  • Zdravljenje KOPB z zdravili = Treatment of COPD with medication
    Košnik, Mitja
The aim of treatment of COPD patients is to reduce symptoms (breathlessness), frequency of exacerbations and the decline of pulmonary function, as well as prolonging their survival. Regular ... inhalation of bronchodilators is the mainstay of drug therapy. The bronchodilator of choice is a long-acting parasympatholytic. Theophylline increases the frequency of complications and causes adverse effects. Inhaled glucocorticoids offer no benefit for the majority of COPD patients. However, it is important to differentiate COPD fromchronic poorly reversible asthma. Oral glucocorticoids are prescribed onlyfor a few days during COPD exacerbations. If, during an exacerbation, the patient expectorates a more purulent sputum, an antibiotic effective against S. pneumoniae, H. influenzae and M. catarrhalis should be prescribed. Giving up smoking reduces the rate of decline of pulmonary function. The long-term success of quitting cigarettes is lower than 10%. Nicotine patches and bupropione are useful for reducing withdrawal symptoms. Long-term oxygen therapy prolongs the life of patients with respiratory insufficiency due to COPD.
