    A shortage of appropriate microvascular grafts is a common problem in the clinical practice. It is encountered with increasing frequency in procedures such as myocardial revascularization, ... reimplantation of digits or limbs and free flaps. The report opens with a brief survery of the development and importance of microvascular surgery. The main factors responsible for the success or failure of a reconstructive or revascularization procedure are discussed. Basic concepts of the development and treatment of cardiovascular diseases are presented. In the study, microvascular graft patency has been evaluated in white rats at intervals of up to three months. Arterial grafts obtained from the infrarenal part of rat abdominal aorta were compared with alloplastic grafts made of Gore-Tex (polytetraflouroethylene, PTFE). The PTFE graft dimensions were as followsČ internal diameter 1 mm, wall thickness 0.5 mm, external diameter 2 mm, lenght 3-4 mm. The operations were carried out according to the basic principles of microvasular surgery. Graft patency was assessed by clinical, histologic and scanning electron microscopy (SEM) methods. A patency rate of 94 per cent was achieved in arterial grafts, compared to 92 per cent seen in PTFE grafts. One out of 16 arterial grafts andtwo out of 25 alloplastic grafts became obliterated. Statistical analysis of the results using the Fisher exact test revealed no significant differencesin the frequency of graft obliteration between the two groups. Grafts that were patent three weeks after placement remained patent at three months. Minor operative trauma and a well-adapted anastomosis were associated with minor reactive histologic and SEM changes, ready infiltration of blood and tissue elements into the Gore-Tex material, neointima formation and good graft patency.
