    The results of monitoring antimicrobial resistance among invasive isolates from patients in Slovenia during the European Antimicrobial Resistance Surveillance System (EARSS) project are presented. ... From the second half of 2000 till the end of 2004, 4028 first isolates per patient per year from bloodcultures and cerebrospinal fluids were included in the EARSS database after removing any duplicate records on a quarterly and yearly basis. Out of these, 1346 isolates of Staphylococcus aureus, 635 of Streptococcus pneumoniae(51 from cerebrospinal fluids), 1781 of Escherichia coli (4 from cerebrospinal fluids) and 266 isolates of enterococci (Enterococcus faecalis and E. faecium) were included. A decrease in MRSA isolates was observed: from 21.4% in the year 2000 to 11.8% in 2004. In twelve Slovene hospitals, the proportions of MRSA were from 7.7% (39 isolates) to 25% (48 isolates). In S. pneumoniae isolates, the proportion of penicillin non-susceptible isolates decreased from 22.5% in 2000 to 14.5% in 2003, and then increased back to 25.3% in 2004. Highly penicillin-resistant isolates were detected in 1% of cases in 2002 and in 2.4% in 2004. Out of 166 isolates of S. pneumoniae in 2004, 13 (7.8%) had dual penicillin and erythromycin resistance, 29 (17.5%) single penicillin and 6 (3.6%) single erythromycin resistance. In a four year follow-up, 63.3% (1127) of invasive isolates of E. coli were reported in patients aged 65 years or over. In this age group, the proportion of ciprofloxacin resistance was 12.7%. 17 out of 1781 (1%) isolates were extended-spectrum beta-lactamase (ESBL) positive. E. faecalis was the predominant isolate among enterococci in all four years with the 68.4% proportion. An increase in the proportion of E. faecium from 25.9% in 2000 to 37.4% in 2004 was also observed. No vancomycin-resistant enterococci were reported.
