La carence en vitamine B12 est fréquente après chirurgie de l’obésité. L’objectif était de déterminer les pratiques actuelles en centre spécialisé de l’obésité en termes de dosage, supplémentation, ...et prise en charge d’un déficit ou d’une carence en B12 après chirurgie de l’obésité.
En 2021, une évaluation des pratiques professionnelles portant sur la supplémentation en B12 en post-chirurgie bariatrique des patients a été réalisée auprès des médecins nutritionnistes de 37 centres spécialisés de l’obésité (CSO) français.
Parmi les 41 réponses collectées provenant de 29 centres, soit un taux de réponse de 78 %, le suivi du dosage de la B12 en postopératoire était toujours utilisé en routine. La voie de supplémentation en B12 envisagée en première intention après chirurgie bariatrique était majoritairement orale, puis intramusculaire, puis sous cutanée, avec respectivement 38 (93 %), 9 (22 %), et 3 réponses (7 %). En prévention du déficit en B12, le schéma le plus utilisé en traitement d’entretien est 1mg par semaine, par voie orale (n=21 ; 51 %). Les autres schémas utilisés dans ce cadre étaient minoritaires et parfois propres à un seul centre. En cas d’apparition d’un déficit en B12, les deux schémas les plus utilisés en traitement d’attaque sont 1mg par jour par voie orale pendant 15jours (n=8 ; 20 %), et 1mg par semaine par voie intramusculaire pendant 1 mois (n=4 ; 10 %).
Cet état des lieux a mis en évidence une grande diversité des pratiques professionnelles de supplémentation en vitamine B12 en post-chirurgie bariatrique, avec une tendance à favoriser la voie orale. Cela est dû au manque de preuves et de recommandations claires. Une étude randomisée comparative permettrait d’identifier la meilleure voie et posologie de supplémentation en vitamine B12 après chirurgie bariatrique.
Objective
The aim of this study was to investigate the respective influence of genetic and nongenetic factors on morphine dose requirements and adverse effects after colorectal surgery.
Methods
...Seventy‐four patients who planned to undergo colorectal surgery were included in this pilot study. The cumulative 24‐hour postoperative dose of morphine and postoperative nausea or vomiting requiring the antiemetic ondansetron were the 2 clinical end points. The association of patient characteristics, A118G μ‐opioid receptor (OPRM1) single‐nucleotide polymorphism (SNP); T802C uridine diphosphate–glucuronosyltransferase 2B7 (UGT2B7) SNP; and 2 adenosine triphosphate–binding cassette, subfamily B, member 1 (ABCB1) (multidrug resistance 1 MDR1) exonic SNPs (G2677T/A and C3435T) with study end points was investigated.
Results
Age, creatinine clearance, and the regular use of psychotropic agents were found to be significantly associated with postoperative morphine dose requirements by univariate analysis. Multivariate analysis identified that age (P = .01) and the use of psychotropic agents before surgery (P = .03) were positively associated with a higher rate of morphine consumption. A higher weight (P = .05) and the ABCB1 homozygous GG‐CC diplotype (P = .03) were significantly associated with fewer morphine side effects by univariate analysis. The homozygous ABCB1 diplotype (GG‐CC) conferred an odds ratio of 0.12 (95% confidence interval, 0.01–0.98) with regard to the use of ondansetron for postoperative nausea or vomiting. Multivariate analysis identified that the ABCB1 GG‐CC diplotype was the only borderline‐significant (P = .07) predictive factor of morphine side effects.
Conclusion
Age and prior use of psychotropic agents are associated with postoperative morphine dose requirements. Whether ABCB1 polymorphisms might predict morphine side effects remains to be determined.

Le trimethylamine-N-oxyde (TMAO) est une molécule issue du métabolisme par le microbiote intestinal de différents micronutriments tels que la bétaïne, la choline ou la carnitine. Des modèles ...expérimentaux murins semblent montrer que des concentrations élevées de cette molécule pourrait être responsable d’un vieillissement cérébral accéléré et d’une altération des performances cognitives. Les atteintes cognitives sont fréquentes dans le trouble de l’usage d’alcool (TUAL), notamment modéré à sévère, et potentiellement associées à une dysbiose intestinale. À notre connaissance, aucune étude n’a porté sur l’évaluation de ce métabolite chez les patients TUAL. L’objectif de ce travail est donc d’évaluer les concentrations sériques de TMAO dans une population de patients TUAL abstinents.
Trente patients TUAL pris en charge pour sevrage alcoolique, et quinze sujets contrôles, ont été inclus (ALCOBRAIN, NCT01455207). Les participants ont tous bénéficié d’un bilan biologique complet à l’entrée dans le service, permettant en particulier une évaluation de la fibrose hépatique (Fibromètre®). Le dosage sérique de TMAO a été fait par chromatographie liquide couplée à la spectrométrie de masse en tandem (LC-MS/MS). La sévérité du TUAL a été évaluée avec le score Alcohol Use Disorders Identification Test (AUDIT), et celle du sevrage grâce au score de Cushman durant l’hospitalisation. Une évaluation cognitive globale (Montreal Cognitive Assessment MoCA) était réalisée en fin de sevrage.
Les résultats montrent une grande variabilité interindividuelle de la concentration sérique de TMAO chez les patients TUAL (0,6 à 67,4μM), moins importante chez les sujets contrôles (2,2 à 10,2μM). Trois sous-groupes de patients TUAL ont été définis suivant la concentration en TMAO : tertile 1, TMAO<2,55μM ; tertile 2, 2,55μM≤TMAO≤6,76μM et tertile 3, TMAO>6,76μM. Au plan biologique, les trois sous-groupes sont significativement différents par rapport au groupe contrôle pour l’albumine, la γ-glutamyl-transférase (γ-GT), l’acide hyaluronique, l’urée sérique, et le volume globulaire moyen. Les patients TUAL du tertile 1 présentent une ammoniémie, un ratio ASAT/ALAT, une α2-macroglobulinémie, et un pourcentage de fibrose significativement plus élevés par rapport au groupe contrôle. Cependant, le score de fibrose n’est pas différent entre les tertiles. Par ailleurs, l’hémoglobinémie et la créatininémie sont significativement plus basses pour les patients TUAL du tertile 1 par rapport au groupe contrôle, mais pas pour les autres tertiles. Le score AUDIT, le score de Cushman, et le score MoCA en fin de sevrage ne sont pas différents entre les trois sous-groupes de patients TUAL.
Cette étude suggère qu’une faible concentration sérique de TMAO est associée à des altérations biologiques plus importantes chez les patients TUAL. Cependant, aucun lien n’est mis en évidence entre, d’une part, la concentration de TMAO et la sévérité du TUAL et du sevrage, et d’autre part, entre la concentration de TMAO et les performances cognitives des patients TUAL en fin de sevrage.
Background/aim The number of diabetic people never ceased to increase over the last decades. Cardiovascular (CV) disease is the primary cause of death in diabetic patients. Many studies have ...investigated the effects of glycemic variability on CV events, and some of these proposed glycemic variability as one of the factors associated with CV disease. Simple models can be used to investigate molecular mechanisms involved. The aim of this study was to investigate the impact of glucose fluctuations on cellular and mitochondrial functions in HL-1 cardiac cell line. Methods/results HL-1 cells were subjected to a 12 hours treatment. This treatment consisted in a several glucose concentrations exposure: 2 mM (low glucose LG), 5 mM (normal glucose NG), and 25 mM (high glucose HG). A fourth condition consisted in glucose fluctuations, every two hours, between 2 and 25 mM (intermittent high glucose IHG). After treatment, cells were harvested and several cellular and mitochondrial mechanisms were tested. We measured the oxygen consumption on permeabilized cells using polarography, with different substrates. IHG and HG conditions significantly enhance oxygen consumption compared with LG condition. We also evaluated activation of p38 and Akt proteins by Western blot. IHG significantly increased p38 and Akt phosphorylation compared with LG and NG conditions, respectively. Mitochondrial membrane potential was also determined, using a mitochondria specific fluorescent probe, TMRE. Mitochondrial membrane potential was significantly increased for IHG condition compared to LG and NG. Conclusion These data indicate that glucose fluctuations increase both p38 and Akt activation in HL-1 cells. Oxygen consumption and mitochondrial membrane potential were also increased for cells treated with IHG. Further studies to explore the cellular and mitochondrial mechanisms involved are required, particularly oxidative stress and cell death.
Remifentanil is a powerful mu-opioid (MOP) receptor agonist used in anaesthesia with a very short half-life. However, per-operative perfusion of remifentanil was shown to increase morphine ...consumption during post-operative period to relieve pain. In the present study, we aimed to describe the cellular mechanisms responsible for this apparent reduction of morphine efficacy. For this purpose, we first examined the pharmacological properties of both remifentanil and morphine at the MOP receptor, endogenously expressed in the human neuroblastoma SH-SY5Y cell line, to regulate adenylyl cyclase and the MAP kinase ERK1/2 pathway, their potency to promote MOP receptor phosphorylation, arrestin 3-CFP (cyan fluorescent protein) recruitment and receptor trafficking during acute and sustained exposure. In the second part of this work, we studied the effects of a prior exposure of remifentanil on morphine-induced inhibition of cAMP accumulation, activation of ERK1/2 and analgesia. We showed that sustained exposure to remifentanil promoted a rapid desensitization of opioid receptors on both signalling pathways and a pretreatment with this agonist reduced signal transduction produced by a second challenge with morphine. While both opioid agonists promoted Ser375 phosphorylation on MOP receptor, remifentanil induced a rapid internalization of opioid receptors compared to morphine but without detectable arrestin 3-CFP translocation to the plasma membrane in our experimental conditions. Lastly, a cross-tolerance between remifentanil and morphine was observed in mice using the hot plate test. Our in vitro and in vivo data thus demonstrated that remifentanil produced a rapid desensitization and internalization of the MOP receptor that would reduce the anti-nociceptive effects of morphine.

Les mucites sont une complication fréquente au cours des traitements par radio-chimiothérapie concomitante des néoplasies ORL, avec des conséquences médicoéconomiques. Mais les traitements restent ...symptomatiques et peu efficaces. Dans notre pratique, des dosages en micronutriments sont réalisés mais demeurent coûteux (≈150€/patient) et leur pertinence non analysée. L’objectif principal de cette étude était d’évaluer l’impact de déficiences en micronutriments sur la survenue de mucites ulcérées (≥grade 2) chez des patients traités par radio-chimiothérapie concomitante. Les objectifs secondaires étaient d’évaluer les autres facteurs de risques de mucites ulcérées et d’analyser l’évolution du statut nutritionnel des patients.
Étude pilote observationnelle sur des patients suivis entre 2015 et 2016 pour une néoplasie ORL et traités par radio-chimiothérapie concomitante. Les données recueillies étaient le sexe, l’âge, l’intoxication alcoolo-tabagique, les traitements préalables, les dosages en micronutriments. La survenue d’une mucite (échelle OMS pour le grade) diagnostiquée par l’oncologue et les bilans nutritionnels de la diététicienne étaient colligés à j0 et j35 du début du traitement. Les micronutriments dosés étaient le zinc, le sélénium, et les vitamines B6, A, E et C. Une déficience était définie par un taux inférieur aux seuils du laboratoire. Devant le faible effectif de la population, l’analyse des facteurs prédictifs de mucite a été réalisée en distinguant le groupe 1 « pas de mucite ou mucite légère (stades 0,1) »,versus le groupe 2 « mucites ulcérées (stades 2, 3, 4) » définies par la présence d’ulcérations associées à des douleurs endobuccales. Statistiques : test du Chi2 ou Fisher (p˂0,05).
Vingt-huit patients ont été inclus (d’âge 58±12 ans, sex-ratio H/F=4,6) ; 85 % présentaient une intoxication tabagique et 80 % un mésusage de l’alcool. Une dénutrition était présente chez 75 % des patients, dont 11 % sévèrement. La moitié des patients bénéficiait d’un support nutritionnel (29 % oral, 21 % nutrition entérale NE). Chez 83 % des patients, au moins une déficience était retrouvée, principalement en vitamine C (68 %), sélénium (33 %) et vitamine B6 (25 %). Aucun patient n’était déficitaire en zinc. Une mucite était décrite chez 86 % des patients. Le G2 représentait 68 % des patients, contre 32 % pour le G1. Il n’y avait pas de lien entre les déficiences en micronutriments et la survenue de mucites ulcérées (p=NS). Il n’a pas été retrouvé de différence significative en analyse univariée pour les autres facteurs prédictifs étudiés (âge, dénutrition,…) sur la survenue de mucites ulcérées. En fin de traitement, 89 % des patients étaient dénutris dont 22 % sévèrement et tous bénéficiaient d’un support nutritionnel (44 %NE).
Notre étude n’a pas démontré de lien entre les déficits en micronutriments et le risque de mucite, possiblement par la faible puissance. Mais, du fait de la fréquence très élevée des déficiences en micronutriments et des dosages coûteux, il semble indiqué de supplémenter en systématique cette population à risque important de mucite (≈10€ pour une supplémentation d’1 mois).
Abstract Aims In addition to hyperglycaemia, glycaemic variability seems to be associated with poor outcomes after acute myocardial infarction. This study explored the impact of glycaemic variability ...in diabetic Wistar rats subjected to myocardial ischaemia/reperfusion. Methods Animals with streptozotocin-induced diabetes received insulin either to maintain stable hyperglycaemia (Dh group) or to generate glycaemic variability (Dv). After experimental myocardial ischaemia/reperfusion was surgically induced, 7T cardiac magnetic resonance imaging (CMR) was performed at weeks 1 (w1) and 3 (w3). Results Twenty-six rats were randomized sham group (S): n = 5; control group (C): n = 7; Dh group: n = 6; and Dv group: n = 8. The mean amplitude of glucose reflecting glycaemic variability was higher in the Dv than in the Dh group (9.1 ± 2.7 mmol/L vs 5.9 ± 1.9 mmol/L; P < 0.05). CMR assessment at w3 revealed ventricular enlargement in both Dh and Dv groups compared with the C and S groups (end-diastolic volume: 1.60 ± 0.22 and 1.36 ± 0.30 mL/kg compared with 1.11 ± 0.13 and 0.87 ± 0.11 mL/kg, respectively; P < 0.05). Circumferential strain was altered between w1 and w3 in the remote area only in the Dv group, resulting in a lower value in this group than in the S, C and Dh groups (−0.11 ± 0.01 vs −0.17 ± 0.05, −0.15 ± 0.03 and −0.16 ± 0.03, respectively; P < 0.05). In addition, at w3, oedema was also higher in the remote area in the Dv than in the C group (18.3 ± 4.9 ms vs 14.5 ± 1.7 ms, respectively; P < 0.05). Conclusion In the context of experimental myocardial ischaemia/reperfusion, our results suggest that glycaemic variability might have a potentially deleterious impact on myocardial outcomes beyond the classical glucose metrics.
Background and purpose:
δ‐Opioid receptors (DOP receptors) could represent a novel target in the treatment of depressive disorders. To explore this new field of interest, the development of highly ...selective DOP receptor agonists is essential. UFP‐512 H‐Dmt‐Tic‐NH‐CH(CH2‐COOH)‐Bid, was recently shown to behave in vitro as a selective and potent DOP receptor agonist and to promote antidepressant‐ and anxiolytic‐like effects in vivo (Vergura et al., 2007). Here, we have characterized the pharmacological properties of UFP‐512 and established a link between desensitization and tolerance.
Experimental approach:
Studies were performed in the human neuroblastoma SK‐N‐BE cells to establish i) binding parameters for UFP‐512 ii) signalling pathways activated after acute and chronic treatment iii) regulation (phosphorylation and trafficking) of human DOP (hDOP) receptors after sustained activation by UFP‐512. In vivo, we studied UFP‐512‐induced antidepressant‐like effects after acute or chronic treatment in the mouse forced swimming test.
Key results:
In vitro, UFP‐512 was a high affinity agonist for DOP receptors. While UFP‐512 induced marked phosphorylation of DOP receptors on Ser363, we observed a low desensitization of the cAMP pathway, associated with receptor endocytosis and recycling without any reduction on extracellular signal‐regulated protein kinase 1/2 activation. In vivo, acute administration of UFP‐512 produced an antidepressant‐like effect, without any sign of tolerance after chronic administration.
Conclusions and implications:
There was a correlation between weak desensitization, significant internalization and recycling of the human DOP receptors and lack of tolerance to UFP‐512. This suggests that this compound would be a promising drug prototype for exploring innovative treatments for mood disorders.

A substitution at nucleotide position 3090 which was detected only in urine sediment and muscle of the patient and was not found in her mother’s blood cells and urine sample. We suggest that this novel de novo mutation in the 16S ribosomal RNA, a nucleotide which is highly conserved in different species, would impair mitochondrial protein synthesis and would cause a severe myopathy.
