Reliable non-invasive biomarkers for the surveillance of patients at risk of hepatocellular carcinoma (HCC) development represent an unmet medical need. Recently, the liver-cancer-specific isoform of ...serine protease inhibitor Kazal (LC-SPIK) has been proposed as a valuable biomarker for the detection of HCC in patients with chronic liver disease of viral etiology. In the present study, we assessed the diagnostic accuracy of LC-SPIK, alone or in combination with standard serologic biomarkers (i.e., alpha-fetoprotein and protein induced by vitamin K absence or antagonist-II, PIVKA-II), for the detection of HCC among patients with dysmetabolic liver disease. A total of 120 patients with non-alcoholic fatty liver disease (NAFLD), including 62 patients with a diagnosis of HCC and 58 with cirrhosis but without tumor, were retrospectively analyzed. The serum levels of LC-SPIK were measured by enzyme-linked immunosorbent assay (ImCare Biotech, Doylestown, PA). The serum LC-SPIK values were significantly different between patients with HCC (24.3, 17.6−39.8 ng/mL) and those with cirrhosis but without tumor (11.7, 8.7−18.2 ng/mL) (p < 0.001). By receiver operating characteristic curve analysis, we observed an area under the curve (AUC) of 0.841 for the detection of HCC; the combination with PIVKA-II further increased the accuracy to AUC = 0.926 (cross-validation). The promising results observed in the present pilot study foster additional research to investigate the usefulness of LC-SPIK for the stratification of the risk of HCC development in patients with NAFLD and advanced liver disease.
LC-SPIK is a liver cancer-specific isoform of Serine Protease Inhibitor Kazal and has been proposed as a new biomarker for the detection of HCC given its unique 3D structure, which differs from ...normal pancreatic SPIK. An ELISA technology based on its unique structure was developed to use LC-SPIK as an effective biomarker for the clinical diagnosis of HCC. AFP, the most widely used biomarker for HCC surveillance currently, suffers from poor clinical performance, especially in the detection of early-stage HCC. In one case-control study, which included 164 HCC patients and 324 controls, LC-SPIK had an AUC of 0.87 compared to only 0.70 for AFP in distinguishing HCC from liver disease controls (cirrhosis, HBV/HCV). LC-SPIK also performed significantly better than AFP for the 81 patients with early-stage HCC (BCLC stage 0 and A), with an AUC of 0.85 compared to only 0.61 for AFP. Cirrhosis is the major risk factor for HCC; about 80% of patients with newly diagnosed HCC have preexisting cirrhosis. LC-SPIK's clinical performance was also studied in HCC patients with viral and non-viral cirrhosis, including cirrhosis caused by metabolic dysfunction-associated steatotic liver disease (MASLD) and alcoholic liver disease (ALD). In a total of 163 viral cirrhosis patients with 93 HCC patients (50 early-stage), LC-SPIK had an AUC of 0.85, while AFP had an AUC of 0.70. For patients with early-stage HCC, LC-SPIK had a similar AUC of 0.83, while AFP had an AUC of only 0.60. For 120 patients with nonviral cirrhosis, including 62 HCC (23 early-stage) patients, LC-SPIK had an AUC of 0.84, while AFP had an AUC of only 0.72. For the 23 patients with early-stage HCC, LC-SPIK had a similar AUC of 0.83, while the AUC for AFP decreased to 0.65. All these results suggest that LC-SPIK exhibits significantly better performance in the detection of HCC than AFP in all etiologies of liver diseases. In addition, LC-SPIK accurately detected the presence of HCC in 71-91% of HCC patients with false-negative AFP test results in viral-associated HCC and non-viral-associated HCC.
“Wind-up”, a condition related to chronic pain, is a form of plasticity in spinal dorsal horn that can be observed during electrical stimulation of pain receptors at low frequencies (0.3–3 Hz). In ...this paper, we present a computational model to explain several aspects of wind-up. The core of this model is the interplay of spike-time-dependent plasticity (STDP), short-term synaptic plasticity (STP), and different propagation velocities of the three afferent fibers (C, Aδ, and Aβ). We utilize Izhikevich’s simple spiking neuron to model a dorsal horn neuron (DHN) of the spinal cord. To achieve the expected results, the model parameters need to adapt to the frequency response which is motivated by biological results. The adaptation is performed by a genetic algorithm (GA), and the resulting optimized values interestingly lie in biological ranges. Based on the proposed model, we suggest that STP may be the origin of the band-pass behavior of wind-up between 0.3 and 3 Hz; while the STDP-based long-term plasticity can be responsible for the synaptic potentiation leading to wind-up, or similar phenomena such as central sensitization. Understanding the mechanisms underlying wind-up generation might allow clarification of the molecular mechanisms of pain signaling and development of strategies, such as transcutaneous electrical nerve stimulation (TENS), for pain treatment.
Sexual reproduction in plants requires elongation of the pollen tube through the transmitting tissues toward the ovary. Tube growth rate is a major determinant of pollen competitive ability. We ...report that a K super(+) channel of the Shaker family in Arabidopsis, SPIK, plays an important role in pollen tube development. SPIK was found to be specifically expressed in pollen. When SPIK was heterologously expressed in COS cells, its product formed hyperpolarization-activated K super(+) channels. Disruption (T-DNA insertion) of the SPIK coding sequence strongly affected inwardly rectifying K super(+)-channel activity in the pollen-grain plasma membrane. Measurements of membrane potential in growing pollen tubes yielded data compatible with a contribution of SPIK to K super(+) influx. In vitro pollen germination assays were performed, revealing that the disruption results in impaired pollen tube growth. Analysis of the transmission rate of the disrupted allele in the progeny of heterozygous plants revealed a decrease in pollen competitive ability, the probability of fertilization by mutant pollen being similar to 1.6 times lower than that by wild-type pollen. The whole set of data supports the hypothesis that functional expression of SPIK plays a role in K super(+) uptake in the growing pollen tube, and thereby in tube development and pollen competitive ability.

Många av arbetarna på Vänersborgs Tändsticksfabrik bodde på Sjöberget, strax öster om centrala stan, nära fabriken. Här på Sjöberget, med en spridd bebyggelse av små stugor bor man trångt. Rummet visar hur det kan ha sett ut i en av stugorna, kallad Johannisberg.
Året är 1900 och här lever makarna August och Kristina Jacobsson med sin tioåriga dotter Hedvig. Ett par år tidigare har också Kristinas vuxne son Nils, som blivit änkeman, flyttat hit med sin femårige son Oscar. I ett rum på 16 kvadratmeter bor alltså fem personer. Kök delas med en annan familj som hyr den lilla stugans andra rum.
Då man endast har tillgång till ett rum får det fylla många funktioner. Vid bordet har Kristina sin arbetsplats som hemarbeterska för Tändsticksfabriken. Här sitter hon och limmar tändsticksaskar som hängs upp i taket för torkning. Utrymmet delar hon med dottern Hedvig som på kvällarna sitter här för att göra sina läxor. I hörnet står August fiskeredskap. Gäddorna han får i Vänern ger en viktig extrainkomst. På den lilla kaminen, som värmer rummet, kan man också laga mat om köket är upptaget av grannen. Golvet är lekplats för familjens minste, Oscar, på natten rullas madrasserna ut och då blir det sovplats istället. Kristina och August delar på kökssoffan. Sommartid brukar Nils och Oscar sova i en liten kammare men på vintern är det för kallt. Det är inte särskilt varmt i det stora rummet heller, det tillhör morgonrutinen att knacka hål på isen i vattenhinken.
