated that the participating Iranian students were more afraid of death and less likely to give care to dying persons than the Swedish participants.
It is suggested that theoretical education should be individualized and culturally sensitive in order to positively influence the students' attitudes, and promote professional development.
