  • Obravnava otrok s febrilno nevtropenijo = Management of the child with febrile neutropenia

    Febrile neutropenia is the most common complication of cancer treatment in children. In neutropenic patients the infection is clinically and/or microbiologically confirmed in 40 - 60 % of cases, ... whereas in half of the patients the reason for the elevated temperature remains unknown. The clinicalsigns of infection are only mild or even absent in neutropenic patients and elevated temperature is often the only sign of infection. Becauseuntreated infections in immunocompromised patients are often fulminant,prompt diagnostic investigations and management are needed. Different initial antibiotic regimens are used, mostly monotherapy or combination therapy, which include beta-lactam antibiotics, broad-spectrum antipseudomonal cephalosporins, as well as aminoglycosides and, recently, carbapenemes. During the past two decades there has been a shift to Gram-positive organisms. For this reason, and the emergence of methicillin-resistant strains, the dilemma still remains as to whether, and inwhich situations, to include glycopeptide antibiotics in the initial regimen.
