A 75-year-old woman is hospitalised for sepsis. The diagnosis of cholecystitis is made and an antibiotic therapy is debuted. The clinical worsening leads to realise an urgent cholecystectomy. A ...sepsis like shock persisted. The antibiotherapy was changed and a second abdominal look made. A severe lactic acidosis persisted since the cholecystectomy despite a continuous hemofiltration. The diagnosis of type B lactic acidosis secondary to malignancy was suspected. An osteomedullar biopsy revelled B-cell lymphoma EBV induced.
Une femme de 75ans est hospitalisée pour sepsis. Le diagnostic de cholécystite aiguë est retenu et un traitement antibiotique débuté. L’aggravation clinique conduit à réaliser une cholécystectomie en ...urgence. Il persiste un état de choc d’allure septique en postopératoire conduisant après un élargissement de l’antibiothérapie à une réintervention sans résultat. Une acidose lactique sévère persiste depuis la cholécystectomie sous épuration extrarénale continue faisant évoquer une acidose lactique de type B secondaire à une hémopathie sous-jacente. Une biopsie ostéomédullaire viendra confirmer le diagnostic de lymphome B à grandes cellules EBV induit du sujet âgé.

A 44-year-old woman had to undergo repeat laparoscopy, four hours after laparoscopic sigmoidectomy, because of persistent bleeding from the drain. The bleeding caused perioperative shock and ...necessitated transfusional support. The following day she developed dyspnea, revealing pulmonary oedema. As her respiratory status continued to deteriorate and the bleeding persisted, she was transferred to the intensive care unit on day 2, after corrective laparotomy. The echocardiogram ruled out cardiogenic pulmonary oedema. No inhalation had occurred during the three anaesthetic procedures, and onset several hours after transfusion argued against lesional pulmonary oedema secondary to packed red cell transfusion. Given the context of repeat surgery by laparoscopy because of active bleeding, a diagnosis of lesional pulmonary oedema complicating CO(2) embolism was made. She received symptomatic treatment and her respiratory status gradually improved. Four months later her clinical status was normal. The different causes of postoperative pulmonary oedema and the pathophysiologic mechanisms of pulmonary lesions induced by gas embolism are discussed.
Une patiente de 44 ans opérée d'une sigmoïdectomie par laparoscopie a dû être reprise au bloc opératoire, à la quatrième heure, par laparoscopie, pour un saignement persistant par la lame de ...drainage. Un collapsus peropératoire attribué à l'hémorragie a justifié d'une transfusion sanguine. L'évolution a été marquée par l'apparition à j1 d'une dyspnée révélant un œdème pulmonaire. L'aggravation respiratoire et la persistance du saignement ont imposé le transfert en réanimation à j2 après reprise chirurgicale par laparotomie. L'échographie cardiaque a permis d'éliminer un œdème pulmonaire cardiogénique. Aucun épisode d'inhalation n'a été constaté au décours des trois anesthésies. La survenue plusieurs heures après la transfusion a plaidé contre le diagnostic d'œdème pulmonaire lésionnel secondaire à la transfusion de concentrés globulaires. Compte tenu d'une reprise chirurgicale par laparoscopie dans un contexte de saignement actif, le diagnostic d'œdème pulmonaire lésionnel compliquant une embolie de CO
2 a été retenu. La patiente a bénéficié d'un traitement symptomatique permettant une amélioration progressive de l'état respiratoire. À quatre mois, l'état clinique était normal. Les données de la littérature ont permis de discuter les différentes étiologies d'œdème pulmonaire postopératoire et les mécanismes physiopathologiques des lésions pulmonaires induites par les embolies gazeuses.
