Gain in VO2 peak after cardiac rehabilitation (CR) following an acute coronary syndrome (ACS), is associated with reduced mortality and morbidity. We have previously shown in CR, that gain in VO2 ...peak is reduced in Type 2 diabetic patients and that response to CR is impaired by hyperglycemia.
We set up a prospective multicenter study (DARE) whose primary objective was to determine whether good glycemic control during CR may improve the gain in VO2 peak. Sixty four type 2 diabetic patients, referred to CR after a recent ACS, were randomized to insulin intensive therapy or a control group with continuation of the pre-CR antidiabetic treatment. The primary objective was to study the effect of glycemic control during CR on the improvement of peak VO2 by comparing first the 2 treatment groups (insulin intensive vs. control) and second, 2 pre-specified glycemic control groups according to the final fructosamine level (below and above the median).
At the end of the CR program, the gain in VO2 peak and the final fructosamine level (assessing glycemic level during CR) were not different between the 2 treatment groups. However, patients who had final fructosamine level below the median value, assessing good glycemic control during CR, showed significantly higher gain in VO2 peak (3.5 ± 2.4 vs. 1.7 ± 2.4 ml/kg/min,p = 0.014) and ventilatory threshold (2.7 ± 2.5 vs. 1.2 ± 1.9 ml/kg/min,p = 0.04) and a higher proportion of good CR-responders (relative gain in VO2 peak ≥ 16 %): 66 % vs. 36 %, p = 0.011. In multivariate analysis, gain in VO2 peak was associated with final fructosamine level (p = 0.010) but not with age, gender, duration of diabetes, type of ACS, insulin treatment or basal fructosamine.
The DARE study shows that, in type 2 diabetes, good glycemic control during CR is an independent factor associated with gain in VO2 peak. This emphasizes the need for good glycemic control in CR for type 2 diabetic patients.

Dans cette revue rétrospective, les circonstances diagnostiques, les modalités évolutives et le pronostic de 16 enfants atteints de néphropathie tubulo-interstitielle associée à une uvéite (TINU) ...provenant de 11 centres ont été analysés. Les critères d’inclusion étaient la présence d’une néphropathie tubulo-interstitielle sur des critères biologiques typiques et sur biopsie rénale réalisée pour 14 enfants et d’une uvéite. L’âge moyen d’apparition de l’uvéite était de 12,7ans, tous étaient symptomatiques avec une majorité de localisation antérieure. Dans la majorité des cas l’uvéite succédait à l’atteinte rénale, avec un délai moyen de 2,2 mois. Dans la plupart des cas l’évolution de l’uvéite était récidivante ou chronique, avec un bon pronostic visuel final en fin de suivi. L’âge moyen au moment du diagnostic de l’atteinte rénale était de 12,9ans. Au moment du diagnostic, la clairance de la créatinine moyenne (DFGe Schwartz 2009) était de 44,4mL/min/1m273. À plus de 12 mois de suivi, 5 patients sur 8 présentaient un DFGe inférieur à 90mL/min/1m273. Tous les patients ont été traités par corticothérapie orale d’une durée moyenne de 4 mois ; 6 patients ont reçu des bolus de solumédrol, et 3 patients un traitement immunosuppresseur. Dans le TINU syndrome, l’uvéite est symptomatique, majoritairement antérieure et bilatérale et le pronostic visuel final était favorable pour la majorité des patients. L’évolution rénale à plus de 12 mois de suivi n’était pas favorable pour 5 patients sur 8, avec un DFGe à moins de 90mL/min/1m273.

O-oxime substituent is explained. 23 primary amines are obtained, with high enantioselectivity (e.e.=98%), good yield (74%) on preparative scale. A mechanistic explanation is proposed.

Une comorbidité entre les troubles neurodéveloppementaux est repérée de façon très fréquente chez les enfants d’âge scolaire. Plusieurs modèles se fondent sur l’existence de déficits sous-jacents ...communs pour tenter d’expliquer l’apparition concomitante de ces troubles. Afin de contribuer à une meilleure compréhension de la comorbidité entre la dyslexie et le trouble de l’acquisition de la coordination (TAC), trois recherches ont été réalisées au cours de cette thèse. Elles sont toutes basées sur l’analyse de la graphomotricité, en termes de processus et de produit. L’objectif principal de ce travail était de repérer les processus à l’œuvre dans les tâches graphomotrices qui pourraient être affectés dans ces deux troubles, tout en tenant compte des particularités de chacun. Nous avons fait l’hypothèse, en référence au modèle du déficit d’apprentissage procédural (Nicolson et Fawett, 2007), que les enfants avec des difficultés en lecture et en motricité se différencieraient de ceux sans difficulté lors d’un apprentissage graphomoteur. Les résultats montrent que la sévérité et la nature des atteintes influencent les performances des enfants pour un ensemble varié de tâches graphomotrices et pour l’apprentissage graphomoteur. L’ensemble des résultats traduit un certain nombre de particularités communes chez les enfants avec une dyslexie et/ou un TAC, au niveau de l’intégration visuo-motrice, ainsi que de l’apprentissage et de l’automatisation de gestes graphomoteurs. En outre, les travaux de cette thèse mettent en relief la pertinence de l’approche dimensionnelle pour étudier la complexité des profils d’enfants avec des troubles comorbides .
Comorbidity between neurodevelopmental disorders is frequently found in children of school age. Several models are based on the existence of common underlying disorders to try and explain the concomitant appearance of these disorders. Three studies, all based on graphomotoricity analyses in terms of the processes and products, were conducted during this thesis so as to contribute to a better understanding of comorbidity between dyslexia and developmental coordination disorder (DCD). The main objective was to identify the processes involved during graphomotoricity tasks that could be affected in these two disorders, taking into account the peculiarities of each. We hypothesised, referring to the procedural learning disorder model (Nicolson & Fawcett, 2007) that children with reading and motor difficulties would differentiate themselves from those without difficulty for graphomotor learning. enabling us to assess their reading and motor levels. Results showed that the severity and nature of the disorders influenced the children’s performance for a diverse set of graphomotricity tasks and graphomotor learning. The overall results reflected a number of common features in children with dyslexia and / or DCD, concerning visuo-motor integration, as well as graphomotor gestures in learning and automation. In addition, the studies carried out during this thesis emphasised the relevance of the dimensional approach so as to study the complexity of the profiles of children with comorbid disorders.
