Sorting nexins (SNXs) comprise a family of proteins characterized by the presence of a phox‐homology domain, which mediates the association of these proteins with phosphoinositides and recruits them ...to specific membranes or vesicular structures within cells. Although only limited information about SNXs and their functions is available, they seem to be involved in membrane trafficking and sorting processes by directly binding to target proteins such as certain growth factor receptors. We show that SNX17 binds to the intracellular domain of some members of the low‐density lipoprotein receptor (LDLR) family such as LDLR, VLDLR, ApoER2 and LDLR‐related protein. SNX17 resides on distinct vesicular structures partially overlapping with endosomal compartments characterized by the presence of EEA1 and rab4. Using rhodamine‐labeled LDL, it was possible to demonstrate that during endocytosis, LDL passes through SNX17‐positive compartments. Functional studies on the LDLR pathway showed that SNX17 enhances the endocytosis rate of this receptor. Our results identify SNX17 as a novel adaptor protein for LDLR family members and define a novel mechanism for modulation of their endocytic activity.
Contexte : Tant l’enseignement du savoir-faire que celui du savoir-être, en particulier la communication médecin-patient peuvent être améliorés. Objectifs : Décrire et documenter l’intérêt des ...étudiants pour des dispositifs de formation à la communication et à la relation médecin-patient associant l’art dramatique et la simulation. Méthodes : Trente-trois étudiants en médecine (diplôme de formation approfondie en sciences médicales 1 et 2) ont participé à des ateliers de communication associant successivement une approche théâtrale comportant plusieurs exercices et une mise en situation professionnelle de communication en situation de consultation avec un patient simulé. Résultats : La grande majorité (92 %) des étudiants ayant participé à ces ateliers ont estimé que l’expérience leur a été bénéfique tant sur le plan du savoir-faire que du savoir-être et 94 % ont validé l’intérêt de ces ateliers pour leur pratique ultérieure. Les exercices théâtraux les plus plébiscités étaient l’histoire en cercle, l’histoire à plusieurs et l’histoire avec contrainte. Conclusion : L’association d’outils pédagogiques contextualisants, comme l’expérience théâtrale, le recours aux patients simulés en situation de problématiques cliniques fréquentes ou complexes, à des étudiants observateurs, le recours à la vidéo, et le débriefing entre les différents acteurs, recueille une très forte adhésion et détermine un grand bénéfice ressenti auprès des apprenants. Les résultats très positifs de cette expérience nous encouragent à étendre cette approche pédagogique intégrative à l’ensemble des étudiants du deuxième cycle des études médicales.
Purpose: Teaching doctor-patient communication needs improvement, in the medical curriculum, both in terms of skill and attitudes. Objectives: The objectives were to describe and evaluate the students’ interest in doctor-patient communication training, using teaching tools combining acting technics and simulation. Methods: Thirty-three medical students (fourth- and fifth-years) participated in these communication workshops starting with a theatrical approach and then a medical consultation simulation with standardized patients. Results: The majority of the students (92%) who participated at these workshops felt that the experience improved their skills and their know-how. Ninety-four percent validated the influence of these workshops on their future practice. Conclusion: This form of teaching using a theatrical approach and standardized patient, which embodies frequent or complex clinical problems, using video, student observers and debriefing between all the participants, gather a very strong adherence and a great benefit for the learners. The positive results of this experience encourage us to extend this teaching to all fourth- and fifth-year medical students.
Les angioedèmes (AE) bradykiniques sont soit associés à un déficit héréditaire ou acquis en C1Inhibiteur (C1Inh) soit associés à un C1Inh normal dans certaines formes héréditaires et en cas ...d’iatrogénie (AE secondaire aux inhibiteurs de l’enzyme de conversion de l’angiotensine). Quand il existe une forte suspicion clinique d’angioedème bradykinique, une exploration de C1Inh doit être réalisée. Cette exploration doit toujours comporter un dosage fonctionnel et pondéral de C1Inh ainsi qu’un dosage du C4. Un déficit pathologique en C1Inh peut être affirmé lorsque les taux sont inférieurs à 50 % des valeurs normales sur 2 prélèvements distincts. Si le déficit en C1Inh se confirme, des examens plus approfondis doivent être réalisés afin de déterminer le caractère héréditaire ou acquis du déficit. Le dosage du C1q et la recherche d’un anticorps anti-C1Inh peuvent alors être proposés. La recherche d’une mutation sur le gène SERPING1 s’impose, même en l’absence d’antécédents familiaux. L’absence de déficit en C1Inh n’exclut pas un angioedème héréditaire. Dans ce cas-là, après validation du tableau clinique par un expert du CREAK, la recherche de mutation sur les gènes F12 et PLG sera proposée.
Bradykinin mediated angioedema (BK-AE) can be associated either with C1Inhibitor deficiency (hereditary and acquired forms), either with normal C1Inh (hereditary form and drug induced AE as angiotensin converting enzyme inhibitors…). In case of high clinical suspicion of BK-AE, C1Inh exploration must be done at first: C1Inh function and antigenemy as well as C4 concentration. C1Inh deficiency is significant if the tests are below 50 % of the normal values and controlled a second time. In case of C1Inh deficiency, you have to identify hereditary from acquired forms. C1q and anti-C1Inh antibody tests are useful for acquired BK-AE. SERPING1 gene screening must be done if a hereditary angioedema is suspected, even if there is no family context (de novo mutation 15 %). If a hereditary BK-AE with normal C1Inh is suspected, F12 and PLG gene screening is suitable.
