  • Določitev meril za oceno nezadostnosti materničnega vratu s transvaginalnim ultrazvočnim pregledom med nosečnostjo = Determination of criteria for assessment of uterine cervix incompetence by transvaginal ultrasonography during pregnancy
    Lužnik, Marijan
Background. With regard to statistic data of percentage of cervical cerclage and of premature births in different Slovenian obstetrics' departments our intention was to lower the percentage of ... cerclage at the Department of Gynecology and Obstetrics of General Hospital Slovenj Gradec for 75 percent. We wished to achieve this objective by establishing sonographyc criteria for assessment of cervical incompetence in second trimester of pregnancy. Methods. On the basis of repeated in-patient gynecological examinations incompetent cervix was diagnosed and ultrasound evaluation of cervical length and shape of the internal cervical os was performed in 57 singleton pregnant women. Statistic data was used to define frequency distribution of cervical length in a group as approximated standardized normal distribution. Criterion for the cervix incompetence was defined as value for standardized deviation z = -1 and z = - 1/2 for cases with marked funneling of the internal os respectively. Results. Calculated mean cervical length in the group was 32.8mmand standard deviation 8.57mm. Criterion for the cervix incompetence wascervical length 20mm and for cases with marked funneling of the internal os25mm. Observing these criteria for the cervix incompetence 74% (42/57) pregnant women with competent cervix were eliminated from the group. Conclusions. On the basis of statistic analysis of values of ultrasound measurement of cervical length and ultrasonically detected anatomical changes on internal os in pregnancy, ultrasound criteria for cervical incompetence were defined. With regard to ultrasound criteria for cervical incompetence in pregnancy, the objectivity of indications for cervical cerclage was increased, enabling us to reduce the percentage of cerclage in our hospital.
