Background
Determining the humoral immunogenicity of tozinameran (BNT162b2) in patients requiring chronic renal replacement therapy, and its impact on COVID-19 morbidity several months after ...vaccination, may guide risk assessment and changes in vaccination policy.
Methods
In a prospective post-vaccination cohort study with up to 5 months follow-up we studied outpatient dialysis and kidney transplant patients and respective healthcare teams. Outcomes were anti S1/S2 antibody responses to vaccine or infection, and infection rate during follow-up.
Results
One hundred seventy-five dialysis patients (40% women, 65 ± 15 years), 252 kidney transplant patients (33% women, 54 ± 14 years) and 71 controls (65% women, 44 ± 14 years) were followed. Three months or longer after vaccination we detected anti S1/S2 IgG antibodies in 79% of dialysis patients, 42% of transplant recipients and 100% of controls, whereas respective rates after infection were 94%, 69% and 100%. Predictors of non-response were older age, diabetes, history of cancer, lower lymphocyte count and lower vitamin-D levels. Factors associated with lower antibody levels in dialysis patients were modality (hemodialysis vs peritoneal) and high serum ferritin levels. In transplant patients, hypertension and higher calcineurin or mTOR inhibitor drug levels were linked with lower antibody response. Vaccination was associated with fewer subsequent infections (HR 0.23, p < 0.05). Moreover, higher antibody levels (particularly above 59 AU/ml) were associated with fewer events, with a HR 0.41 for each unit increased in log

titer (p < 0.05).
Conclusions
Dialysis patients, and more strikingly kidney transplant recipients, mounted reduced antibody response to COVID-19 mRNA vaccination. Lesser humoral response was associated with more infections. Measures to identify and protect non-responsive patients are required.

OBJECTIVESThe purpose of this study was to assess the incremental value of preoperative breast MRI over mammography and US in depicting the accurate extent of disease in invasive duct carcinoma (IDC) ...compared to invasive lobular carcinoma (ILC). PATIENTS AND METHODSRetrospective analysis of pre-operative mammography, US and MRI was performed in 239 patients with either IDC (n = 193) or ILC (n = 46). Images were evaluated for solitary, multifocal or multi centric disease and compared for concordance with postsurgical pathology. Discordance was documented as either overestimation or underestimation. Two tailed paired T and Fischer's exact tests were used for analysis. RESULTSMultifocality was present on pathology in 35% and 61% of patients with IDC and ILC (P < .05) and multicentricity in 23% and 41% respectively (P = .84). In ILC, MRI demonstrated better concordance with pathology compared to mammography and US (89%, 44%, 49% for multifocality P < .05 and 80.5%, 63%, 71% for multicentricity P = .3). For IDC, concordance with pathology for all modalities was similar (65%-76%). Among discordant cases, underestimation was significantly more common for mammography and US, while MRI more frequently overestimated disease extent. MRI very rarely overestimated multifocal disease in ILC (2%). CONCLUSIONMRI demonstrates an 80% to 90% concordance rate with pathology for ILC, superior to mammography and US. The addition of MRI in IDC patients may decrease underestimation of disease extent and potentially contribute to a reduction in post-operative residual disease.

The significance of age in determining one's mental and physical abilities has already been under dispute in ancient Rome, and likely before then. In his dialogue Cato Majore De Senectutem (Old Cato on old age), written by Marcus Tullius Cicero in 44 BC, the rightful approach towards one's age (ageism), and specifically the approach towards old age, is discussed. 62 year-old Cicero, an accomplished politician, orator and philosopher, contends with social perceptions of old age in Rome. These seek to allow the biological age of the Roman citizen dictate the manner and extent of his involvement in the city. In other words, Cicero feels secluded from the political sphere, his natural habitat thus far, when the divided city of Rome is in need of leadership and counsel more than even. The dialogue can be read in different ways: It can be seen as a social manifest on the proper place of the aging man in Rome, as well as the relation between the political and social apparatus in Rome and its individuals. Alternately, it can be read as a private consoling text written by Cicero for himself and his aging friend Atticus. Either way, the dialogue presents through Cato the Old characteristics of an old man turning him into an ideal politician and leader, or at least advisor for the younger generation. There is similarity between the old man described, and the ideal politician known from other Ciceronian texts. These allow the reader to use the dialogue in order to draw upon the characteristics of an ideal leader in the Republic of Rome according to Cicero. The significance of the text is not restricted to historical study of ancient social structures in Ancient Rome. Issues relating to age-based social exclusion, or allowing age to determine the place of the aged in the near and public environment, are also present in modern social discourse. Either as a private or public matter, Cicero's dialogue illuminates additional aspects that may be excluded from current discussions on the matter. Using the dialogue to draw unequivocal conclusions concerning the "proper" attitude towards the aging individuals in modern society is, of course, not permissible. However, the glance that Cicero allows to the soul of an old man secluded from active professional and social activity is important. Optimal response to medical, social and personal needs of aging citizens in the state, without giving in to an ageist approach, is a challenge. Cicero reminds us to aspire to see things also from the aging citizen's point of view, and recognize the potential and advantages that he holds for the state and the society.
