IL-10 is an anti-inflammatory cytokine that regulates the extent of host immunity to infection by exerting suppressive effects on different cell types. Herpes viruses induce IL-10 to modulate the ...virus-host balance towards their own benefit, resulting in prolonged virus persistence. To define the cellular and molecular players involved in IL-10 modulation of herpes virus-specific immunity, we studied mouse cytomegalovirus (MCMV) infection. Here we demonstrate that IL-10 specifically curtails the MCMV-specific CD4 T cell response by suppressing the bidirectional crosstalk between NK cells and myeloid dendritic cells (DCs). In absence of IL-10, NK cells licensed DCs to effectively prime MCMV-specific CD4 T cells and we defined the pro-inflammatory cytokines IL-12, IFN-γ and TNF-α as well as NK cell activating receptors NKG2D and NCR-1 to regulate this bidirectional NK/DC interplay. Consequently, markedly enhanced priming of MCMV-specific CD4 T cells in Il10(-/-) mice led to faster control of lytic viral replication, but this came at the expense of TNF-α mediated immunopathology. Taken together, our data show that early induction of IL-10 during MCMV infection critically regulates the strength of the innate-adaptive immune cell crosstalk, thereby impacting beneficially on the ensuing virus-host balance for both the virus and the host.
Background IL-10 is a pleiotropic cytokine and potent negative regulator of both innate and adaptive immune responses. Consequently, IL-10–deficient (IL-10−/− ) mice have enhanced contact ...hypersensitivity (CHS) to topical hapten. Objective Although the importance of IL-10 production by (regulatory) T cells and Langerhans cells in regulating CHS has been established by cell type–specific il-10 gene targeting, it remains elusive to what extent IL-10 controls dendritic cell (DC) function in vivo. Methods To this aim, we generated mice with a DC-specific deletion of the IL-10 receptor (IL-10R). Results Despite the ability of IL-10 to inhibit DC maturation in vitro , DCs of resting DC-IL10R−/− mice retained their immature phenotype in vivo . In contrast, IL-10R−/− DCs produced increased levels of proinflammatory cytokines and IL-10 after in vitro stimulation. Induction of CHS was indistinguishable from that seen in control mice at 24 hours after hapten challenge but resulted in increased ear swelling at 48 hours and delayed resolution of the inflammatory reaction. Adoptive T-cell transfer experiments revealed that only T-cell reactivation and not sensitization by IL-10R−/− DCs leads to enhanced CHS. Accordingly, the expression of proinflammatory cytokines and IL-10 was augmented in the skin of DC-IL10R−/− mice after hapten challenge. Conclusion Our data demonstrate that IL-10 signaling in DCs is dispensable during naive T-cell priming but is critical to prevent an exaggerated effector T-cell response in the skin.
The pivotal role of TGF-beta in Langerhans cell (LC) development has been previously established in TGF-beta-deficient mice, which lack epidermal LCs. As to whether TGF-beta also governs LC ...homeostasis and function remains elusive. To assess the role of TGF-beta-mediated control of cutaneous dendritic cells (DCs) in vivo, we generated mice with a conditional knockout of the TGF-beta receptor 1 (TbetaR1) under a DC-specific promoter (DC-TbetaR1(del) mice). While initial LC seeding occurred in DC-TbetaR1(del) mice, the cells disappeared from the epidermis during the first week of life. TbetaR1-deficient LCs demonstrated spontaneous maturation and gained migratory potential based on increased surface expression of MHC class II, costimulatory molecules, and CCR7 and downregulation of E-cadherin. In parallel to their early loss from the epidermis, migrating LCs were reduced in the dermis and skin-draining lymph nodes of adult DC-TbetaR1(del) mice, whereas the number of Langerin(+) dermal DCs was similar to wild-type. In the absence of LCs, low-dose contact hypersensitivity in DC-TbetaR1(del) mice was significantly diminished. In contrast, ear swelling was restored to wild-type levels when a higher hapten dose was applied to efficiently target TbetaR1-deficient dermal DCs. In conclusion, TGF-beta inhibits in vivo LC maturation and migratory phenotype, identifying TGF-beta as a critical factor controlling LC homeostasis in the steady state.
Although IL-10 promotes a regulatory phenotype of CD11c+ dendritic cells and macrophages in vitro, the role of IL-10 signaling in CD11c+ cells to maintain intestinal tolerance in vivo remains ...elusive. To this aim, we generated mice with a CD11c-specific deletion of the IL-10 receptor alpha (Cd11ccreIl10rafl/fl). In contrast to the colon, the small intestine of Cd11ccreIl10rafl/fl mice exhibited spontaneous crypt hyperplasia, increased numbers of intraepithelial lymphocytes and lamina propria T cells, associated with elevated levels of T cell-derived IFNγ and IL-17A. Whereas naive mucosal T-cell priming was not affected and oral tolerance to ovalbumin was intact, augmented T-cell function in the lamina propria was associated with elevated numbers of locally dividing T cells, expression of T-cell attracting chemokines and reduced T-cell apoptosis. Upon stimulation, intestinal IL-10Rα deficient CD11c+ cells exhibited increased activation associated with enhanced IL-6 and TNFα production. Following colonization with Helicobacter hepaticus Cd11ccreIl10rafl/fl mice developed severe large intestinal inflammation characterized by infiltrating T cells and increased levels of Il17a, Ifng, and Il12p40. Altogether these findings demonstrate a critical role of IL-10 signaling in CD11c+ cells to control small intestinal immune homeostasis by limiting reactivation of local memory T cells and to protect against Helicobacter hepaticus-induced colitis.

Sarcoidosis is a systemic granulomatous disorder of unknown aetiology. It may rarely affect the gastrointestinal tract.
We reported a 54-year-old woman with a delayed diagnosis of duodenal ...sarcoidosis. She presented with gastric and right upper abdominal pain associated with vomiting and marked weight loss. Abdominal computed tomographic scan showed non-compressive retroperitoneal lymph nodes and histological examination revealed non-caseating epithelioid granulomas typical of sarcoidosis. Diagnosis of duodenal sarcoidosis was obtained at the third gastroscopy. The patient's condition improved quickly with corticosteroid therapy.
Gastrointestinal sarcoidosis should be looked for in patients with digestive symptoms and another sarcoid localisation. Furthermore, it is important to repeat gastroscopy to confirm diagnosis because treatment improved most patients.
L'utilisation des essais de non-infériorité est de plus en plus fréquente dans la validation des traitements anticoagulants. Un essai de non-infériorité consiste à montrer que deux traitements ne ...sont pas trop différents, et que cette différence reste inférieure à un seuil préalablement fixé. Ce seuil correspond à la quantité d'efficacité que l'on peut consentir à perdre, étant donné les autres avantages du nouveau traitement. L'objectif de ce travail est de faire une étude descriptive des essais de non-infériorité réalisés jusqu'à présent, dans le cadre de la maladie thromboembolique veineuse, en se focalisant sur la détermination des seuils de non-infériorité utilisés.
Nous avons identifié les essais de non-infériorité publiés dans la maladie thromboembolique veineuse par une recherche systématique sur PubMed. Nous avons calculé la perte d'efficacité consentie en nous référant à la valeur de l'efficacité minimale démontrée du traitement de référence contre placebo.
Nous avons trouvé neuf essais de non-infériorité portant sur la maladie thromboembolique veineuse. La perte d'efficacité maximale tolérée était en moyenne de 434
% avec une médiane à 357
%. Pour 85
% des valeurs, la perte d'efficacité consentie est supérieure à 100
%.
Les trois quarts des essais de non-infériorité dans la maladie thromboembolique concluent à l'équivalence alors qu'il existe une perte d'efficacité potentielle de la totalité de l'effet du traitement standard par rapport au placebo. Les résultats des essais de non-infériorité doivent donc être interprétés avec prudence. Le choix du seuil de non-infériorité doit être plus rigoureux et être positionné par rapport à l'efficacité minimale démontrée du traitement de référence.
Equivalence trials are actually frequently used to prove non-inferiority in anticoagulant therapy. Equivalence trials consist to demonstrate that two treatments are not too much different. This difference has to be under a margin previously determined. The margin corresponds to an efficacy loss that is defined to be acceptable, in accordance to the advantages due to the new treatment. The aim of this work is to explore the equivalence trial published in the thromboembolic disease by focus on the non-inferiority margin used.

Le syndrome catastrophique des antiphospholipides (CAPS) est une entité rare et grave du syndrome des antiphospholipides (SAPL), caractérisé par l’apparition concomitante de thromboses ...microvasculaires de plusieurs organes en mois d’une semaine. Nous rapportons la première observation à notre connaissance de CAPS familial, chez deux sœurs SAPL connues.
La première sœur est née en 1965. Elle est suivie pour un SAPL primaire compliqué d’un AVC sylvien droit à l’âge de ans, traitée par fluindione et kardegic et associé à syndrome d’Evans sous corticothérapie et imurel. À l’âge de 50ans, elle consulte aux urgences pour troubles phasiques et surdité brutale. Elle est en cours de relais AVK-héparine pour une chirurgie programmée de myomes utérins. L’IRM révèle un accident ischémique récent sylvien superficiel gauche et des lésions semi-récentes sylviennes droite, en plus des images séquellaires. Les troponines sont élevées à 1000ng/L, en lien avec une nécrose myocardique d’origine microcirculatoire en IRM. Un scanner montre des plages d’ischémies spléniques et rénales récentes, sans nette insuffisance rénale ou hypertension. Les APL sont positifs avec la présence d’un anticoagulant circulant Lupique (ACC), d’anticorps anticardiolipides (ACL)>2024U/mL (n<20U/mL) et d’antiB2GP1+>6100U/mL (n<20U/mL) Il n’y a pas de schizocytes ou d’Ac anti-PF4 pas d’anti-DNA natif, pas de consommation du complément et pas de récidive du syndrome d’Evans. Le diagnostic de CAPS est posé devant la positivité des APL et les microthromboses cérébrale, rénale, splénique et cardiaque dans un délai de moins d’une semaine. La patiente est traitée par corticothérapie orale à 1mg/kg, héparine à dose curative et échanges plasmatiques, avec une bonne efficacité sur l’ensemble des symptômes. La patiente garde seulement des séquelles phasiques de son AVC. Le typage HLA réalisé ultérieurement retrouve l’Ag DQ7 (DQB1×03:01) La deuxième sœur est née en 1968. Elle est suivie pour un SAPL compliqué de TVP et EP à l’âge de 23ans, traité par AVK. À 42ans, elle bénéficie d’une hystérectomie pour fibrome utérins et présente dans les suites immédiates des douleurs abdominales révélant une ischémie du grêle et biliaire (TDM abdominale, acide lactique élevé et constatations peropératoires cœlioscopiques). Le tableau se complique de douleurs thoraciques en lien avec un épanchement pleural bilatéral et une ischémie myocardique (troponines à 0,99ug/L, NT proBNP à 4800ng/L) avec une altération de la FEVG. Puis, elle présente une détresse respiratoire aiguë sans cause infectieuse retrouvée évoquant une atteinte pulmonaire microcirculatoire sur le scanner. Une thrombopénie s’installe concomitamment avec des taux minimaux à 61G/L ; les Ac antiPF4, Les schizocytes et les Ac anti-ADAMTS 13, les Ac antiDNA natifs sont négatifs. Le diagnostic de CAPS est alors posé devant la présence d’ACC, ACL à 61,6GPL, (n<23 GPL) et antiB2GP1 à 89UI/mL (n<10UI/mL) associé à des microthromboses digestive, pulmonaire et cardiaque survenu dans un délai de moins d’une semaine. Le traitement consiste en une corticothérapie à 2mg/kg, une anticoagulation curative par héparine et des IgIV à 2mg/kg. La situation s’améliore progressivement, avec un retour à l’état antérieur sans séquelle en quelques semaines. Par ailleurs, le typage HLA réalisé ultérieurement retrouve l’Ag DQ7 (DQB1×03:01). À noter que la mère des patientes est lupique et a présenté de multiples TVP et EP sans facteur déclenchant (APL non recherché à l’époque).
La prévalence de formes familiales de SAPL est très faible probablement autour de 5 %. Des études en population suggèrent une susceptibilité génétique à développer un SAPL. Certaines de ces familles présentent un variant HLA particulier de type DR4, DR7, DR53 ou DQ7. Seulement 0,8 % des SAPL se compliquent de CAPS, et cette observation est la première à notre connaissance d’un CAPS familial chez deux sœurs dont la mère a probablement également un SAPL. Les facteurs déclenchant du CAPS dans un SAPL sont multiples. Chez ces deux sœurs, la chirurgie est le facteur le plus probable. L’existence de deux cas d’une pathologie aussi rare dans une même famille ne peut pas être liée seulement au hasard, et des facteurs de susceptibilité génétique rentrent très probablement en jeu. Nos patientes présentent toutes les deux le variant DQ7, répertorié dans les SAPL familiaux sans les autres variants. On peut se poser la question de son rôle comme facteur de favorisant dans la survenue du CAPS dans ces cas familiaux. À l’avenir, l’étude du typage HLA II des patients ayant un CAPS pourrait permettre de préciser ces données.
Cette observation de CAPS familial survenant chez 2 sœurs pose la question d’un facteur de susceptibilité génétique lié à la présence du HLA DQ7 favorisant l’apparition de la pathologie.

La sarcoïdose est une granulomatose systémique sans étiologie connue, touchant rarement le tube digestif.
Une femme de 54 ans consultait pour des douleurs épigastriques et de l’hypochondre droit ...évoluant depuis plusieurs mois associées à des vomissements responsables d’une perte de poids importante. Les explorations retrouvaient des adénopathies rétropéritonéales non compressives avec granulomes épithélioïdes et gigantocellulaires sans nécrose caséeuse à l’examen anatomo-pathologique, compatibles avec une sarcoïdose. La localisation duodénale n’était retrouvée qu’à la troisième gastroscopie. La corticothérapie entraînait une amélioration rapide.
Une localisation digestive de la sarcoïdose doit être évoquée chez un patient ayant des troubles digestifs et une sarcoïdose avec d’autres localisations. Les gastroscopies doivent être répétées pour permettre le diagnostic et le traitement permet le plus souvent une amélioration clinique franche et rapide.
