We report a case of a patient who had a bilateral silicone breast implant (SBI) placed on the anterior surface of the pectoralis major muscle and presented with extensive swelling of the left breast ...and induration. Invasive ductal breast carcinoma was diagnosed based on core needle biopsy results. Imaging revealed direct tumor invasion into the capsule, massive fluid accumulation around the SBI, morphological changes suggestive of compression impaction and breakage of the outermost shell of the SBI, and nodules suggestive of capsular dissemination. A total mastectomy involving both the SBI and capsule was performed. Pathological examination of the resected specimen confirmed direct tumor invasion and dissemination into the capsule. This showed that the SBI was not damaged, and that a large amount of bloody exudate had accumulated between the SBI and capsule. In cases such as this where capsular dissemination as a nodule is suspected on prior imaging, or where direct tumor invasion into the capsule and fluid accumulation between the SBI and capsule are observed, it is necessary to perform a total mastectomy in addition to complete resection of the capsule surrounding the implant without allowing the fluid within the capsule to leak, keeping in mind the capsular dissemination.
2D inorganic bimolecular crystals, consisting of two different inorganic molecules, are expected to possess novel physical and chemical properties due to the synergistic effect of the individual ...components. However, 2D inorganic bimolecular crystals remain unexploited because of the difficulties in preparation arising from non‐typical layered structures and intricate intermolecular interactions. Here, the synthesis of 2D inorganic bimolecular crystal SbI3·3S8 nanobelts via a facile vertical microspacing sublimation strategy is reported. The as‐synthesized SbI3·3S8 nanobelts exhibit strong in‐plane anisotropy of phonon vibrations and intramolecular vibrations as well as show anisotropic light absorption with a high dichroism ratio of 3.9. Furthermore, it is revealed that the second harmonic generation intensity of SbI3·3S8 nanobelts is highly dependent on the excitation wavelength and crystallographic orientation. This work can inspire the growth of more 2D inorganic bimolecular crystals and excite potential applications for bimolecular optoelectronic devices.

To ensure optimal utilization and bioavailability, iron uptake, transport, subcellular localization, and assimilation are tightly regulated in plants. In this work, we examine recent advances in our ...understanding of cellular responses to Fe deficiency. We then use intracellular mechanisms of Fe homeostasis to discuss how formalizing cell biology knowledge via a mathematical model can advance discovery even when quantitative data is limited. Using simulation-based inference to identify plausible systems mechanisms that conform to known emergent phenotypes can yield novel, testable hypotheses to guide targeted experiments. However, this approach relies on the accurate encoding of domain-expert knowledge in exploratory mathematical models. We argue that this would be facilitated by fostering more “systems thinking” life scientists and that diversifying your research team may be a practical path to achieve that goal.
This paper studies the performance of nineteen private commercial banks and two government-owned banks in Bangladesh during the period 2005–2008 using a slacks-based inefficiency measure and the ...directional technology distance function. Performance is measured assuming a black-box production structure and then the black-box is opened and examined using a two stage network production structure. Current period performance in maximizing desirable loans and securities investments and minimizing bad loans depends on how efficiently inputs at one stage of production are transformed into intermediate outputs which are used at a subsequent stage of production. In addition, current period production of the intermediate output is constrained by the amount of non-performing loans that were generated in an earlier period.
Silent brain infarcts (SBIs) are frequently identified after transcatheter aortic valve implantation (TAVI), when patients are screened with diffusion-weighted magnetic resonance imaging (DW-MRI). ...Outside the cardiac literature, SBIs have been correlated with progressive cognitive dysfunction; however, their prognostic utility after TAVI remains uncertain. This study's main goals were to explore (i) the incidence of and potential risk factors for SBI after TAVI; and (ii) the effect of SBI on early post-procedural cognitive dysfunction (PCD).
A systematic literature review was performed to identify all publications reporting SBI incidence, as detected by DW-MRI after TAVI. Silent brain infarct incidence, baseline characteristics, and the incidence of early PCD were evaluated via meta-analysis and meta-regression models. We identified 39 relevant studies encapsulating 2408 patients. Out of 2171 patients who underwent post-procedural DW-MRI, 1601 were found to have at least one new SBI (pooled effect size 0.76, 95% CI: 0.72-0.81). The incidence of reported stroke with focal neurological deficits was 3%. Meta-regression noted that diabetes, chronic renal disease, 3-Tesla MRI, and pre-dilation were associated with increased SBI risk. The prevalence of early PCD increased during follow-up, from 16% at 10.0 ± 6.3 days to 26% at 6.1 ± 1.7 months and meta-regression suggested an association between the mean number of new SBI and incidence of PCD. The use of cerebral embolic protection devices (CEPDs) appeared to decrease the volume of SBI, but not their overall incidence.
Silent brain infarcts are common after TAVI; and diabetes, kidney disease, and pre-dilation increase overall SBI risk. While higher numbers of new SBIs appear to adversely affect early neurocognitive outcomes, long-term follow-up studies remain necessary as TAVI expands to low-risk patient populations. The use of CEPD did not result in a significant decrease in the occurrence of SBI.
Fluid migration in geological materials, a subject of great interest in various geophysical applications, has been interpreted through multiple numerical methods. Taking advantage of both a ...volume‐based method and a boundary integral method, we innovate a hybrid spectral‐boundary‐integral and finite‐difference method (SBI‐FDM) to describe the fluid injection and propagation in the fault structure. The coupling of the two methods is established from the consistent exchange of the pressure gradient from the volume‐based method and the boundary flux applied in the SBI boundary. The hybrid method benefits from its capability to truncate the domain and capture the fluid‐induced pore pressure in geological systems without modeling the whole bulk. After establishing the numerical framework, we verify the SBI‐FDM under both homogeneous and heterogeneous mediums using an analytical solution and the FDM results, respectively. After model verification, a sensitivity test showcases the stability of the numerical scheme. A speedup comparison of the SBI‐FDM against the FDM is presented. We observe that the proposed method can achieve better computational efficiency with up to one thousand times speedup in our test. Utilizing the proposed hybrid method, we also perform parametric studies to reveal the significance of mobility contrast between the damage zone and host rock, as well as the role of fault width during fault injection.

In this work, all-electron ab initio calculations on the SbI molecule have been carried out with the multi-reference configuration interaction plus the Davidson correction (MRCI+Q) method. In the calculations, total 34 Λ-S states have been obtained for the first time. From calculated PECs, the derived spectroscopic constants well agree with available experimental values. It should be mentioned that A3Π is typically repulsive, which much differs from the cases of lighter analogs SbX(X=F, Cl, and Br). The molecule is expected to possess a strong spin-orbit coupling effect because both Sb and I atoms have a large atomic number (Z). Therefore, this effect is included by using the state interaction method. As a result, there are 74 Ω states generated from Λ-S states. For X3Σ−, the determined energy splitting of 980 cm−1 is in good agreement with the measured value of 965 cm−1. The adiabatic excitation energy Te of b1Σ+ is greatly improved after considering the SOC effect. Transition properties of SbI are evaluated. The obtained results demonstrate that SOC effects have a great impact on the transitions a1Δ-X3Σ− and b1Σ+-X3Σ−. Both transitions borrow transition dipole moments (TDMs) from spin-allowed Λ-S transitions. The radiative lifetimes of a1Δ and b1Σ+ are determined to be 2.28 ms and 96.84 μs, respectively.
Introducción/Objetivos: existen múltiples instrumentos para evaluar el Síndrome de Quemarse por el Trabajo (burnout) en profesiones relacionadas con el servicio a otras personas. Sin embargo, hay ...limitaciones metodológicas para estudiar este fenómeno en poblaciones de trabajadores generales. El presente estudio tiene como objetivo ofrecer evidencia de la validez y fiabilidad del “Cuestionario para la Evaluación de Quemarse por el Trabajo”, en su versión Desencanto Profesional (CESQT-DP), la cual puede ser aplicada a trabajadores de cualquier ocupación, empleando para ello una muestra colombiana. Método: se efectuó un estudio psicométrico con una muestra multiocupacional de 2991 trabajadores. Se realizaron análisis factoriales confirmatorios, análisis de invarianza por sexo, validez convergente y discriminante y de consistencia interna. Resultados: los resultados muestran la estructura factorial, replicando las cuatro dimensiones hipotetizadas; se evidencia invarianza del instrumento según el sexo, y muestras de la validez convergente, discriminante y fiabilidad. Discusión: el CESQT-DP cuenta con altas calidades psicométricas y puede emplearse para evaluar el burnout en empleados en cualquier ocupación en Colombia.
Autoimmune/inflammatory syndrome induced by adjuvants (ASIA) consists of a wide spectrum of symptoms and immunological features that are believed to develop in predisposed individuals after exposure ...to an adjuvant, including a silicone breast implant (SBI). Different autoimmune diseases (AIDs) have been associated with ASIA, but ASIA development after SBI in women with Hashimoto thyroiditis (HT) and familial autoimmunity has rarely been described.
A 37-year-old woman presented in 2019 with arthralgia, sicca symptoms, fatigue, + antinuclear antibody (ANA), + anti SSA, and + anticardiolipin Immunoglobulin G (IgG) antibodies. She was diagnosed with HT and vitamin D deficiency in 2012. The familial autoimmunity was present: the patient's mother had been diagnosed with systemic lupus erythematosus and secondary Sjogren's syndrome and her grandmother with cutaneous lupus and pernicious anemia. In 2017, the patient had a cosmetic SBI procedure that was complicated by repeated right breast capsulitis. After 2 years of irregular visits due to COVID-19, she presented with + ANA, + anticentromere antibodies both in sera and seroma, sicca syndrome, arthralgias, twinkling in extremities, abnormal capillaroscopic findings, and reduced diffusing capacity of the lungs for carbon monoxide. She was diagnosed with ASIA, and antimalarial and corticosteroid therapy were introduced.
In patients with HT and familial autoimmunity, SBI should be carefully considered due to the possibility of ASIA development. Hashimoto thyroiditis, familial autoimmunity, and ASIA seem to be interconnected in the complex mosaic of autoimmunity in predisposed individuals.
