  • Mikroalbuminurija in nema ishemija srčne mišice pri osebah s sladkorno boleznijo tipa 2 = Microalbuminuria and silent myocardial ishaemia in type 2 diabetes

    Prognosis of type 2 diabetes depends mainly on cardiovascular complications. Among these, coronary artery disease is the leading cause of death. Diabetics have commonly several risk factors for ... development of atheroslerosis. Microalbuminuria denotes an increased urinary albumin excretion and is a sign for general endothelial dysfunction. Mortality in type 2 diabetes patients with microalbuminuria is higher and is mainly due to cardiovascular disease. Using EKG exerase testing we have compared the prevalence and severity of silent myocardial ischemia in 30 type 2 diabetics with microalbuminuria and in30 diabetics without microalbuminuria. All patients were free of symptomaticcoronary artery disease. Patients with microalbuminuria had a higher prevalence of ischemic EKG response, defined as >_ 1 mm ST depression:18 out of 30 patients with microalbuminuria versus 9 out of 30 patients without microalbuminuria, p = 0.019. Patients with microalbuminuria had reduced exercise time in comparison to patients without microalbumniuria (9,5 +- 2.5 min vs.10,5 +- 2,2 min, p = 0.09) and reduced maximal work intensity (8,0 +- 2,2 MET vs. 8,9 +- 2,4 MET, p = 0.17). Asymptomatic type 2 diabetic patients fulfilling two or more of the following criteria should all be submitted to exercise testing: age more than 60 years, hypertension, hyperlipidaemia,smoker status, abnormal resting electrocardiogram, peripheral vascular disease, autonomic neuropathy and/or microalbuminuria. It is very likely that non-invasive testingfor coronary disease in type 2 diabeteics could identify a group at very high risk who might benefit from aggressive management of risk factors.
