Malgré l’évolution des modèles théoriques du handicap, tels que la Classification internationale du fonctionnement (CIF), l’évaluation des restrictions de participation reste actuellement peu ...développée. L’objectif de cette recherche était de développer un outil permettant de décrire les restrictions de participation rencontrées dans le handicap psychique et cognitif en tenant compte des facteurs environnementaux (FE), et d’en effectuer une étude exploratoire en vue de travaux de validation ultérieurs. La méthode a été développée à partir d’une analyse critique de la littérature internationale et du travail concerté de chercheurs, cliniciens et associations d’usagers. La grille de mesure de l’activité et de la participation (G-MAP) a été administrée à 16 sujets traumatisés crâniens et 15 sujets souffrant de troubles schizophréniques. Les limitations d’activités (LA), les restrictions de participation et les FE (soutien social SS, attitudes, systèmes et politiques) sont cotés de manière ordinale pour chacun des 26 items de la G-MAP remplie à l’issue d’un entretien semi-dirigé avec le sujet. Les analyses de cohérence interne suggèrent que les facteurs de LA et de restrictions de participation sont fiables. Le taux de réponse élevé et la variabilité des réponses soulignent la pertinence des FE dans l’évaluation des restrictions de la participation (RP).
Participation assessment remains incomplete when considering traumatic brain injury and schizophrenia, often lacking theoretical basis. By integrating environmental factors (EF), the ICF taxonomy of activity limitations (AL) and participation restriction (PR) provides an interesting theoretical framework. The present study was aimed at conducting an exploratory study of a new, ICF-derived, assessment tool of PR designed for psychic and cognitive disability and taking into account environment. The Grille de Mesure de l’Activité et de la Participation (G-MAP) was developed from a critical review of international literature. A group of experts (clinicians, searchers, users) selected the items. The G-MAP was then administered to 16 subjects with brain injury and 15 subjects with schizophrenia to explore items relevance, face validity and internal consistency. A semi-structured interview allows for rating AL, PR and EF on ordinal scales for each of the 26 items. Internal consistency analyses suggest that factors are reliable. Response rate and variability of responses raise encouraging arguments for the relevance of EF in participation assessment and the use of G-MAP in cognitive and psychic disabilities.
La cognition sociale, i.e. notre capacité à attribuer des états mentaux à autrui et à identifier ses émotions, est souvent perturbée dans certaines pathologies telles que le traumatisme crânien (TC). ...Cette capacité est traditionnellement évaluée à l’aide de tâches sous format « papier-crayon » n’impliquant pas le participant dans une situation d’interaction sociale. Pourtant, la cognition sociale est fondamentale dans nos interactions sociales car elle nous permet de comprendre le discours et le comportement d’autrui. Ainsi, dans ce travail de thèse, nous avons évalué les capacités d’attribution d’états mentaux et de reconnaissance émotionnelle de personnes ayant subi un TC à l’aide de tâches les impliquant activement dans une situation de communication (tâche de communication référentielle et tâche EViCog), ce qui n’a jamais été fait dans cette pathologie. La tâche EViCog (Evaluation de la cognition sociale en interaction virtuelle), créée pour cette étude, permet d’avoir des conversations audio-visuelles avec des humains virtuels, qui expriment des émotions et produisent du discours nécessitant d’inférer leurs états mentaux. Les résultats ont montré que les difficultés des personnes TC étaient encore plus importantes pour la tâche en situation d’interaction (tâche EViCog) par rapport à des tâches traditionnelles au format « papier-crayon ». Par ailleurs, en situation d’interaction, les performances de cognition sociale semblent dépendre en partie des capacités mnésiques (mémoire autobiographique et du contexte), ainsi que des fonctions exécutives, alors que pour les tâches traditionnelles, les performances ne seraient expliquées que par certaines capacités exécutives.
Social cognition, i.e. the ability to attribute mental states to others and to identify emotions, is often impaired in various pathologies, such as traumatic brain injury (TBI). This ability is traditionally assessed with “paper-and-pencil” tasks that do not involve the participant in a social interaction situation. However, social cognition is central in our daily social interactions, as it helps us understand others’ speech and behavior. Thus, in this study, we assessed mental state attribution and emotion recognition abilities of TBI participants, using tasks that involve the participant in a communication situation (referential communication task and EViCog task). The EViCog task (social cognition evaluation in virtual interaction), designed for this research, provides audio-visual conversations with virtual humans, which express emotions and produce speech requiring mental state inference. The results showed that the difficulties of the TBI participants were even more important for the task in interaction situation (EViCog task) compared to traditional tasks in paper-and-pencil format. Moreover, in interaction situation, social cognition performance seemed to rely on mnemonic abilities (autobiographical memory and context memory), and on executive functions, while for traditional tasks, performances were only explained by some executive abilities.
To assess the clinical outcomes associated with the inclusion of Integrated Psychological Treatment (IPT) standard medical therapy in nine regular clinical settings, nine clinical teams integrated ...the complete IPT program (six hierarchically arranged subprograms) with their respective standard medical therapies for outpatients with schizophrenia. A total of 90 patients, young adults to long-term mentally ill patients, participated in the program. Patients were evaluated using standardized instruments at four time points: (1) prior to including the IPT program, (2) after the first three IPT subprograms, (3) at the end of IPT, and (4) 3 to 4 months post-IPT. The IPT program was associated with positive results. Patients improved in terms of overall symptoms, subjective experiences, cognitive and social functioning, and quality of life. IPT is one of the most up-to-date programs to address the rehabilitation needs of persons suffering from schizophrenia. Our experience in nine clinical settings suggests that IPT can successfully be included as part of standard medical therapy in the rehabilitation of patients with schizophrenia.
