Abstract The microvascular network formed by the capillaries supplies the tissues and permits their function. It provides a considerable surface area for exchanges between blood and tissues. All ...pathological conditions cause changes in the microcirculation. These changes can be used as imaging biomarkers for the diagnosis of lesions and optimisation of treatment. Among the many imaging techniques developed to study the microcirculation, the analysis of the tissue kinetics of intravenously injected contrast agents is the most widely used, either as positive enhancement for CT, T1-weighted MRI and ultrasound – dynamic contrast-enhanced-imaging (DCE-imaging) – or negative enhancement in T2*-weighted brain MRI – dynamic susceptibility contrast-MRI (DSC-MRI) –. Acquisition involves an injection of contrast agent during the acquisition of a dynamic series of images on a zone of interest. These kinetics may be analyzed visually, to define qualitative criteria, or with software using mathematical modelling, to extract quantitative physiological parameters. The results depend on the acquisition conditions (type of imaging device, imaging mode, frequency and total duration of acquisition), the type of contrast agent, the data pre-processing (motion correction, conversion of the signal into concentration) and the data analysis method. Because of these multiple choices it is necessary to understand the physiological processes involved and understand the advantages and limits of each strategy.
La médecine factuelle fait appel à des descriptions analytiques réalisées selon des critères scientifiques rigoureux. Idéalement, on veut repérer chez le patient un signe pathognomonique quantifiable ...ou biomarqueur de la maladie et en suivre les variations sous traitement. Souvent, le biomarqueur nécessite une analyse destructive du tissu vivant. Les techniques d’imagerie proposent un compromis entre la précision de l’information et l’innocuité de la méthode. Idéalement, l’image localise et quantifie un paramètre précis sans modifier l’équilibre physiologique du sujet. L’intérêt de l’imagerie du petit animal est triple : – inventer, tester et optimiser des techniques d’imagerie transposables à l’homme ; – décrire l’histoire naturelle d’une pathologie par une approche non invasive, donc répétable au cours du temps ; – obtenir un biomarqueur non invasif de l’efficacité thérapeutique. Comparée aux techniques in vitro, l’imagerie paye son caractère non invasif par un accroissement de la complexité et du coût des examens. En outre, une modalité d’imagerie ne donne accès qu’à quelques paramètres biologiques à la fois, ce qui est souvent insuffisant. Il est donc important de valider, en amont de la clinique, les nouvelles approches multimodales d’imagerie chez les modèles animaux des pathologies humaines. Dans cet exposé, nous présenterons à partir d’un exemple concret les recherches menées dans notre laboratoire en imagerie multimodale (IRM, imagerie optique et 18FDG-TEP) chez un modèle murin de paragangliomes liés à une mutation du gène SDHB , et le transfert de l’une d’entre elles en clinique humaine.
Objectif Les mutations du gène SDHB sont un facteur de malignité et de mauvais pronostic des paragangliomes. L’élaboration de modèles animaux est nécessaire pour l’évaluation préclinique de ...thérapeutiques innovantes. Matériel et méthodes Une lignée murine de cellules chromaffines immortalisées (imCC) Sdhb–/− a été générée dans notre laboratoire et implantée en sous-cutané dans le « fatpad » de souris nudes NMRI ( n = 15). Un groupe contrôle ( n = 16) a été réalisé par implantation d’imCC sauvages (WT). L’angiogenèse tumorale a été évaluée par imagerie optique in vivo grâce à un traceur fluorescent ciblant les intégrines αv β3 , la microcirculation par IRM dynamique de contraste (IRM-DCE) et le métabolisme par spectroscopie par résonance magnétique (1 H-MRS) et par18 FDG-PET. Résultats L’expression des intégrines est plus marquée dans le groupe de tumeurs Sdhb–/− et est associée, en IRM-DCE, à un rehaussement tumoral nettement plus important que dans les tumeurs WT. Les tumeurs Sdhb–/− présentent de plus une accumulation de succinate mesurable par1 H-MRS. Enfin, la captation du glucose en18 FDG-PET est également plus marquée dans le groupe Sdhb–/− . L’évolution sous traitement antiangiogénique est en cours d’analyse. Discussion L’augmentation de la vascularisation et de la captation du glucose des tumeurs Sdhb–/− est évaluable in vivo par IRM et imagerie optique. Ces résultats permettent de mettre en place des études précliniques de réponse précoce au traitement. La détection de l’accumulation de succinate par MRS pourrait permettre par un diagnostic « moléculaire » préopératoire afin d’identifier précocement les patients à risque.
Abstract Background Thermal ablation is a widely used minimally invasive treatment modality for different cancers. However, lack of a real-time imaging system for accurate evaluation of the procedure ...is one of the reasons of local recurrences. Shear waves elastography (SWE) is a new ultrasound (US) imaging modality to quantify tissue stiffness. The aim of the study was to assess the feasibility and accuracy of US elastography for quantitative monitoring of thermal ablation and to determine the elasticity threshold predictive of coagulation necrosis. Methods A total of 29 in vivo thermal lesions were performed in pig livers with radiofrequency system. SWE and B-mode images were acquired simultaneously. Liver elasticity was quantified by using SWE data and expressed in kilopascal. After the procedure, pathologic analysis of treated tissues was compared with US images. The sensitivity and positive predictive value of the SWE maps of tissue elasticity were calculated and compared with the boundaries of the pale coagulation necrosis areas found at pathology. Results The liver mean elasticity values before and after thermal therapy were 6.4 ± 0.3 and 38.1 ± 2.5 kPa, respectively ( P < 0.0001). For a threshold of 20 kPa, sensitivity (i.e., the rate of pixels correctly detected as necrosed tissue) was 0.8, and the positive predictive value (i.e., the rate of pixels in the elastographic map >20 kPa that actually developed coagulation necrosis) was 0.83. Conclusions Tissue areas with coagulation necrosis are significantly stiffer than the surrounding tissue. SWE permits the real-time detection of coagulation necrosis produced by radiofrequency and could potentially be used to monitor US-guided thermal ablation.
Objective
To assess stiffness in a human breast cancer implanted in mice using shear wave elastography (SWE) during tumour growth and to correlate the results with pathology.
Methods
Local ethics ...committee for animal research approval was obtained. A human invasive ductal carcinoma was implanted subcutaneously in 24 athymic nude female mice. Ultrasound was longitudinally performed in 22 tumours, every 1–2 weeks. Maximum diameter and mean stiffness were collected. Seven tumours were measured both in vivo and ex vivo. Tumours of different sizes were removed for pathological analysis on which the percentages of viable cellular tissue, fibrosis and necrosis were measured.
Results
A total of 63 SWE measurements were performed. Stiffness increased during tumour growth with an excellent correlation with size (

= 0.94,

< 0.0001). No differences were found between the values of stiffness in vivo and ex vivo (

= 0.81). There was a significant correlation between elasticity and fibrosis (
r
= 0.83,

< 0.0001), a negative correlation with necrosis (

Conclusion
Fibrosis plays an important role in stiffness as measured by SWE, whereas necrosis is correlated with softness.

Abstract Background Ultrasound (US) is the primary imaging modality for the diagnosis of placenta accreta, but it is not sufficiently accurate. MRI morphologic criteria have recently emerged as a ...useful tool in this setting, but their analysis is too subjective. Recent studies suggest that gadolinium enhancement may help to distinguish between the stretched myometrium and placenta within a scar area. However, objective MRI criteria are still required for prenatal diagnosis of placenta accreta. The purpose of this study was to assess the diagnostic value of dynamic contrast gadolinium enhancement (DCE) MRI patterns for placenta accreta. Materials and methods MR images were acquired with a 1.5-T unit at 30–35 weeks of gestation in women with a history of Caesarian section, a low-lying anterior placenta, and US features compatible with placenta accreta. Sagittal, axial and coronal SSFP (Steady State Free Precession) sequences were acquired before injection. Then, contrast-enhanced dynamic T1-weighted images were acquired through the entire cross-sectional area of the placenta. Images were obtained sequentially at 10- to 14-s intervals for 2 min, beginning simultaneously with the bolus injection. Functional analysis was performed retrospectively, and tissular relative enhancement parameters were extracted from the recorded images. The suspected area of accreta (SAA) was placed in the region of the previous scar, and a control area (CA) of similar size was placed on the same image plane, as far as possible from the SAA. Semi-quantitative analysis of DCE-MR images was based on the kinetic enhancement curves in these two regions of interest (ROI). Three tissular relative enhancement parameters were compared according to the pregnancy outcomes, namely time to peak, maximal signal intensity, and area under the enhancement curve. Results We studied 9 women (43%) with accreta and 12 women (57%) with a normal placenta. All three tissular relative enhancement parameters differed significantly between the two groups (p < 10−3 ). Conclusion The use of dynamic contrast-enhanced MRI at 30–35 weeks of gestation in women with a high risk of placenta accreta allows the extraction of tissular enhancement parameters that differ significantly between placenta accreta and normal placenta. It therefore provides objective parameters on which to base the diagnosis and patient management.
Les progrès dans la compréhension de l’oncogenèse des phéochromocytomes et des paragangliomes métastatiques (PPGLm) mutés SDHB nous permettent d’envisager le recours à de nouvelles thérapies ciblées ...dans cette pathologie rare dont le pronostic reste réservé.
La réponse à plusieurs thérapies a été évaluée dans un modèle in vivo d’allogreffes de cellules chromaffines murines spontanément immortalisées (imCC) avec inactivation du gène Sdhb. Différentes imageries multimodales ont été réalisées, dont une spectroscopie par résonnance magnétique permettant de monitorer le taux de succinate in vivo. Un modèle d’allogreffes des cellules imCC Sdhb-/- avec inactivation de HIF2α (imCC Sdhb-/- shHIF2α) a également été analysé.
Après 14jours de traitement, l’ IACS (inhibiteur du complexe I de la chaîne respiratoire mitochondriale), le Sunitinib (inhibiteur de tyrosine kinase à activité anti-angiogénique) et le Talazoparib (inhibiteur de PARP) à posologie élevée permettent une réduction significative du volume tumoral. Seuls les traitements par IACS et Sunitinib sont bien tolérés. La combinaison du Talazoparib avec un agent alkylant, le Temozolomide, et la combinaison de l’IACS avec le Sunitinib potentialisent ces effets anti-tumoraux au prix de toxicités plus importantes. L’efficacité anti-tumorale du Sunitinib est corrélée à une diminution du succinate in vivo. Contrairement à l’inhibition de la croissance tumorale observée dans les tumeurs imCC Sdhb-/- shHIF2α, les deux inhibiteurs pharmacologiques de HIF2α (PT2385 et PT2977) ne semblent avoir que peu d’action anti-tumorale dans ce modèle.
Ces travaux préliminaires ouvrent la voie vers de nouvelles options thérapeutiques dans les PPGLm SDHB-dépendants.
