    Infantile spasms (IS) consist of stereotype clinical pattern of spasms despitevarious etiologic possibilities. IS are age related epileptic syndrome.Prognosis is generally poor - 90% of children ... develop severe mental deterioration with elements of autism, neurological disability and poorly controlled epilepsy with frequet epileptic seizures. Tehnological advances have increased our ability to diagnose specific brain disorders as cortical dysplasias. The application of SPECT has demonstrated the occurrence of focal cortical disturbance- various types of cortical dysplasias at the onset of spasms. IS are probably initiated by cortical epileptic discharges, which propagate to the brainsteam and other subcortical areas. When the epileptogenic cortex is unilateral an infant with resistant spasms, resective surgery may be considered. For most infants, however, surgery is not an optionand the search for most effective anticounvulsant continues.
