A quality-control YouTube-based study using the recognized quality scoring systems.

The Internet is a widely and increasingly used source for obtaining medical information both by patients and clinicians. YouTube, in particular, manifests itself as a leading source with its ease of access to information and visual advantage for Internet users.
The first 50 videos returned by the YouTube search engine in response to "kyphosis" keyword query were included in the study and categorized under seven and six groups, based on their source and content. The popularity of the videos was evaluated with a new index called the video power index (VPI). The quality, educational quality, and accuracy of the source of information were measured using the Journal of American Medical Association (JAMA) score, Global Quality Score (GQS), and Kyphosis-Specific Score (KSS).
Videos had a mean duration of 397 seconds and a mean number of 131,644 views, with a total viewing number of 6,582,221. The source (uploader) in 36% of the videos was a trainer and the content in 46% of the videos was exercise training. Seventy-two percent of the videos were about postural kyphosis. Videos had a mean JAMA score of 1.36 (range: 1-4), GQS of 1.68 (range: 1-5), and KSS of 3.02 (range: 0-32). The academic group had the highest scores and the lowest VPIs.
Online information on kyphosis is low quality and its contents are of unknown source and accuracy. In order to keep the balance in sharing the right information with the patient, clinicians should possess knowledge about the online information related to their field and should contribute to the development of optimal medical videos.

Abstract
Objective
Hospital engagement in electronic health information exchange (HIE) has increased over recent years. We aimed to 1) determine the change in adoption of 3 types of information ...exchange: secure messaging, provider portals, and use of an HIE; and 2) to assess if growth in each approach corresponded to increased ability to access and integrate patient information from outside providers.
Methods
Panel analysis of all nonfederal, acute care hospitals in the United States using hospital- and year-fixed effects. The sample consisted of 1917 hospitals that responded to the American Hospital Association Information Technology Supplement every year from 2014 to 2016.
Results
Adoption of each approach increased by 9–15 percentage points over the study period. The average number of HIE approaches used by each hospital increased from 1.0 to 1.4. Adoption of each approach was associated with increased likelihood that providers routinely had necessary outside information of 4.2–12.7 percentage points and 4.5–13.3 percentage points increase in information integration. Secure messaging was associated with the largest increase in both. Adoption of 1 approach increased the likelihood of having outside information by 10.3 percentage points, while adopting a second approach further increased the likelihood by 9.5 percentage points. Trends in number of approaches and integration were similar.
Discussion/Conclusion
No single HIE tool provided high levels of usable, integrated health information. Instead, hospitals benefited from adopting multiple tools. Policy initiatives that reduce the complexity of enabling high value HIE could result in broader adoption of HIE and use of information to inform care.
Nowadays, a number of mechanisms and tools are being used by health care organizations and physicians to electronically exchange the personal health information of patients. The main objectives of ...different methods of health information exchange (HIE) are to reduce health care costs, minimize medical errors, and improve the coordination of interorganizational information exchange across health care entities. The main challenges associated with the common HIE systems are privacy concerns, security risks, low visibility of system transparency, and lack of patient control. Blockchain technology is likely to disrupt the current information exchange models utilized in the health care industry.
Little is known about patients' perceptions and attitudes toward the implementation of blockchain-enabled HIE networks, and it is still not clear if patients (as one of the main HIE stakeholders) are likely to opt in to the applications of this technology in HIE initiatives. Thus, this study aimed at exploring the core value of blockchain technology in the health care industry from health care consumers' views.
To recognize the potential applications of blockchain technology in health care practices, we designed 16 information exchange scenarios for controlled Web-based experiments. Overall, 2013 respondents participated in 16 Web-based experiments. Each experiment described an information exchange condition characterized by 4 exchange mechanisms (ie, direct, lookup, patient-centered, and blockchain), 2 types of health information (ie, sensitive vs nonsensitive), and 2 types of privacy policy (weak vs strong).
The findings show that there are significant differences in patients' perceptions of various exchange mechanisms with regard to patient privacy concern, trust in competency and integrity, opt-in intention, and willingness to share information. Interestingly, participants hold a favorable attitude toward the implementation of blockchain-based exchange mechanisms for privacy protection, coordination, and information exchange purposes. This study proposed the potentials and limitations of a blockchain-based attempt in the HIE context.
The results of this research should be of interest to both academics and practitioners. The findings propose potential limitations of a blockchain-based HIE that should be addressed by health care organizations to exchange personal health information in a secure and private manner. This study can contribute to the research in the blockchain area and enrich the literature on the use of blockchain in HIE efforts. Practitioners can also identify how to leverage the benefit of blockchain to promote HIE initiatives nationwide.
The HL7® fast healthcare interoperability resources (FHIR®) specification has emerged as the leading interoperability standard for the exchange of healthcare data. We conducted a scoping review to ...identify trends and gaps in the use of FHIR for clinical research.
We reviewed published literature, federally funded project databases, application websites, and other sources to discover FHIR-based papers, projects, and tools (collectively, "FHIR projects") available to support clinical research activities.
Our search identified 203 different FHIR projects applicable to clinical research. Most were associated with preparations to conduct research, such as data mapping to and from FHIR formats (n = 66, 32.5%) and managing ontologies with FHIR (n = 30, 14.8%), or post-study data activities, such as sharing data using repositories or registries (n = 24, 11.8%), general research data sharing (n = 23, 11.3%), and management of genomic data (n = 21, 10.3%). With the exception of phenotyping (n = 19, 9.4%), fewer FHIR-based projects focused on needs within the clinical research process itself.
Funding and usage of FHIR-enabled solutions for research are expanding, but most projects appear focused on establishing data pipelines and linking clinical systems such as electronic health records, patient-facing data systems, and registries, possibly due to the relative newness of FHIR and the incentives for FHIR integration in health information systems. Fewer FHIR projects were associated with research-only activities.
The FHIR standard is becoming an essential component of the clinical research enterprise. To develop FHIR's full potential for clinical research, funding and operational stakeholders should address gaps in FHIR-based research tools and methods.

Information needs motivate human information behavior. Knowledge of information needs is critical for user-centered information behavior research and system design. In consumer health information behavior research, there is a lack of understanding of how consumer health information needs (CHIN) is measured in empirical studies. This study is a systematic review of empirical quantitative studies on CHIN, with a focus on how CHIN is defined and operationalized. A search of six academic databases and citation-track of relevant articles identified a total of 216 relevant articles. These articles were analyzed using the qualitative content analysis method. We found that few included articles explicitly defined either CHIN or information needs in general. When definitions were given, they were from a cognitive perspective and largely ignored the multidimensionality of the concept. Consistent with this cognitive-centered conceptualization, CHIN was operationalized primarily as information topics, with some articles also measuring several additional attributes, including level of importance, fulfilment, amount of information needed, and frequency of needs. These findings suggest that CHIN is undertheorized. To address this gap, future studies should attend to social and emotional dimensions of CHIN, such as motivations, goals, activities, and emotions. Further, more research is needed to understand how CHIN is related to consumer health information seeking behavior and to the social and environmental context in which the needs arise.
Objective
The current study aims to explore the trend of Internet health information seeking (IHIS) in cancer survivors and the relationship between four dimensions of digital divide (eg, mental ...access, material access, skills access, and usage access) and IHIS.
Methods
Data from three iterations (2011, 2013, and 2017) of Health Information National Trends Survey were analyzed. Only cancer survivors (2011: n=563; 2013: n=459; 2017: n=504) were included. ANOVA and multivariate regressions were performed.
Results
The rate of IHIS among cancer survivors has increased from 53.5% in 2011 to 69.2% in 2017. Also, across the three survey years, material access (eg, physical Internet access), usage access (eg, eHealth activities), and mental access (eg, trust in online health information) have remained in a significant and positive relationship with IHIS. Skills access (eg, health information seeking skills) and the other mental access factor, self‐efficacy in health information seeking, were not significantly associated with IHIS over time.
Conclusions
This research revealed a rising trend of IHIS adoption in cancer survivors, and demonstrated significant relationships between digital divide and IHIS. We call for targeted interventions to reduce the digital divide barriers for cancer survivors.
While the healthcare industry is undergoing disruptive digital transformation, data breaches involving health information are not usually the result of integration of new technologies. Based on ...published industry reports, fundamental security safeguards are still considered to be lacking with many documented data breaches occurring as the result of device and equipment theft, human error, hacking, ransomware attacks and misuse. Health information is considered to be one of the most attractive targets for cybercriminals due to its inherent sensitivity, but digital investigations of incidents involving health information are often constrained by the lack of the necessary infrastructure forensic readiness. Following the analysis of healthcare data breach causes and threats, we describe the associated digital forensic readiness challenges in the context of the most significant incident causes. With specific focus on privilege misuse, we present a conceptual architecture for forensic audit logging to assist with capture of the relevant digital artefacts in support of possible future digital investigations.
eHealth literacy is defined as the ability of people to use emerging information and communications technologies to improve or enable health and health care.
The goal of this study was to explore ...whether literacy disparities are diminished or enhanced in the search for health information on the Internet. The study focused on (1) traditional digital divide variables, such as sociodemographic characteristics, digital access, and digital literacy, (2) information search processes, and (3) the outcomes of Internet use for health information purposes.
We used a countrywide representative random-digital-dial telephone household survey of the Israeli adult population (18 years and older, N = 4286). We measured eHealth literacy; Internet access; digital literacy; sociodemographic factors; perceived health; presence of chronic diseases; as well as health information sources, content, search strategies, and evaluation criteria used by consumers.
Respondents who were highly eHealth literate tended to be younger and more educated than their less eHealth-literate counterparts. They were also more active consumers of all types of information on the Internet, used more search strategies, and scrutinized information more carefully than did the less eHealth-literate respondents. Finally, respondents who were highly eHealth literate gained more positive outcomes from the information search in terms of cognitive, instrumental (self-management of health care needs, health behaviors, and better use of health insurance), and interpersonal (interacting with their physician) gains.
The present study documented differences between respondents high and low in eHealth literacy in terms of background attributes, information consumption, and outcomes of the information search. The association of eHealth literacy with background attributes indicates that the Internet reinforces existing social differences. The more comprehensive and sophisticated use of the Internet and the subsequent increased gains among the high eHealth literate create new inequalities in the domain of digital health information. There is a need to educate at-risk and needy groups (eg, chronically ill) and to design technology in a mode befitting more consumers.

It is critical to understand ...the factors that shape consumers' evaluation of web-based health information quality and the role that it plays in their appraisal and use of health information and information systems.
This paper aimed to identify the antecedents and consequences of consumers' evaluation of web-based health information quality as a means to consolidate the related research stream and to inform future studies on web-based health information quality.
We systematically searched 10 databases, examined reference lists, and conducted manual searches. Empirical studies that investigated consumers' evaluation of web-based health information quality, credibility, or trust and their respective relationships with antecedents or consequences were included.
We included 147 studies reported in 136 papers in the analysis. Among the antecedents of web-based health information quality, system navigability (ρ=0.56), aesthetics (ρ=0.49), and ease of understanding (ρ=0.49) had the strongest relationships with web-based health information quality. The strongest consequences of web-based health information quality were consumers' intentions to use health information systems (ρ=0.58) and satisfaction with health information (ρ=0.46). Web-based health information quality relationships were moderated by numerous cultural dimensions, research designs, and publication moderators.
Consumers largely rely on peripheral cues and less on cues that require more information processing (eg, content comprehensiveness) to determine web-based health information quality. Surprisingly, the relationships between individual differences and web-based health information quality are trivial. Web-based health information quality has stronger effects on cognitive appraisals and behavioral intentions than on behavior. Despite efforts to include various moderators, a substantial amount of variance is still unexplained, indicating a need to study additional moderators. This meta-analysis provides broad and consistent evidence for web-based health information quality relationships that have been fractured and incongruent in empirical studies.
Context: The United States has invested billions of dollars to encourage the adoption of and implement the information technologies necessary for health information exchange (HIE), enabling providers ...to efficiently and effectively share patient information with other providers. Health care providers now have multiple options for obtaining and sharing patient information. Community HIEs facilitate information sharing for a broad group of providers within a region. Enterprise HIEs are operated by health systems and share information among affiliated hospitals and providers. We sought to identify why hospitals and health systems choose either to participate in community HIEs or to establish enterprise HIEs. Methods: We conducted semistructured interviews with 40 policymakers, community and enterprise HIE leaders, and health care executives from 19 different organizations. Our qualitative analysis used a general inductive and comparative approach to identify factors influencing participation in, and the success of, each approach to HIE. Findings: Enterprise HIEs support health systems' strategic goals through the control of an information technology network consisting of desired trading partners. Community HIEs support obtaining patient information from the broadest set of providers, but with more dispersed benefits to all participants, the community, and patients. Although not an either/or decision, community and enterprise HIEs compete for finite organizational resources like time, skilled staff, and money. Both approaches face challenges due to vendor costs and lessthan-interoperable technology. Conclusions: Both community and enterprise HIEs support aggregating clinical data and following patients across settings. Although they can be complementary, community and enterprise HIEs nonetheless compete for providers' attention and organizational resources. Health policymakers might try to encourage the type of widespread information exchange pursued by community HIEs, but the business case for enterprise HIEs clearly is stronger. The sustainability of a community HIE, potentially a public good, may necessitate ongoing public funding and supportive regulation.
