Models and photographs were taken before operation, surgical obturator was fabricated for the surgical area provided and the patient was fed without the need to use the nasogastric probe. Hollow bulb obturator was applied to the patient within the 3 months following the operation. The lack of anatomical structures to provide adequate support and retention for prothesis in patients with total resection of maxillary is one of the biggest problems encountered during obturator prothesis fabricating. ÖZETBu çalışmada, total üst çene rezeksiyonu yapılan bir hastaya uygulanan protetik tedaviler hakkında bilgi verilmiştir.Yassı hücreli karsinom (Squamous cell carcinoma) tanısıyla Kulak Burun Boğaz uzmanı tarafından kliniğimize gönderilen hastadan operasyon öncesi modeller ve fotoğraflar alınmış, ameliyat alanı değerlendirilerek cerrahi obturatör hazırlanmış, nasogastrik sonda kullanma ihtiyacı olmadan hastanın beslenmesi sağlanmıştır. Operasyondan 3 ay sonra hastaya hollow bulb obturatörü yapılmıştır.Total üst çene rezeksiyonlu hastalarda protez için yeterli destek ve retansiyonu sağlayacak anatomik yapıların olmaması, obturatör protez yaparken karşılaşılan en büyük sorunlardan biridir. Giriş: Çene-yüz protezlerinin yenilenmesi gerekliliğini doğuran başlıca sebeplerden biri, protezde kullanılan silikon elastomerlerin fiziksel ve mekanik özelliklerinin zayıflamasıdır. Bu özellikler ...içinde sertlik, protezin çevre doku ile uyumlu hareketini etkilediğinden ve çevre dokularda irritasyona neden olabildiğinden dolayı önemlidir. Amaç: Bu çalışmanın amacı, çene-yüz protezinde kullanılan silikon elastomere eklenen renklendirici ajanın ve miktarının silikonun sertliği üzerine etkisini araştırmak. Gereç ve Yöntemler: Bu çalışmada ilave reaksiyonlu oda sıcaklığında vulkanize olan, Shore A 20 sertliğine sahip Derma-sil silikon kullanılmıştır. Okre renk likit boya ağırlıkça %5, %10 ve %15 oranında silikon karışıma ilave edilerek 3 grup oluşturulmuştur. Dördüncü gruba boya eklenmeyerek kontrol grubu oluşturulmuştur. Amerikan Test ve Materyaller Topluluğu D2240 standardına uygun olarak sertlik ölçüm cihazı ile örneklerin Shore A sertlik değerleri ölçülüp 4 grup arasındaki farklılık analiz edilmiştir. Bulgular: %5, %10 ve %15 oranlarında boya eklenmiş 3 grup ile kontrol grubu arasında sertlik açısından anlamlı fark bulunmuştur (p=0,021). Sonuç: Silikon elastomere boya ilavesi, malzemenin sertliğini arttırmıştır. Bu çalışmada tek tip sertliğe sahip silikon ve tek tip boya kullanıldığından farklı türde ve renkte boyaların farklı silikon türlerinin sertliği üzerine etkisi araştırılmalıdır.
Introduction: One of the main reasons for the need to replace the maxillofacial prosthesis is the reduction of the physical and mechanical properties of the silicone elastomers used in the prosthesis. Among these properties, hardness is important because it affects the movement of the prosthesis in harmony with the surrounding tissue and can cause irritation in the surrounding tissues. Aim: The aim of this study is to investigate the effect of coloring agent and its amount added to silicone used in maxillofacial prosthesis on the hardness of silicone. Materials and Methods: In the study, Derma-sil silicone that vulcanizes at high temperature and has Shore A hardness of 20 was used. Ochre liquid dye was added to the silicone mixture in the ratio of 5%, 10% and 15% by weight forming 3 groups. The dye was not added to the fourth group and a control group was formed. Shore A hardness measurement was performed using a hardness tester device according to American Society for Testing of Materials D2240 standards and the difference between 4 groups was analyzed. Results: There was a significant difference in hardness between the 3 groups with 5%, 10% and 15% dye added and the control group. Conclusion: The addition of dye to the silicone elastomer increased the hardness of the material. Since one hardness choice of silicone and one type of dye is used in this study, the effect of different types and colors of dyes on the hardness of different silicone types should be investigated.
An earthflow near Cenes de la Vega (southern Spain) was first observed after a heavy rain in January 2010 in an area where a spring had existed. Initially, the landslide affected only the lower part ...of the slope. Displaced materials accumulated over terraces and the river course, damming it. A year later, a new rainy period reactivated (or increased the activity of) the instability. It progressed in two ways: by sliding and involving progressively higher parts of the slope with the head scarp approaching the top of the slope, and by widening in the central part of the body. Near the top of the slope, there is a pipeline that supplies water to the city of Granada. The most probable triggering factor of the landslide seems to be a combination of persistent water leakage from the pipe, which has saturated the materials, and intense rainfall.
Glutamate is the most widely distributed and a major excitatory neurotransmitter in the CNS. It has been found to play a critical role in various physiological functions in which increased glutamate ...or its subsequent stimulation is thought to have a role in pathophysiological mechanism of various CNS diseases like epilepsy, stroke, depression and pain. Early attempts to develop glutamatergic antagonists failed in clinical studies due to nonselective or competitive antagonism and have a lot of safety issues like loss of cognitive functions, psychomimetic effect and sedation. Neuropathic pain can be described as pain associated with damage or permanent alteration of the peripheral or central nervous system. At present, there are very few effective therapies for neuropathic pain. The current approach includes targeting specific or alternate binding sites of glutamate receptors, resulting in reduced CNS liabilities. Targeting the glutamatergic system shows a better efficacy and fewer side effects, compared with classical drugs for the treatment of neuropathic pain. This review discusses the various targets on glutamatergic system, which includes the receptors, transporters and enzymes, for the treatment of neuropathic pain and their advantages over classical glutamatergic antagonists. The review also highlights the newer drugs in clinical trials for neuropathic pain.
We present an improved method for determining the mass of neutron stars in eclipsing X-ray pulsar binaries and apply the method to six systems, namely, Vela X-1, 4U 1538-52, SMC X-1, LMC X-4, Cen ...X-3, and Her X-1. In previous studies to determine neutron star mass, the X-ray eclipse duration has been approximated analytically by assuming that the companion star is spherical with an effective Roche lobe radius. We use a numerical code based on Roche geometry with various optimizers to analyze the published data for these systems, which we supplement with new spectroscopic and photometric data for 4U 1538-52. This allows us to model the eclipse duration more accurately and thus calculate an improved value for the neutron star mass. The derived neutron star mass also depends on the assumed Roche lobe filling factor Delta *b of the companion star, where Delta *b = 1 indicates a completely filled Roche lobe. In previous work a range of Delta *b between 0.9 and 1.0 was usually adopted. We use optical ellipsoidal light-curve data to constrain Delta *b. We find neutron star masses of 1.77 ? 0.08 M for Vela X-,0.87 ? 0.07 M for 4U 1538-52 (eccentric orbit), 1.00 ? 0.10 M for 4U 1538-52 (circular orbit), 1.04 ? 0.09 M for SMC X-1, 1.29 ? 0.05 M for LMC X-4, 1.49 ? 0.08 M for Cen X-3, and 1.07 ? 0.36 M for Her X-1. We discuss the limits of the approximations that were used to derive the earlier mass determinations, and we comment on the implications our new masses have for observationally refining the upper and lower bounds of the neutron star mass distribution.
Amaç: Van-Karagündüz Höyüğü birinci yapı katında yer alan ve Ortaçağ Döneme tarihlendirilen mezarlık alanından çıkarılan diş ve çenelere sahip 188 erişkin bireye ait iskelet kalıntıları; dişlerde ve ...çenelerde iz bırakan patolojik lezyonları incelemek amacıyla araştırılmıştır. Gereç ve Yöntem: Çalışma materyalimizi 81 kadın (% 18,62) ve 107 erkek (% 24,60) olmak üzere toplam 188 erişkin bireye ait 2734 daimi diş ve 283 çene oluşturmaktadır. Bulgular: Erişkinlerin daimi dişlerinde, diş aşınması % 95,54, diş çürüğü % 6,36, hypoplasia % 24,91, diştaşı % 25,90, apse % 1,86, periyodontal hastalıklar içinde değerlendirilen alveol kaybı % 40,64 ve ölüm öncesi (premortem) diş kaybı % 18,73 oranında görülmektedir. Sonuç: Genel olarak tarım ve hayvancılığa dayalı bir yaşam biçimine sahip olduğu düşünülen Karagündüz Ortaçağ toplumunun beslenme rejimini, genellikle iyi öğütülmüş, fakat biraz lifli ve sert besinler oluşturmaktadır.
Midyat Aktaş Mevkii iskeletleri, 2013 yılında Mardin ili Midyat ilçesi Aktaş Mevkii Mor Hobil-Mor Abrohom manastırının kuzeyinde yer alan ve Roma Dönemine tarihlendirilen dört adet oda mezardan ele ...geçirilmiştir. Çalışmanın amacı, Midyat, Aktaş Mevkii Bizans Dönemi bireylerinin diş, üst ve alt çenelerine ait patolojileri incelenerek, bireylerin yaşam biçimi, ağız sağlığı, beslenme alışkanlıkları ve demografik yapısını tahmin etmektir. Çalışmada, Midyat, Aktaş Mevkii oda mezarlardaki 319 adet üst ve alt çenelerinden elde edilen veriler kullanılmıştır. Sonuç olarak, toplumda 319 adet birey olduğu (% 9,09’u bebek, % 20,06’sı çocuk, % 5,64’ü kadın, % 14,42’si erkek, % 50,78’i cinsiyeti bilinmeyen) saptanmıştır. Aktaş Mevkii bireylerinin diş ve çene patolojileri, % 6,19’u çürük, % 9,29’u apse, % 42,95’i ölüm öncesi diş kayıplarından oluşmaktadır. Toplumun beslenme diyetinin hayvansal proteine yönelik olduğu söylenebilir.
