    Therapeutic strategy of stroke includes prevention, treatment of acute stroke and rehabilitation after stroke. Pirmary stroke prevention is based on the avoidance and monitoring of the nowasays ... well-defined risk factors. In the secondary phase of stroke prevention, antiplatelet drugs still play the primary role. Carotid endarterectomy showed to be superior to medical management in patients with 70% or greater stenosis. The most realiable principle in acute stroke treatment is restoration of brain perfusion in orderto prevent ischemic damage.. Thrombolytic therapy seems to be the most important way of restoration of the brain perfusion since arterial occlusion is the cause of the majority of ischemic strokes. Study of ancrod (a defirbrogenating agent - a snake venom) is under investigation, showing good perspective. In certain cases of acute stroke, anticoagulants (e.g.. warfarineand heparine) may also halt the progression of thrombotic damage. Antiaggregation agents studies show that, besides anticogulation treatment, aspirin should be started after the stroke as soon as possible. Early treatment prevents about 10 deaths or recurrent strokes in each 1000 patients in the first four weeks. Various neuroprotective measures have not shown to beeffective in acute stroke or are still under investigation. General measuresare still an important part of acute stroke treatment.
