Background: Patients with Inflammatory
Bowel Disease (IBD) are at increased risk of hepatitis
B virus (HBV) infection as well as a lower
response to vaccination. This study aimed to analyze
the ...immune response after vaccination against
HBV in patients diagnosed with IBD and its
associated factors.
Methods: A retrospective observational study
was conducted on patients with IBD treated
at the vaccination clinic for at-risk patients at the
Complejo Hospitalario Universitario de Albacete
during the period 2011-2018. Immune response after
vaccination and associated factors were determined
using logistic regression models.
Results: 231 patients were included. HBV
Vaccination had an optimal immune response in
82.7% of the patients. The likelihood of response to
vaccination increased in those diagnosed with ulcerative
colitis (OR 2.90; 95% CI 1.11-7.61) and
decreased with age (80% lower in those aged 40-
55 years (OR 0.20; 95% CI 0.05-0.83) and 88%
lower in those over 55 years of age (OR 0.12; 95%
CI 0.03-0.53) compared to those under 40 years of
age) and pharmacological immunosuppression (OR
0.20; 95% CI 0.58-0.71).
Conclusions: The decrease in the immunogenicity
of the vaccine against hepatitis B in patients
with IBD after the beginning of immunosuppressive
treatment, as well as with age, make early vaccination
a priority in this kind of patients.
Fundamentos: Los pacientes con Enfermedad
Inflamatoria Intestinal (EII) tienen más riesgo de infección
por el virus de la hepatitis B (VHB) así como
menor respuesta frente a la vacunación. El objetivo
de este estudio fue analizar la respuesta inmunitaria
tras la vacunación frente al VHB en los pacientes
diagnosticados de EII y sus factores asociados.
Métodos: Se realizó un estudio observacional
retrospectivo sobre los pacientes con EII atendidos
en la consulta de vacunas de pacientes de riesgo en
el Complejo Hospitalario Universitario de Albacete
durante el período 2011-2018. Se determinó la respuesta
serológica tras la vacunación y los factores
asociados mediante modelos de regresión logística.
Resultados: Se incluyeron 231 pacientes. La
vacunación frente al VHB tuvo una respuesta inmunitaria
óptima en un 82,7% de los pacientes.
La probabilidad de respuesta a la vacunación aumentó
en aquellos diagnosticados de colitis ulcerosa
(OR 2,90; IC95% 1,11-7,61) y se redujo con la
edad (80% menor en los de 40-55 años (OR 0,20;
IC95% 0,05-0,83) y 88% menor en mayores de 55
años (OR 0,12; IC95% 0,03-0,53) frente a los menores
de 40 años) y con la inmunosupresión farmacológica
(OR 0,20; IC95% 0,58-0,71).
Conclusiones: La disminución de la inmunogenicidad
de la vacuna frente a la hepatitis B en los
pacientes con EII tras el inicio del tratamiento inmunosupresor,
así como con la edad, hacen prioritaria la
vacunación temprana en este tipo de pacientes.
Background: the prevalence of chronic viral hepatitis in Spain could vary because of the immigrants coming from countries having an elevated with a higher endemicity of hepatitis B and C virus. ...Hepatitis B and hepatitis C virus infections are an important health problem worldwide taking into account their chronic consequences. The aim of this study was to know the impact of immigration in the prevalence of chronic viral hepatitis in Spain. Methods: qualitative of scientific papers searching in Medline and MEDES-MEDicina, with date limit January 1998- December 2012 and only papers in English and Spanish. Results: data from 19 original articles were analyzed. The prevalences of hepatitis B and C virus infections in the immigrant population, on the whole, are higher than Spanish population. Immigrants from Africa and East European countries presented the higher prevalence of Hepatitis B and Hepatitis C virus infection, whereas the Latin American-origin population displayed the lowest one. Conclusion: the prevalences of hepatitis B and C virus infections in the immigrant population suggest they could have a substantial public health impact in Spain.
Fundamentos: En España la prevalencia de la hepatitis crónicas de origen viral puede variar a causa de los inmigrantes procedentes de áreas de elevada prevalencia de infección por virus B y C de la hepatitis. La infección por estos virus es un problema importante de salud pública global por los procesos crónicos que originan. El objetivo del estudio fue conocer el impacto de la inmigración en la prevalencia de las hepatitis crónicas virales en España. Métodos: Revisión bibliográfica cualitativa de la literatura científica sobre el tema publicada entre enero de 1998 y diciembre de 2012 utilizando las bases Medline y MEDES-MEDicina. Resultados: Se analizaron los datos procedentes de 19 artículos originales. En conjunto la prevalencia de infección por los virus B y C de la hepatitis fue mayor en la población emigrante que la descrita para la población general española. Los emigrantes de África y Europa del Este presentaron las mayores prevalencias y los inmigrantes iberoamericanos las menores. Conclusiones: La prevalencia de las infecciones por virus B y C de la hepatitis en inmigrantes sugiere que podrían tener un importante impacto en la salud pública en España.
Los tumores hepáticos malignos (THM) pediátricos son el resultado final de la combinación variable de los factores de riesgo (FR) constitucionales y ambientales. La presente revisión pretende ...divulgar los principales FR asociados a los THM pediátricos para fomentar su prevención primaria y el diagnóstico precoz en los síndromes constitucionales.
Revisión sistemática de la bibliografía de los últimos 25 años, obtenida del Medline, Embase, Cancerlit, Lilacs y SciElo. El perfil de búsqueda utilizado fue la combinación de “etiology/risk factor/epidemiology” and “malignant liver tumors/hepatic cancer” or “hepatoblastoma/hepatocarcionoma”.
Los THM en la época pediátrica constituyen el 1% del total de cánceres pediátricos. Los dos tipos principales, el hepatoblastoma (HB) y el hepatocarcinoma (HC), engloban el 98-99% de los THM. Diversos FR constitucionales y ambientales están asociados a un mayor riesgo de desarrollar dichas neoplasias. Los principales FR constitucionales son los siguientes: a) síndrome de Beckwith-Wiedemann (BW); b) síndrome de hemihiperplasia aislada (HA); c) poliposis adenomatosa familiar; d) hemocromatosis; e) tirosinemia hereditaria tipo 1; f) deficiencia de alfa-1 antitripsina; g) porfirias; h) cirrosis; i) esteatosis no alcohólica, y j) colangitis esclerosante primaria. Los principales FR ambientales son los siguientes: a) virus de las hepatitis B (VHB) y C (VHC); b) aflatoxina B1 (AFB1); c) radiación ionizante; d) alcohol; e) tratamientos hormonales; f) exposiciones laborales a solventes, pesticidas, cloruro de vinilo y metales; g) tabaquismo; h) arsénico; i) prematuridad y muy bajo peso al nacimiento, y j) tremátodos.
El cribado clínico, analítico y ecográfico de los principales síndromes genéticos comentados, especialmente el de BW y HA, durante los primeros años de vida, facilitan el diagnóstico precoz del HB. La vacunación universal de los recién nacidos con el VHB, constituye el pilar básico de la prevención primaria de los THM. El control del VHB y VBC en la sangre, hemoderivados, donaciones de órganos y en drogadictos también es de gran utilidad. Otras medidas efectivas en la prevención primaria de los THM son: la reducción/ eliminación de alimentos con AFB1, la abstención alcohólica pediátrica y la reducción de las exposiciones prenatales al tabaco, radiaciones ionizantes, solventes, pesticidas, cloruro de vinilo, metales y tratamientos hormonales.
Pediatric Hepatic Malignancies (PHMs) are the result of the interaction between constitutional and environmental risk factors (RFs). We review the evidence on the main RFs associated to PHMs.
Systematic review of the literature published in the last 25 years on Medline, Embase, Cancerlit, Lilacs and SciElo using the following key words: “etiology/risk factor/epidemiology” and “malignant liver tumors/hepatic cancer” or “hepatoblastoma/hepatocarcionoma”.
PHMs account for 1% of all pediatric malignancies. The main types, hepatoblastoma (HB) and hepatocarcionma (HCC) make up 98-99% of PHM. The main constitutional RFs are: a) Beckwith-Wiedemann (BW) syndrome; b) isolated hemihyperplasia syndrome (IHS); c) adenomatous polyps of the colon; d) hemochromatosis; e) Hereditary Tyrosinemia Type 1; f) α-1-antitrypsin deficiency; g) porphyrias; h) cirrhosis; i) nonalcoholic steatosis; and j) primary sclerosing cholangitis. The main environmental RFs are: a) hepatitis B virus (HBV) and C virus (HCV); b) B1 aflatoxin (B1AF); c) ionizing radiation; d) alcohol; e) hormonal treatments; f) occupational exposure to pesticides, solvents, vinyl chloride and metals; g) smoking; h) arsenic; i) prematury and very low birth weight; and j) trematodes.
The clinical, analytical and ultrasound screening facilitate the early diagnosis of HB in the previously mentioned genetic syndromes, particularly BW and IHS during the first years of life. HBV universal vaccination of newborns provides the biggest opportunity to prevent a substantial proportion of PHMs. Also systematic monitoring of HBV and HCV in blood, hemoderivates, donated organs and drug addicts, are very useful. Other effective measures are: the reduction/elimination of B1AF in food, zero alcohol intake during childhood and adolescence as well decreasing prenatal exposure to the tobacco, solvents, pesticides, vinyl chloride, metals, ionizing radiation and hormonal treatments.
İmmunsuprese kronik hepatit C'lilerde artmış okkült HBV enfeksiyon bildirimleri mevcuttur. Bu çalışmada, immunsuprese olmayan ve antiviral tedavi başlanmamış kronik hepatit C'lilerde hepatit B virüsü ...ile karşılaşma sıklığı araştırıldı. Gereç ve yöntem: Hepatit C virüs antikoru ve hepatit C virüs RNA pozitifliği ile kronik hepatit C tanı sı almış 22 olgu (9 kadın/13 erkek, yaş ortalaması: 52±9.7 yıl) bu çalı şmaya alındı. Tüm olgularda hepatit profili mikropartikül enzim immun assay, hepatit B virüs DNA'sı ve hepatit C virüs RNA'sı ise real-time polymerase chain reaksiyonu yöntemiyle bakıldı. Bulgular: Hepatit C virüsüne bağlı kronik hepatitlilerin %68.2'sinin anti-HBc antikor pozitifliği ile hepatit B virüs enfeksiyonu ile karşılaştığı, %31.8'inin ise hepatit B virüsü ile karşılaşmamış olduğu saptandı. Hepatit C virüsüne bağlı kronik hepatitli 22 olgudan sadece 2 olguda (%9.1) hepatit B yüzey antijenin pozitif olduğu ve buna göre hepatit B virüs ile karşılaşmış olan 15 olgunun 3'ünde (%20) hepatit B virüs DNA'sının pozitifliği ile birlikte hepatit B virüsünün kalıcılığı belirlendi. Sonuç: Bizim sonuçlarımıza göre, kronik C hepatitlilerde hepatit B virüsü ile karşılaşma sık ve enfeksiyon kalıcılığı genel populasyondan daha yüksektir. Hastalığın ciddiyeti ve prognozuna katkısı olabileceğinden, özellikle de hepatit B açısından endemik bölgelerde sensitif yöntemler ile okkült hepatit B virüs enfeksiyonu araştırılmalıdır.
We aimed to investigate the rate of hepatitis B virus exposure in subjects with chronic hepatitis C infection who were immunocompetent and not treated with antiviral agent. Materials and methods: Twenty-two patients with chronic hepatitis C (9 female, 13 male, mean age: 52±9.7) were enrolled into this study. Hepatitis B profile was investigated by EIA and hepatitis C virus RNA and hepatitis B virus DNA by real time PCR. Results: AntiHBc antibody positivity, which suggested exposure to hepatitis B virus, was 68.2% in chronic hepatitis C patients. Four patients (18.2%) in the study group were seronegative and 3 patients (13.6%) were vaccinated against hepatitis B virus infection. Only 2 (9.1%) of 22 patients were HBsAg positive, and taking into account hepatitis B virus DNA positivity, 3 patients (20%) had replicative hepatitis B virus infection of 15 patients who were exposed to hepatitis B virus. Conclusions: According to our results, the rate of hepatitis B virus exposure and permanency of hepatitis B virus infection are higher in chronic hepatitis C patients than in the general population. Especially in regions endemic for hepatitis B virus, the occult hepatitis B virus infection, which can increase the severity or deteriorate the prognosis of the disease, should be investigated by sensitive PCR technique in patients with chronic hepatitis C virus infection.
