The repetitive high-energy shocks themselves ... together with interruption of precordial compression compromise the post resuscitation myocardial function and survival. Therefore, our intent was to evolve an objective measurement that would have the potential for guiding more optimal timing of electrical defibrillation which, in turn, would decrease the number of electrical shocks and the total energy delivered to the heart. Consequently, post resuscitation myocardial dysfunction would be minimized. Wedeveloped such non-invasive electrocardiographic prognosticator, amplitude spectrum area (AMSA), representing the area under the curve of the amplitude frequency spectrum obtained from ventricular fibrillation waveforms by fast Fourier analyses. In addition, this prognosticator should be insensitive to artifacts resulting from precordial compression, therefore obviating the need for interruption of precordial compression for rhythm analysis. An establishedporcine model of cardiac arrest due to ventricular fibrillation wasutilized. Five-second electrocardiographic filtered signal which preceded the electrical shock was recorded during uninterrupted precordial compression and analysed for each measurement. Retrospectively, in a derivation group of 55 animals we derived AMSA utilizing an ECG anatyses technique and identified a threshold va(ue of AMSA at which there was uniform predictability of successful resuscitation.Type of material - dissertation ; adult, seriousPublication and manufacture - Ljubljana : [A.
