There have been many attempts to find a less painful surgical method of treating hemorrhoids as against those available in standard surgical procedures. A novel technique of hemorrhoidal ablation by ...radiofrequency is described, which is followed by suture fixation of the hemorrhoidal mass.
This non-randomized, retrospective study describes the clinical outcome of the procedure performed in 1650 patients over a period of 5 years. An Ellman dual frequency radiofrequency generator was used for ablation of hemorrhoids.
The operation time ranged between 6 to 8 minutes. Mean hospital stay was 9 hours. The immediate postoperative complication included retention of urine, wound infection and perianal thrombosis. The mean period of incapacity for work was 10 days. Late complications included development of anal tags, anal papillae and recurrence in 2% patients. There was no incidence of anal stricture or continence disorder.
The procedure advocated by the author can be opted as an alternative to conventional surgical procedures. This day care procedure is simple to perform, allows the patients to return to normal activities in a short span of time with lesser pain and has fewer postoperative complications.
Hand-assisted laparoscopic surgery has been used to perform nephrectomies. This report contrasts the efficacy, postoperative morbidity, length of stay, analgesic use, and time to recovery for ...hand-assisted laparoscopic nephrectomy, to standard laparoscopic and open nephrectomy. The technique for hand-assisted laparoscopic nephrectomy used at two institutions is described. The results from these two institutions are contrasted to results in the literature for standard laparoscopic and open nephrectomy. Standard and hand-assisted laparoscopic nephrectomy seem similar in terms of efficacy of surgery, time of surgery, estimated blood loss, length of stay, and time to full recovery. The two laparoscopic techniques seem to show advantage over open surgery in respect to shorter hospital stay, faster full recovery, and less analgesic use. The operative time for the laparoscopic surgeries is longer than the open surgery operating time. Hand-assisted laparoscopic surgery seems to be equivalent to standard laparoscopy. As urologists around the world are trained in hand-assisted laparoscopic nephrectomy, a more refined look at these early results will be possible.

This report introduced how to use the ultrasonically activated instrument, Harmonic Scalpel® (Johnson & Johnson K.K.), for small animal surgical procedures. The report also showed some cases operated with the instrument.
Upper dorsal thoracoscopic sympathectomy, the treatment of choice for primary palmar hyperhidrosis, is not devoid of long-term complications, like Horner's syndrome and postoperative neuralgia. To assess this hypothesis, a study was undertaken to evaluate the use of harmonic scalpel, which does not dissipate heat.
Sixteen patients with primary palmar hyperhidrosis underwent upper dorsal thoracoscopic sympathectomy using the harmonic scalpel on one side and diathermy on the other. Follow-up was made two years postoperatively.
The length of the procedure with each instrument was similar. There was no localization of postoperative pain, which could be attributed to either device. No Horner's syndrome or postoperative neuralgia occurred.
The present study proved the safe use of harmonic scalpel for upper dorsal thoracoscopic sympathectomy, but did not detect any important advantage of either instrument over diathermy.
