    The prevalence and characteristics of childhood headache syndromes are to someextent different from those in adults, but no major differences in their pathophysiology have been postulated. ... Contemporary views on migraine and tension type headache are presented. Pain as the primary clinical problem in chronic headache syndromes can be a consequence of irritation of either extra-or intra-cranial structures (the latter being especially located at the cranial base). Such irritation explains well the various symptomatic headaches; it is also thought to be the source of pain in migraine and clusterheadache, originating from a pathological response mediated through thetrigeminovascular system. In periodic tension type headache the pain supposedly originates from irritated extracranial structures during physicallyor psychologically stressful situations; in the chronic variety the repeated episodes of pain are thought to derange the pain modulatory systems in the brain, thus leading to chronic pain. Our knowledge of headache pathophysiology is still only fragmentary, and research has not yet yielded any diagnostic procedure that would help support a clinical diagnosis of a primary headache syndrome. Research based on pathophysiological insights has, however, already led to the development of specifically designed drugs.
