This study compared the self-assessed health-related quality of life (HRQoL) and degree of depression between patients with chronic neuropathic nonodontogenic orofacial pain (NOFP) and healthy ...controls using the Short Form Survey (SF-36) health status questionnaire and Beck Depression Inventory II (BDI-II). This controlled cross-sectional study included 100 patients and 119 healthy controls. The diagnostic protocol recorded the following: 1) pain intensity using a visual analog scale for the time of examination and during the one-month prior; 2) evidence for neuropathic pain using the Leeds questionnaire for neuropathic signs and symptoms (LANSS); 3) emotional status using the BDI-II; and 4) HRQoL using the SF-36 questionnaire. The mean LANSS score was 17.18 in the patient group and 0.0 in the control group. The mean BDI-II score was 18.31 in the patient group and 5.87 in the control group. The SF-36 scores were shown with Mann-Whitney U testing to have statistically significant differences between the patient and healthy control groups in all categories. Vitality was the only SF-36 category in which the patient group scored higher than the control group. In conclusion, NOFP significantly reduces the self-reported HRQoL. NOFP is also related to the development of depression, but does not affect its severity. There is a significant correlation between depression and low quality of life in patients with NOFP.
Patient safety culture (PCS) has a crucial impact on the safety practices of healthcare delivery systems. The purpose of this study was to assess the state of PSC in Croatian hospitals and compare it ...with hospitals in the United States. The study was conducted in three public general
hospitals in Croatia using the Croatian translation of the Hospital Survey of Patient Safety Culture (HSOPSC). A comparison of the results from Croatian and American hospitals was performed using a T-square test. We found statistically significant differences in all 12 PSC dimensions. Croatian
responses were more positive in the two dimensions of Handoff s and Transitions and Overall Perceptions of Patient Safety. In the remaining ten dimensions, Croatian responses were less positive than in US hospitals, with the most prominent areas being Nonpunitive Response to Error, Frequency of
Events Reported, Communication Openness, Teamwork within Units, Feedback & Communication about Error, Management Support for Patient Safety, and Staffing. Our findings show that PSC is significantly lower in Croatian than in American hospitals, particularly in the areas of Nonpunitive
Response to Error, Leadership, Teamwork, Communication Openness and Staffing. This suggests that a more comprehensive system for the improvement of patient safety within the framework of the Croatian healthcare system needs to be developed. Our findings also help confirm that HSOPSC is a
useful and appropriate tool for the assessment of PSC. HSOPSC highlights the PSC components in need of improvement and should be considered for use in national and international benchmarking.
Migraine and tension type headache are the most common disabling primary
headache disorders. Epidemiological studies have documented their high prevalence and high socioeconomic
and personal impacts. ...According to recent data, migraine ranks as the third most prevalent
disorder and seventh-highest specific cause of disability worldwide. Tension-type headache has lifetime
prevalence in the general population ranging between 30% and 78% in different studies. According
to the International Classification of Headache Disorders, 3rd edition, there also are many other
headaches but their incidence in general population is lower than the previously mentioned headaches.
Trigeminal nerve and upper cervical segments (C1-C3) are included in pain control of the head region
and often evaluated in headache studies in order to improve differential diagnosis and headache treatment.
In our study, we evaluated the potential role of electromyographic (EMG) blink reflex in establishing
diagnosis of headache and evaluation of trigeminal nerve dysfunction as the possible underlying
pathomorphological headache mechanism. Our study included 60 patients with different types of
primary headaches and 30 control subjects. Statistical analysis was performed by use of χ2-test and
statistical significance was set at p<0.001. Study results showed that patients with trigeminal dysfunction
in EMG blink reflex had a 5.6-fold higher risk of developing headache in comparison to subjects
with normal EMG blink reflex finding.
Gledano globalno, ali i općenito u Hrvatskoj te na razini poduzeća Hrvatske šume d.o.o., ozljede nastale kao rezultat prometnih nesreća predstavljaju značajan javnozdarsvtveni i ekonomski trošak. ...Obveza liječnika koji je prvi pregledao ozlijeđenog u bilo kojem događaju, pa tako i prometnoj nesreći ili događaju koji ima obilježja ozljede na radu, je prijavljivanje ozljeda policiji. U predmetnoj prijavi, uz opće podatke, liječnik je dužan kvalificirati tjelesnu ozljedu. O klalifikaciji predmetne ozljede ovosit će sve daljne radnje tijekom kriminalističkog istraživanja, te hoće li se pokrenut prekršajni, kazneni ili disciplinski postupak. Na temelju navedenog, primarni cilj istraživanja odnosi se na izradu okvira za kvalitetnije, točnije i ujednačenije kvalificiranje ozljeda sudionika prometnih nesreća. Sekundarni cilj istraživanja vezan je za prikaz trenda i kvalifikacije ostalih ozljeda pri radu u prometu unutar poduzeća koje gospodari državnim šumama. Kod analize općih mehanizama ozljeđivanja i povezanih ozljeda tj. kvalifikacije ozljeda sudionika prometnih nesreća, primijenjena je metoda analize, sinteze, kompilacije i klasifikacije, a kod analize numeričkih podataka primijenjena je tehnika deskriptivne statistike. Rezultati istraživanja vezani za kvalifikaciju ozljeda prikazani su unutar četiri pregledne tablice glede mehanizma nastupanja ozljeda kod frontalnih sudara, bočnih sudara, sudara u vožnji u slijedu i kod prevrtanja vozila. Najveći broj kvalificiranih ozljeda (n=58) utvrđen je kod frontalnih sudara, kod kojih je utvrđen i najveći broj mehanizama ozljeda (n=4). Najmanji broj mehanizama ozljeđivanja (n=1) i najmanji broj kvalificiranih ozljeda (n=7) identificiran je kod sudara u vožnji u slijedu. Udio priznatih ostalih ozljeda pri radu u prometu (službeno putovanje, dolazak na posao, odlazak s posla) unutar poduzeću Hrvatske šume d.o.o. za promatrano razdoblje, u odnosu na ukupan broj ozljeda, kreće se u rasponu od 5,14 % do 2,59 %. Spoznaje i rezultati predmetnog istraživanja široj stručnoj i znanstvenoj zajednici postavljaju potencijalni okvir za ujednačavanjem i podizanjem razine kvalitete kvalifikacije ozljeda kod četiri osnovna tipa prometnih nesreća.
The research shows that injuries in the world, as well as in Croatia, especially those resulting from traffic accidents, which are in the company Hrvatske šume Ltd. recorded as others, ie categorized along with other injuries at work, represent a significant public health and economic problem, for the wider community, but also for the company Hrvatske šume Ltd. At the same time, the planned measures and activities (primarily the National Road Safety Program of the Republic of Croatia, but also other plans from the state and company level) from year to year do not bring the expected results, ie the number of casualties is still high. Subject research seeks to mitigate the consequences of accidents, through the development of criteria for more accurate and precise classification of injuries as a prerequisite for many procedures, from treatment, rehabilitation, but also rights from employment and other relations, or proceedings before prosecutors, courts and insurance companies. The research covers an area that has not been researched so far or has been researched to a lesser extent in the Republic of Croatia and surrounding countries, and in addition to determining the framework for injury qualification, which should determine the cause-and-effect relationship of certain types of accidents (consecutive driving, side collision, head-on collision and overturning) and injuries that occur in them, with experiential and scientifically based notes related to individual injuries. In this way, the results of the research are an excellent tool, especially for those who are the first to qualify the injuries that a participant sustains in a car accident, or for the doctors who were the first to help the victims. The authors understand the complexity and severity of the moment in which these doctors qualify injuries (sometimes due to the severity of injuries and inability to contact with the injured). Namely, incorrect qualification in the first stage (although in possible future proceedings may be changed by expertise, which is not uncommon) may jeopardize the further course of treatment, rehabilitation, lead to unfounded criminal or misdemeanor proceedings or its absence even though it should have been conducted, and others numerous administrative and judicial proceedings of the participants in the accidents. Which for an individual and the community, and thus the company Hrvatske šume can have significant administrative difficulties and significant material costs. In addition to the primary goal of the research and its focus on doctors who are the first to qualify injuries, it should be useful for accident research services (police, state attorney’s office, courts, insurance companies, but also professional services of Hrvatske šume Ltd.), because they also offer the possibility of the reverse procedure, ie verification, especially in cases when the accident is subsequently reported, or the driver has left the scene, or there are doubts about the nature of the injured participant. Namely, the results of the research enable and instruct everyone involved in these processes, how to connect an individual injury - check with damage to the vehicle or suspect that it occurred there, or how to conduct a quality criminal investigation in this area, fix and describe damage and injuries. Ltd. (but also all other services from the police, HGSS, to firefighters) who are not always able to call an ambulance in case of accidents with vehicles (because accidents happen on forest roads, often without a cell phone signal), or the arrival of an ambulance due to the distance is justifiably long, and the injured should be assisted, this work is extremely useful, because in addition to the qualification it describes the occurrence of damage in / on the vehicle and possible related injuries, and notes on how to recognize individual injuries. First aid to the injured, which, as we know, can in some cases save the life of the injured, but also reduce the consequences of the injury. The results of the research are classified into four overview tables for injury mechanisms in frontal collisions, side collisions, sequential collisions and vehicle overturns. The highest number of qualified injuries was found in frontal collisions (frontal) in which the largest number of injury mechanisms (4), and a total of 58 different injuries were identified, of which 24 qualify as ordinary serious bodily injury, 23 as particularly serious bodily injury, 10 as bodily injury and 1 as particularly severe with fatal consequences. For them, a total of 30 notes were entered in the tables, on how they are reported, what to pay attention to, etc. This indicates that these types of accidents are the most severe in terms of consequences (which are the most numerous for Hrvatske šume Ltd. when it comes to traffic accidents). In side collisions, the two most common mechanisms of injury were identified, and a total of 20 different injuries were identified, of which 8 qualify as ordinary serious bodily injury, 9 as particularly serious bodily injury and 3 as bodily injury. For these injuries, 10 notes are entered in the table. When the vehicle overturned, the three most common mechanisms of injury were identified, and a total of 16 different injuries were identified, of which 9 qualify as ordinary grievous bodily harm, 3 as particularly grievous bodily harm and 4 as bodily injury. 7 notes are entered in the table for these injuries. One injury mechanism was identified for collisions while driving in a sequence, and a total of 7 injuries were identified, of which 2 qualify as ordinary serious bodily injury and 5 as bodily injury. There are 2 notes for these injuries in the table. The presented statistical-analytical indicators of the company Hrvatske šume Ltd., along with other results of this research, are a good basis for further monitoring of injuries in the company, and can be a good basis for further identification (but also research on vehicle ergonomics and vehicle selection and protection, reduce the number and consequences of injuries) and assess the level of protection and safety at work in forest production, ie on the way to it, and increase competitiveness by developing mechanisms to improve educational, technical and safety competencies of human resources. And certainly, like the Ministry of the Interior, the indicators on the types of injuries to the god of uniformity should be harmonized with the terms from the Criminal Code described in this paper (especially severe, etc.).

Key words: Neuropathic pain; Orofacial pain; Nonodontogenic pain; Quality of life; Depression Cilj istrazivanja bio je usporediti procijenjenu sa zdravljem povezanu kvalitetu zivota i stupanj depresije ispitanika s kronicnom neuropatskom neodontogenom orofacijalnom boli (NOFP) s rezultatima zdravih ispitanika kontrolne skupine. U studiju je ukljuceno 100 ispitanika srednje dobi od 56,95+ or -13,58 godina s klinickom dijagnozom NOFP u trajanju od najmanje sest mjeseci i 119 zdravih ispitanika srednje dobi od 57,21+ or -13,87 godina koji su bili kontrolna skupina. Primijenjen je standardni dijagnosticki protokol: 1) odredivanje intenziteta boli vizualno numerickom ljestvicom u trenutku ispitivanja te tijekom protekloga mjeseca; 2) procjena prisutnosti neuropatske boli Leedskim upitnikom neuropatskih znakova i simptoma (LANSS); 3) procjena emocionalnog statusa Beckovim inventarom depresije II (BDI-II); 4) procjena o zdravlju ovisne kvalitete zivota (HRQoL) upitnikom SF-36. Prosjecan rezultat LANSS za skupinu oboljelih iznosio je 17,18, a za kontrolnu skupinu 0. Prosjecan rezultat BDI-II u skupini oboljelih bio je 18,31 prema 5,87 u kontrolnoj skupini. Mann-Whitneyjevim U testom svaka od devet kategorija koje mjere SF-36 statisticki se znacajno razlikovala izmedu bolesnih i zdravih ispitanika. U svim kategorijama osim jedne (vitalnost) kontrolna skupina imala je visi indeks u odnosu na skupinu s NOFP. Rezultat kontrolne skupine bio je veci od 60% u sest od devet kategorija, dok skupina oboljelih nije prelazila granicu od 60% niti u jednoj kategoriji. Kronicna NOFP uzrokuje depresiju i utjece na gotovo sve odrednice kvalitete zivota mjerene upitnikom SF-36. Nije dokazan utjecaj na stupanj depresije. Background: Leiomyosarcoma (LMS) is a rare malignancy that originates from smooth muscle. The most common sites of metastases include the lungs, liver, kidney, and skin. Notably, metastases of LMS to ...the central nervous system/or spine are extremely rare. When a cervical spinal LMS lesion was encountered, the patient successfully underwent gross total tumor resection with negative margins. Case Description: A 63-year-old female had undergone an anterior cervical C5–C7 diskectomy and fusion 18 years ago and resection of a retroperitoneal LMS 3 years ago. She newly presented with right-sided numbness and pain of 2 months duration that correlated with a focal right-sided C5-level hemiparesis (i.e., 4/5 motor strength). When the cervical magnetic resonance demonstrated a right-sided C5 intralaminar mass with extension into the C5–C6 foramen, she underwent posterior tumor resection; pathologically, this proved to be an LMS metastasis. Respectively, 1- and six months postoperatively, follow-up magnetic resonance imaging scans showed no tumor recurrence; she tolerated adjuvant oncological treatment accompanied by physical therapy. However, in one postoperative year, the lesion recurred, and she is presently under consideration for additional surgical management. Conclusion: Gross total surgical resection is the first line of treatment for patients with metastatic LMS. Here, a patient with a C5 laminar/C5–C6 foraminal bony LMS metastasis underwent posterior tumor resection accompanied by adjuvant oncological treatment but exhibited disease recurrence within one postoperative year.

Given that not one of the literary sources does not directly mention the medical personnel within the various military units, epigraphical discoveries widely represent the main source of our knowledge on the subject. On the other hand, the archaeological exploration of military garrisons offers proof of the medical care of Roman soldiers. If at first it appears that Roman military medicine is perfectly obvious and clear, actually this is not the case as many questions remain to be answered and debated. In all this, Croatia has its own archaeological perspective, where notably, one site stands out, which could hold a key role according to the layout of buildings within the garrison including its hospital.

This study aimed to answer the question whether it is possible to predict the quality of life in individuals with gastrointestinal cancer based on the number of life events, perceived stress levels ...and coping strategies. The study included 60 individuals (44 male and 16 female) aged 48 to 87 years, with malignant gastrointestinal tract diseases (56 with colon or rectal cancer, 2 with stomach cancer and 2 with pancreatic cancer). The following instruments were used: Questionnaire on General Information and Lifestyle Habits (developed for the purpose of this study); Scale for Measuring Quality of Life; Coping Inventory for Stressful Situations; and Life Events Scale. Results showed that the number of life events, perceived stress levels, emotion-oriented coping and avoidance were not predictive for the quality of life. Education was the only predictor for factors contributing to the quality of life (predictive variables on the Scale for Measuring Quality of Life). Task-oriented coping and education were predictors for satisfaction with past life and task-oriented coping was predictive for criterion variables on the Scale for Measuring Quality of Life and for factor related to future expectations and comparison to other people, but the proportion of explained variance was modest. The results of this study suggest that it is important to consider other variables (e.g., personality traits and sociodemographic factors) in predicting the quality of life and psychotherapeutic work with gastrointestinal cancer patients. It is important to bear in mind that there is no universally good individual coping strategy that is acceptable in all situations, but that coping flexibility or the ability to adequately change coping strategies in response to situational demands is by far more important.
Bursae around the knee reduce friction between moving structures. When involution of suprapatellar septum fails to occur (“complete septum”), then suprapatellar bursa may stay completely separated ...from the knee joint cavity to form a cystic cavity. In the case of the increased volume, suprapatellar bursitis can cause painful suprapatellar swelling. The aim is to point to the possibility of arthroscopic decompression of suprapatellar cyst. In the case presented, the indication for knee arthroscopy was based on clinical examination, ultrasonography and magnetic resonance studies. The preoperative diagnosis was verified intra-operatively, and intra-articular cyst decompression was performed by arthroscopy. At the final examination 8 months postoperatively, the patient still had no pain, swelling and had full range of motion. This minimally invasive operative procedure resulted in a satisfactory anatomic and functional outcome. In this case report, we present a patient with suprapatellar cyst submitted to arthroscopic cyst decompression.
