  • Kirurška revaskularizacija na spodnjih okončinah pri bolnikih s sladkorno boleznijo = Surgical revascularisation of lower limbs in patients with diabetes mellitus
    Gasparini, Mladen ; Klokočovnik, Tomislav
Epidemiological data about the frequencies of arterial peripheral vascular disease among diabetics are discussed. Factors affecting the long-term resultsand complications are also considered. ... Concerning patients with intermittent claudication a classification of morphological changes in arteries of lower limbs is proposed and recommendation of treatment modalitiesare discussed. Results of surgical treatment of diabetics and nondiabetics operated due to intermittent claudication are compared. Most papers regarding limb ischemia among diabetics are dealing with the revascularisation in critical ischemia. In the vast majority of cases results of surgical revascularisation are comparable between diabetics and nondiabetics. By-pass surgery on crural and pedal artery has also acceptable long-term patency rate in diabetics. Acute ischemic events in the lower limb may be classified according to the latest SVS/ISCV recommendations, which suggests different treatment approaches. Contrasting the good immediate surgical results is a relatively high postoperative mortality among diabetic patients. The authors conclude that although diabetics have a higher possibility of developing incapacitaiting peripheral vascular disease, they should not have less possibility of proper treatment.
