We report 5 cases of acute heart failure (AHF) related to multiple sclerosis (MS) relapses. AHF was inaugural in 3 patients, always preceded or accompanied by signs of brainstem dysfunction; it was ...severe, requiring intensive care management. Echocardiography showed left ventricular hypokinesis. No other cause of AHF has been found. All patients showed a new medullary lesion on brain magnetic resonance imaging. All had rapid and complete recovery of ventricular function after intravenous corticosteroids. We concluded that the cases represent a takotsubo phenomenon. Physicians should be aware of rare cases of takotsubo cardiomyopathy in MS relapses. Ann Neurol 2017;81:754–758
Risk factors associated with the severity of coronavirus disease 2019 (COVID-19) in patients with multiple sclerosis (MS) are unknown. Disease-modifying therapies (DMTs) may modify the risk of ...developing a severe COVID-19 infection, beside identified risk factors such as age and comorbidities.
To describe the clinical characteristics and outcomes in patients with MS and COVID-19 and identify factors associated with COVID-19 severity.
The Covisep registry is a multicenter, retrospective, observational cohort study conducted in MS expert centers and general hospitals and with neurologists collaborating with MS expert centers and members of the Société Francophone de la Sclérose en Plaques. The study included patients with MS presenting with a confirmed or highly suspected diagnosis of COVID-19 between March 1, 2020, and May 21, 2020.
COVID-19 diagnosed with a polymerase chain reaction test on a nasopharyngeal swab, thoracic computed tomography, or typical symptoms.
The main outcome was COVID-19 severity assessed on a 7-point ordinal scale (ranging from 1 not hospitalized with no limitations on activities to 7 death) with a cutoff at 3 (hospitalized and not requiring supplemental oxygen). We collected demographics, neurological history, Expanded Disability Severity Scale score (EDSS; ranging from 0 to 10, with cutoffs at 3 and 6), comorbidities, COVID-19 characteristics, and outcomes. Univariate and multivariate logistic regression models were used to estimate the association of collected variables with COVID-19 outcomes.
A total of 347 patients (mean SD age, 44.6 12.8 years, 249 women; mean SD disease duration, 13.5 10.0 years) were analyzed. Seventy-three patients (21.0%) had a COVID-19 severity score of 3 or more, and 12 patients (3.5%) died of COVID-19. The median EDSS was 2.0 (range, 0-9.5), and 284 patients (81.8%) were receiving DMT. There was a higher proportion of patients with a COVID-19 severity score of 3 or more among patients with no DMT relative to patients receiving DMTs (46.0% vs 15.5%; P < .001). Multivariate logistic regression models determined that age (odds ratio per 10 years: 1.9 95% CI, 1.4-2.5), EDSS (OR for EDSS ≥6, 6.3 95% CI. 2.8-14.4), and obesity (OR, 3.0 95% CI, 1.0-8.7) were independent risk factors for a COVID-19 severity score of 3 or more (indicating hospitalization or higher severity). The EDSS was associated with the highest variability of COVID-19 severe outcome (R2, 0.2), followed by age (R2, 0.06) and obesity (R2, 0.01).
In this registry-based cohort study of patients with MS, age, EDSS, and obesity were independent risk factors for severe COVID-19; there was no association found between DMTs exposure and COVID-19 severity. The identification of these risk factors should provide the rationale for an individual strategy regarding clinical management of patients with MS during the COVID-19 pandemic.
La maladie associée aux anticorps anti-MOG (MOGAD) est une affection démyélinisante rare du système nerveux central ayant un tropisme optico-médullaire, notamment chez l’adulte, pouvant être ...monophasique ou à rechutes.
L’objectif était d’évaluer la présence de BOC dans le LCR comme un possible facteur de risque de sévérité des poussées et/ou de rechutes de la maladie.
Les données de 190 patients issus du registre français des NMO (NOMADMUS) ont été analysées après répartition en deux groupes selon la présence de bandes oligoclonales au sein du LCR. Nous avons décrit et comparé entre les groupes les caractéristiques générales, cliniques et d’imagerie, et avons également précisé l’évolution radiologique. Des courbes de survies ont été réalisées pour comparer le risque de rechute entre les groupes.
La présence de BOC dans le LCR n’était pas associée à un pronostic fonctionnel plus sévère, après mesure de l’EDSS ou de l’acuité visuelle. Le taux de rechutes était plus élevé dans le groupe BOC+(p=0,049) avec une différence se mettant en place essentiellement lors de la première année (p=0,007). Un phénotype de SEP sur l’IRM cérébrale était plus souvent décrit dans le groupe BOC+(p=0,014).
Les patients MOGAD BOC+soulèvent la question du risque d’erreur diagnostique pour une SEP. Cependant, malgré la plus forte proportion de phénotype de SEP sur l’IRM cérébrale initiale, près de 2/3 des patients du groupe BOC+décrivaient une amélioration ou disparition des lésions à l’imagerie, constituant un drapeau rouge pour le diagnostic de SEP.
Les patients atteints de MOGAD BOC+sont plus à risque de rechute. Une vigilance particulière reste nécessaire pour l’exclusion du diagnostic de SEP.
To characterise the response to treatment of inaugural optic neuritis (ON) in patients with myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD).
We searched the French MOGAD ...database for adults with inaugural ON with a detailed report of acute treatment modalities and measures of high-contrast best-corrected visual acuity (BCVA) at nadir and after 3 months. Predictors of visual outcomes were assessed by multivariable analysis.
Among 245 patients with at least one episode of ON, 82 fulfilled all criteria, and data on the peripapillary retinal nerve fibre layer (pRNFL) were available for 44. All patients received methylprednisolone (MP), combined with plasma exchange in 18. After 3 months, 75 of 82 (91%) patients retained full BCVA recovery, and median (range) pRNFL of the affected eye was 72 µm (40-102). Failure to regain 0.0 logarithmic minimum angle of resolution vision (Snellen 20/20) at 3 months was associated with time to first MP treatment ≥10 days (OR 16, 95% CI 1.14 to 213, p=0.01). pRNFL thickness after 3 months was related to better BCVA at nadir and time to first MP treatment <10 days (r2=19%, p=0.004 and r2=11%, p=0.03, respectively).
Time to MP affects functional but also structural visual outcomes of ON in MOGAD.

Hepatitis E virus (HEV) is a major cause of acute hepatitis worldwide. However, our understanding of the source of contamination is incomplete and the frequency of neurological manifestations in still unknown.
200 eligible cases reported to the French National Reference Center from January 2015 to December 2015 were prospectively included in this case-control study (1 case: 1 control, matched for sex, age and area of living) to investigate the risk of infection. We documented the factors associated with their HEV infection and clinical manifestations.
The 200 HEV-infected patients included 137 who were immunocompetent and 63 immunocompromised. The factors associated with an HEV infection were contact with farm animals, eating pork liver sausage and eating unpeeled fruit. The 33 patients (16.5%) who reported neurological symptoms included 14 with neuropathic pain suggesting small fiber neuropathy, 9 with painless sensory disorders, 6 with Parsonage–Turner syndrome, one Guillain–Barre syndrome, one meningitis, one encephalitis and one diplopia. Neurological manifestations were more frequent in immunocompetent patients (22.6% vs 3.2%, p < 0.001).
This study highlights the risk of HEV transmission by the environment in industrialized countries. The higher frequency of neurological disorders in immunocompetent patients suggests pathophysiological mechanisms involving the immune system.
Les maladies associées aux anticorps anti-MOG sont fréquentes en pédiatrie. La récupération somatique au décours est souvent bonne, mais les séquelles cognitives sont mal décrites et imparfaitement ...évaluées.
L’objectif de notre étude était de décrire, dans une cohorte de patients MOGAD pédiatriques, l’impact de la maladie sur les capacités cognitives et le cursus scolaire.
Il s’agissait d’une étude observationnelle, rétrospective, monocentrique, sur une cohorte de patients MOGAD à début pédiatrique. Les caractéristiques démographiques (dont pré et néonatales, et sur le développement psychomoteur), cliniques, biologiques, d’imagerie, scolaires, ainsi que les évaluations cognitives par l’échelle du WISC-V étaient recueillis.
Cinquante et un patients ont été inclus, avec un âge médian de 8 ans au diagnostic. L’encéphalomyélite aiguë disséminée était la présentation la plus fréquente (54,6 %), suivie de la névrite optique (35 %). Le taux de récidive était de 27,4 %. Au cours du suivi, une augmentation significative des adaptations scolaires et de la nécessité d’une auxiliaire de vie scolaire était notée. Les évaluations par WISC-V (10 patients), montraient des résultats hétérogènes, dans la moyenne basse.
Parmi les patients ayant une scolarité adaptée, 75 % avaient un phénotype initial d’ADEM, 25 % de névrite optique. Ceci suggère une association entre phénotype initial, topographie des lésions, et difficultés cognitives. Aucun facteur de risque pré ou néonatal de difficulté cognitive n’a été mis en évidence.
Les atteintes cognitives sont fréquentes au décours des MOGAD pédiatriques. Elles se traduisent par des difficultés scolaires et des résultats faibles au WISC-V, suggérant l’intérêt d’un dépistage systématique.
La maladie associée aux anticorps anti-MOG (MOGAD) est une maladie inflammatoire rare du système nerveux central touchant préférentiellement l’enfant et le jeune adulte.
L’objectif de cette étude ...était de décrire les caractères cliniques et paracliniques et d’évaluer le pronostic d’une population de patients présentant une MOGAD ayant débutée après l’âge de 50 ans.
Nous avons conduit une étude observationnelle, rétrospective, nationale, multicentrique, incluant les patients ayant présenté un premier épisode de MOGAD après l’âge de 50 ans, et pour lesquels le test sérique était réalisé au laboratoire de référence à Lyon. Nous avons inclus 77 patients, issus de la base NOMADMUS, pour lesquels les données cliniques et paracliniques étaient disponibles.
Au total, 77 patients ont été inclus. L’âge médian était de 57 ans avec 88 % de patients de moins de 70 ans. La névrite optique était l’atteinte initiale la plus fréquente. Les patients étaient fréquemment traités, en phase aiguë (88,2 %), au long cours (72 % ont reçu un traitement de fond), avec 40 effets indésirables identifiés, dont 12 graves. Au total, 44,1 % des patients ont rechuté après 2 ans de suivi et l’EDSS médian au dernier suivi était à 2.
Le pronostic était globalement comparable aux patients plus jeunes, avec un risque de rechute et un handicap modéré, et semblable entre les patients monophasiques et ceux à rechutes. Par ailleurs, des effets secondaires, parfois graves, liés aux traitements de fond ont été observés, devant faire discuter au cas par cas l’intérêt de traiter ces patients au long cours.
À la différence de la SEP ou la NMO, les patients débutant la maladie après 50 ans ont une présentation clinique et un risque évolutif similaire aux patients plus jeunes.
La maladie du spectre des anticorps anti-MOG ou MOGAD (pour Myelin oligodendrocyte glycoprotein antibody-associated disease) est une maladie inflammatoire démyélinisante du système nerveux central ...dont les critères diagnostiques ont été mis à jour en 2023. La détection des anticorps anti-MOG est un élément clé du diagnostic de cette pathologie et différentes techniques existent à l’heure actuelle, reposant sur l’utilisation de cell-based assays sur cellules fixées (FCBA) ou vivantes (LCBA). Dans cette étude, nous avons comparé deux techniques (FCBA et LCBA) puis analysé les données cliniques des patients ayant des résultats discordants. Parmi les 759 échantillons testés, 233 étaient positifs par FCBA et/ou LCBA : 65 % (151/233) étaient positifs avec les deux techniques, 30 % (71/233) étaient positifs par FCBA de façon isolée et 5 % (11/233) étaient positifs par LCBA de façon isolée. Nous avons ensuite exploré les données cliniques de 56 % (40/71) des patients positifs par FCBA mais négatifs par LCBA, et le diagnostic de MOGAD a été retenu pour 42,5 % d’entre eux. Nos données montrent ainsi une bonne concordance entre les deux techniques et l’utilisation d’une technique FCBA a permis de réévaluer le diagnostic de MOGAD pour un nombre important de patients. Néanmoins, cette technique manque de spécificité. Une stratégie de dépistage utilisant en première intention le FCBA avec confirmation par LCBA des sérums positifs mais également des sérums négatifs pour lesquels la suspicion clinique de MOGAD est forte, semble la plus adaptée pour limiter le risque de faux positifs et de faux négatifs.
Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a neurological inflammatory disorder in which the diagnosis relies on MOG antibodies presence in patients with a compatible phenotype. Previous studies comparing live cell-based assay (LCBA) and fixed cell-based assay (FCBA) reported a lowerspecificity in FCBA and similar sensitivity. Aim of the study was to compare FCBA and LCBA and to characterize patients tested negative on LCBA but positive on FCBA in a French cohort. Overall, 233/759 samples were tested positive for MOG antibodies : 65% (151/233) were positive with both techniques, 5% (11/233) were positive only on LCBA, 30 (71/233) were positive only on FCBA. Clinical information was available for 40/71 patients. Of these 40 patients final diagnosis was MOGAD in 17/40 (42,5%). Our data show a good agreement between the two techniques and the use of FCBA helped the diagnosis of MOGAD in some patients. However, this technique lacks of specificity and a good screening strategy would be to use FCBA as a screening test, and to confirm, on LCBA, positive results as well as negative results when the clinical context is highly evocative of MOGAD.
Recent studies suggest that maintenance intravenous immunoglobulin (IVIG) may be an effective treatment to prevent relapses in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD); ...however, most of these studies had pediatric cohorts, and few studies have evaluated IVIG in adult patients.
To determine the association of maintenance IVIG with the prevention of disease relapse in a large adult cohort of patients with MOGAD.
This was a retrospective cohort study conducted from January 1, 2010, to October 31, 2021. Patients were recruited from 14 hospitals in 9 countries and were included in the analysis if they (1) had a history of 1 or more central nervous system demyelinating attacks consistent with MOGAD, (2) had MOG-IgG seropositivity tested by cell-based assay, and (3) were age 18 years or older when starting IVIG treatment. These patients were retrospectively evaluated for a history of maintenance IVIG treatment.

Of the 876 adult patients initially identified with MOGAD, 59 (median range age, 36 18-69 years; 33 women 56%) were treated with maintenance IVIG. IVIG was initiated as first-line immunotherapy in 15 patients (25%) and as second-line therapy in 37 patients (63%) owing to failure of prior immunotherapy and in 7 patients (12%) owing to intolerance to prior immunotherapy. The median (range) annualized relapse rate before IVIG treatment was 1.4 (0-6.1), compared with a median (range) annualized relapse rate while receiving IVIG of 0 (0-3) (t108 = 7.14; P < .001). Twenty patients (34%) had at least 1 relapse while receiving IVIG with a median (range) time to first relapse of 1 (0.03-4.8) years, and 17 patients (29%) were treated with concomitant maintenance immunotherapy. Only 5 of 29 patients (17%) who received 1 g/kg of IVIG every 4 weeks or more experienced disease relapse compared with 15 of 30 patients (50%) treated with lower or less frequent dosing (hazard ratio, 3.31; 95% CI, 1.19-9.09; P = .02). At final follow-up, 52 patients (88%) were still receiving maintenance IVIG with a median (range) duration of 1.7 (0.5-9.9) years of therapy. Seven of 59 patients (12%) discontinued IVIG therapy: 4 (57%) for inefficacy, 2 (29%) for adverse effects, and 1 (14%) for a trial not receiving therapy after a period of disease inactivity.
Results of this retrospective, multicenter, cohort study of adult patients with MOGAD suggest that maintenance IVIG was associated with a reduction in disease relapse. Less frequent and lower dosing of IVIG may be associated with treatment failure. Future prospective randomized clinical trials are warranted to confirm these findings.
Le diagnostic de maladie associée aux anticorps anti-MOG repose sur le dosage sérique des anticorps anti-MOG. Ces anticorps sont parfois retrouvés uniquement dans le LCR avec une valeur qui reste ...incertaine.
Décrire les caractéristiques cliniques, biologiques, radiologiques des patients présentant des anticorps anti-MOG dans le LCR, en particulier ceux pour lesquels les anticorps sont détectés exclusivement dans le LCR (CSF only).
Nous avons conduit une étude nationale rétrospective identifiant les patients présentant une positivité des anticorps anti-MOG dans le LCR par test CBA, au sein du laboratoire de référence français de Lyon. Nous avons identifié 24 patients CSF only et 50 patients doubles positifs (test positif dans le sérum et le LCR). Pour chaque patient, les données cliniques et biologiques de la maladie ont été recueillies et les IRM ont été analysées.
Les patients CSF-only étaient majoritairement des femmes. Leur présentation clinique initiale incluait ADEM (21 %), myélites (29 %), névrite optiques (33 %), atteintes du tronc cérébral (21 %), atteintes des nerfs crâniens (12 %). 60 % des patients présentaient des bandes oligoclonales. L’IRM retrouvait des atteintes leptoméningées (56 %), de l’area postrema (33 %), du chiasma optique (44 %), des nerfs crâniens (56 %). 54 % des patients ont rechuté, 37 % ont développé un phénotype NMO. Le pronostic initial semblait plus péjoratif.
Le profil des patients positifs pour l’anticorps anti-MOG dans le LCR est proche des patients MOGAD séropositifs, bien que certaines particularités clinico-radiologiques aient pu être identifiées, en particulier chez les patients CSF only.
De futures études incluant des groupes contrôles seront indispensables pour préciser la spécificité des anticorps anti-MOG dans le LCR.
Notre travail souligne l’intérêt de tester les anticorps anti-MOG dans le LCR chez les patients présentant un tableau évocateur de MOGAD et séronégatifs, ou présentant une atteinte démyélinisante atypique.
