    Descriptions of possible parkinsonism appeared in ancient medical texts from India, China, Egypt, Greece and Rome; however, the disorder has been nosologically defined by James Parkinson in 1817. It ... can be divided into various groups: idiopathic Parkinson's disease, parkinsonism plus, secondary parkinsonism and psychogenic parkinsonism. The pathogenetic mechanism (decreased dopamine due to cell degeneration in substantia nigra) has been clarified, however, the questions about the cause of the degeneration remain unanswered, as well as the reasons for the selective vulnerability of dopaminergic cells in substantia nigra. Possible causes include aging, geneticand environmental factors, such as infection or toxins; a combined mechanism is also plausible. Extrapolating from epidemiological studies, theremay be approximately 4000 parkinsonian patients in Slovenia.
