The median nerve is a mixed sensory and motor nerve. It is classically described as the nerve of pronation, of thumb, index finger, middle finger and wrist flexion, of thumb antepulsion and ...opposition, as well as the nerve of sensation for the palmar aspect of the first three fingers. It takes its name from its middle position at the end of the brachial plexus and the forearm. During its course from its origin at the brachial plexus to its terminal branches, it runs through various narrow passages where it could be compressed, such as the carpal tunnel or the pronator teres. The objective of this review is to summarize the current knowledge on the median nerve's anatomy: anatomical variations (branches, median-ulnar communicating branches), fascicular microanatomy, vascularization, anatomy of compression sites, embryology, ultrasonographic anatomy. The links between its anatomy and clinical, surgical or diagnostic applications are emphasized throughout this review.
Le nerf médian est un nerf mixte, somatomoteur et somatosensible. Il est classiquement décrit comme le nerf de la pronation, de la flexion des trois premiers doigts et du poignet (sauf la flexion ulnaire), de l’antépulsion et de l’opposition du pouce ainsi que celui de la sensibilité de la face palmaire des trois premiers doigts. Il tire son nom de sa position médiane à la fois au niveau de la terminaison du plexus brachial mais aussi au niveau de l’avant-bras. Au cours de son trajet depuis sa naissance à la sortie du plexus brachial jusqu’à ses branches terminales, il emprunte des lieux de passage dans lesquels il est à risque d’être comprimé, comme le canal carpien ou le rond pronateur. L’objectif de ce travail est de présenter une synthèse des connaissances sur l’anatomie du nerf médian: variations anatomiques (branches, rameaux communicants médio-ulnaires), micro-anatomie fasciculaire, vascularisation, anatomie des sites de compression potentiels, embryologie, anatomie échographique. Tout au long de ce travail, un lien est fait entre l’anatomie et les applications cliniques, chirurgicales ou diagnostiques.
Hamatolunate impingement syndrome is an uncommon cause of ulnar-sided wrist pain in the general population. Often misdiagnosed and untreated by non-specialized physicians, it is an important source ...of chronic ulnar wrist pain in golfers. The purpose of this retrospective study was to report results of arthroscopic burring of the apex of the hamate for hamatolunate impingement, whether isolated or not, in golf players, with a minimum of six months follow-up. Fifteen golf players (10 amateur, 2 semi-professional and 3 professional players), aged 40–61 years, with ulnar carpal pain implicating hamatolunate impingement with Viegas type-II carpal configuration, were included. Treatment consisted in arthroscopic burring of the apex of the hamate. At an average follow-up of 11 months (range, 6–24 months), all patients were satisfied with functional results, except 1 with persistent pain and stiffness; 93% returned to sport to their prior level. Mean range of motion was improved by 17 ° for wrist flexion (range, 15 ° to 30 °) and 15 ° for wrist extension (range, 10 ° to 25 °). All patients except 1 recovered grip strength, improving from 27 kg (range, 12–53) preoperatively to 35 kg (range, 17–61) at last-follow-up, and ulnar-sided pain was alleviated during golf practice. Return to prior sport level was possible by 5.5 months for professional players and by 9 months for amateurs. Arthroscopic burring of the apex of the hamate provided good clinical results for function and pain, with fairly rapid return to sport. Hamatolunate chondritis does not always mean pathology but represents the natural progression of Viegas type II wrists.
Les conflits hamato-lunaires constituent une cause exceptionnelle des douleurs ulnaires du poignet dans la population générale. Chez de nombreux golfeurs, les douleurs chroniques du versant ulnaire du poignet sont souvent négligées, parfois mal étiquetées par des praticiens non avertis, et sont source de gêne dans le sport. L’objectif de cette étude était de présenter les résultats postopératoires sur la fonction du poignet d’une série de golfeurs opérés d’un conflit hamato-lunaire, isolé ou non, avec résection arthroscopique de l’apex de l’hamatum avec suivi de 6 mois minimum. Il s’agissait d’une étude rétrospective de 15 golfeurs (10 amateurs, 2 semi-professionnels et 3 professionnels) de 40 à 61 ans présentant des douleurs ulnaires du carpe en rapport avec un conflit hamato-lunaire sur carpe de type Viegas II. Une résection de l’apex de l’hamatum était réalisée sous arthroscopie. Le suivi moyen était de 11 mois (6–24). Tous les patients sauf un (du fait de douleurs résiduelles et d’une raideur persistante) étaient satisfaits des résultats fonctionnels; 93% des patients avaient repris le sport à leur niveau habituel. Les mobilités moyennes étaient améliorées de 17 ° (15–30) en flexion de poignet et de 15 ° (10–25) en extension de poignet. Tous les patients sauf un avaient récupéré leur force de préhension, de 27 kg (12–53) en préopératoire à 35 kg (17–61) au dernier recul,et les douleurs étaient soulagées pour la pratique du golf. Le retour au sport au niveau antérieur était possible à 5,5 mois chez les professionnels contre 9 mois chez les amateurs. La prise en charge arthroscopique du conflit hamato-lunaire chez le golfeur par résection de l’apex de l’hamatum procure de bons résultats cliniques sur la fonction du poignet et la douleur, et permet un retour au sport assez rapide. La chondrite hamato-lunaire n’est pas systématiquement synonyme de pathologie, plutôt d’une évolution naturelle dans les carpes de configuration Viegas II.
Les lésions post-traumatiques du carpe (rupture scapho-lunaire, pseudarthrose scaphoïdienne) évoluent souvent vers une arthrose invalidante (SNAC/SLAC wrist). La résection de première rangée du carpe ...(RPC) est une option fiable mais dont le pronostic est réservé en cas d’arthrose du radius distal ou de la tête du capitatum. Dans une telle situation, on peut alors être amené à réaliser une arthrodèse radio-carpienne qui sacrifie les mobilités du poignet. Afin de contourner cette limite nous proposons une nouvelle procédure consistant à réaliser une RPC sous arthroscopie puis à remplacer la première rangée par une greffon tendineux.
C’était une étude sur 10 spécimens cadavériques.
Une broche scapho-luno-triquétrale était introduite via un court abord dans la tabatière anatomique. Cette broche servait de guide à une mèche canulée (9mm) permettant de réaliser un tunnel trans-première rangée. L’arthroscope et une fraise motorisée étaient introduits dans ce tunnel (via ses extrémités radiale et ulnaire). La RPC était réalisée sous contrôle arthroscopique. Les tendons graciles et semi-tendineux étaient ensuite prélevés, repliés et suturés pour obtenir un greffon de longueur et de diamètre équivalent à la première rangée. Le greffon était ensuite faufilé par la voie radiale et fixé à la capsule.
La durée moyenne d’intervention était de 75minutes (±13minutes). Entre le pré- et le postopératoire, il n’y avait pas de différence significative d’amplitude de mouvement ni de hauteur du carpe. Lors du radiocinéma, le greffon (radio-marqué) était parfaitement stable entre le radius et la deuxième rangée du carpe (qui n’était plus en contact avec le radius). Les branches sensitives radiale et ulnaire, le nerf médian et l’artère radiale étaient intacts à la fin de la procédure.
Cette technique pourrait représenter une solution en cas de SNAC/SLAC wrist avec arthrose de la tête du capitatum ou de la glène radiale. Elle permet aussi de préserver la hauteur du carpe. La réalisation tout-arthroscopique permettrait d’éviter l’œdème majeur observé lors d’abord dorsal du carpe, mais aussi d’assurer la stabilité du greffon puisque les ligaments radio-carpiens sont préservés. Le prélèvement sur un autre site anatomique et le devenir du transplant in vivo représentent deux limites à discuter.
Cette étude anatomique ouvre la voie à une expérimentation clinique.
L’utilisation de plaque antérieure verrouillée dans l’ostéosynthèse des fractures de l’extrémité distale du radius (EDR) avec vis verrouillées épiphysaires monocorticales longues est communément ...admise, mais elle présente un risque de lésions des tendons extenseurs par d’éventuelles vis saillantes de la corticale postérieure du radius. L’objectif de cette étude est d’évaluer l’efficacité de vis verrouillées épiphysaires courtes, de longueur unique, dans la prévention de la pénétration de la corticale dorsale. L’objectif secondaire est de déterminer si cette stratégie à un impact sur la stabilité comparée à l’utilisation de vis épiphysaires monocorticales longues.
Une étude prospective, monocentrique, non randomisée a été menée chez des patients opérés pour une fracture d’EDR par plaque antérieure verrouillée.
Les patients du groupe A étaient opérés avec des vis verrouillées épiphysaires courtes (16mm et 18mm respectivement chez les femmes et les hommes), les patients du groupe B étaient opérés avec des vis verrouillées monocorticales distales longues (100 % de l’épaisseur du radius distal). Une échographie était réalisée par un radiologue à 3 mois pour rechercher le nombre et la longueur des vis proéminentes de la corticale dorsale du radius. Des radiographies étaient pratiquées à 6 semaines pour évaluer la stabilité selon les variations de l’antéversion radiale, la pente radiale et l’index radio-ulnaire.
Chaque groupe était composé de 35 patients. Dans le groupe A, il y avait 0 % de vis dépassant en dorsal de plus de 1mm contre 7 % dans le groupe B (p<0,05). Il n’y avait pas de différence significative concernant la stabilité au niveau de la variation moyenne d’antéversion palmaire (0,3° vs 0,2 ; non significatif NS), de pente radiale (0,5° vs 0,3° ; NS), de l’index radio-ulnaire (0,2mm vs 0,1mm ; NS).
À stabilité égale dans les plaques antérieures verrouillées pour les fractures d’EDR, les vis épiphysaires courtes devraient être préférées aux vis longues dans un soucis de prévention des lésions tendineuses. Basée sur 75 % de l’épaisseur moyenne de l’EDR, une longueur unique de vis pour chaque sexe (16mm et 18mm respectivement chez les femmes et les hommes) semble être une solution à la fois simple à mettre en œuvre et efficace en termes de résultats.
Severe collateral ligament sprain of the metacarpophalangeal joint (MCP) of the fingers is rare. Clinical examination is mandatory to diagnose severity. The purpose of our study was to validate a ...clinical sign for MCP joint laxity: the spontaneous lateral laxity sign (SLLS), recently described by Meyer et al. to diagnose severe MCP radial collateral ligament (RCL) sprain, with a comparison with the already validated rotational laxity test (RLT). SLLS and RLT were assessed before RCL transection, after RCL transection and after RCL repair on 40 long fingers in 10 fresh thawed cadavers. SLLS was performed with the elbow on the table, wrist in 70° flexion, in neutral pronation-supination, hands drooping passively with the dorsal side toward the examiner and the ulnar side toward the table. The MCP joints were at rest, in passive slight extension. Positive results were defined as an overlap of the damaged finger on the next, or as an increased abduction of the little finger. Correlation between the two tests was calculated. SLLS was positive in 0% of cases before RCL transection, 100% after transection and 0% after repair. Mean arcs of pronation and supination on RLT were 16 and 19.5 mm before section, 24 and 33 mm after section (52% and 69% increase compared to preoperative data), and 17 and 21 mm after repair (7% and 8% increase). Correlation between the two tests was 100%. The spontaneous lateral laxity sign is a simple and reliable clinical sign for diagnosing complete long-finger MCP RCL tears requiring surgery.

Les entorses graves des ligaments collatéraux de l’articulation métacarpo-phalangienne (MCP) des doigts longs sont rares. L’examen clinique est essentiel dans le diagnostic de sévérité de ces lésions. L’objectif de cette étude était de valider un nouveau signe clinique de laxité MCP (le signe de laxité latérale spontanée, SLLS) récemment décrit par Meyer et al. en comparaison avec le test de laxité rotatoire (RLT) déjà décrit, dans la recherche des entorses graves du ligament collatéral radial (RCL) des MCP. Quarante doigts longs de 10 cadavres frais décongelés ont été inclus. Le RLT et le SLLS étaient pratiqués trois fois sur chaque doigt long: avant section du RCL de la MCP, après section du RCL, et après sa réparation. Le SLLS était recherché avec le coude du patient reposant sur la table, l’avant-bras en position neutre (sans pronation ni supination), le poignet fléchi à 70°, dos de la main vers l’examinateur, les MCP au repos, en extension passive. Le test était positif en cas de chevauchement spontané du doigt atteint sur le suivant ou d’hyperabduction spontanée du cinquième doigt. La concordance des résultats des deux tests était calculée. Le SLLS n’était positif dans aucun cas (0%) avant section du RCL. Il était positif dans tous les cas (100%) après section du RCL. Il était négatif dans tous les cas (0%) après réparation. Les arcs de mobilité moyens en pronation et supination du RLT étaient de 16 et 19.5 mm avant section du RCL, de 24 et 33 mm après section (soit 52% et 69% d’augmentation en comparaison aux données préopératoires), de 17 et 21 mm après réparation (soit 7% et 8% d’augmentation). La concordance des deux tests était de 100%. Le signe de laxité latérale spontanée est un test clinique simple et fiable permettant de diagnostiquer les entorses graves du RCL des MCP des doigts longs nécessitant une réparation chirurgicale.
3, étude cas-témoins.
Purpose
Volar locking plates, used in distal radius fracture (DRF), present a risk of injuring extensor tendons with screws penetrating the dorsal cortex of the radius. Actually, even when aiming to ...use maximum-length unicortical locking screws, some still could be bicortical. We hypothesize the use of only short unicortical screws would allow a proper stabilization of the radial epiphysis without the risk of dorsal cortex penetration.
Materials and methods
A prospective monocentric non-randomized study was conducted. Patients with DRF (excepted for partial dorsal joint fractures) were treated in group A with short locking epiphyseal screws (16 mm for females, 18 mm for males) and in group B with full-length unicortical locking screws. Ultrasound was done 3 months postoperatively to evaluate the number and length of prominent dorsal screws. X-rays were performed after 6 weeks to assess stability according to volar tilt and radial inclination variations.
Results
There were 37 patients in group A and 39 in group B with 148 and 156 epiphyseal screws, respectively. In group A, there were 0% of dorsal penetrating screws against 6.5% (10 screws from 8 patients) in group B (
p
< 0.05). There was no significant difference for the stability between the groups: mean volar tilt variation ( − 0.6° vs. − 0.7°) and mean radial inclination variation ( − 0.4° vs. − 0.4°).
Conclusion
For a same stability with volar locking plates for DRF, short epiphyseal locking screws should be preferred to full-length unicortical screws in order to prevent extensor tendon injuries. Based on 75% of distal radial average anteroposterior width for each sex, screw lengths of 16 mm for females and 18 mm for males seem to be the length to use.

The understanding of the processes occurring in Nature has been a continuing concern throughout the history of mankind. Intellectual tools employed towards this goal were specific for each period and ...have been largely based on the prevailing paradigms that reigned in the past. In this work we present evidence that supports the idea of viruses as key agents mediating natural processes linked to the evolution of organisms, particularly those involved in the flux of genes in the environment. This point of view tinges our perception of Nature and prompts us to include “viral” creativity and plasticity among the tools we employ to analyze those processes far beyond actual paradigms. Experimental data to support this proposal arose during the study of the interaction of the human pathogen, herpes simplex virus (HSV) with sulfated polysaccharides during multiplication of the virus in vitro. Sulfated polysaccharides are the main chemical structures found in carrageenans (CGNs) that are natural products obtained from seaweeds, which proved to be strong inhibitors for the virus. Here we describe the interaction between virus and CGNs as a suitable scenario for the emergence of viral variants which proved to be markedly attenuated for mice. A striking feature of these variants is that they showed changes at the level of conserved regions of the genome such as the DNA polymerase and thymidine kinase genes. In view of these findings, the importance of HSV evolution towards attenuated variants by the action of polysaccharides is also discussed. Attenuation may be considered part of a natural evolutionary process enabling the virus to contribute with valuable information for the host.

We have examined the effects of embedded pitch adapters on signal formation in n-substrate silicon microstrip sensors with data from beam tests and simulation. According to simulation, the presence ...of the pitch adapter metal layer changes the electric field inside the sensor, resulting in slowed signal formation on the nearby strips and a pick-up effect on the pitch adapter. This can result in an inefficiency to detect particles passing through the pitch adapter region. All these effects have been observed in the beam test data.

The performance of prototype active-edge VTT sensors bump-bonded to the Timepix ASIC is presented. Non-irradiated sensors of thicknesses 100–200μm and pixel-to-edge distances of 50μm and 100μm were ...probed with a beam of charged hadrons with sub-pixel precision using the Timepix telescope assembled at the SPS at CERN. The sensors are shown to be highly efficient up to a few micrometers from the physical edge of the sensor. The distortion of the electric field lines at the edge of the sensors is studied by reconstructing the streamlines of the electric field using two-pixel clusters. These results are supported by TCAD simulations. The reconstructed streamlines are used to study the field distortion as a function of the bias voltage and to apply corrections to the cluster positions at the edge.
Immune evasion strategies are important for the onset and the maintenance of viral infections. Many viruses have evolved mechanisms to counteract or suppress the host immune response. We have ...previously characterized two syncytial (syn) variants of Herpes simplex 1 (HSV-1) strain F, syn14-1 and syn17-2, obtained by selective pressure with a natural carrageenan. These variants showed a differential pathology in vaginal and respiratory mucosa infection in comparison with parental strain. In this paper, we evaluated the modulation of immune response in respiratory mucosa by these HSV-1 variants. We observed altered levels of Tumor Necrosis Factor-α and Interleukin-6 in lungs of animals infected with the syn14-1 and syn17-2 variants compared with the parental strain. Also, we detected differences in the recruitment of immune cells to the lung in syn variants infected mice. Both variants exhibit one point mutation in the sequence of the gene of glycoprotein D detected in the ectodomain of syn14-1 and the cytoplasmic tail of syn17-2. Results obtained in the present study contribute to the characterization of HSV-1 syn variants and the participation of the cellular inflammatory response in viral pathogenesis.
•HSV-1 syn variants arise during in vitro serial passages with carrageenans.•The pathology of HSV-1 depends on modulation of the innate immune response.•Low level of TNF-α is not due to an overproduction of anti-inflammatory cytokine.•Virulence of variants by intranasal route correlates with an enhanced replication.
