Adult stem cells are undifferentiated cells that can be mobilized from the bone marrow or other organs, home into injured tissues, and differentiate into different cell phenotypes to serve in a ...repairing capacity. Furthermore, these cells can respond to inflammation and oxidative stress by exhibiting immunomodulatory properties. The protective and reparative roles of mesenchymal stem cells (MSCs), very small embryonic-like stem cells (VSELs), and endothelial progenitor cells (EPCs) have primarily been examined and characterized in auto-immune and cardiovascular diseases. Obstructive sleep apnea (OSA) is a very prevalent disease (4-5% of adult population and 2-3% of children) characterized by an abnormal increase in upper airway collapsibility. Recurrent airway obstructions elicit arterial oxygen desaturations, increased inspiratory efforts, and sleep fragmentation, which have been associated with important long-term neurocognitive, metabolic, and cardiovascular consequences. Since inflammation, oxidative stress and endothelial dysfunction are key factors in the development of the morbid consequences of OSA, bone marrow-derived stem cells could be important modulators of the morbid phenotype by affording a protective role. This mini-review is focused on the recent data available on EPCs, VSELs, and MSCs in both animal models and patients with OSA.
El síndrome de la apnea/hipopnea del sueño (SAHS) es una enfermedad caracterizada por el colapso repetitivo de forma parcial o total de la vía aérea superior (VAS). Numerosos autores han sugerido que ...los daños observados en la VAS de pacientes con SAHS podrían ser provocados por el estrés mecánico provocado por fenómenos como el ronquido y los eventos apneicos. Concretamente estos estímulos mecánicos asociados al SAHS son vibración (ronquido) y presión intraluminal negativa decreciente con colapso y reapertura (eventos apneicos). Por otro lado, durante el tratamiento con CPAP, la pared nasal está sometida a una compresión que podría explicar el agravamiento y aparición de los síntomas nasales experimentados por estos pacientes.El objetivo del primer estudio es determinar si el estímulo mecánico vibratorio asociado al ronquido induce inflamación en la VAS en un modelo en rata in vivo. La VAS de 12 ratas se canuló a nivel traqueal y se sometió a vibración (60 Hz; ± 10 cmH2O) durante 3 h con un patrón temporal consistente en 1 s de vibración seguido de 3 s sin vibración. Una segunda canulación, dirigida hacia los pulmones, permitió al animal respirar espontáneamente durante todo el experimento. En un grupo control (n=12) los animales se instrumentaron del mismo modo con la única diferencia de que no se aplicó vibración a la VAS. Mediante PCR en tiempo real, se observó un incremento significativo en la expresión de ambos mediadores proinflamatorios tras la aplicación de vibración: TNF-alpha: 6.01 veces (p = 0.005) y MIP-2: 2.38 veces (p = 0.021) respecto al grupo control. La aplicación durante 3 h de un estímulo mecánico vibratorio con las características típicas del ronquido es capaz de inducir un proceso local proinflamatorio en la VAS.Los objetivos del segundo estudio son, desarrollar un modelo en rata que permita la aplicación intratraqueal de presión negativa intermitente capaz de inducir el colapso y reapertura de la VAS y determinar si el estímulo mecánico induce inflamación local en la VAS. La VAS de 14 ratas fue sometida a un patrón temporal de 1 s de presión negativa inspiratoria seguida de 2 s de presión positiva espiratoria durante 5 h. En un grupo control de 14 ratas, los animales fueron instrumentados similarmente pero no se aplicó estímulo mecánico. El modelo desarrollado consiguió simular el estímulo de colapso y reapertura que ocurre en la VAS de pacientes con apnea obstructiva del sueño. Después de 5 h de estímulo, mediante PCR en tiempo real se observó un incremento de expresión génica de los marcadores inflamatorios MIP-2, TNF-alpha, IL-1 y P-selectina (~ 40, 24, 47 y 7 veces, respectivamente) respecto al grupo control. Incrementos similares se observaron en la expresión génica del tejido de paladar blando. Estos resultados podrían explicar la inflamación observada en la VAS de los pacientes con apnea obstructiva del sueño.El objetivo del tercer estudio fue demostrar si la aplicación no invasiva de CPAP nasal es un estímulo mecánico suficiente para inducir una respuesta inflamatoria local. 32 ratas macho de la cepa Sprague-Dawley (250-300 g) distribuidas en 4 grupos fueron sometidas a CPAP nasal (10 cmH2O) y control durante 3 h y 5 h. Tras la aplicación de CPAP nasal durante 5 h se observó un incremento (p=0.006) en el porcentaje de neutrófilos presente en la mucosa nasal (3.51 %) respecto al grupo control (1.12 %). Por otro lado, mediante PCR en tiempo real se observó un incremento de los mediadores inflamatorios MIP-2 (2.28 veces; p=0.034) tras 3h de aplicación de CPAP nasal respecto al grupo control y 5.56 veces; p=0.002) tras 5h. La inflamación generada tras la aplicación de CPAP nasal podría explicar la aparición y/o agravamiento de los síntomas riníticos observados en los pacientes tratados con esta técnica terapéutica.

. Five of the 12 PMOs show definitive evidence of excess, implying a disk fraction of 42%, albeit with a large statistical uncertainty given the small sample. Comparing with measurements for higher-mass objects, the disk fraction does not decline substantially with decreasing mass in the substellar domain, consistent with previous findings. Thus, free-floating PMOs have the potential to form their own miniature planetary systems. We note that only one of the six lowest-mass objects in NGC 1333, with spectral type L0 or later, has a confirmed disk. Reviewing the literature, we find that the lowest-mass free-floating objects with firm disk detections have masses ∼0.01
M

(or ∼10

). It is not clear yet whether even lower-mass objects harbor disks. If not, it may indicate that ∼10
