La protéinose alvéolaire pulmonaire (PAP) est une maladie rare caractérisée par l’accumulation alvéolaire de composants du surfactant qui gêne les échanges gazeux. Les PAP sont séparées en trois ...groupes : les PAP auto-immunes caractérisées par la présence d’anticorps sériques anti-GM-CSF qui sont les plus fréquentes ; les PAP secondaires à une maladie sous-jacente, le plus souvent hématologique, mais aussi à l’inhalation de toxiques et à des infections ; enfin, les PAP d’origine génétique. Le diagnostic de PAP est évoqué sur le scanner thoracique avec une pneumopathie infiltrante diffuse ayant un aspect classique de crazy paving, puis confirmé par l’aspect laiteux du lavage bronchoalvéolaire, et la présence de matériel lipoprotéinacé PAS positif. La confirmation du diagnostic de PAP par biopsie pulmonaire chirurgicale est désormais rarement nécessaire. La positivité des anticorps sériques anti-GM-CSF signe le caractère auto-immun de la maladie. Le traitement de référence des PAP auto-immunes est le « grand lavage pulmonaire thérapeutique ». En cas de maladie réfractaire, d’autres traitements sont disponibles, tels que le GM-CSF en injection sous-cutanée ou en aérosol et le rituximab. L’évolution de la maladie est imprévisible. Une évolution spontanément favorable ou même une guérison spontanée peuvent survenir et la survie actuarielle à 5ans est de 95 %. Les principales complications sont infectieuses.
Alveolar proteinosis (AP) is a rare disease characterized by alveolar accumulation of surfactant components, which impairs gas exchange. AP is classified into three groups: auto-immune AP defined by the presence of plasma autoantibodies anti-GM-CSF, the most frequent form (90 % of all AP); secondary AP, mainly occurring as a consequence of haematological diseases, or following on from toxic inhalation or infections, and genetic AP, which affects almost exclusively children. AP diagnosis is suspected where chest CT-scan demonstrates interstitial lung disease with a crazy paving aspect; and confirmed by bronchoalveolar lavage, which has a milky appearance and contains periodic acid Schiff positive proteinaceous alveolar deposits. The use of surgical lung biopsy to confirm AP is less frequent nowadays. In this context, positive antibodies against GM-CSF indicates an auto-immune etiology of the AP. Concerning management, whole lung lavage is the gold standard therapy. In refractory AP, new treatments are available such as subcutaneous or inhaled GM-CSF supplementation, or rituximab infusions. The clinical course is unpredictable. Spontaneous improvement or even cure can occur, and the 5-year actuarial survival is 95 %. The most frequent complications are infectious etiology.
La pneumopathie d’hypersensibilité (PHS) est une pneumopathie interstitielle diffuse (PID) qui résulte d’une réaction d’hypersensibilité de type III ou IV déclenchée par l’exposition répétée à des ...antigènes de l’environnement, chez des individus prédisposés. Les formes les plus fréquentes sont la maladie des éleveurs d’oiseaux et la maladie du poumon de fermier. Les antigènes incriminés sont cependant très variés et de nouvelles causes de PHS sont fréquemment mises en évidence. Bien que relativement rare, la PHS semble sous diagnostiquée du fait de sa présentation très hétérogène dans ses formes non fibrosante ou fibrosante. L’identification des cas de PHS est cependant indispensable afin de faire bénéficier au patient de la stratégie thérapeutique adaptée. L’aide d’un conseiller médical en environnement est parfois cruciale dans ce contexte. Le diagnostic de PHS peut être considéré comme quasi certain en cas d’association d’une exposition, d’un aspect en tomodensitométrie haute résolution (TDM-HR) présentant des caractéristiques typiques et d’une lymphocytose au lavage broncho-alvéolaire (LBA) supérieure à 30 %. Dans tous les autres cas, la réalisation d’investigations à visée histologique, dont le résultat sera discuté en discussion multidisciplinaire (DMD), est indispensable. Une fois le diagnostic posé, l’éviction antigénique est la règle lorsqu’elle est possible. La corticothérapie systémique est le traitement médical de première ligne en cas de forme sévère hypoxémiante afin de prévenir l’apparition de lésions fibrosantes. La mise en place d’une thérapeutique anti-fibrosante est désormais possible, après démonstration du caractère progressif de la pathologie et échec des thérapeutiques immunomodulatrices/immunosuppressives.

The most commonly known forms are bird fancier's disease and farmer's lung. However, the antigens involved are widely diverse. Therefore, the list of causes of HP is frequently expanding. HP seems to be under-diagnosed owing to its highly heterogeneous presentation in both the non-fibrotic and fibrotic subtypes and could represent up to 15% of all ILDs encountered in clinical practice. However, the recognition of HP cases is essential to ensure appropriate therapy for the patient. Home health care workers’ intervention is sometimes critical in this context. In case of confirmed exposure, the diagnosis could be considered with high confidence if the high-resolution computed tomography (HR-CT) shows a typical HP pattern associated with a lymphocytosis over 30% in the broncho-alveolar lavage (BAL). In all other situations, the patients should undergo further investigations and additional histopathological sampling should be considered and submitted to a multidisciplinary team discussion. After diagnosis, antigenic eviction is the rule whenever possible. Corticosteroid treatment is the first-line medical treatment for severe forms and aims to prevent the development of fibrosis. Anti-fibrotic therapy is now an option for patients with progressive ILD and failure of immunomodulatory/immunosuppressive therapies.
Idiopathic pulmonary fibrosis is a disease with a poor prognosis and has been associated with increased lung cancer incidence.
We report the case of a Caucasian 75-year-old woman, a former smoker, ...hospitalized for breathlessness with a chest computed tomography scan showing an interstitial lung disease. A surgical lung biopsy was performed, confirming a pattern of usual interstitial pneumonia but also numerous disseminated foci of well-differentiated focally invasive squamous cell carcinoma without hypermetabolic lung nodule, mass, or enlarged lymph node visualized on chest computed tomography or positron emission tomography scan. Nintedanib was started for its antifibrotic and antitumor properties, without any other antineoplastic treatment. Three years after initiation of nintedanib, clinical, functional, and computed tomography scan evaluations were stable, and there was no evidence for evolution of the squamous cell carcinoma.
Data are scarce regarding the benefit of nintedanib in patients with idiopathic pulmonary fibrosis-associated lung cancer, and it is unclear whether nintedanib could have a preventive role in lung carcinogenesis in idiopathic pulmonary fibrosis patients. This experience could help the scientific community in case of similar incidental findings.

The oral antifibrotic agent, pirfenidone (PFD), 5-methyl-l-phenyl-1H-pyridine, is used to treat idiopathic pulmonary fibrosis (IPF), a chronic and fatal lung disease. In trials, PFD reduces disease ...progression and decreases mortality. The most common adverse events of PFD are skin manifestations (25%), described as photosensitivity or rash, but they are not well characterized. The objective of the present real-life study was to address the question of skin manifestations in patients treated with PFD for IPF.
Thymomas are rare intrathoracic malignancies that may be aggressive and difficult to treat. Knowledge and level of evidence for treatment strategies are mainly based on retrospective studies or ...expert opinion. Currently there is no strong evidence that postoperative radiotherapy after complete resection of localized thymoma is associated with survival benefit in patients. RADIORYTHMIC is a phase III, randomized trial aiming at comparing postoperative radiotherapy versus surveillance after complete resection of Masaoka-Koga stage IIb/III thymoma. Systematic central pathologic review will be performed before patient enrollment as per the RYTHMIC network pathway.
Three hundred fourteen patients will be included; randomization 1:1 will attribute either postoperative radiotherapy (50-54 Gy to the mediastinum using intensity-modulated radiation therapy or proton beam therapy) or surveillance. Stratification criteria include histologic grading (thymoma type A, AB, B1 vs B2, B3), stage, and delivery of preoperative chemotherapy. Patient recruitment will be mainly made through the French RYTHMIC network of 15 expert centers participating in a nationwide multidisciplinary tumor board. Follow-up will last 7 years. The primary endpoint is recurrence-free survival. Secondary objectives include overall survival, assessment of acute and late toxicities, and analysis of prognostic and predictive biomarkers.
The first patient will be enrolled in January 2021, with results expected in 2028.
Au cours de la FPI, la dénutrition est fréquente mais son impact sur le devenir du patient est mal connu. But : déterminer si la dénutrition au moment du diagnostic influence l’hospitalisation et la ...mortalité toutes causes confondues, et les exacerbations aiguës au cours du suivi.
L’état nutritionnel de 153 patients ambulatoires porteurs d’une FPI nouvellement diagnostiquée consécutifs a été évalué en mesurant l’indice de masse corporelle (IMC), l’indice de masse maigre (FFMI, mesuré par BIA) et la prise alimentaire (Score d’Evaluation facile des ingesta, SEFI®). La dénutrition a été définie par un FFMI<17 (hommes) ou<15 (femmes). Le jour de l’évaluation nutritionnelle a été considéré comme date de début du suivi. Pour déterminer les facteurs associés à l’hospitalisation et la mortalité toutes causes confondues, des analyses de régression de Cox univariées ont été effectuées et les variables avec p<0,2 ont été incluses dans une analyse multivariée pas-à-pas.
Plus d’1/4 (26 % ; 40/153) des patients était dénutris à l’inclusion. La dénutrition était encore plus fréquente chez les patients avec IMC<25 (62 %). Les patients dénutris (FFMI bas) étaient plus susceptibles d’être hospitalisés (hazard ratio (HR)=1,98 intervalle de confiance (IC) 95 %, 1,15 ; 3,41, p=0,014) et/ou de mourir (HR=1,79 1,11 ; 2,89, p=0,016), mais les exacerbations aiguës n’étaient pas plus fréquentes qu’en cas de FFMI normal. Une diminution de l’apport alimentaire (SEFI®<7) à l’inclusion était associée à plus d’hospitalisation et de mortalité toutes causes confondues au cours du suivi. L’hospitalisation et la mortalité toutes causes confondues étaient indépendamment associées à un score GAP (Gender Age Physiology) plus élevé (HR=1,24 IC95 %, 1,01 ; 1,52, p=0,0434 ; HR=1,71 1,37 ; 2,14, p<0,0001, respectivement), un IMC plus faible (HR=0,89 0,83 ; 0,96, p=0,003 ; HR=0,89 0,82 ; 0,96, p=0,003) et une consommation alimentaire plus faible (score SEFI® plus bas) (HR=0,81 0,71 ; 0,93, p=0,003 ; HR=0,72 0,64 ; 0,81, p<0,0001).
Au moment du diagnostic de FPI, la dénutrition et une prise alimentaire insuffisante sont associées à une augmentation des hospitalisations et de la mortalité. D’autres études sont nécessaires pour déterminer si des interventions nutritionnelles pourraient améliorer le devenir clinique des patients atteints de FPI.
