Rapporter et analyser les différentes présentations cliniques, les diagnostics étiologiques, le pronostic et le traitement des patients atteints de sclérites.
Étude rétrospective, monocentrique à ...partir d’une série de 32 patients adressés pour le diagnostic étiologique d’une sclérite dans un centre tertiaire.
L’âge moyen au diagnostic de sclérite était de 46,8 ans (extrême : 22 à 77 ans). Il s’agissait de 19 femmes et 13 hommes. Vingt-six patients (81 %) avaient une atteinte antérieure (15 nodulaires, 8 diffuses et 3 nécrosantes), six (19 %) avaient une atteinte postérieure. La sclérite était unilatérale chez 24 patients (75 %). Le bilan révélait une maladie systémique chez 12 patients (37,5 %) : granulomatose avec polyangéite (n=3), maladie de Behçet probable (n=2), rhumatisme inflammatoire indéterminé (n=2), rhumatisme psoriasique (n=1), spondylarthrite ankylosante (n=1), sarcoïdose (n=1), syndrome de Cogan (n=1) et rectocolite hémorragique (n=1). Une origine herpétique était retenue chez 6 patients (18,8 %) devant un contexte clinique évocateur et l’efficacité du traitement d’épreuve par valaciclovir. Vingt-huit patients (87,5 %) ont reçu un traitement systémique associé à un traitement topique. La première ligne thérapeutique a comporté des anti-inflammatoires non stéroïdiens chez 15 patients (47 %) et des corticoïdes chez 8 patients (25 %). Six patients ont reçu un traitement immunosuppresseur. Après une durée moyenne de suivi de 16,3 mois, l’acuité visuelle finale était abaissée chez 6 patients (19 %).
La proportion de diagnostic dans notre série est proche des principales séries de la littérature. Un traitement par valaciclovir peut être efficace chez des patients suspects de sclérite d’origine herpétique.
To report on the various clinical presentations, etiological diagnosis, prognosis and treatment of patients with scleritis evaluated at a tertiary care eye center.
Retrospective, monocentric study on a series of 32 patients in a tertiary center.
The mean age of included patients with scleritis was 46.8 years (range, 22 to 77 years). Nineteen patients were women and 13 were men. Twenty-six patients (81%) had anterior scleritis (15 nodular, 8 diffuse and 3 necrotizing), six (19%) had posterior scleritis. Unilateral inflammation was present in 24 patients (75%). Twelve out of the 32 patients (37.5%) had an underlying systemic disease: granulomatosis with polyangiitis (n=3), Behçet's disease (n=2), unspecified inflammatory arthritis (n=2), psoriatic arthritis (n=1), ankylosing spondylitis (n=1), sarcoidosis (n=1), Cogan's syndrome (n=1) and ulcerative colitis (n=1). Six patients (18.8%) were suspected of having infectious disease with herpes virus: clinical context and positive treatment response with oral valacyclovir. Systemic agents and topical agents were required in 28 patients (87.5%). The first line therapy was mainly oral non-steroidal anti-inflammatory drugs in 15 patients (47%) and oral corticosteroids in 8 (25%). Immunosuppressive drugs were required in 6 patients. The mean follow-up was 16.3 months. Six patients (19%) had a decrease in visual acuity.
The number of systemic disease in our series is similar to the main series in the literature. Treatment with valaciclovir might be effective in patients with suspected herpes simplex scleritis.
To assess clinical features, treatment and outcome of Hypothalamo-pituitary (HP) sarcoidosis and to determine whether HP is associated with a particular clinical phenotype of sarcoidosis.

Retrospective chart review. Each patient was matched with two controls.
Twenty-four patients were identified (10 women, 14 men). Their median age at the sarcoidosis diagnosis was 31.5 years (range: 8-69 years). HP involvement occurred in the course of a previously known sarcoidosis in 11 cases (46%), whereas it preceded the diagnosis in 13 patients (54%). All but two patients had anterior pituitary dysfunction, 12 patients presented with diabetes insipidus. The most common hormonal features were gonadotropin deficiency (n=21), TSH deficiency (n=15) and hyperprolactinemia (n=12). Magnetic Resonance Imaging (MRI) revealed infundibulum involvement (n=8), pituitary stalk thickness (n=12) and involvement of the pituitary gland (n=14). All but two patients received prednisone. After a mean follow-up of 4 years, only two patients recovered from hormonal deficiencies. MRI abnormalities improved or disappeared in 12 cases under corticosteroid. There was no correlation between the hormonal dysfunctions and the radiologic outcomes. Patients with HP sarcoidosis had significantly more frequent sinonasal localizations and neurosarcoidosis and required a systemic treatment more frequently than controls.
Although HP sarcoidosis is unusual, physicians should be aware that such specific localization could be the first manifestation of sarcoidosis. HP involvement is associated with general severity of sarcoidosis. MRI abnormalities can improve or disappear under corticosteroid treatment, but most endocrine defects are irreversible.

Background
Studies on sarcoidosis in elderly patients are scarce and none have specifically evaluated patients aged ≥75 at onset.
Aim
We aimed to analyse the characteristics of patients with ...sarcoidosis diagnosed after 75 and to compare them with those of younger patients.
Design
Multicenter case–control study comparing elderly-onset sarcoidosis (EOS) with young-onset sarcoidosis (YOS) seen at Lyon University Hospitals between 2006 and 2018.
Methods
Using our institutional database, we included 34 patients in the EOS group and compared them with 102 controls from the YOS group in a 1:3 ratio. Demographic characteristics, medical history, clinical presentation, laboratory and imaging findings, sites of biopsies, histological analyses, treatments and outcomes were recorded using a comprehensive questionnaire.
Results
There were more Caucasians in the EOS group (94.1% vs. 59.8%; P < 0.001), who had significantly more comorbidities (mean, 3.1 ± 2 vs. 1.1 ± 1.6; P < 0.001). In the EOS group, there was less pulmonary involvement (26.5% vs. 49%; P = 0.022), less lymphadenopathy (2.9% vs. 16.7%; P = 0.041), no erythema nodosum (0% vs. 12.8%; P = 0.029) and no arthralgia (0% vs. 25.5%; P = 0.001). Conversely, uveitis was more common in the EOS group (55.9% vs. 20.6%; P < 0.001). Pathological confirmation was obtained significantly less frequently in the EOS group (67.7% vs. 85.3%; P = 0.023). Corticosteroid-related side effects were significantly more common in the EOS group (100% vs. 75.9%; P = 0.030).
Conclusion
Epidemiology and clinical presentation of EOS differs from YOS, including more comorbidities and more uveitis. Elderly patients are more prone to corticosteroid side effects.
To describe the main characteristics and the treatment of cryptococcosis in patients with sarcoidosis.
Multicenter study including all patients notified at the French National Reference Center for ...Invasive Mycoses and Antifungals.
Retrospective chart review. Each case was compared with two controls without opportunistic infections.
Eighteen cases of cryptococcosis complicating sarcoidosis were analyzed (13 men and 5 women). With 2749 cases of cryptococcosis registered in France during the inclusion period of this study, sarcoidosis accounted for 0.6% of all the cryptococcosis patients and for 2.9% of the cryptococcosis HIV-seronegative patients. Cryptococcosis and sarcoidosis were diagnosed concomitantly in four cases; while sarcoidosis was previously known in 14/18 patients, including 12 patients (67%) treated with steroids. The median rate of CD4 T cells was 145 per mm(3) (range: 55-1300) and not related to steroid treatment. Thirteen patients had cryptococcal meningitis (72%), three osteoarticular (17%) and four disseminated infections (22%). Sixteen patients (89%) presented a complete response to antifungal therapy. After a mean follow-up of 6 years, no death was attributable to cryptococcosis. Extra-thoracic sarcoidosis and steroids were independent risk factors of cryptococcosis in a logistic regression model adjusted with the sex of the patients.
Cryptococcosis is a significant opportunistic infection during extra-thoracic sarcoidosis, which occurs in one-third of the cases in patients without any treatment; it is not associated to severe CD4 lymphocytopenia and has a good prognosis.
To assess the clinical features, treatment and outcome of oral sarcoidosis and to determine whether oral involvement is associated with a particular clinical phenotype of sarcoidosis.

Retrospective chart review. Each patient was matched with four controls.
Twelve patients (9 women, 3 men) were identified. Their median age at sarcoidosis diagnosis was 38 years. Oral involvement was the first clinical evidence of sarcoidosis in seven cases and was a relapse symptom in five cases. Clinical presentations were nodules (n = 7) or ulcers (n = 5) and were mostly solitary. The tongue was the commonest site affected (n = 4), followed by lips (n = 3), oral mucosa (n = 2), palate (n = 2) and gingiva (n = 1). Patients with oral sarcoidosis were significantly younger and had more frequent lacrimal or salivary glands and upper airway tract clinical involvement than the controls; increased angiotensin-converting enzyme was less frequent in oral sarcoidosis. Multiple treatments of oral sarcoidosis were used: no treatment (n = 3), surgery (n = 2), corticosteroids (n = 7), hydroxychloroquine (n = 3), methotrexate (n = 2), doxycycline (n = 1). Methotrexate was efficient in one patient, hydroxychloroquine showed benefit in only 1 out of 3 patients. Three patients presented oral relapses. After a mean follow-up of 6 years, 10 patients experienced a complete (n = 7) or partial (n = 3) remission of oral sarcoidosis; stability was observed in the remaining two cases.
Although oral manifestations of sarcoidosis are unusual, physicians should be aware that this specific localization is frequently the first manifestation of the disease. Treatment modalities range from observation in asymptomatic patients to immunosuppressants for severe involvement.
Background: Spinal cord involvement in sarcoidosis is rare, occurring in <1% of patients with sarcoidosis.
Methods: We report seven cases of spinal cord sarcoidosis, seen in two French hospitals ...over a 13‐year period. Presentation of disease, methods of diagnosis and response to treatment, with quantification according to the reduction of the modified Rankin scale (MRS), were noted.
Results: Six patients presented insidious paresthesias or weakness and one a sudden paraplegia. Average MRS at diagnosis was to 2. Spine MRI showed one or several intramedullary lesions in all cases. Diagnosis was confirmed by extra‐neural tissue biopsies in all cases, including mediastinoscopy (two patients), coelioscopy (one patient), bronchoscopy (one patient), salivary gland biopsy (one patient) and skin biopsy (two patients). The average follow‐up for the group was 49.4 months. All patients responded to corticosteroid therapy with a median reduction of MRS of one point. Five patients received immunosuppressive therapy: cyclophosphamide (two patients), methotrexate (two patients), azathioprine (one patient), mycophenolate mofetyl (one patient), with an inconstant benefit. Patients who received cyclophosphamide presented severe fungaemia.
Conclusion: Based on our study and literature analysis, we propose an algorithm for treatment of spinal cord sarcoidosis.
Approximativement 20 % des lupiques présentent une thrombopénie immunologique (TI). La TI cliniquement significative (TICS) secondaire au lupus érythémateux systémique (LES) (TICS-LES) concernerait ...quant à elle 5 % des patients 1. La prise en charge de la TICS-LES ne fait pas consensus à l’ère des traitements récemment développés pour la TI primaire.
Étude rétrospective dans 15 services de médecine interne du Centre-Est et Sud, incluant des adultes lupiques (critères ACR et SLICC), présentant une TICS (TI<30 G/L ou avec hémorragies), pris en charge entre 2000 et 2020. Les thrombopénies d’autres causes étaient exclues. La sévérité des événements hémorragiques (EH) était évaluée par les scores OMS et de Khellaf et al. 2, l’activité du LES par le SLEDAI, le test de Farr et le complément. Les sujets présentant ≥1EH OMS>2 ou Khellaf>8 définissaient le groupe « EH majeur (EHM) ». La réponse (R)/non réponse (NR) à chacun des traitements de la TICS était définie selon le consensus international 3. Les traitements de fond (TF) comprenaient les immunosuppresseurs (IS), le rituximab (RTX), et la splénectomie. Les événements de morbi-mortalité étaient analysés. Les variables qualitatives étaient comparées selon le Test de Fisher, la non-significativité (NS) définie par p>0,05.
Quatre vingt dix patients (80 femmes) avec un suivi médian de 80 mois 6–446 étaient inclus. Le diagnostic de TI précédait celui de LES (28 %), était concomitant (47 %), ou lui succédait (25 %). Sept malades (8 %) présentaient un LES « hématologique pur ». Chez 34 des 52 (65,4 %) patients chez qui l’information était disponible, le LES était actif lors de la TICS. 54 malades (62 %) présentaient des anticorps antiphospholipides (APL), 16 (18 %) un syndrome des APL. Au total, 215 EH chez 79 malades étaient répertoriés ;45 révélaient la TI. Le groupe EHM comprenait 25 malades. Au cours du suivi, 11 patients (9 EHM) recevaient≥1 transfusion de concentrés érythrocytaires et 14 (7 EHM) des concentrés plaquettaires. Le groupe EHM motivait plus d’hospitalisations (256 sur les 485 répertoriées, p<0,05) et plus de TF différents (1,6 vs 0,68 en moyenne). Les patients recevaient 4,2 lignes de traitements en moyenne 1–11. 88 étaient traités par hydroxychloroquine (HCQ) et 87 par corticostéroïdes (CS). Chez 20 malades (dont 2/25 EHM), HCQ et CS suffisaient au contrôle de la TICS. 29 patients ont reçu au moins 1 IS, le plus souvent (19/29) l’azathioprine (AZA), 11/19 comme TF initial. Le taux de R à l’AZA était de 74 % et pour 8/19 (42 %) l’AZA était toujours en cours aux dernières nouvelles. 9/29 recevaient du mycophenolate mofetil (MMF) avec un taux de R de 89 %, le plus souvent (5/9) après d’autres TF RTX+≥1 IS (n=2) ; splénectomie+RTX+≥1 IS (n=3) avec une R obtenue chez les 5. 4 des 6 et 1 des 2 patients respectivement traités par méthotrexate et ciclosporine étaient NR. 34 des 35 patients ayant reçu du RTX présentaient une TICS à l’initiation. Le taux de R au RTX était de 76 %. Dans 6 cas le RTX était prescrit après splénectomie (R : 5/6), chez 16 avant tout autre TF (R : 11/16, 69 %). Il s’agissait du dernier TF chez 13/34 (47 %). Le RTX était plus souvent prescrit dans le groupe EHM (OR : 7,66 ; IC 95 % 2,5 ;26,0 ; p<0,001). 19 patients (21 %) ont été splénectomisés, 1 était réfractaire. Chez 8/19 (42 %), il s’agissait du dernier TF. Fourteen patients recevaient un des deux agonistes du récepteur de la thrombopoïétine (aRTPO), un les 2. On notait 5R/6 patients traités par romiplostim et 9 R/10 traités par eltrombopag. L’aRTPO était prescrit en ≥5e ligne de traitement dans 13 cas, en 3e ligne chez les 2 autres. Les aRTPO étaient significativement plus prescrits dans le groupe EHM (OR : 3,83 ; IC 95 %1,05 ; 14,46 ; p=0,025) et moins en présence d’APL (OR : 0,27 ; IC 95 %0,065 ; 0,9749 ; p=0,04). On ne notait pas plus de thromboses sous aRTPO. Dix-huit patients (20 %) ont présenté ≥1 thrombose (27 événements), 9/25 (36 %) dans le groupe EHM, 12 avec APL et 1 sous aRTPO (triple positivité APL). 18 patients ont présenté≥1 infection (36 événements) dont 7 du groupe EHM (NS). Parmi les 3 décès répertoriés (tous du groupe EHM), 1 était attribuable à la TICS-LES.
28 % des TICS-LES présentent ici ≥1 EHM et une morbidité plus élevée (plus d’hospitalisations, de TF, de transfusions). Parmi les TF, l’AZA souvent proposé en première intention, permet 74 % de R. Le MMF semble rentable comme IS de recours. La proportion de R au RTX parait meilleure après échec d’un IS ou de la splénectomie qui est à éviter avec seulement 42 % de R persistante et un surrisque thrombotique et de poussée lupique.
La maladie de Kawasaki de l’adulte (MKA) est une forme rare de vascularite dont la gravité est liée à l’atteinte coronaire. Nous décrivons dans cette étude les conséquences cardio-vasculaires à long ...terme de la MKA.
Les données de suivi cardio-vasculaires des patients de l’Observatoire Français de la MKA ont été collectées rétrospectivement.
26 patients ont été inclus avec un âge médian au diagnostic de 30 ans. Au diagnostic, 8 patients présentaient des symptômes cardio-vasculaires : douleur thoracique (5/8), insuffisance cardiaque droite (3/8) ou gauche (2/8) et 5 patients avaient une élévation de la troponine sanguine. Douze patients avaient une atteinte cardiovasculaire au diagnostic : 8 péricardites, 6 artérites coronaires sténosantes, 5 myocardites, 3 anévrysmes coronariens. Treize patients présentaient au moins un facteur de risque cardio-vasculaire, le plus fréquent étant le tabagisme (12/13). Dix-sept patients ont reçus des immunoglobulines intraveineuses au diagnostic et 19 un traitement anti-inflammatoire. La durée médiane de suivi était de 60 mois (10 à 192). Deux patients ont présenté un évènement cardiovasculaire relié à la MKA durant le suivi : deux infarctus du myocarde, survenus respectivement chez un patient non traité au diagnostic et un patient avec séquelles coronaires connues de sa MKA. Aucun décès n’est survenu. La dernière échocardiographie de suivi (disponible pour 23 patients) retrouvait une fraction d’éjection ventriculaire gauche normale. Un seul des 4 patients ayant bénéficié de test fonctionnel de dépistage de l’ischémie myocardique a présenté un résultat anormal. Deux patients ont bénéficié d’une coronarographie durant le suivi dont une seule était anormale. Neuf patients bénéficient encore d’un traitement par antiagrégant plaquettaire.
Cette étude apporte les premières données de suivi cardiovasculaires dans la MKA. Un diagnostic et un traitement précoces semblent importants pour prévenir les complications à long terme, de même qu’un dépistage et une prise en charge systématique des facteurs de risque cardio-vasculaires.
Peu d’études ont analysé les caractéristiques et traitements des thrombopénies immunologiques cliniquement significatives (TICS) secondaires au lupus érythémateux systémique (LES).
Étude ...observationnelle rétrospective multicentrique, basée sur un appel à observations au sein des centres de médecine interne des régions Centre-Est et Sud incluant des adultes avec LES (critères ACR 1997 et SLICC 2012), thrombopénie<30 G/L et manifestations hémorragiques pris en charge entre 2000 et 2018. Les thrombopénies non attribuables au LES étaient exclues. La gravité des événements hémorragiques (EH) était évaluée par les scores OMS et de Khellaf et al. 1, l’activité du LES par le SLEDAI. Un score OMS>2 ou Khellaf>8 définissait les EH majeurs (EHM). Les immunosuppresseurs conventionnels (IS), le rituximab (RTX), et les agonistes des récepteurs de la thrombopoïétine (aRTPO) étaient définis comme traitements spécifiques (TS) et la réponse (R) de la TICS selon le consensus 2. Les variables qualitatives étaient comparées selon le Test de Fisher, la non-significativité (NS) définie par p>0,05.
Soixante-quatorze patients ont été inclus dont 65 femmes (88 %). L’âge médian au diagnostic était de 28 ans et la durée de suivi médiane de 83 mois. Le diagnostic de TI précédait celui de LES dans 18 cas (24 %), était concomitant dans 35 cas (47 %), et postérieur dans 19 cas (26 %). Au diagnostic de LES 70 patients présentaient des symptômes cutanéo-articulaires, 11 des sérites, 9 une néphropathie lupique, 7 un neurolupus, 3 une anémie hémolytique auto-immune (AHAI), 1 érythroblastopénie ; 47 malades (64 %) présentaient des anticorps antiphospholipides (APL), 11 un syndrome des APL.
Au total, 179 EH étaient répertoriés, 37 révélaient la TI (50 % des patients). Dix-neuf patients (26 %) présentaient≥1 EHM. Ce « groupe EHM » présentait plus d’EH cumulés (5,4 vs 1,4/patient), un LES plus actif (SLEDAI moyen : 8 vs 5,7) et des sérites (32 % vs 9 % ; p=0,027), une moindre prévalence d’APL (42 % vs 70 % ; p=0,03), et un recours transfusionnel plus fréquent (p=0,02). Il cumulait aussi la moitié des transfusions (8/16) et la majorité des hospitalisations (288/388, 61 %) réalisées.
Tous recevaient une corticothérapie, 71 de l’hydroxychloroquine, et 34≥1 IS : azathioprine chez 21 (13 R) ; mycophénolate mofetil (MMF) chez 11 (11 R) ; méthotrexate chez 8 (4 R) ; cyclophosphamide chez 8 (7 R) ; ciclosporine chez 1 (non R). Le MMF permettait une R persistante : en première ligne (2 cas), après échec du RTX (2 cas), du RTX+≥1 IS (4 cas), et du RTX +≥1 IS+≥1 aRTPO (2 cas). Le RTX était administré dans 29 cas (21 R dont 8 justifiant ensuite≥1 TS). Douze patients (15 %) étaient traités par aRTPO (1 par les 2 successivement) : 4/5 R sous romiplostim ; 7/8 R sous eltrombopag. Le belimumab permettait 1 R persistante après échec des IS+RTX+splénectomie. Une splénectomie était réalisée chez 15 malades (R persistante : 7/15) et 42 recevaient des immunoglobulines polyvalentes. La proportion de R aux TS était similaire dans le groupe EHM.
Treize sujets présentaient des complications thrombotiques (9 avec APL ; 1 thrombose veineuse sous aRTPO) ; 14 présentaient 26 complications infectieuses (5 graves) ; 4 devaient être admis en réanimation. Parmi les 3 décès répertoriés 1 seul était directement attribuable à la TICS.
Malgré les nombreux biais inhérents à sa méthodologie, cette étude souligne la morbidité de la TICS secondaire au LES et en particulier du « groupe EHM » et identifie les traitements souvent rentables : MMF, RTX et aRTPO.
Uveitis is a common (20-50%) and early manifestation of sarcoidosis. Typical sarcoid uveitis presents with bilateral mutton fat keratic precipitates, iris nodules, and anterior and posterior ...synechia. Posterior involvement includes vitreitis, vasculitis and choroidal lesions. Long-term complications are common, and cystoid macular edema is the most important and sight-threatening consequence. Diagnostic work-up of sarcoidosis usually includes chest radiography or computed tomography scan, bronchoscopy with bronchoalveolar lavage, angiotensin converting enzyme, lysozyme, gallium scintigraphy and biopsy. The gold standard for the diagnosis of sarcoidosis should be obtained with histologic examination. However, an international workshop has recently established diagnostic criteria of "intraocular sarcoidosis" (sarcoidosis uveitis) on the basis of a combination of suggestive ophthalmological findings and laboratory tests, when biopsy is not performed or is negative. More recent techniques such as PET-scan and endoscopic ultrasound-guided fine-needle biopsy of intrathoracic nodes should be assessed in future prospective studies. Corticosteroids are the mainstay of treatment. Anterior and unilateral intermediate or posterior uveitis are usually treated with topical corticosteroids. Systemic corticosteroids are indicated in uveitis not responding to topical corticosteroids or in the presence of bilateral posterior involvement, especially with macular edema and occlusive vasculitis. In 5 to 20% of the patients who are corticosteroids resistant or require an unacceptable dose to maintain remission, additional immunosuppression is used, including methotrexate, leflunomide and mycophenolate mofetil. As in systemic sarcoidosis, infliximab has been recently suggested for refractory or sight threatening disease.
