Les biopsies spléniques radiologiques sont considérées comme des gestes à haut hémorragiques, conduisant parfois à réaliser de splénectomie d’hémostase en urgence. Sa réalisation n’est donc pas de ...pratique courante et nécessite une expertise radiologique. Nous présentons un cas original de découverte d’une mélioïdose disséminée découverte a l’occasion d’une biopsie splénique radioguidée.
Un patient de 62ans, diabétique de type 2, vivant entre la Thaïlande et la France est hospitalisé pour toux et douleurs basithoracique gauche depuis 9 mois. Il a perdu 15kg en 1 an. Il est apyrétique, mais a eu de la fièvre intermittente il y a quelques mois. Il a eu 2 accidents de scooter compliqués de fractures de côtes à gauche il y a 1 an et demi. Il a réalisé en Thaïlande un TDM abdominal retrouvant une lésion splénique de 7,6×6,2×6,8cm, un épanchement pleural gauche avec atélectasie du lobe inférieur gauche semblant communiquer avec la rate au travers du diaphragme. Il n’a pas de syndrome inflammatoire biologique. Les hémocultures sont stériles. Sérologie VIH, VHB, VHC négative. Sérologies parasitaires dont hydatidose et ecchinococose négatives. L’immunophénotypage lymphocytaire ne retrouve pas de clone suspect. L’ETO dédouane une endocardite. L’hypothèse de l’hématome splénique est écartée après relecture scannographique, du fait de la perforation diaphragmatique atypique et l’absence de signe de résorption. La lésion splénique est hétérogène et pseudo tissulaire. La rate est ascensionnée, collée au diaphragme rendant la splénectomie difficile du fait des risques d’adhérence. La lésion pulmonaire ne semble pas accessible par voie endobronchique et nécessite une thoracotomie. Le TEP TDM montre une hyperfixation intense d’une condensation postérobasale gauche, et de deux lésions intraspléniques confluentes. Nous réaliserons une biopsie splénique radiologique (8 carottes faites au pistolet à biopsie de 18G). Il n’y a pas de complication post geste immédiat ou à distance. La culture bactériologique et mycobactériologique sont négatives, et une PCR et culture Burkholderia reviennent positives. Le patient est traité par CEFTAZIDIME 2g×3/j puis par BACTRIM FORTE 3×/j pour une durée totale de 6 mois pour cette infection a Burkholderia pseudomallei splénique et pulmonaire. L’évolution clinique à 6 mois est satisfaisante avec une diminution de la taille des lésions.
La décision de réalisation de la PCR Burkholderia est motivée par la connaissance de l’épidémiologie infectieuse Thailandaise. La mélioidose est une infection a Burkholderia pseudomallei, (BGN aérobies, non sporulés, non fermentants) endémique d’Asie du Sud Est mais émergente dans les pays occidentaux (considérée comme éradiquée depuis 1945). L’infection est associée à une blessure, à l’ingestion ou à l’inhalation de bactéries en aérosol 1. En 2018, le taux de mortalité globale était de 30 à 35 % chez les patients admis dans les hôpitaux en Thaïlande 1. La prévalence était de 36,8 pour 100 000 patients hospitalisés 1. Ce BGN se culture sur les géloses MacConkey, au sang et au chocolat. Les tests PCR auraient une sensibilité de 94 %, une spécificité de 83 % (réaction croisée faussement positive avec plusieurs espèces de Burkholderia) 1. La CEFTAZIDIME et le MEROPENEM sont les antibiotiques utilisés initialement, puis le COTRIMOXAZOLE (TMP-SMX), la DOXYCYCLINE et l’AMOXYCILLINE-ACIDE CLAVULANIQUE pour le relai oral et la prophylaxie post-exposition 2. L’imagerie thoracique retrouve habituellement des lésions nodulaires multiples en cas de dissémination hématogéne ou des lésions abcédées excavées pseudo tuberculeuses. Les autres localisations sont les abcès hépatiques et spléniques. Le diabète est un facteur de risque de mélioïdose avec un risque relatif d’hospitalisation liée à l’infection de 2,2 (IC à 99 %, 2,10 à 2,23) 3. En Thaïlande, le taux de récidive est estimé à 6 % 1, conduisant à rallonger les traitements et les prophylaxies secondaires.
La mélioïdose est une pathologie émergente dans les pays occidentaux du fait des migrations de population. Notre CHU bénéficie de l’expertise de radiologues entraînés dans la réalisation de biopsie splénique. Cette technique a permis un diagnostic de pathologie inédite dans les pays occidentaux sans recours d’une alternative chirurgicale invasive.

L’objectif de l’étude était de comparer 2 cathéters tunnélisés d’hémodialyse en termes de dysfonctions mécaniques et infectieuses : le Canaud® (Medcomp), bicathéter en silicone et le Palindrome® ...(Covidien), cathéter monobloc en carbothane, symétrique, avec cuff.
Il s’agissait d’une étude pilote prospective observationnelle : tous les patients bénéficiant de la pose d’un cathéter tunnelisé d’hémodialyse au CHU de Toulouse entre septembre 2013 et septembre 2014 ont été inclus et suivis pendant au moins 6mois dans leur centre respectif. Le bon positionnement du cathéter a été contrôlé par un radiologue indépendant. Les dysfonctions mécaniques étaient définies par un débit sanguin<300mL/min, une pression veineuse>250mmHg ou une pression artérielle<–250mmHg lors d’au moins 2 séances d’hémodialyse et nécessitant une intervention (inversion des lignes, utilisation de fibrinolytiques). Le KT/V était relevé à M2, M6, M12, M18. Les complications infectieuses étaient définies par une bactériémie ou une infection du tunnel.
Cent sept cathéters posés chez 107 patients ont été retenus dans notre travail : 40 Canaud®, 67 Palindrome®. Dix patients (25 %) du groupe Canaud® et 40 patients (59,4 %) du groupe Palindrome® ont présenté une dysfonction mécanique (p=0,001) motivant la dépose de 4 Canaud® et 9 Palindrome®. Il n’y avait pas de différence significative en termes de KT/V médian aux temps étudiés, ni en termes de complication infectieuse : 7 patients du groupe Canaud® (17,5 %) et 8 du groupe Palindrome® (12,5 %) ont présenté une complication infectieuse (p=0,480). Ces dernières imposaient la dépose de 4 Canaud® et 4 Palindrome®.
Le cathéter de Canaud® est considéré comme la référence en termes de cathéter de dialyse, permettant un débit sanguin satisfaisant. Le Palindrome®, en carbothane (thermoformable), aux extrémités symétriques laissait espérer de bons résultats. Cependant, les Palindrome® ont été pourvoyeurs de plus de dysfonctions mécaniques. Malgré l’absence de cuff, les Canaud® n’ont pas été pourvoyeurs de plus de complications infectieuses.
Alors qu’il n’y avait pas de différence en termes de complication infectieuse, le Palindrome® était plus à risque de dysfonction mécanique sur la durée du suivi dans notre étude.
Abdominal Aortic Aneurysm (AAA) is a common pathology which treatment decision is mostly made upon measurement of maximal diameter, despite known limitations. Thanks to the collaboration between ...Medical Physicist and Radiologist, this study aimed to identify new biomarkers based on computational fluid dynamics (CFD) analysis to predict AAAs rapid growth risk.
In our multi-centric and prospective study, we included 78 patients with an AAA from September 2012 to June 2014. Patients underwent two CT examinations separated by a one-year interval to assess aneurysms growth. Fifty patients underwent CFD analysis thanks to the Medical Physicist expertise. Based on a 10 ml threshold of total volume growth, we classified patients into slow and rapid growth groups. Aneurysms initial morphological and functional parameters were analyzed including: maximal diameter and surface, thrombus and lumen volumes, maximal wall pressure and wall shear stress (WSS).
There was a significant difference between the two groups regarding aneurysm lumen volume (P = 0.0051) and mean WSS variation (P = 0.0240) unlike maximal diameter (P = 0.71). We found significant correlation of aneurysm volume growth with lumen volume and reduction of the mean WSS variation value (respectively R = 0.47, P = 0.0015 and R = −0.42, P = 0.0062) and total aneurysm volume growth. Combining these parameters, we computed an AAAs growth-predicting model, which featured better area under ROC than the only measurement of maximal diameter (0.78 vs 0.52, P = 0.0031). Depending on the threshold, our model yields either excellent sensitivity (95.00% IC95% 75.1, 99.9) or specificity (90.00% IC95% 73.5, 97.9).
Tight collaboration between Medical Physicist and Radiologists allowed to find a promising biomarker based on CFD which provides better information than maximal diameter to assess rapid AAAs volume growth risk.
Shared decision making is recommended as a strategy to help patients identify what matters most to them and make informed decisions about musculoskeletal care. In part 5 of the Overcoming Overuse ...series, we look at the evidence supporting shared decision making as a strategy to help curb overuse. Using shared decision making in clinical consultations may help to reduce the overuse of options that are not beneficial and to increase use of care supported by evidence. Shared decision making could support clinicians in promoting uptake of active rehabilitation options with a favorable balance of benefits to harms. Shared decision making facilitates conversations about unnecessary tests or treatments and could be a key strategy for overcoming overuse.

AimTo develop and user test an evidence-based patient decision aid for children and adolescents who are considering anterior cruciate ligament (ACL) reconstruction.DesignMixed-methods study ...describing the development of a patient decision aid.SettingA draft decision aid was developed by a multidisciplinary steering group (including various types of health professionals and researchers, and consumers) informed by the best available evidence and existing patient decision aids.ParticipantsPeople who ruptured their ACL when they were under 18 years old (ie, adolescents), their parents, and health professionals who manage these patients. Participants were recruited through social media and the network outreach of the steering group.Primary and secondary outcomesSemistructured interviews and questionnaires were used to gather feedback on the decision aid. The feedback was used to refine the decision aid and assess acceptability. An iterative cycle of interviews, refining the aid according to feedback and further interviews, was used. Interviews were analysed using reflexive thematic analysis.ResultsWe conducted 32 interviews; 16 health professionals (12 physiotherapists, 4 orthopaedic surgeons) and 16 people who ruptured their ACL when they were under 18 years old (7 were adolescents and 9 were adults at the time of the interview). Parents participated in 8 interviews. Most health professionals, patients and parents rated the aid’s acceptability as good-to-excellent. Health professionals and patients agreed on most aspects of the decision aid, but some health professionals had differing views on non-surgical management, risk of harms, treatment protocols and evidence on benefits and harms.ConclusionOur patient decision aid is an acceptable tool to help children and adolescents choose an appropriate management option following ACL rupture with their parents and health professionals. A clinical trial evaluating the potential benefit of this tool for children and adolescents considering ACL reconstruction is warranted.
Background
Diagnostic labels may influence treatment intentions. We examined the effect of labelling low back pain (LBP) on beliefs about imaging, surgery, second opinion, seriousness, recovery, ...work, and physical activities.
Methods
Six‐arm online randomized experiment with blinded participants with and without LBP. Participants received one of six labels: ‘disc bulge’, ‘degeneration’, ‘arthritis’, ‘lumbar sprain’, ‘non‐specific LBP’, ‘episode of back pain’. The primary outcome was the belief about the need for imaging.
Results
A total of 1375 participants (mean SD age, 41.7 years 18.4 years; 748 women 54.4%) were included. The need for imaging was rated lower with the labels ‘episode of back pain’ (4.2 2.9), ‘lumbar sprain’ (4.2 2.9) and ‘non‐specific LBP’ (4.4 3.0) compared to the labels ‘arthritis’ (6.0 2.9), ‘degeneration’ (5.7 3.2) and ‘disc bulge’ (5.7 3.1). The same labels led to higher recovery expectations and lower ratings of need for a second opinion, surgery and perceived seriousness compared to ‘disc bulge’, ‘degeneration’ and ‘arthritis’. Differences were larger amongst participants with current LBP who had a history of seeking care. No differences were found in beliefs about physical activity and work between the six labels.
Conclusions
‘Episode of back pain’, ‘lumbar sprain’ and ‘non‐specific LBP’ reduced need for imaging, surgery and second opinion compared to ‘arthritis’, ‘degeneration’ and ‘disc bulge’ amongst public and patients with LBP as well as reducing the perceived seriousness of LBP and enhancing recovery expectations. The impact of labels appears most relevant amongst those at risk of poor outcomes (participants with current LBP who had a history of seeking care).
In this series on "Overcoming Overuse," we explore the issue of health care overuse and how it may be identified in musculoskeletal physical therapy. In part 2, we frame health care overuse as a ...continuum from overuse to appropriate care, and consider how to measure overuse. We describe how overuse can be defined within a framework of care that is ineffective, inefficient, and misaligned, depending on the perspective of the person delivering or receiving care-the clinician, society, or patient. To ensure that musculoskeletal health care is of high value and sustainable, we encourage physical therapists to reflect on their practice.
