Several investigators have described the effect of bolus injections on sensory block during combined spinal epidural anesthesia. This study investigates the effects of the immediate epidural infusion ...of 0.2% ropivacaine versus 0.9% saline on spinal anesthesia.
Forty-four patients undergoing partial hip replacement were randomly assigned to 2 groups, receiving epidural infusion of 0.2% ropivacaine 10 mL/h (group R, n = 22) or 0.9% NaCl 10 mL/h (group S, n = 22), immediately after spinal anesthesia with 7.5 mg 0.5% hyperbaric bupivacaine. Postoperative analgesia was provided in both groups using a ropivacaine patient-controlled epidural analgesia technique. Sensory block, motor block, postoperative pain scores, ropivacaine consumption, and patient satisfaction were recorded.
There was no difference between the 2 groups in the characteristics of the sensory block. The duration of motor block was prolonged in group R (312 +/- 95 minutes vs 198 +/- 78 minutes; P < .001). Postoperative pain scores and 24-hour ropivacaine consumption were similar among groups. Demand/delivery ratio was 1.6 in group R and 3.5 in group S (P = .048). A significantly higher number of patients in group R described their satisfaction as excellent (20 patients vs 6 patients; P < .001).
Epidural infusion of ropivacaine 0.2% initiated immediately after spinal anesthesia prolonged the regression of motor block but not the regression of sensory block when compared with saline infusion.
Amaç: Patpat tarım aracı Karadeniz bölgesinde ulaşım, sulama, taşıma, çapalama amacıyla kullanılmaktadır. Patpat aracının altında ezilme veya çapalama ekipmanlarının arasına sıkışma neticesinde ...ekstremiteler başta olmak üzere tüm vücutta yaralanmalar meydana gelmektedir. Bu hastalarda oluşabilecek damar yaralanmaları önemli bir morbidite, mortalite sebebidir. Bu çalışmada pat-pat tarım aracı kazası nedeniyle yaralanan ve Kalp Damar Cerrahisi Kliniğinde takip edilen hastaların retrospektif olarak değerlendirilmesi amaçlanmıştır.
Gereç ve Yöntem: Tanımlayıcı tipte planlanan çalışmaya retrospektif olarak 01.01.2008-31.12.2021 tarihleri arasında patpat kazasına bağlı yaralanma nedeniyle xxxxx Üniversitesi Hastanesi Kalp Damar Cerrahisi (KDC) Kliniğinde takip edilen hastalar dahil edildi. Hastane kayıtlarından elde edilen veriler tanımlayıcı istatistikler şeklinde ifade edildi.
Bulgular: Son 14 yılda 32 hastanın patpat kazası nedeniyle KDC kliniğinde opere edildiği belirlendi. Hastaların yaş ortalaması 36,7±21,2 yıldı ve %71,9’u erkekti. Kazaların %62,5’i tarlada meydana gelmişti ve %59,4’ü ezilme, %31,3’ü sıkışma tipindeydi. Hastalarda en sık alt ekstremite (%50,0) yaralanması ve bunlara eşlik eden arter-ven yaralanması (%25,0 popliteal, %18,8 femoral ve % 18,8 iliak) olduğu belirlendi. Hastaların hastanede kalış süreleri ortalama 22,2±14,2 gündü. Hastaların 4’üne (%12,5) ekstremite amputasyonu yapılırken, 25’i (%78,1) tam şifa ile taburcu oldu. Bu 29 hastanın ortalama iş gücü kaybı 78,5±25,1 (35-124) gündü. Hastaların 3’ü (%9,4) ise patpat kazası sonucu hayatını kaybetmişti.
Sonuç: Tarımda kullanılan pat pat aracına bağlı oluşan iş kazaları, başta ekstremiteler olmak üzere tüm vücutta multipl yaralanmalar meydana getirmektedir. Özellikle damar yaralanmasının eşlik ettiği yaralanmaların önemli bir morbidite ve mortalite sebebi olduğu ve çok sayıda iş günü kayıplarına yol açtığı belirlenmiştir.
Objective: In this study, it is aimed to constitute a chronic toxoplasmosis model using toxoplasma gondii (T. gondii) ME49 strain that is supposed to produce tissue cysts, to compare healthy and ...infected mice in terms of behavioral changes. In addition, the relationship between behavioral changes and brain lesions was questioned. Method: Before experimental application, out of a total of twenty-one 2-month- old Swiss albino mice, T. gondiantibody-free, 14 were infected by intraperitoneal (IP) (n=8) or oral (n=6) inoculation of 2x102 Toxoplasma gondii ME49 oocysts. Four months later, fear- and anxiety-related behavioral changes in infected and healthy control groups were comparatively evaluated with plus-maze test. At the end of the experiment (45 day after inoculation), euthanasia was carried out on all mice and their brains were examined histopathologically and immunohistochemically for the presence of T.gondii tissue cysts. Results: Infected mice had low levels of anxiety, they have enteredthe open arms more frequently and have spent more time in the open arms of the maze compared to controls. Pathologically, in the infected group, high rates of gliosis, perivascular cell infiltration, meningitis, and neuron necrosis were observed especially in the parietal and temporal lobes, cornu ammonis, amygdala, and thalamus compared to the other parts of the brain. A higher number of tissue cyst formations were positively correlated with the lesion severity in these parts of the brain. Conclusion: It can be suggested that, tissue cysts and neuropathological changes in chronically infected mice brains have direct concern with behavioral manipulation which results from low anxiety levels. Yontem: Uygulama oncesi serumlarinda anti-T. gondii antikorlarinin olmadigi gosterilen 2 aylik 21 adet erkek Swiss albino fareden deney grubundaki 14'une; intraperitoneal (IP) (n=8) ve oral (n=6) yollarla 2x102 Toxoplasma gondii ME49 ookisti verildikten 4 hafta sonra, anksiyete veya korku ile iliskili davranislari T. gondii ile enfekte fareler ile kontrol grubu saglikli farelerde karsilastirmali olarak degerlendirildi. Deney bitiminde (inokulasyon sonrasi 45'inci gun) farelere otenazi yapilarak, beyinleri histopatolojik olarak ve T. gondiidoku kisti antijeni yonunden immunoperoksidaz test ile incelendi. Bulgular: T. gondii ile enfekte farelerin daha dusuk duzeyde anksiyeteye sahip olduklarini, enfekte olmayan kontrol grubundaki farelere gore daha yuksek oran ve sureyle acik kolu tercih ettiklerini ortaya koydu. Patolojik olarak; enfekte grupta ozellikle parietal ve temporal loblar, kornu ammonis, amigdala, talamus ve cevresinde, beynin diger bolumlerine oranla daha yuksek derecede gliozis, perivaskuler hucre infiltrasyonu, meningitis ve noron nekrozlari gozlendi. Beyinde T. gondii doku kisti yerlesimi de anilan lezyon siddeti ile dogru orantili olarak diger alanlara oranla daha yuksek sayidaydi. Sonuc: Kronik toksoplazmozlu farelerin beyinlerinde sekillenen T.gondiidoku kistleri ve noropatolojik degisikliklerin dogrudan anksiyetenin azaltilmasina yonelik davranis manipulasyonu ile iliskili oldugu one surulebilir.
