Význam tohto Dohovoru spočíva predovšetkým v „zhmotnení“ dlhoročnej iniciatívy zameranej na ochranu práv zdravotne postihnutých osôb na medzinárodnej úrovni, na podklade ktorej je „najväčšej svetovej minorite“ umožnené žiadať rešpektovanie ich ľudských práv. Náš príspevok sa zameriava na analýzu katalógu práv, ktoré majú chrániť osoby so zdravotným postihnutím, presahmi na oblasť poskytovania zdravotnej starostlivosti ako aj jeho dopadom na vnútroštátne politiky štátov, ktoré tento Dohovor transponovali do svojich vnútroštátnych poriadkov. V neposlednom rade je pozornosť venovaná súvisiacej judikatúre Európskeho súdu pre ľudské práva.- On 13 December 2006, the UN adopted the Convention on the Rights of Persons with Disabilities, which in the Slovak republic came into force on 25.06.2010 and as an international treaty takes precedence over the laws and constitution. The importance of this Convention lies primarily in the "materialization" long-standing initiative aimed at protecting the rights of disabled persons at the international level, on the basis of which is "the world's largest minority" able to demand respect for their human rights. Our contribution focuses on the analysis of the catalogue of rights which is supposed to protect a person with a disability, overlaps in area of health care and its impact on national policy states that transposed the Convention into their national law systems. There are some exceptions of this obligation. One group of exceptions is the definition of certain types of health services, in which case the patient does not have the right to choose providers of health services. Another group of exceptions states the reasons for which a provider of health services does not have to enter into a contract with a patient. Od roku 2014 jsou však výsledky orálního glukózového tolerančního testu (oGTT), který je součástí screeningu, hodnoceny podle dvojích kritérií. Zatímco diabetologové a biochemici nově postupují podle doporučení Mezinárodní asociace pro studium diabetu v těhotenství (IADPSG), přejatého v řadě zemí a také WHO v roce 2013, čeští gynekologové zůstávají u původních diagnostických kritérií z roku 2008. Článek rozebírá nejdůležitější argumenty proti přijetí nových kritérií a vysvětluje důležitost jednotného postupu při screeningu a sledování gestačního diabetes mellitus.- Since 2008 there have been screened all pregnant women for gestational diabetes in the Czech Republic. Nevertheless since 2014 the results of the oral glucose tolerance test (which is a part of the screening) have been evaluated by two different sets of diagnostic criteria. While diabetologists as well as biochemists newly use the criteria recommended by the International Association of Diabetes and Pregnancy Study Groups (IADPSG), accepted by various national medical societies and WHO in 2013 as well, czech gynecologists still use the former criteria accepted in 2008. However, economic integration cannot spare it entirely. Financing of urgent treatment of tourists is based on coordination of reimbursement among competent institutions. The Court of Justice established right of patients for public reimbursement of treatment intentionally found in other member states with domestic rates with broad interpretation of basic economic freedoms, albeit measures for cost-containment justify restrictions. The Directive on the application of patients’ rights in cross-border healthcare brings clarification. The Czech Republic has implemented the directive benevolently within short delay. There is no fear of outflow of public money to other member states due to economic, social and linguistic reasons and for absent litigation of patients as clients of public health insurance funds. Opportunity to require prior authorization with reimbursement was thus not used. Increasing complexity of public financing of Czech healthcare is revealed. It is unclear whether rates of reimbursement are fair and regulated prices are feasible. Autoři uvádějí nejnovější úpravy v taxonomii a názvosloví těchto syndromů včetně aktuálních definic a nabízejí pohled na bolestivé syndromy z gynekologické i urologické perspektivy. Článek je doplněn přehlednými diagramy a algoritmy uznávaných mezinárodních společností, které mohou sloužit jako vodítko při diagnostice a léčbě těchto komplexních syndromů.- This review is based on the course presented on AUU meeting held in July 2014 focusing on the conditions known as interstitial cystitis and bladder pain syndrome. The authors are introducing last updates in taxonomy and nomenclature of these syndromes, including current definitions and offering an overview of painful syndromes from both gynaecologic and urologic perspective. Ke každému typu bradykardie navrhují vhodnou diagnostiku a optimální způsob trvalé stimulace. V závěru jsou navržena optimální bezpečnostní opatření, pokud pacient s implantovaným kardiostimulátorem potřebuje podstoupit vyšetření magnetickou rezonancí.- The new European recommendations in permanent cardiac pacing include a new classification of bradycardia indicated for permanent pacing according to inducing mechanism. For each type of bradycardia suggest the appropriate diagnostic method and the optimal mode of permanent stimulation. U podstatné části z těchto pacientek se po onkologické léčbě daří dosáhnout dlouhodobé kompletní remise („vyléčení“), ovšem stále u nich hrozí jednak dlouhodobé komplikace způsobené onkologickou léčbou, jednak recidiva malignity. Dlouhodobá dispenzarizace pacientek s karcinomem prsu je oblastí zájmu nejen onkologů, ale i gynekologů, praktických lékařů a chirurgů. Klíčová slova: dispenzarizace – karcinom prsu – komplikace- In the Czech Republic 7076 new cases of breast cancer were diagnosed in 2011 (132 per 100,000 women). The majority of these patients will achieve long-term complete remission (“cure”) after cancer treatment. However, they still face the risk of long-term complications caused by cancer therapy as well the recurrence of malignancy. Long-term follow-up of patients with breast cancer is an area of interest not only for oncologists, but also for gynecologists, general practitioners, and surgeons. Předkládaný literární přehled je pokusem usnadnit rozhodování v klinické praxi. K problematice lze najít vysoký počet publikací, ale jen jedna splňuje podmínky prospektivní, randomizované, srovnávací studie – PREPIC 1. Během 8 let sledování pacientů byla plicní embolie méně častá ve skupině s kaválním filtrem než ve skupině bez filtru, ovšem za cenu častější návratnosti hluboké žilní trombózy. Jiné observační studie s použitím permanentních filtrů jako ve studii PREPIC 1 došly k podobným výsledkům. Zhruba posledních 10 let jsou k dispozici odstranitelné kavální filtry, které dávají možnost vyhnout se po vynětí pozdním komplikacím zaznamenaným ve studii PREPIC 1. Další studie PREPIC 2 provedená s odstranitelnými filtry má uspořádání, aby dala odpověď, je-li tomu tak. Výsledky jsou dosud k dispozici jen jako abstrakt z konference a mají tedy jen informativní povahu. Nicméně opakování plicní embolie a opakování hluboké žilní trombózy bylo méně časté ve skupině bez filtru než ve skupině se zavedeným filtrem. Vyhodnocení případů z registru RIETE ukázalo jen trend ve prospěch zavedeného filtru v parametru celkové mortality. Zřetelnější je protekční účinek zavedeného filtru u hemodynamicky nestabilních pacientů (kardiogenní šok, masivní embolizace, prudký pokles krevního tlaku), a to jak s trombolytickou léčbou, tak bez ní, jak ukázala analýza z jiného registru hospitalizovaných pacientů v USA. Metaanalýza studií provedených u pacientů s polytraumaty došla k závěru, že filtr je účelné zavést zejména u pacientů, u nichž je kontraindikována antikoagulační léčba. Uvedená data jsou krátce diskutovaná na pozadí existujících doporučení odborných společností. Doporučení Evropské kardiologické společnosti (2014) uvádí, že kavální filtry mohou být použity, když je absolutní kontraindikace k antikoagulační léčbě a vysoké riziko rekurence hluboké žilní trombózy.- The indication for vena cava filters (VCF) in treatment of venous thromboembolism is still controversial. The presented overview should support the practical decisions. Beside of large volume of observational date there is only one study – PREPIC 1 – fulfilling requirements of prospective randomized design. During 8 years of follow up, pulmonary embolism (PE) was less frequent in the group with VCF than in the group without VCF, but at the cost of more frequent deep vein thrombosis (DVT). Other, but observational studies, showed similar results. Since last 10 years retrievable VCF are available. PREPIC 2 study was settled to prove, if use of retrievable VCF and their early removal will decrease the frequency of late complications observed in PREPIC 1. The results are available as conference abstract only, but it was presented that recurrence of DVT and PE was less frequent in group without IVC than with inserted VCF. Evaluation of impact of VCF insertion on mortality from RIETE registry showed only a trend which was in favour of VCF. On the other hand, a protective effect of VCF was demonstrated in hemodynamically unstable patients with PE (cardiogenic shock, massive embolism) with or without thrombolytic therapy evaluating cases from US NIS registry. Metaanalysis of studies in patients with polytrauma showed VCF protection mainly in patients where anticoagulation was contraindicated. Data gained from literature are discussed with existing guidelines. 2014 Recommendations of European Cardiologic Society is thatVC filters may be used when there are absolute contraindications to anticoagulation and a high risk of VTE recurrence.
