Les angiœdèmes héréditaires (AOH), avec ou sans déficit en C1-inhibiteur, sont des maladies rares caractérisées par des crises récurrentes d’œdème non inflammatoire sous-cutané et/ou sous-muqueux. Le ...bérotralstat est le premier inhibiteur hautement sélectif de l’activité protéolytique de la kallicréine plasmatique administré par voie orale. Il a montré son profil favorable d’efficacité et de tolérance lors d’une étude de phase 3, APeX-2. Il est administré par voie orale en une prise quotidienne de 150mg. En France, il est disponible depuis août 2020 d’abord dans le cadre d’une autorisation temporaire d’utilisation (ATU) puis a obtenu son autorisation de mise sur le marché le 30 avril 2021. Il est indiqué dans la prévention des crises d’AOH chez les adultes et les adolescents âgés de 12 ans et plus. Berolife est une étude observationnelle post-ATU visant à collecter les données en vie réelle de tolérance et d’efficacité du bérotralstat. Les résultats intermédiaires de cette étude débutée en septembre 2021 sont présentés ici.
Quatorze investigateurs répartis dans 13 centres hospitaliers ont accepté de participer à cette étude. Les données sont collectées par l’intermédiaire d’un cahier d’observation électronique à différentes étapes : lors de l’initiation du traitement par bérotralstat, à 1 mois, 3 mois, 6 mois et 9 mois de suivi. Les données recueillies incluent des éléments démographiques, l’histoire de la maladie, les informations relatives aux crises d’AOH et la collecte des évènements indésirables (EI), à l’état basal et lors du suivi. Les données présentées ici sont celles disponibles au 26 août 2022. Le recueil des données est toujours en cours.
Les données à l’état basal sont disponibles pour 51 patients dont 8 ayant bénéficié d’une ATU nominative. L’âge moyen est de 41,5±18,6 ans et 61 % sont des femmes (n=31). Concernant le diagnostic, 84 % présentaient un AOH par déficit en C1-inhibiteur (C1INH) de type I (n=42), 6 % de type II (n=3) et 10 % avaient un AOH à C1INH normal (n=5) et un patient de diagnostic non précisé. La majorité des patients avaient une histoire familiale d’AOH (80 %, n=39). Des comorbidités étaient présentes chez 27,5 % des patients (n=14), parmi elles, l’hypertension artérielle systémique était la plus fréquemment rapportée dans 57 % des cas (n=8). Un précédent traitement préventif à long terme était recensé chez 80 % des patients (n=39), dont 20 par danatrol, 16 par acide tranexamique, 6 par lanadelumab et 4 par macroprogestatifs. Le nombre moyen de crises d’AOH au cours des 6 mois précédant la mise sous bérotralstat était de 1,28±1,13 crises/mois, la majorité concernant la sphère abdominale (70 %), d’intensité sévère (47 %). Les données de suivi sous berotralstat sont disponibles pour 27 patients avec une durée moyenne de suivi de 4 mois±2,49. Le nombre de crises mensuelles est de 0,86 à 1 mois (n=7), 1,25 à 0,79 à 3 mois (n=8), et 1,29 à 0,38 à 6 mois (n=10). Deux patients ont été suivis durant 9 mois. Parmi les patients ayant complété des données à 6 mois, la réduction moyenne du nombre de crises mensuelles était de 70 %. Concernant la tolérance du bérotralstat, 16 EI sont survenus chez 11 patients. Aucun EI grave n’a été enregistré. Huit patients ont arrêté le bérotralstat : une patiente en raison d’une grossesse, 5 pour manque de contrôle de la maladie et 2 pour intolérance au cours du premier mois.
L’étude pivot APeX-2 a permis de démontrer le profil favorable de sécurité et d’efficacité du bérotralstat. Les résultats préliminaires de cette étude post-ATU, menée en vraie vie dans les conditions réelles d’utilisation a montré une réduction de la fréquence des crises mensuelles maintenue dans le temps et un profil de tolérance favorable. Ces données corroborent l’étude pivot, démontrant que le berotralstat est un traitement efficace pour les patients atteints d’AOH. Les résultats finaux de cette étude sont attendus pour septembre 2024.

La specificità della rianimazione ...cardiopolmonare intraospedaliera è stata riconosciuta per la prima volta nel 1997, il che spiega il ritardo nell’instaurazione della “catena di sopravvivenza intraospedaliera”. Questa catena di sopravvivenza richiede una formazione con il mantenimento delle competenze del personale ospedaliero nei gesti di rianimazione cardiopolmonare di base e l’uso di un defibrillatore esterno automatizzato, la fornitura di carrelli di urgenza e defibrillatori in diverse zone strategiche degli stabilimenti, una valutazione delle pratiche da parte di un comitato intraospedaliero e un registro di tali eventi. Occorre distinguere tra l’arresto cardiaco del paziente monitorato e quello del paziente non monitorato. L’arresto cardiaco del paziente monitorato è quello che si verifica in sala operatoria o in terapia intensiva: presenta particolarità nella diagnosi e nel trattamento, legate alla presenza del monitoraggio e frequentemente della ventilazione artificiale. In caso di arresto cardiaco legato a un sovradosaggio di anestetici locali durante un’anestesia locoregionale in un paziente non ventilato, sono ora proposti trattamenti “specifici”. L’arresto cardiaco del paziente non monitorato si inserisce nell’algoritmo terapeutico generale dell’arresto cardiaco extraospedaliero. Viceversa, pone un vero e proprio problema di politica medica attraverso l’applicazione della catena di sopravvivenza intraospedaliera raccomandata nelle raccomandazioni internazionali e nazionali.

In this ...study, we describe a case of PAMA after accidental re-exposure to amoxicillin in a patient who developed severe anaphylaxis to this drug three days earlier in the operating room. To our knowledge, this report is the second to document this phenomenon.

Although multiple individual immune parameters have been demonstrated to predict the occurrence of secondary infection after critical illness, significant questions remain with regards to ...the selection, timing and clinical utility of such immune monitoring tests.
Research question
As a sub-study of the
REALISM
study, the
REALIST
score was developed as a pragmatic approach to help clinicians better identify and stratify patients at high risk for secondary infection, using a simple set of relatively available and technically robust biomarkers.
Study design and methods
This is a sub-study of a single-centre prospective cohort study of immune profiling in critically ill adults admitted after severe trauma, major surgery or sepsis/septic shock. For the REALIST score, five immune parameters were pre-emptively selected based on their clinical applicability and technical robustness. Predictive power of different parameters and combinations of parameters was assessed. The main outcome of interest was the occurrence of secondary infection within 30 days.
Results
After excluding statistically redundant and poorly predictive parameters, three parameters remained in the
REALIST
score: mHLA-DR, percentage of immature (CD10
−
CD16
−
) neutrophils and serum IL-10 level. In the cohort of interest (
n
= 189), incidence of secondary infection at day 30 increased from 8% for patients with
REALIST
score of 0 to 46% in patients with a score of 3 abnormal parameters, measured ad D5–7. When adjusted for a priori identified clinical risk factors for secondary infection (SOFA score and invasive mechanical ventilation at D5–7), a higher
REALIST
score was independently associated with increased risk of secondary infection (42 events (22.2%), adjusted HR 3.22 (1.09–9.50),
p
= 0.034) and mortality (10 events (5.3%),
p
= 0.001).
Interpretation
We derived and presented the
REALIST
score, a simple and pragmatic stratification strategy which provides clinicians with a clear assessment of the immune status of their patients. This new tool could help optimize care of these individuals and could contribute in designing future trials of immune stimulation strategies.

Angioedema (AE) due to acquired C1-inhibitor (C1-INH) deficiency (AAE-C1-INH) is related to excessive consumption of C1-INH or to anti-C1-INH antibodies, and is frequently associated with ...lymphoproliferative syndromes or monoclonal gammopathies. Standard of care for prophylactic treatment in this condition is not established. Rituximab may be effective to prevent attacks, especially if the lymphoid hemopathy is controlled, but data are scarce.
To evaluate efficacy of rituximab in AAE-C1-INH.
A retrospective multicenter study was carried out in France, including patients with AAE-C1-INH treated with rituximab between April 2005 and July 2019.
Fifty-five patients with AAE-C1-INH were included in the study, and 23 of them had an anti-C1-INH antibody. A lymphoid malignancy was identified in 39 patients, and a monoclonal gammopathy in 9. There was no associated condition in 7 cases. Thirty patients received rituximab alone or in association with chemotherapy (n = 25). Among 51 patients with available follow-up, 34 patients were in clinical remission and 17 patients had active AE after a median follow-up of 3.9 years (interquartile range, 1.5-7.7). Three patients died. The presence of anti-C1-INH antibodies was associated with a lower probability of AE remission (hazard ratio, 0.29 95% CI, 0.12-0.67; P = .004). Relapse was less frequent in patients with lymphoma (risk ratio, 0.27 95% CI, 0.09-0.80; P = .019) and in patients treated with rituximab and chemotherapy (risk ratio, 0.31 95% CI, 0.12-0.79; P = .014).
Rituximab is an efficient and well-tolerated therapeutic option in AE, especially in lymphoid malignancies and in the absence of detectable anti-C1-INH antibodies.
